De novo CHD7 variant in a CHARGE syndrome preterm infant initially diagnosed as idiopathic hypogonadotropic hypogonadism: a case report and literature review.

BACKGROUND: CHARGE syndrome (CS), a rare and complex autosomal dominant disorder characterized by ocular coloboma, cardiac defects, choanal atresia, growth retardation, genital abnormalities, and ear malformations, is caused primarily by variants in the chromodomain helicase DNA-binding protein 7 (CHD7) gene. The clinical manifestations of CS frequently overlap with those of idiopathic hypogonadotropic hypogonadism (IHH), complicating diagnosis and management. To date, only a few cases of preterm infants with CS have been reported. Here, we describe the clinical features and genetic findings o

Management of severe tracheobronchomalacia with 3D printed resorbable airway splints: An Australian first.

INTRODUCTION: Tracheobronchomalacia (TBM) is the loss of the cartilaginous support of the trachea and/or major bronchi, leading to the dynamic collapse of the airway during respiration. It usually resolves by the age of two. Severe cases often present with recurrent pneumonias requiring hospitalisations, inability to extubate following illness or procedures, or life-threatening events. METHODS: We present the first Australian case of a successful implantation of tracheal splints using 3D printing technology to treat a 21-month-old with severe TBM who failed non-invasive treatments. His diseas

Facial Abnormalities Following Warfarin Exposure In Utero.

In low- and middle-income countries like Uganda, rheumatic heart disease (RHD) remains a leading indication for valve replacement, disproportionately affecting young women of reproductive age. Pregnancy in women with mechanical heart valves (MHVs) poses significant challenges due to the need for anticoagulation therapy, balancing maternal thrombotic risk against fetal safety. Warfarin, though highly effective in preventing valve thrombosis, carries teratogenic risks, including warfarin embryopathy, particularly when used during the first trimester. We report a case of a 21-year-old woman with

Clinical and molecular results in 15 Turkish patients with Wiedemann-Steiner syndrome: identification of eight novel KMT2A variants and a case of dual molecular diagnosis in the CSNK2A1.

UNLABELLED: Wiedemann-Steiner syndrome (WSS) is a rare autosomal dominant neurogenetic disorder caused by monallelic variants in KMT2A gene, characterized by neuromotor developmental delay, intellectual disability, microcephaly, seizures, behavioral disorders, dysmorphic facial features, hirsutism, and systemic anomalies. The KMT2A gene encodes a histone lysine methyltransferase crucial for the regulation of gene expression during early developmental stages. In this study, the clinical and molecular findings of 15 Turkish patients with WSS confirmed by whole exome sequencing are reported. Vari

Crouzonodermoskeletal syndrome requires individualized surgical management: Scoping review of a rare complex craniofacial syndrome.

Crouzonodermoskeletal syndrome (CDSS) is a rare condition characterized by premature fusion of the coronal sutures, midface hypoplasia, beaked nasal deformity, mandibular prognathism, and proptosis. A specific mutation in the Fibroblast Growth Factor 3 (FGFR3) gene causes CDSS (Ala391Glu) and thus provides a diagnostic marker for early identification. Beyond the Crouzon syndrome-like craniofacial appearance, the CDSS phenotype includes acanthosis nigricans, choanal stenosis/atresia, hydrocephalus, Chiari malformation, and achondroplasia, although presentations vary substantially throughout the

Endoscopic Dacryocystorhinostomy in a Patient With Hemi-Arrhinia.

Congenital arrhinia and hemi-arrhinia can present with varying degrees of nasal hypoplasia. There are significant anatomical consequences to the lacrimal system, and treatment of epiphora in these cases is challenging, with few reports of successful external dacryocystorhinostomy or combined endoscopic and external dacryocystorhinostomy. A 12-year-old girl with left hemi-arrhinia presented with left-sided epiphora and chronic dacryocystitis due to an incompletely formed nasolacrimal system. She underwent a left endoscopic dacryocystorhinostomy via a right-sided transnasal approach in conjuncti

Bilateral Choanal Atresia With Facial Deformity.

BACKGROUND: Bilateral choanal atresia is a rare condition. Secondary posterior choanal atresia typically results from trauma, radiotherapy, and chemotherapy, whereas congenital cases are primarily caused by gene mutations. Congenital bilateral choanal atresia often presents as complete nasal congestion and discharge, which, if not promptly diagnosed and treated, can lead to fatal asphyxia. METHODS: This condition is rare and rarely reported in adults, and here we report a case of bilateral posterior nostril atresia in an 18-year-old male. The patient presented with congenital bilateral poster

CHARGE Syndrome in a Six-Month-Old Male Infant: A Case Report.

CHARGE syndrome, short for coloboma, heart defects, atresia choanae, growth retardation, genital abnormalities, and ear anomalies, is a rare congenital disorder caused by genetic mutations. The diagnosis is based on a combination of major criteria, such as coloboma of the iris or choroid, choanal atresia, and hypoplastic semicircular canal, and minor criteria, including rhombencephalic dysfunction, hypothalamic-pituitary dysfunction, and malformations of the middle or external ear. Additional associated anomalies include malformations of mediastinal organs (heart, esophagus) and intellectual d

Standardization of Terminology, Definitions, and Outcome Criteria for Bleeding in Hereditary Hemorrhagic Telangiectasia: International Consensus Report.

Hereditary hemorrhagic telangiectasia (HHT, Osler-Weber-Rendu disease) is the second most common inherited bleeding disorder worldwide, affecting approximately 1 in 5000 people. Development of disease-modifying and efficacious hemostatic agents to treat HHT has finally begun after decades without such medical therapies. However, the lack of consensus on standardized severity definitions, outcome criteria, and terminology remains a major obstacle to clinical investigation and therapeutic development in HHT. Additionally, with the ongoing repurposing of antiangiogenic agents and emerging develop

Nasolabial shape and aesthetics in patients with cleft lip and palate: analysis of 3D facial images.

BACKGROUND/OBJECTIVES: In individuals with cleft lip and palate, nasolabial asymmetry and dysmorphology are associated with lower aesthetic ratings, though this relationship remains unclear. This study examines how deviations in nasolabial shape from the average affect aesthetic ratings. MATERIALS/METHODS: 3D stereophotogrammetric images were taken from 53 children with unilateral cleft lip and palate (UCLP) (34 boys, 19 girls; mean age: 10.5 ± 0.6 years) and 53 age and sex-matched children without cleft. UCLP repair was performed by three experienced surgeons in a single stage at 8.2 ± 2.3 m

Ortho-surgical management of Arhinia syndrome: A case report.

Introduction and importance: Arhinia syndrome (AS) is an extremely rare malformation characterized by typical clinical signs such as a lack of nose, with difficulty in breathing and inability to feed due to airway obstruction. The ortho-surgical management of AS is a big challenge and it requires an interdisciplinary relationship between the orthodontist and a maxillofacial surgeon. The authors reported a case of AS. Case presentation: The only case in Morocco among fewer than 30 cases described in the whole world, male child, 7-year-old. Clinical findings and investigations: Complete absence

A case report of tracheal duplication cyst with steatocystomatous histology: a rare embryological overlap.

BACKGROUND: Tracheal duplication cysts (TDCs) are rare congenital anomalies caused by defects in the duplication of the foregut, that is of endodermal origin. These cysts are usually present during childhood and cases in adults are seen very rarely. Steatocystoma multiplex is a disorder of multiple sebaceous cysts, most commonly from skin appendages, that has ectodermal origin. The presence of the two coexisting in the same place has never been reported in literature. We report the first case of a unique embryological overlap between ectoderm and endoderm in a TDC. CASE PRESENTATION: A 34-yea

A case of Schöpf-Schulz-Passarge syndrome presented with widespread apocrine hidrocystomas showing variable clinical features.

Dear Editor, Genodermatoses are usually diagnosed in early childhood but diagnosis may be delayed in rare cases. Schöpf-Schulz-Passarge syndrome (SSPS) first described in 1971 is a rare type of autosomal recessive ectodermal dysplasia characterized by palmoplantar keratoderma, periocular and eyelid apocrine hidrocystomas, hypodontia, hypotrichosis, and nail dystrophy (1). Fewer than 40 cases have been reported up to 2018 (2) and the number has slightly increased with subsequent case reports. We report here a 73-year-old male patient whose parents were third-degree cousins presented to dermatol

Palliative sedation - revised recommendations.

Palliative sedation is defined as the monitored use of medications intended to induce a state of decreased or absent awareness (unconsciousness) to relieve the burden of otherwise intractable suffering in a manner ethically acceptable to the patient, their family, and healthcare providers. In Switzerland, the prevalence of continuous deep sedation until death increased from 4.7% in 2001 to 17.5% of all deceased in 2013, depending on the research method used and on regional variations. Yet, these numbers may be overestimated due to a lack of understanding of the term "continuous deep sedation"

Prenatal Diagnosis of Crossed Pulmonary Arteries with a Postnatal Diagnosis of CHARGE Syndrome.

Introduction: Crossed pulmonary arteries (CPA) is an abnormality in which the ostium of the left pulmonary artery is located rightward and the ostium of the right pulmonary artery is leftward. Case report: We diagnosed a fetus with CPA prenatally. In fetal echocardiography, left pulmonary artery was seen to pass beneath the ductus and directing toward the left side and pulmonary artery bifurcation could not be demonstrated at the same plane. Postnatal echocardiography reconfirmed the presence of CPA. Bilateral choanal atresia, genital hypoplasia, hearing loss with facial and external ear asymm
