A sudden death case of laryngocele accompanied by infarction of the dorso-lateral portion of the medulla oblongata.

A 70-year-old man, who had a laryngeal tumor discovered by otolaryngological examination during admission for suspicion of facial nerve paralysis was found dead in his bed on the seventh hospital day. Clinical doctors aspirated a large amount of mucous sputum from his larynx during their attempt at resuscitation. Although the direct cause of death was clinically unknown, an autopsy revealed an internal type laryngocele, a type of laryngeal cyst, in the right larynx and infarction of the left dorso-lateral portion of the medulla oblongata. Suffocation resulted from mucous sputum filling his lar

Complications and functional outcome after supracricoid partial laryngectomy with cricohyoidoepiglottopexy.

We present a review of the postoperative course, complications, and functional outcome of 190 patients consecutively treated with supracricoid partial laryngectomy with cricohyoidoepiglottopexy. The average times until removal of the tracheostomy and nasogastric feeding tubes were 9 and 16 days, respectively. The postoperative mortality rate was 1%. Major complications included pneumonia from aspiration, cervical wound infection, symptomatic laryngocele, ruptured pexis, laryngeal chondroradionecrosis, and laryngeal stenosis in 8.5%, 4.2%, 3.1%, 1%, 0.5%, and 0.5% of the patients, respectively.

Supracricoid partial laryngectomies in the elderly: mortality, complications, and functional outcome.

A review of the postoperative course, complications, and functional outcome of 69 elderly patients (older than 65 years of age) consecutively managed with a supracricoid partial laryngectomy between 1977 and 1993 is presented. The tumors were glottic and supraglottic in origin in 53 and 16 patients, respectively. A cricohyoidoepiglottopexy, a tracheo-cricohyoidoepiglottopexy, and a cricohyoidopexy were performed in 48, five, and 16 patients, respectively. Immediate laryngeal, cervical, and medical complications were noted in 5.8%, 10.2%, and 10.2% of patients, respectively. Supracricoid partia

Laryngomucocele as an unusual late complication of subtotal laryngectomy. Case report.

We report an unusual case of laryngomucocele occurring after subtotal laryngectomy. Laryngoceles generally have a congenital origin in a long-preexisting saccule, and their association with laryngeal carcinoma is well known. Laryngocele is usually favored by the increase of intraglottic pressure caused by the laryngeal carcinoma. However, an iatrogenic secondary laryngomucocele occurring after a surgical procedure is uncommon. We report in detail the physiopathologic conditions leading to the creation of this lesion.

[Phyolaryngocele: a case report and review of literature].

A case of pyolaryngocele presented as a lateral cervical mass that produced acute dyspnea requiring tracheotomy. The diagnosis was by direct laryngoscopy, which revealed outflow of purulent material with pressure on the tumor, and was confirmed by CT. Broad-spectrum antibiotic treatment was given and the pyolaryngocele was excised by lateral extramucosal tyrotomy. The literature on laryngocele and pyolaryngocele was reviewed.

Internal jugular phlebectasia.

BACKGROUND: Anterior neck mass which appears on straining should be differentiated from laryngocele, jugular venous system phlebectasia and superior mediastinal cysts or masses. Internal jugular phlebectasia is a fusiform dilatation of the internal jugular vein. PURPOSE: this is an attempt to elucidate the etiology, clinical features, diagnosis, treatment and review of the literature about this rare venous lesion. METHODS: an 11 year old boy presented with soft neck swelling on the right side which appeared on straining. The diagnosis was confirmed on the bases of ultrasonography combined wi

Unusual neck masses secondary to jugular venous abnormalities: case report and discussion.

Thrombosis of the lateral sinus/proximal jugular vein and jugular venous ectasia are infrequent but not uncommon neck masses. Lateral sinus thrombosis was well described in the preantibiotic era, but modern-day surgeons may not be familiar with this entity. Failure to diagnose a neck mass due to jugular vein thrombosis may have deadly consequences. Jugular vein ectasia, which is innocuous, has been mistaken for laryngocele, cystic hygroma, as well as other vascular lesions. However, the management of jugular vein ectasia, which is thought to be caused by incompetence of the bicuspid jugular ve

Saccular aneurysm of the external jugular vein, an unusual cause of neck swelling.

Enlargement of neck swelling on Valsalva maneuver raises the diagnosis of laryngocele or jugular aneurysm. When considering this latter type of lesion, though fusiform cervical venous aneurysms represent a frequent occurrence, sacciform aneurysms are much rarer. We report two exceptional cases of saccular aneurysms of the external jugular vein which presenting symptom was a neck lump that enlarged on Valsalva maneuver. Although computed tomography and selective venography allow accurate diagnosis, ultrasonography with color flow duplex imaging is the gold standard for the diagnosis of such ven

Endoscopic management of a combined laryngocele.

A 70-year-old women presented with hoarseness, foul sputum, and a softneck mass. Clinical and radiographic examination disclosed findings consistent with a combined internal and external laryngopyocele. Antibiotics, throat irrigations, and warm packs applied to the neck resulted in full resolution of the neck mass and subtotal regression of the supraglottic swelling. Endoscopic vestibulectomy was performed with a carbon dioxide laser; neither residual neck mass nor a tract leading to the extralaryngeal neck swelling was detected. The immediate and long-term clinical course has been uneventful.

Fixity of vocal cords and laryngocele in acromegaly.

Acromegalic patients have a reduced life expectancy mainly due to cardio-, cerebrovascular and respiratory disorders and increased prevalence of neoplasias. Particularly, the pathogenesis of respiratory disorders in acromegalics is debated. Laryngeal abnormalities are not yet well clarified even if they are frequently involved in the occurrence of respiratory insufficiency. In this study, we report on a 65 year-old acromegalic male suffering from frequent and severe dyspnea attacks and clinical findings of respiratory upperway obstruction, besides the common acromegalic features. At the extern

[The significance of the valve mechanism in pathology of laryngocele].

We have described a case of pneumolaryngocele (saccular cyst), which was occurred as a tumor on patient's neck only after performating by him Valsalva maneuver, so it looked like inflating a balloon. The tumor was visible about one hour and then gradually and very slowly disappeared (collapsed), what had approved existence of valve mechanism in a pedincule of the laryngocele. We believe that the valve mechanism offers a feasible explanation of the pathomechanism of laryngocele. Long laryngeal saccule obstructed by the valve mechanism (air can pass only in one direction) and changes of an air p

[Apropos of an uncommon disease of the larynx: pyolaryngocele].

The laryngopyocele is a rare complication of laryngocele. About one case of laryngopyocele and review of literature, the authors recall the main characteristics of this affection. Its clinical picture is often alarming. The CT scan, not often made emmergenceley, give a characteristic image. The treatment is mainly surgical but also endoscopic, then facilitated by the use of laser. The fact of thinking to this diagnosis toward a laryngeal dyspnea with fever permit sometimes to avoid tracheotomy then usually made emmergencely.
