BACKGROUND: Excessive dynamic airway collapse (EDAC) and tracheobronchomalacia (TBM) are distinct causes of excessive central airway collapse; EDAC is characterized by excessive anterior displacement of the posterior membrane, whereas TBM is due to weakness of the cartilaginous rings. We compared outcomes after tracheobronchoplasty across these 2 pathologic processes.
METHODS: This was a single-center retrospective study including patients who underwent tracheobronchoplasty between 2018 and 2023. Preoperative dynamic computed tomography scans were reviewed by 4 airway experts, and patients we

Left pulmonary artery (LPA) sling is a rare congenital anomaly in which the LPA abnormally originates from the right pulmonary artery (RPA) and courses between the trachea and esophagus to reach the left pulmonary hilum. This anomaly is frequently associated with tracheobronchial and other cardiovascular anomalies and patients may manifest with varying airway and cardiovascular symptoms. Surgical repair is often required for symptomatic patients. Clinical outcomes largely depend on the extent and severity of coexisting anomalies, particularly tracheobronchial abnormalities. We report 2 autopsy

This case report describes a congenital anomaly involving a right cervical aortic arch and an aberrant subclavian artery (ASA) leading to vascular ring (VR) formation. This anomaly is characterized by the presence of the aortic arch in the cervical region rather than the thoracic cavity. The ASA can contribute to the formation of a VR encircling the trachea and esophagus, potentially causing severe respiratory complications. Early diagnosis and effective management in pediatric patients are crucial, given the life-threatening risk of respiratory compromise. In this case, the patient was diagno

INTRODUCTION: In this study we aim to prove we can compare 4D CT findings in children with clinically suspected tracheomalacia to a normalized 3D Pediatric Airway Atlas to evaluate for potentially clinically useful diagnostic metrics.
METHODS: This retrospective analysis included patients from the ongoing DYMOSA study who underwent 4D CT imaging for clinically suspected tracheomalacia. Tracheal 4D CT images were analyzed and compared to a weight-matched Pediatric Airway Atlas of children with radiographically normal airways. Key metrics included minimum airway cross sectional area, Airway Per

The severity of brachycephalic obstructive airway syndrome (BOAS) can vary in dogs with similar external traits. This study aimed to identify breed-specific upper airway lesions measured on computed tomographic (CT) images and their associations with the BOAS index (a measure of airway obstruction). This was a prospective cross-sectional study including 40 pugs, 52 French bulldogs, and 21 bulldogs. All subjects underwent whole-body barometric plethysmography respiratory function testing to obtain a BOAS index and CT of the head/neck. Eleven validated CT measurements were recorded from nares to

BackgroundVACTERL association is a mnemonically useful acronym for a condition characterized by the sporadic, non-random association of specific birth defects in multiple organ systems. Described in the early 1970s, it is typically defined by the presence of three or more of these congenital malformations: vertebral defects, anal atresia, cardiac defects, tracheoesophageal fistulas, renal anomalies, and limb abnormalities. In addition to these core components, patients may also have other congenital anomalies. VACTERL association does not involve neuro-cognitive impairment unless; associated w

PURPOSE: Evaluation of the effect of supraglottoplasty on co-existing tracheomalacia in pediatric patients with congenital laryngotracheomalacia to establish the venturi effect in vivo.
METHODS: A prospective interventional study was conducted in a tertiary care hospital from 2020 to 2024. All consecutive pediatric patients undergoing supraglottoplasty for congenital laryngomalacia, with co-existing tracheomalacia on pre-operative bronchoscopic assessment were included and were assessed for change in severity of tracheomalacia by bronchoscopy and clinical parameters post-surgery as a comparis

VACTERL association refers to the concomitant occurrence of congenital malformations such as vertebral defects, anorectal malformations, cardiac anomalies, tracheoesophageal fistula, renal anomalies, and limb defects. This case describes a newborn girl who presented with anomalies compatible with this association, such as scoliosis, sacral dysgenesis, right lower phocomelia, and multicystic renal dysplasia, also associated with myelomeningocele, which was surprising as it occurred concomitantly with the other congenital malformations. Although neural tube defects are not part of the classic cr

BACKGROUND: Conventional diagnostic assessment of congenital tracheal stenosis does not always closely correlate with clinical status, especially in asymptomatic children. Clinical decision making is often difficult in these circumstances. Computational fluid dynamics (CFD), in which computed tomography scans are reconstructed to map dynamic flow in the body, has shown promise as a tool for assessing airflow in children with congenital tracheal stenosis when traditional imaging modalities are limited. We sought to establish proof of this concept by applying CFD to congenital tracheal stenosis.

INTRODUCTION: Moebius syndrome is a rare congenital condition involving underdevelopment of cranial nerves VI and VII, often causing facial palsy. Also rare, bronchial stenosis may cause recurrent respiratory infections. There is scarce evidence on the outcomes of pediatric patients undergoing open surgical repair for congenital bronchial stenosis.
PRESENTATION OF CASE: An eight-year-old male with Moebius syndrome and tracheostomy-dependent since infancy, presented with fever, severe respiratory compromise and left lung atelectasis. Bronchoscopy revealed severe stenosis of the left main bronc

Central airway stenosis following congenital heart malformation surgery is a rare but significant cause of postoperative weaning failure. In selected cases, bronchoscopic interventions are effective treatment options for managing these kind of airway obstructions and achieving successful weaning. The data of six pediatric patients who were unable to be weaned from mechanical ventilation due to central airway obstruction following congenital heart malformation surgery were retrospectively analyzed. Rigid and flexible bronchoscopies were performed under general anesthesia for six patients.Six pat

RATIONALE: Tracheomalacia, typically seen in relapsing polychondritis,[1] is rarely reported in association with congenital heart disease (CHD). In patients with pulmonary hypoperfusion-type CHD, surgical repair results in a rapid increase in pulmonary blood flow, predisposing them to mucus retention, airway obstruction, and respiratory distress. We describe acute airway collapse in a patient with double outlet right ventricle and congenital bronchial stenosis following cardiac repair. Surgically augmented pulmonary flow critically narrowed the stenotic airway, rapidly reversed by bronchoscopi

INTRODUCTION: Tracheobronchomalacia (TBM) is the loss of the cartilaginous support of the trachea and/or major bronchi, leading to the dynamic collapse of the airway during respiration. It usually resolves by the age of two. Severe cases often present with recurrent pneumonias requiring hospitalisations, inability to extubate following illness or procedures, or life-threatening events.
METHODS: We present the first Australian case of a successful implantation of tracheal splints using 3D printing technology to treat a 21-month-old with severe TBM who failed non-invasive treatments. His diseas

Bronchiectasis is characterized by the permanent dilation of bronchi, clinically presenting with chronic cough, sputum production, dyspnea, and recurrent exacerbations. Bronchiectasis can occur due to genetic disorders, congenital malformations, endobronchial obstruction of infectious and inflammatory origin, and chronic aspiration, among other causes. Bronchial dilation leads to impaired mucociliary clearance, trapping particles and microorganisms in the airways. Macrophages and epithelial cells release proinflammatory cytokines that promote neutrophil chemotaxis. Neutrophils release enzymes

BACKGROUND: Coarctation of the aorta (CoA) is one of the more prevalent congenital heart diseases (CHD). The conventional treatment approach involves surgical correction of the coarctation. This procedure, however, can be associated with several complications, such as re-stenosis, aneurysm formation and pseudoaneurysm. One rare but significant complication is left bronchial compression, which may result from anterior displacement of the thoracic aorta or compression of the trachea by adherent tissue.
CASE PRESENTATION: We present a case of a 7-month-old postoperative CoA patient who developed

Heterotaxy syndrome is a rare congenital disorder characterized by abnormal arrangement of the thoracic and abdominal organs and is classified into polysplenia (left isomerism) and asplenia (right isomerism) syndromes. Polysplenia is associated with multiple splenic nodules, bilobed lungs, vascular anomalies, and organ malpositioning. In contrast, asplenia features bilateral trilobed lungs and eparterial bronchi, and is commonly associated with severe cardiac defects and high neonatal mortality. Here, we report 2 cases that illustrate the clinical spectrum of heterotaxy syndrome. The first cas

A 13-year-old male with a past medical history of Noonan syndrome, pulmonary valve stenosis status post balloon valvuloplasty and subsequent surgical valvotomy with main pulmonary artery augmentation, splenomegaly, and von Willebrand disease type 1 presents with chest pain, fever, and chronic, recurrent vomiting. At the outlying facility, computed tomography of the chest revealed a right upper lobe pulmonary cavitation. After an extensive workup, he was found to have a right-sided tracheal bronchus via bronchoscopy. Broad-range polymerase chain reaction analysis of the biopsied specimen demons

Heterotaxy syndrome is a congenital condition characterized by abnormal left-right axis patterning of thoracoabdominal organs, frequently accompanied by complex congenital heart disease and extracardiac anomalies. The condition demonstrates considerable phenotypic variability, posing diagnostic and management challenges. We report three pediatric cases of heterotaxy syndrome classified by atrial isomerism: two with left atrial isomerism (LAI) and one with right atrial isomerism (RAI). The first case involved a 22-month-old male with LAI and polysplenia who presented with a transitional atriove

Dynamic airway computed topography (4D-CT) can be used to evaluate the trachea in pediatric patients with tracheomalacia. The 4D-CT enables objective and quantitative evaluation throughout all phases of respiration; however, current systems focus on qualitative review of generated 4D images. Few analytic workflows are available to assist in the extraction of the quantitative geomorphic data generated. In this study, we share a protocol developed within the 3D Slicer framework that performs semi-automatic tracheal segmentation and subsequent geomorphic analysis. This methodology is applied to 4

Laryngotracheal stenosis (LTS) encompasses any airway-narrowing condition at the glottis, supraglottis, subglottis, or trachea. Stenosis in this critical area represents a challenge in clinical practice due to the variety of possible causes and the complexity of therapeutic management. Diagnostic modalities such as endoscopy and imaging techniques have the potential to demonstrate the severity of stenosis and guide treatment. An individualized assessment is required in patients with LTS to determine the utility of these modalities. The primary imaging technique used is thin-section CT, which m
