Esophageal atresia is a congenital disease that results in discontinuity of the esophagus and can occur with or without fistulous connections to the trachea. It is often associated with other anomalies and has multiple long-term health implications. Because of the complexity of care involved in affected children, it is important to diagnose this lesion as early as possible. Surgical repair via either anastomosis of the native esophagus or reconstruction using either the stomach or intestine if this is not possible remain the mainstays of therapy, although variations in technique prevail. BACKGROUND: Excessive dynamic airway collapse (EDAC) and tracheobronchomalacia (TBM) are distinct causes of excessive central airway collapse; EDAC is characterized by excessive anterior displacement of the posterior membrane, whereas TBM is due to weakness of the cartilaginous rings. We compared outcomes after tracheobronchoplasty across these 2 pathologic processes.
METHODS: This was a single-center retrospective study including patients who underwent tracheobronchoplasty between 2018 and 2023. Preoperative dynamic computed tomography scans were reviewed by 4 airway experts, and patients we

Left pulmonary artery (LPA) sling is a rare congenital anomaly in which the LPA abnormally originates from the right pulmonary artery (RPA) and courses between the trachea and esophagus to reach the left pulmonary hilum. This anomaly is frequently associated with tracheobronchial and other cardiovascular anomalies and patients may manifest with varying airway and cardiovascular symptoms. Surgical repair is often required for symptomatic patients. Clinical outcomes largely depend on the extent and severity of coexisting anomalies, particularly tracheobronchial abnormalities. We report 2 autopsy

This case report describes a congenital anomaly involving a right cervical aortic arch and an aberrant subclavian artery (ASA) leading to vascular ring (VR) formation. This anomaly is characterized by the presence of the aortic arch in the cervical region rather than the thoracic cavity. The ASA can contribute to the formation of a VR encircling the trachea and esophagus, potentially causing severe respiratory complications. Early diagnosis and effective management in pediatric patients are crucial, given the life-threatening risk of respiratory compromise. In this case, the patient was diagno

INTRODUCTION: In this study we aim to prove we can compare 4D CT findings in children with clinically suspected tracheomalacia to a normalized 3D Pediatric Airway Atlas to evaluate for potentially clinically useful diagnostic metrics.
METHODS: This retrospective analysis included patients from the ongoing DYMOSA study who underwent 4D CT imaging for clinically suspected tracheomalacia. Tracheal 4D CT images were analyzed and compared to a weight-matched Pediatric Airway Atlas of children with radiographically normal airways. Key metrics included minimum airway cross sectional area, Airway Per

The severity of brachycephalic obstructive airway syndrome (BOAS) can vary in dogs with similar external traits. This study aimed to identify breed-specific upper airway lesions measured on computed tomographic (CT) images and their associations with the BOAS index (a measure of airway obstruction). This was a prospective cross-sectional study including 40 pugs, 52 French bulldogs, and 21 bulldogs. All subjects underwent whole-body barometric plethysmography respiratory function testing to obtain a BOAS index and CT of the head/neck. Eleven validated CT measurements were recorded from nares to

BackgroundVACTERL association is a mnemonically useful acronym for a condition characterized by the sporadic, non-random association of specific birth defects in multiple organ systems. Described in the early 1970s, it is typically defined by the presence of three or more of these congenital malformations: vertebral defects, anal atresia, cardiac defects, tracheoesophageal fistulas, renal anomalies, and limb abnormalities. In addition to these core components, patients may also have other congenital anomalies. VACTERL association does not involve neuro-cognitive impairment unless; associated w

VACTERL association refers to the concomitant occurrence of congenital malformations such as vertebral defects, anorectal malformations, cardiac anomalies, tracheoesophageal fistula, renal anomalies, and limb defects. This case describes a newborn girl who presented with anomalies compatible with this association, such as scoliosis, sacral dysgenesis, right lower phocomelia, and multicystic renal dysplasia, also associated with myelomeningocele, which was surprising as it occurred concomitantly with the other congenital malformations. Although neural tube defects are not part of the classic cr

INTRODUCTION: Tracheobronchomalacia (TBM) is the loss of the cartilaginous support of the trachea and/or major bronchi, leading to the dynamic collapse of the airway during respiration. It usually resolves by the age of two. Severe cases often present with recurrent pneumonias requiring hospitalisations, inability to extubate following illness or procedures, or life-threatening events.
METHODS: We present the first Australian case of a successful implantation of tracheal splints using 3D printing technology to treat a 21-month-old with severe TBM who failed non-invasive treatments. His diseas

Dynamic airway computed topography (4D-CT) can be used to evaluate the trachea in pediatric patients with tracheomalacia. The 4D-CT enables objective and quantitative evaluation throughout all phases of respiration; however, current systems focus on qualitative review of generated 4D images. Few analytic workflows are available to assist in the extraction of the quantitative geomorphic data generated. In this study, we share a protocol developed within the 3D Slicer framework that performs semi-automatic tracheal segmentation and subsequent geomorphic analysis. This methodology is applied to 4

Laryngotracheal stenosis (LTS) encompasses any airway-narrowing condition at the glottis, supraglottis, subglottis, or trachea. Stenosis in this critical area represents a challenge in clinical practice due to the variety of possible causes and the complexity of therapeutic management. Diagnostic modalities such as endoscopy and imaging techniques have the potential to demonstrate the severity of stenosis and guide treatment. An individualized assessment is required in patients with LTS to determine the utility of these modalities. The primary imaging technique used is thin-section CT, which m

BACKGROUND: Tracheal duplication cysts (TDCs) are rare congenital anomalies caused by defects in the duplication of the foregut, that is of endodermal origin. These cysts are usually present during childhood and cases in adults are seen very rarely. Steatocystoma multiplex is a disorder of multiple sebaceous cysts, most commonly from skin appendages, that has ectodermal origin. The presence of the two coexisting in the same place has never been reported in literature. We report the first case of a unique embryological overlap between ectoderm and endoderm in a TDC.

BACKGROUND: Globally, birth defects are the fourth most common cause of neonatal mortality. They cause substantial morbidity, and often long-term disability. Despite this, the impact of birth defects on public health has received little attention in low- and middle-income countries.
AIMS: To report the types, incidence and geographic distribution of birth defects in the East New Britain Province of Papua New Guinea.
METHODS: Data were collected over 3 years on newborns with birth defects seen at Rabaul Hospital, born anywhere in the province. Each affected newborn was examined, the anomaly d

BACKGROUND: In this study we aimed to describe the morphological and pathogenetic differences between tracheal agenesis and tracheal atresia, which are not clearly distinguished from each other in the literature, and to contribute thereby to the understanding and management of these conditions. Both tracheal agenesis and tracheal atresia represent rare disorders of still unknown aetiology that cannot be detected by prenatal ultrasound. If the affected foetuses survive until birth these conditions result in respiratory failure and in futile attempts to rescue the infant's life.
RESULTS: Autops

The trachea is essential for proper airflow to the lungs for gas exchange. Frequent congenital tracheal malformations affect the cartilage, causing the collapse of the central airway during the respiratory cycle. We have shown that Notum, a Wnt ligand de-acylase that attenuates the canonical branch of the Wnt signaling pathway, is necessary for cartilaginous mesenchymal condensations. In Notum deficient tracheas, chondrogenesis is delayed, and the tracheal lumen is narrowed. It is unknown if Notum attenuates non-canonical Wnt signaling. We observed premature tracheal chondrogenesis after mesen

INTRODUCTION: Mounier-Kuhn syndrome or tracheobronchomegaly, is a rare condition that consists of abnormal dilation of the trachea and main bronchi due to a pathological arrangement of smooth muscle fibers in this area.
CASE REPORT: We present the case of a 46-year-old woman with poorly controlled asthma and recurrent infections, who was diagnosed with Mounier-Kuhn syndrome through a computed tomography scan revealing an unusual enlargement of the trachea with associated bronchiectasis.
RESULTS: The diagnosis of Mounier-Kuhn syndrome is radiological, involving measurement of the trachea wher

INTRODUCTION: The postoperative management of single-stage laryngotracheal reconstruction (ssLTR) plays a significant role in the surgery's outcomes. The relatively prolonged period in which the child remains intubated and sedated to allow graft healing may be complicated by pulmonary sequelae, airway obstruction, withdrawal symptoms, and eventually failed extubation. This study aims to assess post-ssLTR practices among pediatric otolaryngologists.
METHOD: An electronic cross-sectional survey was distributed to the American Society of Pediatric Otolaryngology (ASPO) members to elucidate curre

Tracheal bronchus is a rare congenital tracheal abnormality that generally refers to the right upper lobe bronchus of the lung that originates from the trachea. Tracheal bronchus is usually asymptomatic and is often accidentally detected by fiberoptic bronchoscopy or computed tomography for other conditions. Depending on the location of the tracheal bronchial opening and possible anatomical variations, the management of 1-lung ventilation in patients with tracheal bronchus is a significant challenge for anesthesiologists. To provide a reference for anesthesiologists to better manage anesthesia

PURPOSE: Congenital right aortic arch (RAA) and double aortic arch (DAA) were difficult to be diagnosed by traditional fetal echography. However, these two diseases have distinct clinical therapies and long-term outcomes which makes the differential diagnosis of great importance. While fetal screening by traditional fetal echography provides limited information about the branches of the aortic arch. This study aimed to discover and evaluate a novel combination of different echocardiography views in the diagnosis and differentiation of RAA and DAA.
METHODS: From January 2014 to December 2022,
