The Outcomes of Slide Tracheoplasty for Complete Tracheal Rings in Children With a Tracheal Bronchus.

OBJECTIVE: To compare airway presentation and outcomes following slide tracheoplasty for congenital tracheal stenosis with complete tracheal rings in children with and without a tracheal bronchus. STUDY DESIGN: Retrospective cohort study. SETTING: Quaternary pediatric referral center. METHODS: Children undergoing slide tracheoplasty for complete tracheal rings between 2001 and 2021 were included. Clinical variables and outcomes were compared by tracheal bronchus status. Propensity score matching was performed to adjust for baseline differences. Operative success was defined as a symptom-fre

Multimodality imaging and surgical management of a rare combination: type 2A bridging bronchus, pulmonary artery sling, and atrial septal defect, a case report.

We report the case of a 15-year-old female presenting with exertional syncope and a history of refractory respiratory symptoms. Comprehensive preoperative multimodality imaging-including echocardiography, multi-detector computed tomography (MDCT) with three-dimensional reconstruction, and bronchoscopy-revealed a rare complex of congenital anomalies: a type 2 A bridging bronchus (BB), a pulmonary artery sling (PAS), and an atrial septal defect (ASD). Crucially, imaging confirmed the absence of significant tracheal stenosis, thereby obviating the need for tracheoplasty. The patient successfully

Congenital esophageal stenosis related to tracheobronchial remnant: Balloon dilatation versus resection and anastomosis - A systematic review.

INTRODUCTION: Congenital esophageal stenosis (CES) related to tracheobronchial remnant (TBR) is an extremely rare malformation, typically diagnosed in infants and young children. Endoscopic dilatation or resection and anastomosis are both accepted therapeutic options, but the superiority of one versus the other remains controversial. METHODS: We performed a comprehensive literature search in PubMed and MEDLINE using standardized search terms and subject headings from 1969 onwards. Inclusion criteria were patient age 0-18 years, confirmed diagnosis of CES due to TBR, English language, interven

Bilateral Bronchus Suis Tracheoplasty.

Complex tracheal anomalies are life threatening without intervention and often require surgery in the neonatal period to establish a stable airway. Variations of slide tracheoplasty using all autologous tissue are the gold standard in tracheal reconstruction. We have encountered 9 patients whose complex tracheal anomalies required significant variations to slide tracheoplasty. Six of these patients presented with bronchus suis with extreme long segment tracheobronchial stenosis which were effectively repaired using a side-to-side bronchus suis tracheobronchoplasty. This technique has the advan

Echocardiographic diagnosis of a pulmonary artery sling: what kept it hidden for seven years?

Pulmonary artery sling (PAS) is a rare cardiovascular anomaly, occurring in approximately 1 in 17,000 individuals, often leading to severe tracheal compression and respiratory distress. This case report details the diagnosis, treatment, and follow-up of a seven-year-old female patient with PAS, a condition that remained undiagnosed for years despite multiple hospitalisations. Initially admitted at 4 weeks old for neonatal pneumonia, hypoxic encephalopathy, and patent ductus arteriosus (PDA), the patient underwent various treatments and misdiagnoses, including bronchopneumonia and congenital he

Bronchoscopy-Guided Bronchial Blocker for Acute Right Pulmonary Artery Hemorrhage in an Infant With Berry Syndrome: A Case Report.

BACKGROUND Berry syndrome is a rare congenital cardiovascular disorder characterized by typical malformations (eg, right pulmonary artery [RPA] origin from the aorta, aortic arch interruption, and atrial septal defect), most of which require surgical correction in infancy or early childhood. CASE REPORT A 1-week-oldinfant presented with postnatal dyspnea, irregular respiration, and poor weight gain. A diagnosis of Berry syndrome was confirmed via transthoracic echocardiography and cardiac great-vessel computed tomography angiography. The infant first underwent successful initial surgery to cor

Airway Management in Fontan Patients with LPA Stenosis.

Left pulmonary artery (LPA) stenosis is encountered in up to 65% of patients with a Fontan circulation. Stenting of LPA stenosis can be complicated by compression of the left bronchus. We evaluated the effect of periprocedural airway management with three-dimensional rotational angiography (3DRA) and bronchoscopy on the occurrence of left main bronchus compression in Fontan patients undergoing percutaneous treatment for LPA stenosis. All pediatric cardiac catheterizations for LPA stenosis between 2011 and 2024 as well as the pre- and postoperative course were analyzed. In 64 of a total of 109

Cryoextraction for Airway Bleeding After Valve-in-Valve Procedure.

BACKGROUND: Hemoptysis after percutaneous valve implantation is a rare but life-threatening complication. CASE SUMMARY: An 84-year-old woman with prior mechanical aortic and biological tricuspid valve replacements presented with tricuspid stenosis and regurgitation. A transcatheter valve-in-valve procedure using a 24.5-mm Myval was performed successfully. However, she developed severe hemoptysis, hypoxia, and ventilation difficulties shortly after the procedure. Flexible bronchoscopy with cryoprobe-assisted clot extraction was performed, removing large obstructive clots from the bronchi. A se

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Computational fluid dynamics as an objective measure for assessing congenital tracheal stenosis: Proof-of-concept study.

BACKGROUND: Conventional diagnostic assessment of congenital tracheal stenosis does not always closely correlate with clinical status, especially in asymptomatic children. Clinical decision making is often difficult in these circumstances. Computational fluid dynamics (CFD), in which computed tomography scans are reconstructed to map dynamic flow in the body, has shown promise as a tool for assessing airflow in children with congenital tracheal stenosis when traditional imaging modalities are limited. We sought to establish proof of this concept by applying CFD to congenital tracheal stenosis.

Acute stent placement for dynamic airway collapse after cardiac surgery in congenital heart disease with bronchial stenosis: A case report.

RATIONALE: Tracheomalacia, typically seen in relapsing polychondritis,[1] is rarely reported in association with congenital heart disease (CHD). In patients with pulmonary hypoperfusion-type CHD, surgical repair results in a rapid increase in pulmonary blood flow, predisposing them to mucus retention, airway obstruction, and respiratory distress. We describe acute airway collapse in a patient with double outlet right ventricle and congenital bronchial stenosis following cardiac repair. Surgically augmented pulmonary flow critically narrowed the stenotic airway, rapidly reversed by bronchoscopi

Application of fibrobronchoscopy-guided aortic suspension in pediatric patients with bronchial compression resulting from repair of coarctation of the aorta: a case report.

BACKGROUND: Coarctation of the aorta (CoA) is one of the more prevalent congenital heart diseases (CHD). The conventional treatment approach involves surgical correction of the coarctation. This procedure, however, can be associated with several complications, such as re-stenosis, aneurysm formation and pseudoaneurysm. One rare but significant complication is left bronchial compression, which may result from anterior displacement of the thoracic aorta or compression of the trachea by adherent tissue. CASE PRESENTATION: We present a case of a 7-month-old postoperative CoA patient who developed

From multiple spleens to absence: Insights from two cases of heterotaxy syndromes.

Heterotaxy syndrome is a rare congenital disorder characterized by abnormal arrangement of the thoracic and abdominal organs and is classified into polysplenia (left isomerism) and asplenia (right isomerism) syndromes. Polysplenia is associated with multiple splenic nodules, bilobed lungs, vascular anomalies, and organ malpositioning. In contrast, asplenia features bilateral trilobed lungs and eparterial bronchi, and is commonly associated with severe cardiac defects and high neonatal mortality. Here, we report 2 cases that illustrate the clinical spectrum of heterotaxy syndrome. The first cas
