[Diffuse esophageal leiomyomatosis. Apropos of 5 cases with 2 familial cases].

Diffuse esophageal leiomyomatosis is characterized by diffuse muscular hypertrophy chiefly marked in the lower end of the esophagus looks at a large tumor. It involves the entire length of the esophagus and down to the upper part of the stomach (esogastric leiomyomatosis). Diffuse leiomyomatosis is a extremely rare pathology encountered in children (13 previous report cases) and young adults. Il may be isolated or found in association with other intrathoracic and genital localisations or Alport syndrome (nephropathy, sensorineural deafness, ocular lesions). Extensive leiomyomatosis, engulfing

[Chronic obstructive bronchopneumopathy and ectodermal dysplasia].

A boy, 5 year aged, admitted in intensive therapy room because affected by respiratory insufficiency, is reported. Recurrent lower respiratory tract infections and three surgical operation in the lacrimal ducts were present in the anamnesis. Clinical and radiologic features of chronic obstructive pneumonia were present. Hypotrichosis, onychodystrophy, microdontia, associated with absence of muciparous glands and greatly reduced muciparous cells of respiratory tract, were other findings.

Crucial bronchoscopic findings in esophageal atresia and tracheoesophageal fistula.

We performed a brief bronchoscopy in 42 newborn infants with esophageal atresia and tracheoesophageal fistula (TEF). The procedure was carried out in the operating room, usually just prior to thoracotomy for repair of the anomaly. Most infants had a gastrostomy tube placed initially. Observations included: (1) level of fistula, (2) presence of unusual variants, eg, double fistula, trifurcation fistula, (3) presence and severity of tracheobronchitis, and (4) position of the aortic arch. The endoscopic findings influenced the operative technique or management of 24 of the 42 infants (57%), inclu

Reconsideration of the anatomy of sling left pulmonary artery: the association of one form with bridging bronchus and imperforate anus. Anatomic and diagnostic aspects.

In sling (retrotracheal) left pulmonary artery (SLPA), the tracheobronchial pattern is generally considered basically normal. Analysis of dissected specimens and/or bronchograms and other preparations from five studied and 32 reported patients suggests that there are two different forms of SLPA: (1) types 1A and B, with normal TB pattern [with (A) or without (B) a right pre-eparterial (tracheal bronchus)], and the aberrant left pulmonary artery causing TB compression. The tracheal bifurcation in type 1 SLPA is usually demonstrable at the fourth to fifth thoracic vertebral level; (2A) SLPA type

[Morphogenesis of cystic abnormalities of the lung].

The material used was the pathomorphological studies of cystic developmental lung defects with consideration of X-ray and clinical data. In total 126 cases have been studied including: 72 cystic lung hypoplasias, 38 solitary cysts, 8 sequestrations, 8 bronchogenic cysts of mediastinum. Cystic hypoplasia revealed branching of cystic cavities and sometimes their connection with respiratory compartments. The other above mentioned defects reveal sometimes the connection between cysts, bronchi and respiratory compartments. Consequently with the formation of cystic developmental lung defects the dev

Congenital bronchial atresia. A report of 4 cases and a review of the literature.

The clinical, radiographic, and pathologic findings in 82 patients with congenital bronchial atresia (CBA) have been reviewed, and we have discussed 4 additional cases. Most patients are asymptomatic and come to attention because of abnormal radiographic findings of a round or lobulated perihilar, solid, or cystic mass--the mucoid impaction sign. Typically, the region distal to the mass is hyperinflated. Recently, computed tomography has been shown to be diagnostic and its use obviates the need for other more complex imaging modalities or surgical exploration. Excisional surgery has been perfo

Unexpectedly normal cilia and spermatozoa in an infertile man with Kartagener's syndrome.

We investigated a 28-year-old, infertile male patient with the characteristic clinical signs of Kartagener's syndrome (i.e. bronchiectasis, recurrent sinusitis, situs inversus). The patient had tried unsuccessfully for 3 years to become a father, and no cause of infertility was detected in his wife. The functional capacities tested in vitro of nasal and tracheal ciliated cells, spermatozoa and neutrophils were found to be normal. The tracheo-bronchial clearance was not impaired. The ultrastructure of ciliary and flagellar axonemes was normal. This case of Kartagener's syndrome without axonemal

Bronchial compression by a patent ductus arteriosus associated with pulmonary atresia.

Two infants with pulmonary atresia are presented, wherein a large, vertically oriented patent ductus arteriosus, acting as the only source of pulmonary blood supply, compressed the left bronchus, causing significant respiratory distress. Hyperinflation of the left lung was the clue to this circumstance. After bronchographic confirmation, surgical repair was accomplished by placement of aortopulmonary shunts and division of the patent ductus. Other causes of airway compression in congenital heart disease are briefly reviewed. The embryology of the ductus arteriosus is discussed, emphasizing its

A mathematical model for the quantitative study of left to right cardiac shunt.

Non-invasive radioisotope cardiographic techniques have become a useful tool for studying the anatomy and function of the heart. The mathematical model described justifies the phenomenological analysis of pulmonary time-activity histograms and was invented for quantitative study of left to right cardiac shunt. The model also gives a theoretical insight into such an empirically proposed diagnostic method and represents an adequate framework for the understanding of other possible approaches to the problem of left to right shunt.

Plate atelectasis.

Postmortem examination was performed in 15 patients who had plate atelectasis (PA) on their last antemortem radiograph. In 10 patients, corresponding abnormalities were found in the lungs. In all 10, there was peripheral subpleural linear collapse combined with invagination of the overlying pleura; prominent interlobular septa were observed within or bordering the linear atelectasis in nine. The atelectasis and the invaginated pleura both contributed to the linear radiographic density. The bronchi supplying the areas of PA showed no obstruction nor rearrangement. Despite a frequent association

Tracheal bronchus: association with respiratory morbidity in childhood.

An aberrant right upper lobe (RUL) bronchus arising from the trachea (tracheal bronchus) can be responsible for recurrent pneumonia. In this hospital, 2% of children requiring bronchoscopy for respiratory symptoms are found to have a tracheal bronchus, which is frequently thought to be an incidental finding. We reviewed findings in 18 patients to determine when a tracheal bronchus is of clinical significance. The age at presentation ranged from 1 day to 54 months (mean 17 months). The children had recurrent pneumonia (nine), stridor (six), respiratory distress (two) and a thoracic mass (one).

The dyskinetic cilia syndrome in childhood. Modifications of ultrastructural patterns.

The syndrome of cilia dyskinesia is known as a heterogenous ciliary dysfunction caused by morphological defects of the dynein arms, the nexin links, the radial spokes and by the transposition of microtubules. Supernumerary tubules have been regarded as acquired morphological defects on the background of other bronchopathies. The report of a 9-year-old girl with the clinical signs of ciliary dyskinesis is considered to be an attribution to the clinical and pathological features of this syndrome. The girl's history of chronic bronchopulmonary infections and nasal polyposis resistant to therapy m

[Immotile cilia disease with neonatal disclosure. Ultrastructural study].

Recurrent bronchopulmonary and E.N.T. infections in a 2-month old child with complete situs inversus suggested an immotile cilia syndrome. Electron microscopy of the respiratory epithelium cilia demonstrated an ultra structural abnormality (defective radial spokes) typical of this recently discovered syndrome. This case is similar to 3 other cases in infants reported in the literature. It shows that the clinical manifestations of ciliary dysfunction may occur soon after birth and that early detection is desirable for optimal treatment. The type of abnormality detected and the percentage of cil

Atypical bronchial cilia in children with recurrent respiratory tract infections. A comparative ultrastructural study.

Ultrastructurally atypical bronchial cilia are studied and semiquantitatively analysed in 24 children suffering from recurrent respiratory tract infections with or without bronchiectasis. In patients with Kartagener's syndrome normal-looking and shortened dynein arms are present at some axonemal microtubular doublets. This finding suggests that the polymerization or assemblage of dynein molecules on microtubules only is defective but not totally lacking. Bilateral, local and partial absence of dynein arms is demonstrated in some of the patients with acquired unilateral bronchiectases. These pa

[Association between bronchial dilatation and male sterility].

The frequency of associated bronchial dilatation and male sterility is sufficiently high for simple coincidence to be discounted. Initially observed in patients with Kartagener's syndrome, an abnormal frequency of bronchial disorders (with or without ectasia)M was then noted in subjects with azoospermia, especially of the excretory type. Originally proposed by AFZELIUS et al in 1975, an explanation for the association could be the presence of ultrastructural anomalies of the cilia, both in the bronchial mucosa and in the single cilium of the spermatozoid. Other physiopathological explanations

Measurement of bronchial mucociliary clearance.

We briefly review bronchial mucociliary clearance: one of the non-specific host defence mechanisms operating in the lungs, helping to maintain cleanliness and sterility. We describe in detail the measurement in man of bronchial mucociliary clearance by means of the radioaerosol technique used in our laboratory. We report the use of the technique for detecting impairment of tracheobronchial clearance in patients with chronic bronchitis compared with healthy control subjects. We illustrate whole lung and tracheobronchial retention curves in a patient with Kartagener's syndrome - and also prelimi

The immotile-cilia syndrome compared to other obstructive lung diseases: a clue to their pathogenesis.

Twentyfour adults, aged 19-47 years, with congenitally nonfunctioning cilia were investigated by clinical examination, radiography, spirometry, and in most cases by tracheobronchial clearance measurements and ultrastructural examination of respiratory tract cilia and/or sperm tails. They all suffered from chronic bronchitis, rhinitis and sinusitis, and most of them had bronchiectasis and a history of otitis. Most had spirometrically overt obstructive lung disease and four radiological signs of emphysema. Clinical profile, lung function and mucociliary clearance data were compared to correspond

Measurements of tracheobronchial clearance in patients with immotile-cilia syndrome and its value in differential diagnosis.

Tracheobronchial clearance was studied in twenty patients who fulfilled suggested criteria of the immotile-cilia syndrome and in eight other patients suspected of suffering from the syndrome, but who did not fulfill the criteria. Clearance was studied using 6 micrometers teflon aerosol particles tagged with 99mTc and external measurement of the radioactivity. The criteria were based on combinations of a typical history of bronchitis, rhinitis and sinusitis since childhood, hereditary data, situs inversus, sperm immotility and characteristic defects of respiratory tract cilia and sperm tails. The immotile cilia syndrome: explanation for many a clinical mystery.

With the advent of electron microscopy, ciliary abnormalities could be detected, analyzed, and correlated with clinical problems. Kartagener's syndrome, identified over 40 years ago as a triad of sinusitis, bronchiectasis, and situs inversus, was found to actually be a subset of a broader category of diseases: the immotile cilia syndrome. Two other subsets involving multiple organ systems have also been recognized.
