Kartagener's syndrome (situs inversus totalis, bronchiectasis and sinusitis) was demonstrated in two patients. Electron microscopic investigations of the bronchial epithelium showed an accidental arrangement of ciliary axes, but not the expected lack of dynein arms. In both cases severe disturbance of maturation of spermiogenesis, not noted in this syndrome so far, was observed. Whereas one patient is largely free of symptoms after two operations, the other died after the second operation as a consequence of bronchopleural fistulation and bilateral pneumonic complications.

A 17-day-old infant with a right-sided extralobar pulmonary sequestration associated with an intact diaphragmatic hernia sac presented as an acute respiratory emergency. The diagnosis was obscured by the initial resuscitative treatment of cardiorespiratory arrest necessitating positive-pressure ventilation complicated by bilateral tension pneumothoraces prior to transfer. At operation, unique systemic arterial and venous communications to the sequestration from the dome of the liver through the intact hernia sac were found. A bronchial communication to the right main bronchus was also present.

Two cases of congenital esophageal stenosis due to tracheobronchial remnants are reported, one accompanied by anovestibular fistula and the other by microphthalmos with iris coloboma. A survey of congenital esophageal stenosis due to tracheobronchial remnants in Japan revealed that the incidence of associated anomalies in this lesion is 17.3% This value is significantly lower than that reported for esophageal atresia. Our analysis showed that esophageal atresia is the most frequently associated anomaly, followed by anorectal abnormalities. Cardiovascular anomalies are very uncommon. Embryology

The authors describe an infant whose right-lower-lobe bronchus arose from the left main bronchus and bridged the lower mediastinum. Other major congenital anomalies were present as well. Possible explanations for crossed airways, which would seem to contradict the current theory of embryonic lung development, are suggested.

Five children presented with recurrent respiratory illnesses and were noted to have pectus excavatum. In four patients subsequent investigation demonstrated a collapsible segment in the middle third of the left main bronchus, which appeared to lack cartilage support and caused gas trapping in the left lung. The remainder of the bronchial tree was normal. In the remaining child the clinical and physiologic pattern was very similar. In three children there were no other anomalities, but two had congenital heart disease. The association of bronchial anomalies and pectus excavatum may occur more o

Of 219 patients whose primary presenting symptom was stridor, 150 were males, 69 were females. All were under 2 1/2 years of age; more than half were four months of age or younger. Congenital anomalies caused stridor in 191 (87.2%). Congenital laryngeal anomalies accounted for the stridor in 132 (60.3%); there were 35 (16.0%) patients with congenital tracheal anomalies, 11 (5.0%) with congenital bronchial anomalies, 12 (5.5%) with infectious conditions, 12 (5.5%) with internal laryngeal trauma and 15 (6.8%) patients with other conditions. The cause of stridor was undetermined in two patients.

The atrial morphology and venous connections were assessed "blind" in 51 necropsy specimens from patients with visceral heterotaxy. This was compared with bronchial morphology as established by dissection. Six specimens were found to have both atria and bronchi in situs solitus or inversus, and were rejected. In the remainder, atrial isomerism was diagnosed, though this required minor revision of the atrial assessment in two patients. Thirty-four patients had isomeric right atria and bronchi, while 11 had isomeric left atria and bronchi. In seven cases, splenic status was unknown, but in seven

A detailed quantitative analysis was made of the lungs from 8 infants dying with bilateral renal agenesis or dysplasia. Total lung volume was reduced in all cases, particularly in those with renal agenesis. In both groups there was a reduction in number of airway generations, indicating interference with development at between 12 and 16 weeks' gestation. The alveoli in each acinus were reduced in size and, in some cases, number--although their stage of differentiation was normal for age--pointing to a disturbance of growth during later fetal life also. As liquor is largely non-renal in origin

Congenital tracheal bronchus is an uncommon condition that may occur together with other, sometimes clinically significant anomalies of the tracheobronchial tree. Two such patients are reported. Both presented with respiratory distress, and one was initially diagnosed as having a pulmonary cyst. Radiographic demonstration of an upper-lobe cyst, particularly on the right, should suggest a tracheal bronchus.

Localized persistent mucoid impaction as seen on the chest radiograph is a fairly common roentgen finding that always implies segmental bronchial obstruction. Collateral ventilation apparently permits air to enter the lung distal to the obstruction, outlining bronchi distended with trapped mucus or other secretions. This mucoid impaction sign has been seen in a variety of obstructing conditions, including primary and metastatic carcinoma of the lung, bronchial adenoma, tuberculous bronchostenosis, bronchial adenoma, tuberculous bronchostenosis, broncholithiasis, bronchial atresia, sequestratio

Extralobar pulmonary sequestrations occurring in 15 patients were studied. Twelve of the lesions were discovered during the first day of life; all of these patients died. The lesions occurred more often in male patients (11 cases). Polyhydramnios was present in four infants, and these four plus an additional infant had localized or generalized edema. Associated congenital anomalies were present in ten patients, including three examples each of diaphragmatic hernia and bronchogenic cyst. In one case the extralobar pulmonary sequestration was composed entirely of a congenital cystic adenomatoid

The authors examined 143 members of a family, where they found 37 (= 25.87%) of Osler patients. It was only in 30% of these patients that the symptoms occurred before the tenth year of age. As in one patient the symptoms did not appear until the age of 58 years, the possibility cannot be excluded that symptoms of the disease will become manifest even in other, sill younger members of the family in the course of time. Epistaxis was observed in 93% of the cases, nephrorrhagia in no case, hepatopathy and gastrorhagy were found only once in each case. The X-ray examination revealed arteriovenous p

Agenesis of the lung is rare. The cases of ten patients with this malformation are recorded and illustrated. Most had congenital malformation of other organs, especially the heart, as well. All had hypoplasia of the pulmonary artery or its branches. A pantaloon-like appearance of the trachea and the main bronchi on the bronchograms is described. Exercise tolerance was impaired in all patients.

We describe a case of extreme primary pulmonary hypoplasia. No other congenital anomalies and none of the conditions known to be associated with pulmonary hypoplasia were present. Pregnancy had been complicated by substantial polyhydramnios. The hypoplasia was due to a marked deficiency of the respiratory parenchyma in the presence of normal upper airways and bronchi. Virtually no parenchymal development had occurred and there were very few bronchioles, alveolar ducts, and alveoli. The changes differ from those seen in pulmonary hypoplasia secondary to congenital diaphragmatic hernia, bilatera

Segmental lung resection was performed in 5 infants with congenital lung disease. The lesions were cystic adenomatoid malformation in 2, congenital cystic disease in 2, and congenital lobar emphysema in 1. A higher than expected incidence of major segmental bronchovascular anomalies was encountered (3 of 5 patients). Other than prolonged air leak in 1 patient, complications were negligible. All patients were well from one to six and one-half years after operation. Segmentectomy is a sensible operation for congenital pulmonary disease and ultimately may have as its greatest virtue the preservat

This report describes the clinical features and pathologic anatomy seen in an infant with anomalous bronchial branching and other congenital anomalies. The patient died of respiratory insufficiency and infection, possibly fostered by the malformed airway and a coexistent partial anomalous venous return. At autopsy, bronchial drainage of the right middle and lower lobes was provided by a large bronchial branch that originated in the left main-stem bronchus and bridged the mediastinum before entering the contralateral lung. Since crossed anatomic airway relationships after formation of primary b

Two siblings in whom respiratory symptoms developed immediately after birth subsequently were found to have bronchiectasis with strikingly similar distribution of lesions (mainly lower lobes). Inspiratory and expiratory bronchograms performed on one of the siblings demonstrated marked ballooning and collapse of proximal bronchi during tidal breathing. The clinical courses and roentgenographic findings, in the absence of other underlying abnormalities, suggest that the basic disease process was the absence of bronchial cartilage (Williams-Campbell syndrome). This would be the first reported fam

Mucociliary transport in the tracheobronchial tract was studies in 2 subjects by having them inhale a radioactively tagged test aerosol and by taking external measurements of the radioacitivity in the lungs for 2 hours. Both subjects had living, yet immotile seprmatozoa; the sperm tails lacked normal dynein arms, which are essential for the bending movements of sperm tails as well as of other cilia. The subjects were classified as suffering from Kartagener's syndrome (situs inversus, chronic sinusitis, and bronchiectasis) and were shown to have an extremely slow, probably absent transport. The
