Making the Diagnosis of Primary Ciliary Dyskinesia in Australia.

BACKGROUND AND OBJECTIVE: Primary Ciliary Dyskinesia (PCD) is a genetic disorder associated with recurrent infections and irreversible airway damage. No single diagnostic test exists, necessitating use of a diagnostic algorithm that varies internationally. This retrospective study aimed to describe the clinical characteristics of patients, and algorithm used to diagnose PCD in New South Wales between 2014 and 2024. METHODS: A retrospective review of patients diagnosed with PCD at Concord Repatriation General Hospital (CRGH) from 2014 to 2024 was performed. RESULTS: This study included 52 pat

Maternal autoimmune diseases during pregnancy and mortality up to young adulthood in offspring: a register-based cohort study.

BACKGROUND: The implications of maternal autoimmune diseases for offspring survival after infancy remain poorly understood. We aimed to examine the association between maternal autoimmune diseases during pregnancy and all-cause and cause-specific mortality in offspring from birth to early adulthood. METHODS: In this nationwide population-based cohort study, we included all live births in Sweden between 2001 and 2014 and followed individuals until death, emigration, or December 31, 2023. Cox proportional hazards models were used to analyze the association between maternal autoimmune diseases d

Interferon gamma-induced MX1 and IFITM1 inhibit Varicella-Zoster Virus Replication.

UNLABELLED: Interferon gamma (IFN-γ) is a potent cytokine secreted following primary varicella-zoster virus (VZV) infection. Our previous studies demonstrated that pretreatment with IFN-γ completely inhibited VZV replication in lung fibroblast MRC-5 and retinal epithelial ARPE-19 but not in melanoma MeWo cells, suggesting that IFN-γ-stimulated protein(s) inhibit viral replication. Our microarray analysis revealed that 34 interferon-stimulated genes (ISGs) were upregulated by greater than 2.5-fold at 8 h post-treatment in both ARPE-19 and MRC-5 cells compared to those of MeWo cells. The depleti

Secondary pulmonary alveolar proteinosis mimicking progression of interstitial lung disease during treatment for adult-onset still's disease: An autopsy case report.

RATIONALE: Secondary pulmonary alveolar proteinosis (sPAP) is a rare disorder caused by impaired surfactant clearance due to alveolar macrophage dysfunction and is often associated with hematologic disorders or immunosuppressive therapy. In patients with connective tissue disease-associated interstitial lung disease (CTD-ILD), worsening respiratory findings are frequently interpreted as progression of the underlying ILD, which may delay recognition of sPAP, particularly when anti-granulocyte-macrophage colony-stimulating factor (GM-CSF) antibodies are negative. In the present case, preexisting

Two Truncating MITF Variants Expand the Phenotypic Spectrum of COMMAD Syndrome.

Pathogenic variants in MITF (Microphthalmia Transcription Factor) gene are associated with a spectrum of disorders, including Waardenburg syndrome type II and Tietz syndrome. Rare reports suggest that biallelic loss-of-function variants may result in a severe multisystem phenotype. We report an infant presenting with a phenotype consistent with COMMAD-like syndrome (Coloboma, Οsteopetrosis, Μicrophthalmia, Μacrocephaly, Αlbinism, and Deafness) who died in the early neonatal period. In addition to the core COMMAD features, the proband exhibited ectopic ossification, cardiac fibrosis and fibroel

Gastrin and cholecystokinin 2 receptor, and gastrin-releasing peptide receptor in cancer: From pathogenesis to precision medicine.

Gastrin regulates gastric acid secretion and mucosal homeostasis via the cholecystokinin-2 receptor (CCK2R). Aberrant signaling via CCK2R (cholecystokinin family) and the distinct class A GPCR, gastrin-releasing peptide receptor (GRPR; bombesin family), drives oncogenesis across diverse GI and non-GI malignancies through calcium mobilization, MAPK/PI3K activation, EGFR transactivation, and constitutive splice variants like CCK2Ri4sv. Furthermore, precursor forms such as progastrin remodel the tumor microenvironment by interacting with non-CCK binding sites (p78, Annexin A2), converting fibrobl

Variants in CEP135 Cause Congenital Microcephaly and Primary Ciliary Dyskinesia.

INTRODUCTION: Primary ciliary dyskinesia (PCD), a disorder of motile ciliary dysfunction causing chronic respiratory infections, may also rarely present with aspects of non-motile ciliary dysfunction, including retinitis, central nervous system malformations, skeletal dysplasia, and sensorineural hearing loss. However, congenital microcephaly has not been previously associated with PCD. METHODS: We identified three children with congenital microcephaly and chronic respiratory disease consistent with PCD. Comprehensive PCD diagnostic testing and whole exome sequencing were performed. Character

Current Controversies in the Management of Congenital Lung Malformations: A Report from the Congenital Lung Malformation Study Group.

BACKGROUND: There remains considerable variability in the management of congenital lung malformations (CLMs). The CLM Study Group organized a breakout session at the American Pediatric Surgical Association 2026 Annual Meeting to address current controversies in CLM care. METHODS: An expert panel of pediatric surgeons from the United States facilitated discussions on three controversial topics: 1) the role and utility of fetal MRI in the prenatal diagnostic evaluation, 2) non-operative management of asymptomatic CLMs, and 3) indocyanine green (ICG) as an emerging adjunct to enable lung-sparing

Surviving Males With PORCN Variants: Expanding the Clinical, Molecular, and Mechanistic Spectrum.

Pathogenic variants in PORCN cause focal dermal hypoplasia (FDH/Goltz syndrome), an X-linked dominant disorder historically considered lethal in males, with milder presentations now recognized as PORCN non-Goltz spectrum (PONGOS). We report three male patients identified by exome sequencing: one with a mosaic de novo variant (c.727C>T; p.Arg243*) showing FDH features, and two siblings with an inherited non-mosaic variant (c.1315T>G; p.Trp439Gly) from their unaffected carrier mother with a PONGOS phenotype. These cases confirm that male survival is possible with both mosaic and non-mosaic PORCN

Automated Insulin Delivery: Great Strides in the Past, Great Needs for the Future.

Lymphangioleiomyomatosis (LAM) is a progressive cystic lung disease caused by tuberous sclerosis complex 1/2 (TSC1/2) gene mutations in pulmonary mesenchymal cells, resulting in activation of the mechanistic target of rapamycin complex 1 (mTORC1). A subset of patients with LAM develop pulmonary vascular remodeling and pulmonary hypertension. Little, however, is known regarding how LAM cells communicate with endothelial cells (ECs) to trigger vascular remodeling. In end-stage LAM lung explants, we identified EC dysfunction characterized by increased EC proliferation and migration, defective ang

Early outcomes of children with univentricular circulation undergoing Fontan surgery: the EuroFontan registry.

BACKGROUND AND AIMS: The Fontan operation and its modifications have transformed the management of children with functional single ventricle physiology. While outcomes have improved, peri-operative complications remain common. This study aimed to assess early outcomes and complications following Fontan completion in a large international European cohort. METHODS: The EuroFontan registry is a multicentre retrospective study involving 21 congenital heart disease and transplant centres across Europe. Data were collected on patients who underwent Fontan surgery from January 1990 to January 2023.

Surgical Murine Model of Congenital Diaphragmatic Hernia.

INTRODUCTION: Mouse models of congenital diaphragmatic hernia (CDH) are confounded by the teratogenic effects of nitrofen. Large animal surgical models are created later in lung development, making them less suitable to study the pathogenesis of pulmonary malformation. Therefore, we developed a surgical murine model of CDH that does not have the limitations of other models. Here we describe the feasibility of this model. METHODS: The fetuses of E16.5 pregnant C57BL/6 mice underwent thoracotomy and creation of a defect in the left hemidiaphragm. Littermates served as controls. A sham group und

Genetic aetiologies of bronchiectasis revisited: A new diagnosis of primary ciliary dyskinesia in adulthood.

Primary ciliary dyskinesia (PCD) is an inherited condition in which ciliary motility is defective. This may manifest clinically with chronic rhinosinusitis, otitis media and recurrent lower respiratory tract infections that lead to bronchiectasis. Additionally, fertility may be reduced and situs abnormalities can occur. We present a case of a patient in whom a diagnosis of PCD was initially excluded on the basis of two negative nasal nitric oxide tests. A high index of clinical suspicion was maintained and 23 years after his initial presentation to a specialist bronchiectasis service, addition

Public Reporting and Case Selection in Congenital Heart Surgery: Signals From a Multicenter Observational Study.

BACKGROUND: Public reporting for congenital heart surgery promotes transparency and quality improvement but has the potential for the unintended consequence of risk avoidance. To assess this possibility, it is necessary to understand both changes in case selection and outcomes after public reporting. OBJECTIVES: This study sought to examine the association between adoption of public reporting with hospital performance and predicted probability of mortality as a measure of case-mix complexity. METHODS: Using the Inpatient Essentials Database, we identified all congenital heart surgeries perfo

Comparison of prenatal lung volume measurements associated with extracorporeal membrane oxygenation (ECMO) utilization and mortality in congenital diaphragmatic hernia.

BACKGROUND: Prenatal imaging plays a central role in risk stratification for congenital diaphragmatic hernia (CDH). We sought to identify the most accurate predictor of mortality and ECMO utilization at early and late gestation timepoints amongst the various lung volume measurements: lung-to-head ratio (LHR), observed-to-expected (O/E) LHR, O/E total fetal lung volume (TFLV) and percent predicted lung volume (PPLV). METHODS: Retrospective cohort study of patients with isolated left and right-sided CDH at our fetal care center from 2012 to 2023. Early gestation imaging was performed at initial

Pulmonary Restriction in Patients With a Fontan Circulation: Determinants, Deterioration Over Time, and Clinical Implications.

PURPOSE: Pulmonary restriction is common in patients with a Fontan circulation and is associated with impaired quality of life. Studies often fail to accurately define pulmonary restriction, and few have investigated its predictors, longitudinal course, and clinical implication. We investigate the prevalence and determinants of pulmonary restriction in patients with a Fontan circulation, the evolution of pulmonary function over time, and the relationship between pulmonary function and exercise capacity. METHODS: This longitudinal study included 85 patients with a Fontan circulation who underw

The CARMUCI Study Design: A Double-Blind, Cross-Over Sham-Controlled Trial of Indoor Air Purification in People With Cystic Fibrosis and Primary Ciliary Dyskinesia.

BACKGROUND: People with cystic fibrosis (pwCF) and primary ciliary dyskinesia (pwPCD) are particularly vulnerable to the harmful effects of air pollution due to the impairment of mucociliary clearance (MCC). Despite growing evidence supporting the use of indoor air purification in common lung conditions, its role in CF and PCD remains unexplored. METHODS: The Clean Air for Rare MCC dIsorders (CARMUCI) study is a randomized, double-blind, cross-over, sham-controlled trial aiming to evaluate the impact of indoor air purification on the respiratory health of pwCF and pwPCD (children and adults)
