Lung Function Trajectories After Preterm Birth: A Life-Course Approach to Age-Specific Monitoring.

Preterm birth interrupts critical phases of lung development and is associated with long-term alterations in respiratory structure and function. While bronchopulmonary dysplasia (BPD) has traditionally been considered the principal determinant of adverse outcomes, accumulating evidence indicates that prematurity per se contributes substantially to persistent pulmonary impairment. Lung function trajectories in preterm-born children frequently track along lower percentiles from infancy into adolescence and early adulthood, with limited catch-up growth and increased vulnerability to chronic airfl

The role of advanced imaging in the diagnosis and management of scimitar syndrome in pediatric patients.

This imaging review highlights the spectrum of Scimitar Syndrome (SS) as seen on cardiac computed tomography (CT) and cardiovascular magnetic resonance (CMR), illustrating its variants and linking imaging findings with clinical presentation and management strategies. SS is a rare congenital cardiopulmonary anomaly characterized by partial or total right anomalous pulmonary venous drainage to the hepatic vein or inferior vena cava (IVC), hypoplastic right lung and pulmonary artery, abdominal aortopulmonary collateral(s) to the right lower lobe and other cardiac and bronchial malformations. [A rare differential diagnosis for pulmonary calcifications].

A 77-year-old patient developed community-acquired pneumonia four times within two years. During the work-up, the CT chest showed typical signs of dendriform pulmonary ossification (DPO), a rare disease characterized by bone formation in the lung tissue. DPO usually occurs in chronic lung damage, its clinical significance is still unclear, but it could become more relevant with increasing high-resolution diagnostics.

Assessment of pulmonary function in preterm infants without bronchopulmonary dysplasia. A pilot observational study.

INTRODUCTION: Preterm birth is a major cause of neonatal respiratory morbidity and increases the risk of bronchopulmonary dysplasia (BPD). Tidal-breathing flow-volume loops (TBFVL) enable non-invasive early detection of respiratory abnormalities. While there is extensive data on TBFVL in preterm infants with BPD, data in those without BPD are limited. This prospective cohort study evaluates TBFVL and the impact of prematurity-related variables in preterm infants without BPD. MATERIALS AND METHODS: Subjects were recruited from the Neonatology Unit of Buzzi Children's Hospital in Milan, Italy,

Incidence and Mortality Outcomes of Extracorporeal Membrane Oxygenation in Infants With Congenital Lung Malformations: A Registry Study.

Congenital lung malformations (CLMs) are rare developmental anomalies that can cause severe respiratory failure in early infancy, occasionally necessitating extracorporeal membrane oxygenation (ECMO). This study aimed to determine the incidence of ECMO use and identify predictors of survival among infants with isolated CLMs who required ECMO support. We conducted a retrospective cohort study using the Extracorporeal Life Support Organization (ELSO) registry from 2000 to 2024. The primary outcome was survival to hospital discharge. Bivariate and multivariable logistic regression analyses were p

Echocardiographic Characteristics and Detection Rate of Symptomatic Uni-Leaflet Mitral Valve Malformations in a Consecutive Chinese Cohort.

BACKGROUND & AIMS: Congenital uni-leaflet mitral valve (ULMV), a rare cardiac anomaly caused by embryonic atrioventricular valve dysplasia, lacks established epidemiological data and imaging diagnostic criteria for symptomatic cases. This study systematically analysed echocardiographic characteristics of ULMV and, for the first known time, elucidated the detection rate of symptomatic ULMV in a consecutive Chinese cohort. METHOD: A single-centre retrospective cohort study was conducted, enrolling 137,214 consecutive symptomatic cardiac patients who underwent transthoracic echocardiography at T

Metabolic Checkpoints and Lymphoid Neogenesis in Lung Dendritic Cells: Mechanisms Guiding Tolerance and Chronic Lung Inflammation.

Dendritic cells (DCs) are key sentinels in the lung mucosa that interpret environmental signals to either promote tolerance or trigger inflammation, influencing the development of chronic lung diseases. This review highlights recent mechanistic insights showing that metabolic checkpoints serve as upstream regulators of DC fate and activity: inflammatory stimuli activate HIF-1α/mTOR-linked glycolytic pathways that drive maturation, cytokine secretion, antigen presentation, and migration. In contrast, AMPK-related oxidative and lipid metabolism pathways support tolerogenic states that encourage

Preliminary Report of a Nationwide Prospective Cohort of Congenital Diaphragmatic Hernia in Korea.

BACKGROUND: Most congenital diaphragmatic hernia (CDH) cases are identified prenatally, enabling early multidisciplinary interventions, including maternal transfer and preparation for extracorporeal membrane oxygenation (ECMO). These strategies may improve the historically stagnant survival rates in Korea. However, multicenter studies on the epidemiology and treatment outcomes of CDH are lacking. This study introduces the Korean CDH cohort (K-CDH), a nationwide prospective registry, and presents interim findings. METHODS: The cohort comprises data from 6 tertiary centers (4 in Seoul and 2 in

Inhibition of thrombospondin-1 signaling prevents experimental bronchopulmonary dysplasia.

Bronchopulmonary dysplasia (BPD) is a neonatal lung injury characterized by inflammation and alveolar and vascular hypoplasia that currently lacks effective treatment. Thrombospondin (TSP)-1 is an angiostatic and proinflammatory protein, recently implicated in BPD pathogenesis, that both activates transforming growth factor (TGF)-β1 and suppresses nitric oxide (NO) signaling. To gain further insight into the relative importance of downstream effects of TSP-1, our objective in a neonatal rat model of hyperoxia-intermittent hypoxia (H-IH)-induced lung injury was to compare effects of inhibiting

Metabolic Dysfunction-Associated Steatotic Liver Disease and Respiratory Disorders: A Systematic Review of Clinical and Pathophysiological Associations.

PURPOSE OF REVIEW: Metabolic dysfunction–associated steatotic liver disease (MASLD) represents the hepatic manifestation of ectopic fat accumulation and adipose tissue dysfunction. Excess visceral adiposity promotes adipose tissue hypoxia, macrophage infiltration, and chronic low-grade systemic inflammation, generating systemic metabolic stress that may extend to the lung through a proposed liver–lung axis (hepatopulmonary crosstalk). This systematic review synthesizes current evidence on the associations between MASLD and major respiratory diseases, with emphasis on shared obesity-driven mech

Pulmonary Artery Diameters in Children, Teenagers and Young Adults Derived from Quiescent Interval Slice Selective (QISS) Magnetic Resonance Angiography.

The radial Quiescent Interval Slice Selective (QISS) technique, which is used in cardiovascular magnetic resonance (CMR), is a reliable, contrast-free technique for obtaining high-quality vascular images. It is distinguished by its safety, insensitivity to motion and arrhythmias, and its overall simplicity and adaptability. However, previously published studies assessing pulmonary artery (PA) dimensions in children have not yet incorporated this technique. CMR scans of 139 patients (95 males), aged 6 weeks - 20 years, with no evidence of PA disorders were analyzed. Main PA (MPA), right PA (RPA

Diuretics for Preventing Bronchopulmonary Dysplasia in Preterm Infants: A Systematic Review and Meta-Analysis.

INTRODUCTION: Randomized controlled trials (RCTs) have not demonstrated that diuretics prevent bronchopulmonary dysplasia (BPD), but recent observational studies warrant an updated synthesis. We evaluated the preventive effects and safety of diuretics in preterm infants at risk of developing BPD. METHODS: MEDLINE, Embase, Cochrane CENTRAL, and CINAHL were searched through June 2025. We included randomized and non-randomized studies of various diuretic classes with different routes, dosing regimens, and treatment durations. The risk of bias was assessed using the RoB 2 and ROBINS-I and the cer

A Multitask Deep Learning Model for Pediatric Echocardiography Analysis.

BACKGROUND: Congenital heart defects afflict ≈1% of all births worldwide. Although deep learning has shown significant promise in automating and improving adult echocardiography analysis, existing pediatric-based models are often limited to single tasks and specific echocardiographic views. To address this, we introduce EchoAI-Peds, a multitask deep learning model for pediatric echocardiography. Our model was developed using the most comprehensive set of pediatric labels to date and is designed to integrate information from multiple views simultaneously. METHODS: We trained a video-based visi

Developmentally sensitive neuropharmacological effects of dexamethasone in neonatal bronchopulmonary dysplasia-associated brain injury via microglial Acod1-itaconate/IL-1β signaling.

BACKGROUND: Bronchopulmonary dysplasia (BPD) in preterm infants is frequently accompanied by neurodevelopmental impairment, yet the central neuropharmacological actions of dexamethasone (DEX), a commonly used therapy for severe or evolving BPD, remain incompletely understood. In particular, whether DEX exerts timing-dependent neuroprotection in the developing brain and the mechanisms underlying such effects are unclear. METHODS: We investigated the neuroprotective effects of DEX in a neonatal rat double-hit model combining prenatal maternal lipopolysaccharide exposure with postnatal hyperoxia

Innovations in transcatheter interventions for hypoplastic left heart syndrome and borderline left heart structures from fetal to neonatal life: current evidence and systematic review of recent advances (2016-2025).

Alongside advances in surgical and medical care, catheter-based interventions have reshaped the care of fetuses and neonates with hypoplastic left heart syndrome (HLHS) or borderline left heart structures over the last decades. These interventions include fetal aortic valvuloplasty (FAV), fetal and neonatal atrial septal interventions, the hybrid Stage-1 procedure (HS1P), and the emerging total transcatheter Stage-1 procedure (TTS1P). We summarize the current evidence on catheter-based interventions for HLHS and borderline left heart structures from fetal to neonatal life and systematically re

Semaglutide Reverses Ectopic Lipid Accumulation, Impaired Myocardial Perfusion Reserve, and Diastolic Dysfunction in a Mouse Model of Cardiometabolic Heart Disease.

Semaglutide (SEMA) improves cardiometabolic outcomes in obesity, but its mechanisms remain incompletely understood. We examined the effects of SEMA in mice fed a high-fat, high-sucrose diet. Obese mice were treated with SEMA and compared with pair-fed controls to account for reduced dietary intake with SEMA. Multiparametric cardiovascular magnetic resonance was used to assess epicardial adipose tissue volume and composition, myocardial fat fraction, adenosine myocardial perfusion reserve, systolic strain, and diastolic function, with histological evaluation of myocardial fibrosis. Safety of intravascular administration of umbilical-cord-derived mesenchymal stromal cells: an updated systematic review and meta-analysis.

BACKGROUND: Mesenchymal stromal cells (MSCs) have the capacity to self-renew and exert immunomodulatory and paracrine effects. Our previously published systematic review of 55 randomized controlled trials (RCTs) of all MSC sources found an overall favorable safety profile for MSCs aside from more frequent fever in the MSC group. OBJECTIVE: Umbilical cord MSCs (UC-MSCs) possess potential biological and economic advantages over other MSC sources. Since several UC-MSC RCTs have been recently published in a heterogenous group of disorders, we performed a systematic review to specifically examine
