Beyond Heritable PAH: Pulmonary Hypertension in Genetic Syndromes.

Pulmonary hypertension (PH) may complicate a broad range of genetic syndromes beyond the established spectrum of heritable pulmonary arterial hypertension. Although these conditions are individually rare, together they represent an emerging field at the crossroads of developmental biology, vascular medicine, and precision genomics. In many cases, PH may be the presenting feature or may remain unrecognized because it occurs within complex multisystem disorders involving congenital heart disease, developmental lung abnormalities, parenchymal lung disease, vascular malformations, or extra-pulmona

FSTL4 interaction with Gαi proteins activates the Akt-mTOR pathway to drive malignant phenotypes in non-small cell lung cancer.

Non-small cell lung cancer (NSCLC) presents a critical challenge clinically; thus, identifying novel drivers and reliable prognostic biomarkers is imperative. Here, we investigated the clinical relevance, cell-type-specific localization, and mechanistic role of Follistatin-like 4 (FSTL4) in NSCLC. Analysis of The Cancer Genome Atlas (TCGA) and local resected tissue samples show that FSTL4 expression is significantly elevated in NSCLC tissues compared to normal lung tissues, and serves as an independent prognostic indicator for reduced overall survival. Single-cell RNA sequencing confirmed that

SATB2 dysregulation generates a novel circular RNA and drives KRAS-like transcriptional reprogramming and transformation-associated phenotypes.

BACKGROUND: Even though epigenetic factors contribute to oncogenesis, most human cancer models still assume that disease originates from driver DNA mutations. Thus, it is still unclear if non-genetic mechanisms are sufficient to trigger malignant transformation. Special AT-rich binding protein 2 (SATB2) is a chromatin organizer that brings distal DNA elements into close proximity, thus remodeling chromatin structures to reprogram cell-specific and/or developmentally sensitive gene networks. METHODS: Here, we chronically exposed human bronchial lung epithelial cells to ≤ 2 µM inorganic arsenic

Diagnostic Challenges in a Hybrid Congenital Pulmonary Airway Malformation: A Case Report.

Congenital pulmonary airway malformation (CPAM) and bronchopulmonary sequestration are rare congenital lung anomalies that can occasionally coexist as a "hybrid" lesion. We report a case of a 15-year-old female with a two-week history of fever with evening-predominant spikes, cough, and right hypochondrial pain. The pain was aggravated by deep inspiration and coughing. Imaging revealed a right lower lobe consolidation containing air-filled cystic spaces on chest X-ray and computed tomography (CT). An anomalous arterial supply from the descending thoracic aorta on CT angiography was also noted.

Impact of Changes in the Profiles of Patients' Backgrounds on the Indications of Thoracoscopic Lobectomy for Congenital Lung Malformations: A Multicenter Retrospective Study in Southern Japan.

INTRODUCTION: The number of patients with congenital lung malformations (CLM) is increasing because of prenatal diagnosis. We investigated the changes in the patient backgrounds and the surgical approach for lobectomy, evaluating the surgical outcomes of thoracoscopic surgery. METHODS: We enrolled 50 CLM patients who underwent lobectomy between 1995 and 2024. Patients were divided into three 10-year groups to evaluate the trends in patient backgrounds. We also divided them into the thoracotomy and thoracoscopy groups to compare the patient backgrounds and surgical outcomes. RESULTS: An incre

DDIT3, OTUB2, and ASS1 regulate arginine biosynthesis in colorectal cancer cells under arginine deficiency.

Argininosuccinate synthetase 1 (ASS1) is a rate-limiting enzyme in arginine biosynthesis, and its stability is regulated by TRAF2-mediated ubiquitination. Here, we report OTUB2 as a major deubiquitinase to stabilize ASS1, resulting increased arginine biosynthesis in colorectal cancer (CRC) cells; OTUB2 expression is elevated in CRC tissue, and patients with high OTUB2 expression exhibit shorter overall survival. As such, ectopic expression of OTUB2 promotes growth of CRC cells and xenograted tumors and accelerates cell migration and lung metastasis. Moreover, arginine deprivation induces marke

Adult Congenital Pulmonary Airway Malformation Presenting With a Large Cystic Lung Lesion: A Case Report.

Congenital pulmonary airway malformation (CPAM) is a rare developmental lung malformation that is usually diagnosed antenatally or during early childhood. Adult presentation is uncommon and may present a significant diagnostic challenge because of overlapping radiological features with other cystic lung diseases and malignancy. We report the case of a 49-year-old man with an incidentally detected large multiseptated cystic lesion occupying almost the entire right upper lobe. CT and PET-CT suggested a congenital cystic lung lesion without evidence of metabolically active malignancy. Following m

SRGAP2-AS1 Regulates RSV Replication and EMT-Associated Changes Through PTBP1 and ZEB1.

Respiratory syncytial virus (RSV) is a leading cause of lower respiratory tract disease, but the contribution of long non-coding RNAs (lncRNAs) to RSV-induced epithelial-mesenchymal transition (EMT)-associated changes remains unclear. We used transcriptomic screening in RSV-exposed BEAS-2B cells to identify candidate lncRNAs and then examined SRGAP2-AS1 in cell and mouse models. Expression was measured by RT-qPCR, and loss- and gain-of-function experiments were combined with RNA immunoprecipitation, co-immunoprecipitation, Western blot analysis, and TCID50 assays. SRGAP2-AS1 was increased in R

Clinically relevant pathologic classification of Congenital Cystic Lung Malformations.

Congenital cystic lung malformations encompass a diverse group of lesions that arise during in utero development. A variety of pathologic classification schemes have been proposed, but recent advances suggest that gross and microscopic morphologies can often be explained by the underlying mechanism of cyst development. A subset of congenital pulmonary airway malformations, usually with large cysts, is associated with widespread epithelial oncogenic KRAS mutation. These have a long-term risk of developing mucinous adenocarcinoma. Another subset, more commonly with smaller cysts, are associated

Adult Intralobar Pulmonary Sequestration: A Report of Two Cases With Distinct Clinical Presentations.

Adult pulmonary sequestration (PS) is an uncommon congenital lung anomaly that may present with nonspecific respiratory symptoms and misleading radiological findings. We report two adult cases of intralobar pulmonary sequestration with distinct clinical presentations. The first case involved a 54-year-old diabetic smoker admitted for recurrent bronchopulmonary infections and severe recurrent hemoptysis requiring blood transfusions. Thoracic computed tomography (CT) angiography revealed a hypervascular lesion in the left lower lobe supplied by aberrant systemic vessels arising from the descendi

Electrical impedance tomography reveals ventilation and perfusion heterogeneity in infants with bronchopulmonary dysplasia.

OBJECTIVE: Preterm infants have pulmonary ventilation and perfusion abnormalities, yet few imaging modalities can inform clinicians about this ventilation/perfusion (V/Q) relationship. Electrical impedance tomography (EIT) is an imaging technique with V/Q imaging capabilities that has not been well described in infants with BPD. STUDY DESIGN: EIT was performed every 4 weeks in preterm infants for a maximum of 5 visits per infant. Term infants with healthy lungs had one EIT imaging visit as controls. RESULTS: Data were collected from a total of 51 EIT visits. Novel V/Q maps were generated fro

Differential genetic analysis of ectrodactyly in a Fanconi anemia pedigree with FANCA mutations.

Fanconi anemia (FA; OMIM: 227650) is a rare genetic disorder characterized by bone marrow failure, congenital anomalies, and cancer predisposition. While FANCA mutations account for most FA cases, phenotypic overlap with other disorders complicates diagnosis. This study analyzes molecular diagnostic pathways for FANCA-related FA and establishes a hereditary differential diagnosis for ectrodactyly. A Chinese FA family clinical phenotype was collected. The proband and father underwent whole-genome sequencing. Copy number variations (CNVs) in FANCA were assessed by genomic qPCR. Functional charac

Congenital Pulmonary Airway Malformation Volume Ratio in Fetuses With Congenital Cystic Lung Diseases to Predict Surgery Within 7 Days of Birth.

AIM: To identify congenital pulmonary airway malformation (CPAM) volume ratio (CVR) values that can predict the need for early neonatal surgery. METHODS: We retrospectively reviewed 70 infants diagnosed with congenital cystic lung diseases (CCLD) over the past 10 years at our center (10 with CPAM, 25 with intra- and extra-lobar bronchopulmonary sequestrations, and 35 with bronchial atresia). Perinatal features, the maximum CVR during pregnancy, and the final CVR before delivery were compared between the early surgery group (surgery within 1 week of birth) and the follow-up group (surgery afte

Maternal and Neonatal Determinants of Respiratory Outcome Following Second-Trimester PPROM: A Multi-Domain Machine Learning Analysis.

Background: Preterm premature rupture of membranes (PPROM) before 32 weeks of gestation with prolonged latency is associated with substantial neonatal morbidity, including Dry Lung Syndrome (DLS), pulmonary hypoplasia (PH), bronchopulmonary dysplasia (BPD), and death. Accurate individualized risk stratification remains elusive, as the interacting contributions of amniotic fluid dynamics, inflammatory status, and microbiological burden are inadequately captured by traditional statistical approaches. Methods: We performed a retrospective, exploratory-predictive analysis of 66 pregnancies complic

A rare case of multiple congenital anomalies in a neonate: Diagnostic challenges in establishing live birth and the role of comprehensive fetal autopsy.

Congenital malformations remain one of the leading causes of perinatal mortality in India. These anomalies encompass a wide range of chromosomal abnormalities, inborn errors of metabolism, and other Mendelian disorders. We report a case of a four-day-old male neonate presenting with multiple congenital anomalies consistent with features of Trisomy 13, a rare chromosomal disorder, which ultimately led to neonatal death. An important observation was also made regarding the hydrostatic test-a procedure traditionally used to determine whether a newborn was liveborn or stillborn. This test, however

Small partial deletion of a highly GC-rich FOXF1 exon 1 in two deceased siblings with alveolar capillary dysplasia.

Alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV) is a rare lethal lung developmental disorder caused by haploinsufficiency of FOXF1. While larger-sized coding and noncoding copy-number variant (CNV) deletions involving the FOXF1 locus are detected in approximately half of histopathologically-diagnosed ACDMPV patients, small CNVs remain diagnostically challenging. Here, we revisited an unsolved case of familial ACDMPV with two affected siblings. Whole genome sequencing (WGS) of 30-year-old archival lung autopsy tissue analyzed using AI powered platform revealed a 151 b

Prenatal Presentation of Congenital Alveolar Capillary Dysplasia With Misalignment of Pulmonary Veins: A Case Initially Suspected as Posterior Urethral Valves.

Alveolar capillary dysplasia with misalignment of the pulmonary veins (ACDMPV) is a rare, typically lethal pulmonary disorder characterized by underdeveloped pulmonary vascularization. It usually presents within the first 48 h of life with progressive respiratory distress and therapy-resistant persistent pulmonary hypertension of the newborn (PPHN). ACDMPV is associated with genomic alterations at the FOXF1 locus, with most cases arising de novo. We present a case in which suspected urethral valves, managed prenatally with fetal vesicoamniotic shunting, were the first indication of ACDMPV. In

Early dysanapsis in experimental bronchopulmonary dysplasia: implications for lifelong lung disease.

Disruptions during perinatal fetal lung development can lead to postnatal chronic lung diseases such as bronchopulmonary dysplasia (BPD). Along with decreased alveolar and pulmonary vascular growth, abnormal airway growth occurs in BPD. Previous studies in rats have shown that antenatal endotoxin (AN-ETX) mimicking maternal chorioamnionitis causes dysanapsis during the neonatal period. Whether dysanaptic growth alters long-term lung function remains unknown. We hypothesized that antenatal endotoxin causes persistent differences between airway and distal lung growth with age in experimental BPD
