INTRODUCTION: The number of patients with congenital lung malformations (CLM) is increasing because of prenatal diagnosis. We investigated the changes in the patient backgrounds and the surgical approach for lobectomy, evaluating the surgical outcomes of thoracoscopic surgery.
METHODS: We enrolled 50 CLM patients who underwent lobectomy between 1995 and 2024. Patients were divided into three 10-year groups to evaluate the trends in patient backgrounds. We also divided them into the thoracotomy and thoracoscopy groups to compare the patient backgrounds and surgical outcomes.
RESULTS: An incre

Congenital pulmonary airway malformation (CPAM) is a rare developmental lung malformation that is usually diagnosed antenatally or during early childhood. Adult presentation is uncommon and may present a significant diagnostic challenge because of overlapping radiological features with other cystic lung diseases and malignancy. We report the case of a 49-year-old man with an incidentally detected large multiseptated cystic lesion occupying almost the entire right upper lobe. CT and PET-CT suggested a congenital cystic lung lesion without evidence of metabolically active malignancy. Following m

Congenital cystic lung malformations encompass a diverse group of lesions that arise during in utero development. A variety of pathologic classification schemes have been proposed, but recent advances suggest that gross and microscopic morphologies can often be explained by the underlying mechanism of cyst development. A subset of congenital pulmonary airway malformations, usually with large cysts, is associated with widespread epithelial oncogenic KRAS mutation. These have a long-term risk of developing mucinous adenocarcinoma. Another subset, more commonly with smaller cysts, are associated

AIM: To identify congenital pulmonary airway malformation (CPAM) volume ratio (CVR) values that can predict the need for early neonatal surgery.
METHODS: We retrospectively reviewed 70 infants diagnosed with congenital cystic lung diseases (CCLD) over the past 10 years at our center (10 with CPAM, 25 with intra- and extra-lobar bronchopulmonary sequestrations, and 35 with bronchial atresia). Perinatal features, the maximum CVR during pregnancy, and the final CVR before delivery were compared between the early surgery group (surgery within 1 week of birth) and the follow-up group (surgery afte

Congenital pulmonary airway malformation (CPAM) is a rare developmental anomaly of the lung that can present with a wide spectrum of clinical manifestations, ranging from asymptomatic incidental findings to severe respiratory compromise during infancy. We present the case of a one-year-old male infant with a giant multicystic pulmonary lesion involving the right upper and middle lobes, causing significant mediastinal shift and respiratory symptoms. The patient presented with recurrent episodes of lower respiratory tract infection, persistent cough, and progressive shortness of breath. Clinical

CONTEXT.—: Pleuropulmonary blastoma (PPB) is a rare primary lung neoplasm predominantly occurring in infancy and early childhood, which, because of rarity and/or confusion with congenital cystic lung lesions and its variable architectural and morphologic features, can be a diagnostic challenge.
OBJECTIVE.—: To characterize the diagnostic challenges in cases submitted to the International PPB/DICER1 [dicer 1, ribonuclease III] Registry (Registry) with a possible PPB diagnosis.
DESIGN.—: This study reviews lesions submitted to the Registry during a 35-year period. Pathologic diagnoses, ancilla

BACKGROUND: Chronic granulomatous disease (CGD) is a rare primary immunodeficiency disorder caused by defects in phagocyte NADPH oxidase. X-linked CGD, resulting from CYBB gene mutations, accounts for approximately two-thirds of cases. Clinical manifestations typically include recurrent bacterial and fungal infections, granuloma formation, and inflammatory complications. CASE PRESENTATION: We report monochorionic diamniotic twins with a novel CYBB variant (c.1303A > T, p.Lys435Ter). The 7-month-old proband presented with recurrent respiratory infections and was initially misdiagnosed with cong

BACKGROUND: Congenital pulmonary airway malformation (CPAM) is a rare developmental anomaly of the lower respiratory tract, with an estimated incidence ranging from 1 in 10,000 to 1 in 35,000 live births. CPAM may present during infancy with severe respiratory distress and complications such as pneumothorax or recurrent pulmonary infection.
CASE PRESENTATION: We report two female infants aged 7 months and 2 months who presented with severe respiratory distress and radiological findings initially suggestive of pneumothorax. In Case 1, chest computed tomography (CT) demonstrated a large solitar

INTRODUCTION: The patient showed an uneventful recovery post-surgery, underscoring the significance of early intervention in managing rare congenital anomalies. Bronchogenic cysts constitute 13-15 % of congenital cystic lung diseases and 6 % of childhood mediastinal masses. Arising from abnormal foregut development during embryogenesis, these cysts can be asymptomatic or cause serious complications, such as airway or vascular compression.
CASE PRESENTATION: A 10-day-old female newborn presented with a pulmonary cystic lesion in the left upper lung lobe, identified via routine antenatal ultras

"Cystic" lung lesions that arise in utero or early postnatal life encompass congenital pulmonary airway malformations, sequestrations, pleuropulmonary blastoma, and bronchogenic cyst, but this category also includes noncystic lesions such as lobar emphysema and rare fetal neoplasms. Traditional classifications have heavily weighted cyst size as a defining feature. In this review, we emphasize the significance of the etiology in defining categories and driving patient care.

Congenital cystic lung malformation (CCLM) is a rare condition requiring lobectomy in symptomatic pediatric patients. Effective postoperative pain management is crucial to prevent respiratory complications and optimize recovery. We present a 12-kg, 1-year-old patient undergoing right lobectomy for CCLM, where Serratus Posterior Superior Intercostal Plane Block (SPSIPB) was used as part of multimodal analgesia. Postoperatively, the patient had low FLACC (Face, Legs, Activity, Cry, Consolability) scores with minimal opioid use. SPSIPB provided effective thoracic analgesia, demonstrating its pote

BACKGROUND: Congenital cystic lung malformations (CCLM) increasingly require parenchyma-sparing resections. While thoracoscopic segmentectomy offers tissue preservation benefits, its complexity demands advanced preoperative visualization. This study evaluates head-mounted display virtual reality (HMD-VR) for surgical planning in pediatric segmentectomies.
METHODS: Nineteen children (4 to 148 months) underwent VR-assisted thoracoscopic segmentectomy. Patient-specific three-dimensional (3D) models were generated using Medicalholodeck® software, integrating contrast-enhanced CT DICOM data. Surge

Congenital pulmonary airway malformation (CPAM) is a rare developmental anomaly of the lung, most often diagnosed in infancy or early childhood due to recurrent pneumonia or respiratory distress. We report the case of an eight-year-old girl who presented with fever, cough, hemoptysis, and chest pain, and was ultimately found to have pulmonary abscesses in the right upper and middle lobes. She had no prior history of recurrent respiratory infections or neonatal distress. Chest computed tomography revealed multilocular cystic lesions involving two lobes, raising suspicion for CPAM. The patient r

Congenital pulmonary airway malformations (CPAM) compose the major part of congenital lung malformations (CLM) and have traditionally been treated by pulmonary lobectomy. In terms of surgical strategy, lobectomy has conventionally been the preferred treatment for CPAM localized to a single lobe. More recently, alternative approaches including lung-sparing resections (LSR), such as wedge or non-anatomic resections and segmentectomy, have been suggested. In asymptomatic CPAM early surgical resection is often shown to reduce infection and malignancy development. We describe two patients who were

OBJECTIVE: The aim of this study is to explore the practical value of prenatal magnetic resonance imaging (MRI) in the assessment of congenital cystic lung disease in fetuses, to evaluate the relative size of the lesion and the status of lung development, and to make an attempt at utilizing the strength of MRI in post-processing to obtain assessment indicators of the size of the lesion and the status of lung development, with which predictions can be made for the prognosis that these fetuses may face after birth. We retrospectively collected and analyzed the data of fetuses diagnosed with cong

Congenital pulmonary airway malformations (CPAMs) are predominantly identified prenatally or during infancy, with adult-onset cases being considered extremely rare. This case report describes a 22-year-old male who presented with hemoptysis and exertional dyspnea, leading to the diagnosis of CPAM. The patient had experienced small-volume hemoptysis for four years, which escalated to larger volumes and progressive dyspnea one week before hospital admission. A chest CT scan revealed a large 13.2 cm thin-walled cavitary lesion with an air-fluid level in the left lower lobe. The patient underwent

BACKGROUND: Congenital diaphragmatic hernia(CDH) is a rare congenital anomaly characterized by herniation of abdominal contents into thoracic cavity through a defect in diaphragm. While commonly diagnosed prenatally or in neonatal period, late-presenting CDH can occur and may mimic other thoracic emergencies such as tension pneumothorax, complicating diagnosis and management.
CASE PRESENTATION: A two-year old male black child from Ethiopia presented to the emergency department with sudden onset of acute respiratory distress. Initial clinical assessment and chest radiography suggested a diagno

INTRODUCTION: Pleuropulmonary blastoma (PPB) is a rare, aggressive, primary intrathoracic malignancy typically seen in infancy and early childhood. Accurate distinction from congenital cystic lung lesions is crucial due to significant prognostic and therapeutic differences. Cytologic features have rarely been described. Establishing a cytodiagnosis is challenging owing to its rarity, lack of awareness, and multiple morphologic mimics.
MATERIALS AND METHODS: This was a retrospective study conducted over 8 years. The histopathology and cytopathology databases were searched for all pediatric PPB
