An unusual case of orbital tumor with high renin content and severe hypertension is described. The patient was a 15-year-old girl with juvenile hypertension (200-140 mmHg) associated with right exophthalmos and hypokalemia. Ten out of 164 cases of bronchogenic carcinoma showed pathological evidence at necropsy of the ectopic ACTH syndrome. All occurred in association with oat-cell carcinoma, constituting 19% of that group. The pathological features consisted of adrenocortical hyperplasia confined to the zona fasciculata and Crooke's hyaline change in the pituitary. We present a case of marked hyperamylasemia associated with undifferentiated small cell carcinoma of the lung. The serum alpha-amylase had an electrophoretic mobility similar to that of salivary gland enzyme (s-type), and that in tissue extracts from the lung tumor and metastatic skin nodules showed similar migration. Two cases of extracranial heterotopia of the brain are reported. Case 1, in an otherwise normal female baby, had a brain heterotopia in the submandibular region. A case of a benign, cystic intrapulmonary teratoma occurring in the left lobe of a 63-yr-old female is described and the typical clinical symptoms and distinguishing X-ray appearances which may be helpful in diagnosing this rare tumour are mentioned. The connection between the tumor and the segmental bronchus clearly established the true intrapulmonary nature of the lesion in this case and the unusual finding of thymic tissue within the wall supports previous speculation regarding the possible thymic origin of these neoplasms.

A similar distribution of 99Tcm, 45Ca and 85Sr was obtained when injected in rats as 99Tcm-labelled pyrophosphate or ethylene-1-hydroxy-1,1-diphosphonate (EHDP), 45Ca-chloride and 85Sr-choloride respectively. The highest concentrations occurred in bone and teeth. In EHDP-treated rats, isotopes were accumulated at sites of ectopic calcifications in the lung, kidney and stomach wall as well as in mineralized tissues. These results indicate that 99Tcm, when administered as 99Tcm-labelled EHDP accumulates predominatly in the inorganic rather than organic tissue components.

Small fragments of kidney tissue were grafted into the right lung (Xenopus laevis) and the left lung was subsequently removed. This stimulated compensatory hyperplasia (increased mitotic rate) in alveolar tissue of the right lung and in the kidney graft. Monolayer tissue culture has been used as a system in which to study aspects of ectopic hormone secretion. Of a series of twenty-four human bronchial carcinomas, nineteen were successfully established in culture and the supernatant medium from each tested for peptide hormones by radioimmunoassay. Immunoreactive calcitonin was found in extracts of seven out of eight consecutive breast carcinomas and four selected lung carcinomas, but not in extracts of benign breast lesions or normal tissues. This suggests that the high plasma calcitonin levels observed in patients with a wide variety of cancers reflect ectopic production of calcitonin by cancer tissue, and that the radioimmunoassay for calcitonin could prove to be of value in the detection and management of malignant disease, particularly breast cancer.

The formation of heterotopic bone tissue in malignant tumors or in their metastases is extremely rare. In a 60 years old male patient with bronchogenic carcinoma (adenocarcinoma) extensive bone formation was observed within multiple metastases in the skeletal muscles. On the basis of the microscopic findings, the mechanism of bone formation by malignant tumors is discussed. Obviously, proliferation of local mesenchymal tissue and differentiation of mesenchymal cells to osteoblasts is induced by the tumor cells.

Two patients are described whose acromegaly was cured by removal of a bronchial carcinoid tumour. One had an enlarged pituitary fossa and evidence is presented that in this patient the tumour was not secreting growth hormone. The evidence available suggests that the tumour was producing a growth hormone releasing substance. It is suggested that some cases of the pluriglandular syndrome may be secondary to small bronchial carcinoid tumours that produce substances as yet unidentified, that stimulate the growth and hyperactivity of other endocrine tissue.
