Bedside flexible bronchoscopy in the neonatal intensive care unit: Current evidence, clinical applications and future directions.

BACKGROUND: Lung ultrasound and targeted echocardiography let the neonatal team examine lungs and heart at the cot side, but neither shows the airway. Flexible bronchoscopy does, and it is increasingly performed inside the neonatal intensive care unit (NICU). METHODS: Narrative review of published series of flexible bronchoscopy in infants cared for in neonatal units, supported by pediatric intensive care studies and society standards. Certainty of evidence was not graded. RESULTS: Twelve neonatal series (652 infants) were identified; all but one were single-center and most were retrospectiv

Secondary pulmonary alveolar proteinosis mimicking progression of interstitial lung disease during treatment for adult-onset still's disease: An autopsy case report.

RATIONALE: Secondary pulmonary alveolar proteinosis (sPAP) is a rare disorder caused by impaired surfactant clearance due to alveolar macrophage dysfunction and is often associated with hematologic disorders or immunosuppressive therapy. In patients with connective tissue disease-associated interstitial lung disease (CTD-ILD), worsening respiratory findings are frequently interpreted as progression of the underlying ILD, which may delay recognition of sPAP, particularly when anti-granulocyte-macrophage colony-stimulating factor (GM-CSF) antibodies are negative. In the present case, preexisting

Follow-up of neonatal chronic respiratory disease: an update based on the current evidence.

UNLABELLED: Chronic respiratory morbidity following premature birth is associated with significant and long-standing complications affecting airway function, the lung parenchyma and cardiac morphology. The aim of this narrative review was to provide an update on the current evidence for the cardiorespiratory follow-up of extremely preterm infants, particularly those with bronchopulmonary dysplasia. There is currently wide variation in the frequency, components and duration in the follow-up of these infants and an absence of robust evidence to support specific recommendations. Follow-up is most

Lung Ultrasound in Bronchopulmonary Dysplasia: A Comparative Diagnostic Study With CT, Chest X-Ray and Pulmonary Function Tests.

BACKGROUND: Chest x-ray (CXR) and computed tomography (CT) are commonly used for diagnosing bronchopulmonary dysplasia (BPD), though each modality has limitations. CXR has low sensitivity for early parenchymal changes, while CT involves radiation exposure. Lung ultrasound (LUS) has emerged as a promising, radiation-free alternative. OBJECTIVE: To assess the diagnostic accuracy of LUS in BPD compared to CT and CXR, and to evaluate correlations between LUS scores and pulmonary function parameters, including spirometry, interrupter resistance technique (RINT), and nocturnal respiratory monitorin

Endothelial MLKL Inhibition Reduces Hyperoxia-Induced Bronchopulmonary Dysplasia in Neonatal Mice.

Bronchopulmonary dysplasia (BPD) remains a severe complication in premature infants requiring prolonged oxygen therapy, with vascular endothelial dysfunction recognised as a critical contributor to disease progression. Mixed lineage kinase domain-like protein (MLKL)-mediated necroptosis, an essential form of regulated cell death implicated in various pulmonary disorders, has not been fully investigated in the context of BPD. Here, we utilised a neonatal mouse model of hyperoxia exposure to elucidate the role and mechanisms of MLKL-mediated necroptosis in BPD pathogenesis. Our analysis demonstr

Peri and Neonatal Risk Factors and Structural Lung Abnormalities Predict Hypoxic Challenge Test Failure in Infants With Severe Bronchopulmonary Dysplasia.

INTRODUCTION: Infants with severe bronchopulmonary dysplasia (BPD) are at increased risk of hypoxemia and often fail hypoxic challenge testing (HCT). The predictive value of clinical characteristics and structural lung abnormalities on HCT outcomes is unclear. METHODS: This prospective cohort study included preterm-born infants (≤ 32 weeks of gestation) with severe BPD enrolled in a standardized follow-up program. Perinatal and neonatal data were extracted from medical records. At 6 months corrected gestational age, structural lung abnormalities were quantified on chest CT using PRAGMA-BPD sc

Mitochondria-derived peptide MOTS-c alleviates hyperoxia-induced bronchopulmonary dysplasia in neonatal mice by activating Nrf2 pathway.

Oxidative stress play key roles in the pathogenesis of bronchopulmonary dysplasia (BPD). MOTS-c is a mitochondria-derived peptide containing 16 amino acids that is reported to be involved in the treatment of oxidative stress-related diseases. However, whether MOTS-c functions on hyperoxia-induced BPD remains unknown. The purpose of this study was to investigate the potential therapeutic effect and mechanism of MOTS-c on hyperoxia-induced BPD. Here, hyperoxia (70% O2) was used to mimic the murine BPD model. We found that MOTS-c content was reduced in hyperoxia-induced BPD mice. Exogenous MOTS-c

Outpatient assessment of bronchopulmonary dysplasia using point of care lung ultrasound.

BACKGROUND: Lung point-of-care ultrasound (POCUS) is an emerging technology for assessment of neonatal lung diseases. Prior studies have described lung ultrasound scores (LUS) in bronchopulmonary dysplasia (BPD). However, few have assessed LUS in patients with BPD beyond the initial neonatal intensive care unit (NICU) hospitalization. METHODS: We performed a cross-sectional study to evaluate outpatient LUS abnormalities and respiratory morbidity in children with and without a history of BPD. We hypothesized that infants with severe BPD would have higher (more abnormal) LUS compared to healthy

Determinants of declining lung function trajectories from childhood to adulthood after preterm birth.

BACKGROUND: Preterm birth is associated with lifelong respiratory sequelae, yet our understanding of lung function trajectories across the lifespan remains limited. We aimed to identify patterns of spirometry development from childhood to early adulthood in survivors of very preterm birth using novel data-driven methods. METHODS: In this longitudinal study, 116 individuals born ≤32 weeks' gestation and 39 term-born controls underwent spirometry testing at up to four timepoints between 4-23 years of age. Patterns (trajectories) of forced expiratory volume in 1 s (FEV1), forced vital capacity (

PROMISE-impact of maternal and neonatal risk factors on the respiratory outcome of extremely preterm infants following PPROM in the second trimester of pregnancy.

BACKGROUND: Prolonged preterm premature rupture of membranes (PPROM) in the second trimester is associated with heterogeneous neonatal respiratory outcomes, ranging from mild transitional respiratory impairment to dry lung syndrome (DL), pulmonary hypoplasia (PH), bronchopulmonary dysplasia (BPD) and death. Risk assessment is challenging because outcomes are influenced by both gestational maturity and PPROM-specific exposures. This study aimed to evaluate respiratory outcomes after prolonged PPROM using a multifactorial and longitudinal approach that integrates gestational age, amniotic fluid

Treprostinil Is Associated With Early and Sustained Improvement in Group 3 Pulmonary Hypertension and Right Ventricular Function in Children.

Use of subcutaneous treprostinil (TRE) in children with pulmonary hypertension (PH) secondary to lung disease (group 3 PH) is not well described. We hypothesized that initiation of TRE is associated with improvement in PH severity and right ventricular (RV) function with minimal early side effects. We conducted a single-center retrospective analysis of children with group 3 PH treated with TRE between 2006 and 2022. We compared echocardiographic changes in PH severity and RV systolic function over the first 3-6 months of TRE and described adverse effects over the first 48 h of therapy. Forty-o

Inherited TBX4 frameshifting variants predicted to escape nonsense mediated decay in two families with variable phenotypes, including lethal lung developmental disorders.

BACKGROUND: Pathogenic variants involving the transcription factor TBX4 gene have been associated with various skeletal and pulmonary abnormalities, including lethal lung developmental disorders (LLDD). METHODS: Whole-genome sequencing (WGS) with AI-powered platform for variant detection and interpretation followed by Sanger sequencing targeted variant segregation analysis were used. Reverse transcription quantitative PCR (RT-qPCR) and immunohistochemistry (IHC) studies were performed to assess gene and protein expression levels, respectively. RESULTS: We describe two unrelated families with

Lung Function Trajectories After Preterm Birth: A Life-Course Approach to Age-Specific Monitoring.

Preterm birth interrupts critical phases of lung development and is associated with long-term alterations in respiratory structure and function. While bronchopulmonary dysplasia (BPD) has traditionally been considered the principal determinant of adverse outcomes, accumulating evidence indicates that prematurity per se contributes substantially to persistent pulmonary impairment. Lung function trajectories in preterm-born children frequently track along lower percentiles from infancy into adolescence and early adulthood, with limited catch-up growth and increased vulnerability to chronic airfl

Assessment of pulmonary function in preterm infants without bronchopulmonary dysplasia. A pilot observational study.

INTRODUCTION: Preterm birth is a major cause of neonatal respiratory morbidity and increases the risk of bronchopulmonary dysplasia (BPD). Tidal-breathing flow-volume loops (TBFVL) enable non-invasive early detection of respiratory abnormalities. While there is extensive data on TBFVL in preterm infants with BPD, data in those without BPD are limited. This prospective cohort study evaluates TBFVL and the impact of prematurity-related variables in preterm infants without BPD. MATERIALS AND METHODS: Subjects were recruited from the Neonatology Unit of Buzzi Children's Hospital in Milan, Italy,

Echocardiographic Characteristics and Detection Rate of Symptomatic Uni-Leaflet Mitral Valve Malformations in a Consecutive Chinese Cohort.

BACKGROUND & AIMS: Congenital uni-leaflet mitral valve (ULMV), a rare cardiac anomaly caused by embryonic atrioventricular valve dysplasia, lacks established epidemiological data and imaging diagnostic criteria for symptomatic cases. This study systematically analysed echocardiographic characteristics of ULMV and, for the first known time, elucidated the detection rate of symptomatic ULMV in a consecutive Chinese cohort. METHOD: A single-centre retrospective cohort study was conducted, enrolling 137,214 consecutive symptomatic cardiac patients who underwent transthoracic echocardiography at T

Inhibition of thrombospondin-1 signaling prevents experimental bronchopulmonary dysplasia.

Bronchopulmonary dysplasia (BPD) is a neonatal lung injury characterized by inflammation and alveolar and vascular hypoplasia that currently lacks effective treatment. Thrombospondin (TSP)-1 is an angiostatic and proinflammatory protein, recently implicated in BPD pathogenesis, that both activates transforming growth factor (TGF)-β1 and suppresses nitric oxide (NO) signaling. To gain further insight into the relative importance of downstream effects of TSP-1, our objective in a neonatal rat model of hyperoxia-intermittent hypoxia (H-IH)-induced lung injury was to compare effects of inhibiting

Diuretics for Preventing Bronchopulmonary Dysplasia in Preterm Infants: A Systematic Review and Meta-Analysis.

INTRODUCTION: Randomized controlled trials (RCTs) have not demonstrated that diuretics prevent bronchopulmonary dysplasia (BPD), but recent observational studies warrant an updated synthesis. We evaluated the preventive effects and safety of diuretics in preterm infants at risk of developing BPD. METHODS: MEDLINE, Embase, Cochrane CENTRAL, and CINAHL were searched through June 2025. We included randomized and non-randomized studies of various diuretic classes with different routes, dosing regimens, and treatment durations. The risk of bias was assessed using the RoB 2 and ROBINS-I and the cer

Developmentally sensitive neuropharmacological effects of dexamethasone in neonatal bronchopulmonary dysplasia-associated brain injury via microglial Acod1-itaconate/IL-1β signaling.

BACKGROUND: Bronchopulmonary dysplasia (BPD) in preterm infants is frequently accompanied by neurodevelopmental impairment, yet the central neuropharmacological actions of dexamethasone (DEX), a commonly used therapy for severe or evolving BPD, remain incompletely understood. In particular, whether DEX exerts timing-dependent neuroprotection in the developing brain and the mechanisms underlying such effects are unclear. METHODS: We investigated the neuroprotective effects of DEX in a neonatal rat double-hit model combining prenatal maternal lipopolysaccharide exposure with postnatal hyperoxia

Beyond Heritable PAH: Pulmonary Hypertension in Genetic Syndromes.

Pulmonary hypertension (PH) may complicate a broad range of genetic syndromes beyond the established spectrum of heritable pulmonary arterial hypertension. Although these conditions are individually rare, together they represent an emerging field at the crossroads of developmental biology, vascular medicine, and precision genomics. In many cases, PH may be the presenting feature or may remain unrecognized because it occurs within complex multisystem disorders involving congenital heart disease, developmental lung abnormalities, parenchymal lung disease, vascular malformations, or extra-pulmona
