Sodium nitrite prevents impaired postnatal alveolar development.

Deficient nitric oxide (NO) signaling plays a critical role in the pathogenesis of bronchopulmonary dysplasia (BPD); however, clinical trials of inhaled NO (iNO) as a preventive therapy for BPD have shown little to no benefit. A biochemical obstacle to effective NO-based therapy relates to the high reactivity of NO, potentially leading to harmful oxidation and nitration. Hypothesizing that nitrite-based therapy has less potential to produce adverse reactions, we compared the preventive effects of sodium nitrite (NaNO2) and iNO on lung morphology, NO content and signaling, S-nitrosothiols (SNOs

Most infants with prenatal osteogenesis imperfecta diagnosis and poor prognosis survive: experience of a quaternary care osteogenesis imperfecta center.

Osteogenesis imperfecta is a genetic condition with improperly or inadequately produced Type I collagen. Manifestations include bowing deformities, fractures, hydrocephalus, respiratory insufficiency, and feeding difficulty. Moderate or severe OI is often diagnosed prenatally based on ultrasound findings and genetic testing may be labeled as lethal. Here, we present 18 infants with moderate to severely presenting OI who received neonatal care at a single center over a 5 yr period, 10 of which were delivered at our institution. All 18 infants survived to neonatal discharge, with 7 infants requi

Pulmonary disease in paediatric patients with trisomy 21: a review of imaging findings.

AIM: Trisomy 21, also known as Down syndrome, is the most common chromosomal abnormality seen in live births and is associated with congenital abnormalities involving multiple organ systems. While the congenital cardiac and gastrointestinal associations of trisomy 21 are well known, the associated pulmonary radiological findings are less widely described. Our objective is to assess the presence, categories, and prevalence of pulmonary radiological findings in patients with trisomy 21, and to describe and provide reference images of these findings. MATERIALS AND METHODS: A database of patients

Airway Anomalies Predict Risk of Pediatric Pulmonary Hypertension.

INTRODUCTION: Pediatric pulmonary hypertension (PH) significantly contributes to morbidity and mortality due to the progressive nature of the disease in some subtypes, which leads to severe right heart failure, impaired oxygenation, and subsequent complications of chronic hypoxia, and side effects of long-term therapies. Associations between PH and airway anomalies in pediatric patients are complex and multifactorial. Better understanding of these associations will help identify which patients should receive screening for PH by multidisciplinary care teams (pediatric otolaryngology, cardiology

The impact of external environment on pulmonary development - a morphological evaluation of pulmonary tissue in preterm infants.

Preterm birth disrupts the natural progression of pulmonary development, which can trigger functional and morphological consequences that may lead to death or the development of a chronic lung disease. The objective of this research was to evaluate the pulmonary morphological characteristics in 67 preterm neonates who had survived for a minimum of 24 hours. All evaluations were carried out on paraffin-embedded lung tissue, sliced at 5 micrometers, and stained with a standard hematoxylin-eosin stain. From each case, photomicrographs of one square millimeter were assessed, and the quantity of al

Perinatal risk factors and lung function at 8 years of age in extremely preterm infants: Insights from a new Japanese bronchopulmonary dysplasia classification.

BACKGROUND: Advances in neonatal care have considerably improved the survival rates of extremely preterm infants. However, long-term pulmonary sequelae remain a major concern. A revised classification of bronchopulmonary dysplasia (BPD) in Japan highlights histological chorioamnionitis, small for gestational age (SGA), and a bubbly/cystic appearance on chest X-ray (bubbly/cystic CXR) as key risk factors. This study aimed to evaluate the effect of extremely preterm birth on lung function at school age. METHODS: This retrospective study included 283 extremely preterm infants (born at <28 weeks

Development of late pulmonary hypertension after antenatal inflammation in experimental bronchopulmonary dysplasia.

BACKGROUND: Antenatal inflammation due to chorioamnionitis is strongly associated with the development of bronchopulmonary dysplasia (BPD) and BPD-associated pulmonary hypertension (BPD-PH) after preterm birth. However, mechanisms linking antenatal stress with temporal changes in the pulmonary circulation leading to BPD-PH during infancy are incompletely understood. We hypothesized that antenatal inflammation impairs lung alveolar and vascular growth that precedes and increases susceptibility for the development of late BPD-PH. METHODS: Fetal rats received intra-amniotic ETX or saline 2 days

Lung organoids: a new frontier in neonatology and paediatric respiratory medicine.

Great strides have been made in pre-clinical research in recent decades using animal models and cell lines. However, traditional models may fail to translate to humans, resulting in substantial failure rates in drug development. Recent three-dimensional organoid models have borne a good resemblance to the architecture, development and function of tissues, especially for organs with complex cell interactions and dynamics such as the lungs. In 2022, the role of organoids as alternative to animal testing was recognised by the US Food and Drug Administration. We searched Medline and ClinicalTrials

Prolonged Postnatal Hypoxia Impairs Lung Development and Causes Severe Pulmonary Hypertension in Mice.

BACKGROUND: In children born at high altitude, lung development is affected by hypoxia, which can lead to pulmonary hypertension and right ventricular failure. Animal models of postnatal, transient, hypoxia failed to reproduce these conditions seen in children living at high altitude. The aim of this study was to establish a mouse model of pediatric pulmonary hypertension associated with abnormal lung development. METHODS: C57bl/6J mice were exposed to normobaric hypoxia at 11% inspired fraction of oxygen starting on postnatal-days 1 to 4 ("11%-birth"). At age 8 weeks, pulmonary arterial pres

Serial MRI Measures Short-Term Parenchymal Changes in Neonatal Bronchopulmonary Dysplasia.

BACKGROUND: Bronchopulmonary dysplasia (BPD) is the most common neonatal lung parenchymal disease characterized by obstructive and restrictive lung abnormalities. Evaluation of neonatal BPD lung disease using MRI has been shown to be more sensitive to short-term outcomes than standard clinical measures. However, the longitudinal trajectory of parenchymal disease remains poorly characterized. RESEARCH QUESTION: What is the 1-month trajectory of lung parenchymal disease in infants with moderate to severe BPD, as assessed by quantitative MRI measures of lung volume and tissue density? Can We Enhance Long Term Respiratory Outcomes of Neonatal Pulmonary Diseases?

Infants born prematurely and infants born with neonatal pulmonary diseases are at risk for long term abnormal lung functions. In this review we will discuss efforts to enhance long term respiratory outcomes in premature infants, including the use of tracheostomy in those with severe bronchopulmonary dysplasia (BPD). We will further discuss long term respiratory abnormalities in newborns with congenital diaphragmatic hernia (CDH), representing infants with lung hypoplasia.

Increased pulmonary blood flow leads to alveolar dysplasia during the early postnatal developmental stage.

BACKGROUND: Increased pulmonary blood flow (IncPBF), one of the most important features of many children with congenital heart diseases, is well-known as a prerequisite for the induction of pulmonary arterial hypertension. However, due to the lack of neonatal mouse models of IncPBF, it remains largely unknown how IncPBF affects postnatal lung development. METHODS AND RESULTS: A neonatal mouse model of IncPBF was created via abdominal aorta and inferior vena cava fistula microsurgery at postnatal day 7 (P7) and verified by abdominal ultrasound and cardiac ultrasound. Hematoxylin-eosin staining

Changing the Underlying Causes of Recurrent Pneumonia In Children: A Systematic Review.

BACKGROUND: Recurrent pneumonia (RP) is a common diagnosis in children, leading to increased morbidity rates, frequent hospitalizations, and longer treatments. Asthma has traditionally been the most common underlying cause. However, recently, other conditions have emerged. Therefore, our goal was to systematically analyze and compare the changes in the underlying causes of RP. METHODS: We searched four databases from their inception until November 2024 for studies in children under 18 years with RP, where the underlying causes were described. To compare changes in the prevalence of underlying

Ultrasonographic diagnosis of secondary cardiac position alterations led by fetal thoraco-abdominal anomalies: a report of three cases.

BACKGROUND: Congenital diaphragmatic hernia (CDH) and congenital diaphragmatic eventration (CDE) are rare congenital anomalies. They occur in approximately one in 2,000-3,000 and 2,500-5,000 children respectively. It is difficult to distinguish between CDE and right diaphragmatic hernia in fetal period, which can cause changes in the position of the heart and lead to dysplasia of the right lung; absence of the right lung can lead to secondary dextrocardia. These are common in dextrocardia and lung agenesis, which are the main obstacles for diagnosis. CASE DESCRIPTION: This paper reports on th

Prenatal NO2 Exposure and Bronchopulmonary Dysplasia in Offspring: ceRNA Network-Mediated Cell Cycle Dysregulation in Alveolar Type II Cells.

Epidemiological studies have established a correlation between prenatal nitrogen dioxide (NO2) exposure and the incidence of respiratory diseases in offspring. However, the sex-specific outcomes of respiratory system development, critical risk windows, and underlying epigenetic mechanisms remain unclear. In this study, we established a prenatal NO2 exposure model and found that prenatal NO2 exposure induced pulmonary dysfunction and structural abnormalities in neonatal offspring (particularly airflow limitation, airway obstruction, and alveolar development) and impaired airway secretory functi

Structural and Functional Lung Assessment in Neonates With Moderate to Severe Bronchopulmonary Dysplasia Using 3D Ultra-Short Echo Time MRI.

BACKGROUND: 2D multi-slice MRI techniques for evaluating regional ventilation in neonatal lung diseases, including bronchopulmonary dysplasia (BPD), have limited through-plane resolution, potentially missing heterogeneous lung abnormalities. 3D ultra-short echo time (UTE) MRI phase-resolved functional lung (PREFUL) improves spatial resolution but has not been used to evaluate infants with BPD. PURPOSE: To demonstrate the feasibility of 3D UTE MRI for structural and functional assessment in infants with BPD. STUDY TYPE: Retrospective.
