Catheterization and cardiovascular interventions : official journal of the Society for Cardiac Angiography & Interventions2026eng
BACKGROUND: In patients with tetralogy of Fallot (TOF) and multiple aortopulmonary collaterals (MAPCAs), the presence of a ductus arteriosus (DA) supplying all flow to 1 lung and MAPCAs providing all flow to the other lung is a distinct phenotype that has not been well characterized.
AIMS: To characterize the anatomy of the pulmonary circulation and report long-term surgical outcomes in patients with this subtype of TOF/MAPCAs.
METHODS: From our institutional experience with TOF/MAPCAs, we ascertained patients with unilateral ductal origin of a PA who underwent surgery from 11/01 through 4/2

Congenital lung malformations (CLMs) may result in inflammation, malignancy, lung-dependent heart disease, and increased mortality. This study aimed to analyze the epidemiology of congenital lung malformations, changes in the prevalence, treatment patterns, and outcomes, including mortality. This nationwide retrospective research included all patients diagnosed with CLMs during 1986-2015. Histological findings, treatments, and primary outcomes were reviewed. Mortality due to CLMs was analyzed using official death certificates. The detection of CLMs increased from 1.9/100 000 live births (1990)

Congenital lung malformations (CLMs) are rare developmental anomalies that can cause severe respiratory failure in early infancy, occasionally necessitating extracorporeal membrane oxygenation (ECMO). This study aimed to determine the incidence of ECMO use and identify predictors of survival among infants with isolated CLMs who required ECMO support. We conducted a retrospective cohort study using the Extracorporeal Life Support Organization (ELSO) registry from 2000 to 2024. The primary outcome was survival to hospital discharge. Bivariate and multivariable logistic regression analyses were p

BACKGROUND: Most congenital diaphragmatic hernia (CDH) cases are identified prenatally, enabling early multidisciplinary interventions, including maternal transfer and preparation for extracorporeal membrane oxygenation (ECMO). These strategies may improve the historically stagnant survival rates in Korea. However, multicenter studies on the epidemiology and treatment outcomes of CDH are lacking. This study introduces the Korean CDH cohort (K-CDH), a nationwide prospective registry, and presents interim findings.
METHODS: The cohort comprises data from 6 tertiary centers (4 in Seoul and 2 in

BACKGROUND: The QUALIREHAB (Cardiac Rehabilitation in Youth With Congenital Heart Disease) trial demonstrated the efficacy of a hybrid cardiac rehabilitation model in adolescents and young adults with congenital heart disease. This post hoc mixed-methods study aimed to identify baseline predictors of clinical improvement and to characterize key components supporting implementation and scalability.
METHODS: Seventy participants aged 13 to 25 years from the intervention arm were analyzed. Clinical improvement was defined as a clinically meaningful increase in cardiopulmonary fitness (increase i

The radial Quiescent Interval Slice Selective (QISS) technique, which is used in cardiovascular magnetic resonance (CMR), is a reliable, contrast-free technique for obtaining high-quality vascular images. It is distinguished by its safety, insensitivity to motion and arrhythmias, and its overall simplicity and adaptability. However, previously published studies assessing pulmonary artery (PA) dimensions in children have not yet incorporated this technique. CMR scans of 139 patients (95 males), aged 6 weeks - 20 years, with no evidence of PA disorders were analyzed. Main PA (MPA), right PA (RPA

BACKGROUND: Congenital heart defects afflict ≈1% of all births worldwide. Although deep learning has shown significant promise in automating and improving adult echocardiography analysis, existing pediatric-based models are often limited to single tasks and specific echocardiographic views. To address this, we introduce EchoAI-Peds, a multitask deep learning model for pediatric echocardiography. Our model was developed using the most comprehensive set of pediatric labels to date and is designed to integrate information from multiple views simultaneously.
METHODS: We trained a video-based visi

Alongside advances in surgical and medical care, catheter-based interventions have reshaped the care of fetuses and neonates with hypoplastic left heart syndrome (HLHS) or borderline left heart structures over the last decades. These interventions include fetal aortic valvuloplasty (FAV), fetal and neonatal atrial septal interventions, the hybrid Stage-1 procedure (HS1P), and the emerging total transcatheter Stage-1 procedure (TTS1P). We summarize the current evidence on catheter-based interventions for HLHS and borderline left heart structures from fetal to neonatal life and systematically re

Congenital pulmonary airway malformation (CPAM) and bronchopulmonary sequestration are rare congenital lung anomalies that can occasionally coexist as a "hybrid" lesion. We report a case of a 15-year-old female with a two-week history of fever with evening-predominant spikes, cough, and right hypochondrial pain. The pain was aggravated by deep inspiration and coughing. Imaging revealed a right lower lobe consolidation containing air-filled cystic spaces on chest X-ray and computed tomography (CT). An anomalous arterial supply from the descending thoracic aorta on CT angiography was also noted.

INTRODUCTION: The number of patients with congenital lung malformations (CLM) is increasing because of prenatal diagnosis. We investigated the changes in the patient backgrounds and the surgical approach for lobectomy, evaluating the surgical outcomes of thoracoscopic surgery.
METHODS: We enrolled 50 CLM patients who underwent lobectomy between 1995 and 2024. Patients were divided into three 10-year groups to evaluate the trends in patient backgrounds. We also divided them into the thoracotomy and thoracoscopy groups to compare the patient backgrounds and surgical outcomes.
RESULTS: An incre

Congenital cystic lung malformations encompass a diverse group of lesions that arise during in utero development. A variety of pathologic classification schemes have been proposed, but recent advances suggest that gross and microscopic morphologies can often be explained by the underlying mechanism of cyst development. A subset of congenital pulmonary airway malformations, usually with large cysts, is associated with widespread epithelial oncogenic KRAS mutation. These have a long-term risk of developing mucinous adenocarcinoma. Another subset, more commonly with smaller cysts, are associated

Adult pulmonary sequestration (PS) is an uncommon congenital lung anomaly that may present with nonspecific respiratory symptoms and misleading radiological findings. We report two adult cases of intralobar pulmonary sequestration with distinct clinical presentations. The first case involved a 54-year-old diabetic smoker admitted for recurrent bronchopulmonary infections and severe recurrent hemoptysis requiring blood transfusions. Thoracic computed tomography (CT) angiography revealed a hypervascular lesion in the left lower lobe supplied by aberrant systemic vessels arising from the descendi

PURPOSE: Thoracoamniotic shunting (TAS) in fetuses with macrocystic congenital pulmonary airway malformation (CPAM) is mostly performed with pigtail shunts like the rocket shunt or the Harrison fetal bladder stent. The aim of this study was to assess the prenatal course, perinatal outcome and complications of TAS for severe macrocystic CPAM using the Somatex® intrauterine shunt.
METHODS: This was a two center (Cologne/Bonn) observational retrospective study of fetuses that underwent TAS using the Somatex® intrauterine shunt for severe macrocystic CPAM with and without hydrops between 2016-202

Fanconi anemia (FA; OMIM: 227650) is a rare genetic disorder characterized by bone marrow failure, congenital anomalies, and cancer predisposition. While FANCA mutations account for most FA cases, phenotypic overlap with other disorders complicates diagnosis. This study analyzes molecular diagnostic pathways for FANCA-related FA and establishes a hereditary differential diagnosis for ectrodactyly. A Chinese FA family clinical phenotype was collected. The proband and father underwent whole-genome sequencing. Copy number variations (CNVs) in FANCA were assessed by genomic qPCR. Functional charac

Congenital malformations remain one of the leading causes of perinatal mortality in India. These anomalies encompass a wide range of chromosomal abnormalities, inborn errors of metabolism, and other Mendelian disorders. We report a case of a four-day-old male neonate presenting with multiple congenital anomalies consistent with features of Trisomy 13, a rare chromosomal disorder, which ultimately led to neonatal death. An important observation was also made regarding the hydrostatic test-a procedure traditionally used to determine whether a newborn was liveborn or stillborn. This test, however

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is an uncommon cause of myocardial infarction that disproportionately affects women, particularly during pregnancy and the peripartum period. Limited understanding of its underlying pathophysiology hinders the development of effective preventive and therapeutic strategies.
METHODS: This study investigated associations between genetically predicted circulating proteins and tissue-specific RNA levels with genetically predicted SCAD risk using Mendelian randomization and Bayesian colocalization. Genetic scores for >1500 circulating protei

BACKGROUND: Asphyxiating thoracic dystrophy (ATD), or Jeune syndrome, is a rare skeletal dysplasia marked by a severe hypoplastic chest wall and restrictive lung disease, leading to an often-lethal thoracic insufficiency syndrome. Early-onset scoliosis (EOS) can complicate ATD, but details of operative management remain limited. This study aims to analyze the peri- and postoperative course of children with ATD who underwent posterior spinal fusion (PSF).
METHODS: A retrospective review was conducted for all patients with ATD treated for EOS who underwent PSF at our institution. Preoperative s

Lymphangioleiomyomatosis (LAM) is a rare, slowly progressive lung disorder that predominantly affects women of childbearing age. LAM is classified into two subtypes: tuberous sclerosis complex (TSC)-associated LAM (TSC-LAM) and sporadic LAM. Mutations in the TSC1 or TSC2 genes are the underlying cause of both forms. Over the past decade, research and clinical practice in managing LAM have advanced significantly, with breakthroughs in the understanding of disease pathogenesis, biomarker discovery, targeted therapies, and new therapeutic technologies. This article presents a review of the progre
