Hypoxemic respiratory failure is a common reason for admission to the neonatal intensive care unit for term and late preterm infants. Some infants have rare disorders due to genetic mechanisms including surfactant dysfunction disorders, alveolar capillary dysplasia with misalignment of the pulmonary veins, and other developmental lung disorders that result in more severe, persistent, and even fatal disease. In this article, we summarize clinical characteristics of the most common causes of neonatal respiratory failure and of rare genetic causes, to help clinicians differentiate between common

Schwartz-Jampel syndrome (SJS) is associated with neuromuscular and structural abnormalities that can complicate perioperative and anesthetic management. The syndrome has been noted to create increased difficulty in anesthetic airway management with age. We report the perioperative course of a 13-year-old female with presumed SJS undergoing combined anterior and posterior spinal fusion for severe progressive congenital scoliosis. Preoperative evaluation revealed features predictive of a difficult airway, including limited mouth opening and restricted neck mobility, requiring video laryngoscopy

AIM: To compare perioperative and long-term outcomes of lobectomy versus lung-sparing resection in children undergoing surgery for congenital pulmonary airway malformation (CPAM).
METHODS: A retrospective single-center study was conducted including children who underwent primary surgical resection for postnatally confirmed CPAM between 2005 and 2024. Patients with other congenital lung lesions, bilateral disease, syndromic conditions, or incomplete data were excluded. Perioperative variables, postoperative outcomes, redo surgery, and chest wall anomalies were compared between lobectomy and lu

Pulmonary vascular malformations (PVMs) are vascular complications that universally develop in patients with single ventricle congenital heart disease after Glenn surgery, yet the pathogenesis of single ventricle PVMs remains unknown. Using a surgical rat model of Glenn circulation, single-cell RNA sequencing (scRNAseq) on whole lung samples identified dramatic endothelial cell (EC) gene expression changes post-surgery, including decreased all-trans retinoic acid (ATRA) signaling. Many gene expression differences (∼17% of all EC differentially expressed genes), including decreased ATRA signali

BACKGROUND: Pulmonary sequestration (PS) is a congenital lung malformation, which is defined by an anomalous arterial supply from systemic circulation and an absence of communication with the tracheobronchial tree. It is further divided into intralobar and extralobar PS. Pulmonary sequestrations are usually treated surgically, especially if they are symptomatic. Malignant tumors are rarely found within such malformations, mainly adenocarcinomas. We report a case of a malignant melanoma found in a patient operated for intralobar pulmonary sequestration, which is, to the best of our knowledge, t

INTRODUCTION: Esophageal lung is a rare and complex congenital malformation, in which the bronchus of one or several pulmonary lobes arises from the esophagus. It is frequently associated with other malformations. Surgical management needs individual approaches, is often complex and depends on concomitant disorders.
CASE DESCRIPTION: A 3-month-old male was referred for further management of a left lower esophageal pulmonary lobe associated with a laryngotracheal cleft, congenital heart disease, heterotaxy, medialized stomach with additional microgastria and severe gastroesophageal reflux lead

BACKGROUND: Chromosome 17q23.1-q23.2 deletion syndrome is a rare genetic disorder characterized by various congenital defects, including microcephaly, heart and lung defects, limb abnormalities, and mild-to-severe developmental delay. Respiratory distress and severe pulmonary hypertension (PH) are possible initial symptoms. CASE PRESENTATION: We report two cases of full-term neonates with respiratory distress presenting shortly after birth. They both presented with severe PH in the early postnatal period, with oxygenation indices exceeding 70. Their symptoms were temporarily relieved with comp

INTRODUCTION: Primary ciliary dyskinesia (PCD) is a rare, genetic disorder characterized by impaired mucociliary clearance and progressive sino-pulmonary disease. Individuals have chronic, progressive chest symptoms throughout life with wet cough, frequent exacerbations and bronchiectasis. Traditional measures such as lung function and imaging do not capture the impact of PCD from the patient's perspective. This article examined health-related quality of life (HRQoL) measures and their applications in PCD.
METHODS: Published literature on HRQoL in children, adolescents and adults with PCD was

Primary ciliary dyskinesia (PCD) is a rare, hereditary disorder characterized by impaired motile ciliary function, resulting in abnormal mucociliary clearance (MCC). This leads to persistent infection and inflammation, progressive airway damage, and bronchiectasis. Physiotherapy, particularly airway clearance therapy (ACT), is considered a cornerstone of PCD management to compensate for the mechanical MCC deficit, aiming to reduce infection risk, prevent disease progression, and improve quality of life (QOL). However, evidence supporting specific physiotherapy regimens in PCD remains limited.

Hereditary haemorrhagic telangiectasia (HHT) is an autosomal dominant disorder with an estimated prevalence of one in 5000 to one in 7000. Pathogenic variants in three genes, endoglin (ENG), activin receptor-like kinase 1 (ACVRL1) and mothers against decapentaplegic homolog 4 (SMAD4), all part of the transforming growth factor-β signalling pathway, account for over 90% of HHT cases. Clinically, HHT is characterised by recurrent epistaxis, gastrointestinal bleeding from mucocutaneous telangiectasias, and visceral arteriovenous malformations (AVMs), most commonly affecting the lungs, liver and b

BACKGROUND: Prenatal therapy with elexacaftor/tezacaftor/ivacaftor (ETI) has recently emerged as a potential strategy to modify the early natural history of cystic fibrosis (CF). While case reports have described prevention of meconium ileus and pancreatic insufficiency, data on male reproductive outcomes remain extremely limited.
CASE PRESENTATION: We report a male infant with CF (homozygous F508del) whose heterozygous carrier mother initiated ETI at 27+4 weeks of gestation after prenatal diagnosis. Pregnancy was uneventful until preterm delivery at 35+2 weeks. The neonate presented with nor

Variants involving TBX4 are associated with ischiocoxopodopatellar syndrome with or without pulmonary arterial hypertension (ICPPS; also known as small patella syndrome), pulmonary arterial hypertension (PAH), and lethal lung developmental disorders. The variability of penetrance and expressivity of TBX4 variants remains a prominent challenge in understanding their genotype-phenotype correlations. We investigated a five-generation family with 12 affected individuals presenting with isolated ICPPS, lung-related manifestations with features of ICPPS, and other milder abnormalities. Whole-genome

INTRODUCTION: Congenital lung malformations are increasingly recognized through prenatal and early postnatal imaging. Although many affected children are asymptomatic in infancy, CLMs are associated with long-term respiratory and structural complications. The influence of surgical timing, approach, and extent on these outcomes remains uncertain.
MATERIALS AND METHODS: Using the TriNetX platform, we conducted a multicenter retrospective cohort study that included 20,101 children with CLM diagnosed before age 6 and 20,089,085 controls. Propensity score matching was applied. Asthma was the prima

BACKGROUND: Families and clinicians need more information about complications associated with congenital pulmonary malformations (CPMs) to engage in shared decision-making about surgery for asymptomatic children. The study's aim was to prospectively describe symptoms in children with a CPM and to identify markers associated with surgical removal because of symptoms.
METHODS: MALFPULM is a prospective population-based nationally representative cohort including pregnant women whose fetus had a CPM. Events occurring between birth and 2 years of age were compared between children who were not ope

Pulmonary arteriovenous malformation (PAVM) is a congenital vascular anomaly that forms a shunt between the pulmonary artery and vein. It is frequently detected incidentally in asymptomatic patients. We encountered a 69-year-old female patient with bilateral PAVMs that were incidentally detected on abdominal computed tomography( CT) performed for a detailed examination of appendicitis. Abdominal CT revealed nodular opacities with contrast enhancement in the middle lobe of the right lung and the lingular segment of the left lung. They were further examined, and a diagnosis of PAVMs measuring 14
