Acute Kidney Injury in Non-falciparum Malaria.

Acute kidney injury (AKI) complicates non-falciparum malaria, particularly that from Plasmodium knowlesi. AKI (any KDIGO stage) is present in 20-30% of hospitalized patients with knowlesi malaria, with age >45 years having a sixfold risk of AKI. WHO-defined severe AKI (creatinine >265μmol/L) is found in ∼2.5% of adult knowlesi hospitalizations and 60% of deaths, with pathogenesis linked with intravascular hemolysis, endothelial activation, glycocalyx degradation and acute tubular necrosis (ATN). Paracetamol may have a renoprotective effect in severe knowlesi AKI, including reductions in medium

Multi-pronged surveillance to understand the spatiotemporal correlations among macaques, vectors and humans in Plasmodium knowlesi malaria transmission.

BACKGROUND: Plasmodium knowlesi, a non-human primate (NHP) malaria parasite, has become a major public health concern in Malaysia and is now the leading cause of human malaria infections in the country. The transmission of P. knowlesi involves a complex cycle among humans, non-human primates and vectors. Numerous studies have focused on these hosts individually, but comprehensive research that integrates field data from all three hosts is lacking. This study aims to integrate multi-pronged surveillance data from macaques, vectors and human blood samples to better understand the epidemiology of

Mass spectrometry-based metabolomics uncovers distinct metabolic signatures and potential therapeutic targets in Plasmodium knowlesi.

Malaria remains a major global health challenge, caused by several Plasmodium species and transmitted via mosquito vectors. Among these, Plasmodium knowlesi is notable for its zoonotic nature, capable of infecting both macaques and humans. The incidence of P. knowlesi infections has been rising, particularly in Southeast Asia, raising public health concerns. However, compared to other Plasmodium species, the biology, pathophysiology, and transmission dynamics of P. knowlesi remain poorly understood. Given the absence of a licensed vaccine and the increasing threat of drug resistance, a deeper

Low-volume Plasmodium blood sample processing protocols for untargeted transcriptomics optimized using Plasmodium knowlesi.

As the importance of transcriptional variation and regulation for Plasmodium becomes more apparent, advances for non-falciparum species are hindered by our reliance on natural infections to study parasite biology. Untargeted transcriptomic research is also complicated by low-parasite densities and high proportions of human genetic material, highlighting the need for optimized sample processing protocols. In this study, we used a Plasmodium knowlesi culture diluted in whole blood as a mock Plasmodium vivax natural infection to compare white blood cell (WBC), rRNA and globin depletion methods an

Persistent low-density infection in areas at risk of malaria reintroduction in Malaysia.

Malaysia successfully achieved zero indigenous human malaria cases since 2018. However, challenges persist from Plasmodium knowlesi (zoonotic malaria) and low-density infections, posing reintroduction risks in previously malaria-free areas. Addressing these hidden infections is critical for sustaining Malaysia's elimination gains. This study investigated the persistence of low-density malaria transmission in high-risk localities declared malaria-free for at least three consecutive years. A community-based cross-sectional survey was conducted from June to October 2020 in 23 high-receptivity loc

Plasmodium knowlesi and global changes: socio-ecological drivers and challenges.

Plasmodium knowlesi, a malaria species predominantly found in Southeast Asia, is influenced by climate change in its transmission dynamics. Variations in temperature and precipitation impact both mosquito vectors and the parasites they carry. Additionally, deforestation and alterations in land use, often driven by climate change, facilitate the zoonotic transmission of the disease from simian hosts to humans. Climatic fluctuations also influence human and community behaviours, potentially altering exposure risks. Although increased awareness of climate risks may prompt behavioural changes, the

Histone lactylation: A new epigenetic mark in the malaria parasite Plasmodium.

Epigenetic processes play important roles in the biology of the malaria parasite Plasmodium falciparum. Here, we characterised a new epigenetic mark, histone lactylation, for the first time in Plasmodium: it was found in two human malaria parasites, P. falciparum and P. knowlesi, and also in vivo in two rodent malaria models, P. yoelii and P. berghei. Histones were increasingly lactylated in response to elevated lactate levels in vitro and in vivo, making this mark uniquely well-placed to act as a metabolic sensor, since severe falciparum malaria characteristically leads to hyperlactataemia in

Plasmodium knowlesi gametocyte carriage and load among malaria patients at Kapit Hospital in Sarawak, Malaysian Borneo.

The number of human infections with Plasmodium knowlesi, a malaria parasite typically found in long-tailed and pig-tailed macaques, have increased and P. knowlesi has become the sole cause of indigenous cases of malaria in Malaysia since 2018. The reasons for the increase are multifactorial and could include human-to-human transmission through mosquito bites. Such transmission would require viable gametocytes circulating in the blood of infected individuals as this is the only parasite blood stage transmissible to mosquitoes. The objectives of this study were to determine the proportion of P.

A cold-chain free loop-mediated isothermal amplification test for detection of human malaria in point-of-care settings.

Malaria diagnosis in low-resource settings demands robust and cold-chain free molecular tests. To overcome infrastructure limitations of conventional loop mediated isothermal amplification (LAMP), we developed a refrigeration-independent assay for Plasmodium knowlesi detection. By using sugar-based enzyme preservation approach, sucrose shows most effectively to maintain Bst DNA polymerase activity during room-temperature storage, outperforming equivalent concentrations of glucose and galactose. The sucrose-preserved LAMP reagent demonstrated consistent performance for 12 days. This optimized a

Plasmodium knowlesi Heat Shock Protein 90s: In Silico Analysis Reveals Unique Druggable Structural Features.

The increasing threat of zoonotic malaria parasites of humans, such as Plasmodium knowlesi, make the search for improved pharmacotherapy imperative. Using protein sequence and structural analyses, phylogenetics, protein network mapping, protein-ligand interaction, and small molecule docking studies, we have identified for the first time the predicted structure, function, and druggability of the P. knowlesi heat shock protein 90s (PkHsp90s). Four isoforms were identified (in the cytosol, endoplasmic reticulum, mitochondrion, and apicoplast), and key structural differences were elucidated compar

Performance evaluation of the automated haematology analyzer XN-31 for malaria diagnosis in Plasmodium vivax-dominant regions of Thailand.

BACKGROUND: The innovative automated haematology analyzer XN-31 has demonstrated comparable or, in some settings, superior performance to microscopy and rapid diagnostic tests (RDTs), and good concordance with polymerase chain reaction (PCR) in various endemic areas, particularly where Plasmodium falciparum is prevalent. The XN-31 applies the principle of flow cytometry to measure cell size and nucleic acid content to generate a two-dimensional cytogram in approximately one minute. This technique identifies P. falciparum and other species and provides % parasitaemia (MI-RBC%). This study evalu

In silico profiling of the plasmodium knowlesi 32 kDa antigen: Diversity, epitope prediction, and structural modeling.

The emergence of Plasmodium knowlesi malaria in Southeast Asia, particularly Malaysia, necessitates effective interventions. However, high polymorphism often limits the utility of key merozoite surface proteins (MSPs) as viable vaccine candidates. Pk32, a predicted glycosylphosphatidylinositol (GPI)-anchored protein in P. knowlesi, shares homology with the relatively conserved Plasmodium vivax Pv32, suggesting it may be a promising vaccine candidate. We analyzed the genetic diversity, polymorphism, and signatures of natural selection and 3-D structure using 39 full-length Pk32 sequences, prima

Submicroscopic Burden of Zoonotic Plasmodium knowlesi Malaria on Mursala Island and Plasmodium falciparum and Plasmodium vivax Transmission in Mainland North Sumatra, Indonesia.

Accurate molecular tools are essential for estimating zoonotic malaria transmission in Southeast Asia. This study applied ultrasensitive reverse-transcriptase real-time polymerase chain reaction (PCR) to detect zoonotic malaria in febrile patients from health facilities across three mainland districts (Batubara, Tanjung Balai, and Central Tapanuli) and separately on Mursala Island, North Sumatra, Indonesia. Among 64 participants on Mursala, 7 (10.9%) adults had Plasmodium knowlesi infections (including 5 agricultural workers and 2 adults with severe WHO anemia), and 2 (3.1%) adults had Plasmod

Rapid Detection of Knowlesi Malaria Using Room-Temperature Stable and Ready-to-Use Colorimetric Loop-Mediated Isothermal Amplification Reagents.

A room temperature-stable and ready-to-use dried-format loop-mediated isothermal amplification (LAMP) diagnostic tool is proposed for detecting knowlesi malaria in the present study. Traditional LAMP assays require a cold chain to maintain the stability of their reagents during storage and transport. This requirement limits the use of LAMP-based methods in environments with limited infrastructure. Results can be obtained by directly observing color changes without the need for specialized instruments; positive samples appear yellow, whereas negative reactions remain pink. A visualized hybrid PCR-LAMP assay for the detection of human Plasmodium species.

Combining the advantages of PCR and LAMP, we described a new technique, namely PCR-LAMP, for malaria diagnosis. The whole process of DNA amplification can be completed in 35 min. This hybrid amplification technique markedly improved the sensitivity of detection compared to the classic single PCR or LAMP assay alone. PCR-LAMP assay had a detection limit of 1 copy/µL for P. knowlesi and P. ovale, 0.1 copy/µL for P. vivax, P. falciparum and P. malariae, respectively. To facilitate the endpoint detection, xylenol orange was added. Positive samples were indicated in orange while negative reactions

A multi-criteria framework for disease surveillance site selection: case study for Plasmodium knowlesi malaria in Indonesia.

Disease surveillance aims to collect data at different times or locations, to assist public health authorities to respond appropriately. Surveillance of the simian malaria parasite, Plasmodium knowlesi, is sparse in some endemic areas and the spatial extent of transmission is uncertain. Zoonotic transmission of Plasmodium knowlesi has been demonstrated throughout Southeast Asia and represents a major hurdle to regional malaria elimination efforts. Given an arbitrary spatial prediction of relative disease risk, we develop a flexible framework for surveillance site selection, drawing on principl

Genetic polymorphism and clustering of the Plasmodium cynomolgi Duffy binding protein 1 region II of recent macaque isolates from Peninsular Malaysia.

Plasmodium cynomolgi is a simian malaria parasite that has been increasingly infecting humans. It is naturally present in the long-tailed and pig-tailed macaques in Southeast Asia. The P. cynomolgi Duffy binding protein 1 region II [PcDBP1(II)] plays an essential role in the invasion of the parasite into host erythrocytes. This study investigated the genetic polymorphism, natural selection and haplotype clustering of PcDBP1(II) from wild macaque isolates in Peninsular Malaysia. The genomic DNA of 50 P. cynomolgi isolates was extracted from the macaque blood samples. Their PcDBP1(II) gene was a

Updating estimates of Plasmodium knowlesi malaria risk in response to changing land use patterns across Southeast Asia.

BACKGROUND: Plasmodium knowlesi is a zoonotic parasite that causes malaria in humans. The pathogen has a natural host reservoir in certain macaque species and is transmitted to humans via mosquitoes of the Anopheles Leucosphyrus Group. The risk of human P. knowlesi infection varies across Southeast Asia and is dependent upon environmental factors. Understanding this geographic variation in risk is important both for enabling appropriate diagnosis and treatment of the disease and for improving the planning and evaluation of malaria elimination. However, the data available on P. knowlesi occurre
