Early neonatal death due to prenatally undiagnosed congenital subglottic stenosis in Noonan syndrome.

INTRODUCTION: Congenital subglottic stenosis is a rare malformation of the laryngeal airway. In its severe forms, it can result in immediate neonatal respiratory failure and death if not managed emergently at birth. CASE PRESENTATION: A pregnant woman, whose fetus was suspected of having Noonan syndrome, underwent a cesarean delivery. Notably, subglottic stenosis was not detected on prenatal imaging. The neonate exhibited immediate and severe respiratory distress, and upper airway obstruction was diagnosed during neonatal resuscitation, leading to failed intubation and subsequent neonatal dem

AAV-Mediated Base Editing for Correction of RSPH4A Mutations in Primary Ciliary Dyskinesia: A Proof-of-Concept Study.

Primary ciliary dyskinesia (PCD) is a rare, genetically heterogeneous disorder, with abnormal ciliary motility, usually due to an ultrastructural defect, with chronic airway infections. Currently, no curative therapy exists for PCD. Given the prevalence of single nucleotide variants (SNVs) among causative mutations, we evaluated a novel base-editing approach. Specifically, we used a nickase Cas9 fused to adenosine deaminase to correct mutations in the radial spoke head component 4 A (RSPH4A) gene, causing PCD. We selected two PCD patients sharing the same SNV in RSPH4A, one with compound heter

Follow-Up of Children With Primary Ciliary Dyskinesia.

Primary ciliary dyskinesia (PCD) is a genetically heterogeneous disorder resulting in defective ciliary motility, which leads to a spectrum of clinical manifestations, including chronic upper and lower respiratory infections, middle ear disease, situs abnormalities, congenital heart defects, and infertility. The development of standardized management plans for pediatric patients with PCD is hindered by the lack of high-quality evidence regarding treatment and follow-up protocols, alongside significant inter-individual variability in disease severity and presentation. Moreover, the diagnosis of

Left pulmonary artery sling in an infant: a vascular anomaly with a high impact on breathing.

An infant presented with persistent stridor and recurrent respiratory distress since birth. Imaging revealed a left pulmonary artery sling causing significant tracheal compression. Surgical reimplantation of the left pulmonary artery to the main pulmonary artery with concurrent slide tracheoplasty resulted in full recovery. This case highlights the importance of early recognition of vascular anomalies in infants presenting with unexplained airway obstruction.

A Case Report: Seven-Year Follow-Up of a Patient With Kartagener Syndrome Undergoing Bilateral Lung Transplantation.

Kartagener syndrome (KS) is a rare genetic disorder inherited in an autosomal recessive manner. Bronchiectasis is a major contributor to the morbidity associated with this syndrome. In end-stage cases of KS, bilateral lung transplantation (DLuTx) is considered a viable treatment option. This case report presents a 7-year follow-up of a KS patient who developed end-stage respiratory failure and subsequently underwent DLuTx. The case includes both surgical and postoperative challenges, ultimately resulting in a significant improvement in the patient's quality of life.

Effects of MEK-inhibitor treatment in infants with lymphatic abnormalities in noonan syndrome.

Noonan syndrome (NS) is a genetic disorder associated with dysregulation of the RAS/MAPK signaling pathway and is frequently accompanied by lymphatic abnormalities. These lymphatic complications can lead to severe clinical manifestations, including chylous effusions and respiratory compromise, particularly in infants. Management of such complications is often challenging, and conventional therapies may be insufficient in severe cases. Recently, targeted therapies affecting the RAS/MAPK pathway, such as MEK inhibitors, have emerged as a potential treatment option for patients with refractory ly

Impulse oscillometry indices are associated with functional impairment and progressive pulmonary fibrosis across interstitial lung diseases.

BACKGROUND: Respiratory oscillometry provides effort-independent indices of airway heterogeneity (R5-20) and respiratory system stiffness (AX). Their clinical relevance in interstitial lung diseases (ILD) remains unclear and was the aim of our study. RESEARCH DESIGN AND METHODS: A cross-sectional study including patients with ILD who underwent oscillometry, pulmonary function tests (PFTs), and six-minute walking distance (6MWD) at a single visit. R5-20 and AX were analyzed as continuous variables and as thresholds derived from receiver operating characteristic (ROC) analysis for progressive p

Combined laryngocele: an atypical cause of acute airway obstruction.

The authors present a case of a combined laryngocele that manifested as acute upper airway obstruction requiring tracheostomy. The initial clinical presentation and imaging were suggestive of a laryngeal neoplasm; however, a combined laryngocele was subsequently diagnosed.A complete transoral resection of the laryngocele was performed using a CO₂ LASER. The postoperative course was uneventful, allowing for tracheostomy closure and no recurrence was observed during the follow-up period.

Shunt estimation in pneumonia of different aetiologies: a non-invasive physiological assessment.

INTRODUCTION: COVID-19 has challenged traditional models of respiratory failure, and several studies have suggested that gas exchange impairment in COVID-19 pneumonia may involve mechanisms beyond anatomical shunt. This has renewed interest in non-invasive physiological tools to explore gas exchange abnormalities, including the evaluation of shunt, dead space and ventilation/perfusion mismatch. MATERIAL AND METHODS: This prospective cross-sectional study was conducted between October 2020 and January 2021 at Papa Giovanni XXIII Hospital (Bergamo, Italy), enrolling adults with acute respirator

Retroperitoneal Bronchogenic Cysts: A Case Series.

BACKGROUND Bronchogenic cysts are a rare congenital malformation of primitive foregut development, predominantly located in the thorax. However, they can also be found in various atypical locations, including the retroperitoneal space. Retroperitoneal bronchogenic cysts are exceedingly rare. The diagnosis of retroperitoneal bronchogenic cysts presents considerable difficulty owing to their rarity and the absence of distinctive clinical and imaging characteristics. CASE REPORT This study reports 6 surgically confirmed cases of retroperitoneal bronchogenic cysts at our institution. One patient p

Lung mucus burden and treatment response assessed by MRI in the post-modulator era.

BACKGROUND: Mucus is believed to be central to the progressive nature of cystic fibrosis lung disease. Poor clearance leads to plugging, inflammation, and infection, driving a cycle that eventually results in irreversible lung damage. We hypothesized that patients with higher mucus burden would demonstrate greater improvement in spirometry and ventilation when treated with modulator therapy. METHODS: We retrospectively analyzed data collected in 29 participants prior to and following initiation of elexacaftor/tezacaftor/ivacaftor (ETI) therapy. Participants performed 1H and 129Xe MRI, multipl

[Normal Delivery by Intracytoplasmic Sperm Injection with Epididymal Sperm in a Patient with Azoospermia and Kartagener Syndrome].

Kartagener syndrome is defined as a primary ciliary dyskinesia with visceral inversion, bronchiectasis, and chronic sinusitis. Male patients with primary ciliary dyskinesia including Kartagener syndrome are also associated with male infertility, mainly because of sperm tail immobility. We report a case in a 36-year-old man with Kartagener syndrome and azoospermia. An examination indicated obstructive azoospermia. Therefore, testicular sperm extraction and microsurgical epididymal sperm aspiration were performed, and sperm retrieval was successful. Motile epididymal spermatozoa were used for the

First neonatal case of primary ciliary dyskinesia caused by a pathogenic DRC1 variant presenting with recurrent atelectasis and airway hypersecretion: a case report and literature review.

BACKGROUND: Primary ciliary dyskinesia (PCD) is a relatively rare hereditary disorder. Pathogenic variants in DRC1 are prevalent in East Asian populations and are frequently associated with situs solitus, which often obscures clinical suspicion. More than 80% of affected neonates with PCD present with respiratory distress; however, definitive diagnosis often requires specialized investigations, and many cases remain undiagnosed. Because PCD can progress to chronic obstructive lung disease and bronchiectasis, early diagnosis and appropriate management are essential. We report the first neonatal

Primary Ciliary Dyskinesia With Situs Inversus in Twin Premature Infants: Two Case Reports.

AIMS/BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare autosomal recessive disorder characterised by structural or functional abnormalities of the cilia, which prevent proper oscillation. Patients typically present with symptoms such as chronic respiratory infections, bronchiectasis, and, in approximately half of the cases, inversion of internal organs. Due to its diverse clinical manifestations and lack of specificity, PCD is often misdiagnosed or underdiagnosed, and diagnosis in the neonatal period remains notably challenging. Therefore, this study aimed to analyse the clinical characte

Diagnostic pitfall: Right frontal brain abscess presenting as presumed cervical pathology in a patient with Kartagener syndrome - A case report.

BACKGROUND: Kartagener syndrome (KS), a phenotypic subset of primary ciliary dyskinesia, is characterized by defective motile cilia, impaired mucociliary clearance, chronic sinus disease, bronchiectasis, and situs abnormalities. Although respiratory manifestations predominate, central nervous system complications such as brain abscess are rare but potentially life-threatening. Because early neurological manifestations may be focal and non-specific, diagnosis can be delayed when symptoms are initially attributed to cervical or peripheral pathology. CASE DESCRIPTION: We describe a 27-year-old m

Oxygen-enhanced MRI and multiple breath washout with Short extension reveal cystic fibrosis lung disease progression despite triple modulator therapy.

INTRODUCTION: The current cystic fibrosis (CF) care era, while hugely welcome, raises new challenges, particularly the need for more sensitive pulmonary outcome measures. Seeking further optimisation, we previously developed a Short extension to multiple breath washout measure (MBWShX) which captures previously overlooked, under-ventilated lung units but lacks regional information. Functional lung MRI addresses this limitation. We hypothesised these measures would be more sensitive to change in tracking CF lung disease than usual clinical respiratory function tests. METHODS: Forty-six people
