BACKGROUND: Chronic granulomatous disease (CGD) is a rare primary immunodeficiency disorder caused by defects in phagocyte NADPH oxidase. X-linked CGD, resulting from CYBB gene mutations, accounts for approximately two-thirds of cases. Clinical manifestations typically include recurrent bacterial and fungal infections, granuloma formation, and inflammatory complications. CASE PRESENTATION: We report monochorionic diamniotic twins with a novel CYBB variant (c.1303A > T, p.Lys435Ter). The 7-month-old proband presented with recurrent respiratory infections and was initially misdiagnosed with cong

BACKGROUND: MRI revealed a high prevalence of lung abnormalities in children with primary ciliary dyskinesia (PCD). However, longitudinal imaging data on the onset and progression of PCD are lacking.
RESEARCH QUESTION: When do abnormalities in lung morphologic features and perfusion in patients with PCD first emerge, and how do they progress longitudinally from infancy through adulthood as assessed by MRI?
STUDY DESIGN AND METHODS: One hundred eighty-nine MRI examinations (mean [SD] examinations per patient, 2.4 [1.7] examinations; range, 1-10 examinations) from 75 patients with PCD (mean [S

BACKGROUND: Horseshoe lung (HL) and scimitar syndrome are rare congenital cardiopulmonary anomalies that may coexist, posing complex diagnostic and management challenges. The following case presents the clinical manifestation of repeated hemoptysis episodes and syncope, a combination that has rarely been reported in this context. Moreover, there were treatment challenges regarding the utility of minimally invasive techniques in the iatrogenic occlusion of aberrant vessels, which has more recently been suggested in the literature to yield comparable outcomes compared with open surgeries.

Primary ciliary dyskinesia (PCD) is a rare inherited disorder characterised by impaired ciliary function, leading to chronic upper and lower airway disease from early life. Limited awareness of the condition contributes to delayed diagnosis, with some individuals first diagnosed in adulthood following specialised testing. We describe 91 adults referred to the PCD diagnostic service at the Royal Children's Hospital in Melbourne, Australia over a 10-year period. Twenty-eight adults were diagnosed with PCD (31% referrals). The most common reason for referral was bronchiectasis. These findings hig

BACKGROUND: Bronchogenic cysts often require surgical excision in children because of the risk of progressive enlargement and complications. Although minimally invasive surgery is preferred, pediatric thoracic procedures remain technically challenging because of limited intrathoracic working space.
CASE DESCRIPTION: A 6-year-old girl with an asymptomatic mediastinal mass demonstrated interval growth of a cystic lesion in the right superior mediastinum on serial imaging. Single-port robotic resection was performed using the da Vinci SP system through a single intercostal incision with low-pres

Bronchogenic cysts are rare congenital anomalies arising from abnormal budding of the ventral foregut during early embryogenesis. They are typically located in the mediastinum or pulmonary parenchyma and are mostly asymptomatic. However, complications such as infection, compression, hemorrhage, or rupture may cause significant morbidity. Surgical resection is recommended in symptomatic or complicated cysts and may also be justified in asymptomatic patients to prevent future complications. We report the case of a 40-year-old male who presented with chest pain and dysphagia. A presumptive diagno

Ciliated foregut cysts are rare congenital lesions derived from abnormal budding of the primitive foregut and are most commonly identified in the mediastinum or liver, where they may also be referred to as bronchogenic-type cysts. Occurrence within the gallbladder is extremely uncommon, with only a limited number of cases reported in the literature. We present the case of a 29-year-old male who presented with right upper quadrant abdominal pain, nausea, and vomiting. Computed tomography and ultrasound revealed a cystic lesion adjacent to the gallbladder that was initially interpreted radiologi

TREC-NBS identifies patients with inborn errors of immunity (IEI) and syndromic features, but uncertainty remains regarding their immunological management. To address this, syndromic patients detected by TREC-NBS in Germany between August 2019 and April 2024 were systematically analyzed, including phenotype, treatment, and outcomes. National registries were screened, and data were completed by the treating centres. A total of 77 syndromic patients were identified, with 22 different gene defects found in 72 individuals (93.5%). Primary thymic deficiency was present in 64% (49/77), most commonly

BACKGROUND: Pericardial cysts are a rare form of congenital mediastinal cystic lesion. They are usually asymptomatic and are often discovered incidentally, although some patients may present with a variety of different symptoms. The presence of small, asymptomatic pericardial cysts does not need treatment; however, large and/or symptomatic cysts may require surgical intervention.
CASE SUMMARY: We report a case of a 42-year-old man with a 7-year history of dysphagia and non-specific thoracic complaints. Multiple gastroenterological examinations and proton pump inhibitor therapies did not find

Technical difficulties encountered during endotracheal tube (ETT) extubation are rare but can precipitate catastrophic airway complications. We present the case of a 3-month-old male infant weighing 5.4 kg who underwent video-assisted thoracoscopic surgery (VATS) for a bronchogenic cyst excision. Following the procedure, initial attempts at extubation failed. Flexible fiberoptic bronchoscopy (FOB) identified that the ETT had been inadvertently sutured to the tracheal wall. Since this Vicryl suture also secured the surgical repair, extubation was delayed to avoid disrupting the anastomosis. On

Bronchogenic cysts (BC) are rare congenital malformations that result from abnormal budding of the primitive foregut. Most of these cysts are located in the mediastinum or the lungs. Rarely, abdominal BCs result from embryonic budding migrating below the diaphragm prior to the fusion of the pleuroperitoneal membranes. We present a case of a 34-year-old male patient undergoing evaluation for bronchopneumopathy. Preoperative imaging revealed a well-circumscribed cystic lesion adjacent to the colon, which failed to provide a definitive diagnosis due to its atypical location and nonspecific radiol

Bronchogenic cysts are congenital foregut malformations most often found in the mediastinum or lung parenchyma. Abdominal locations are exceedingly rare and most commonly found in the left retroperitoneum. They may mimic other intra-abdominal masses and pose a preoperative diagnostic challenge. A 17-year-old female presented with 4 days of diffuse, mild abdominal pain. Ultrasound and MRI identified a 5.5 × 3.8 × 3.1 cm unilocular, hypoechoic cyst adjacent to the terminal ileum at the ileocecal valve. Preoperative differential diagnoses included an ileal duplication cyst. Single-incision laparo

Bronchogenic cysts are rare congenital foregut malformations that most commonly arise in the mediastinum. Intrapulmonary bronchogenic cysts are less frequent and may present diagnostic challenges when located adjacent to mediastinal structures. We report a case of an intrapulmonary bronchogenic cyst that closely mimicked a paraesophageal mediastinal cyst on preoperative imaging. A 72-year-old man was referred to our department after a cystic lesion was incidentally detected during a routine health examination. CT demonstrated a 37-mm well-circumscribed cystic lesion in the right lower thorax.

Bronchogenic cysts are rare benign congenital lesions, most commonly located within the thoracic cavity, particularly in the mediastinum or pulmonary parenchyma. Subcutaneous presternal bronchogenic cysts are exceedingly uncommon, with fewer than 80 cases reported in the literature since their first description in 1945. We report a rare case of a 14- year- old girl presenting with a progressive presternal midline swelling since childhood, previously managed by needle aspiration with subsequent recurrence. Thoracic computed tomography suggested a thyroglossal duct cyst. Complete surgical excisi

BACKGROUND: Primary Ciliary Dyskinesia (PCD) and predominantly antibody deficiencies (PAD) are among the important causes of recurrent respiratory tract infections from childhood and require different treatment strategies. The primary aim of this study was to investigate the coexistence of PCD and PAD in a cohort of patients diagnosed with PCD at a tertiary care hospital. Our secondary aim was to determine the association between the ultrastructural phenotype and the type of immune disorder.
METHODS: Sixty-two patients with PCD were enrolled in the study. Complete blood count, serum immunoglo

Bronchogenic cysts, a subgroup of foregut duplication cysts, are rare congenital malformations of ventral foregut development. These cysts can be found ectopically along the entire developmental pathway of the foregut. Less than 1% of bronchogenic cysts are found in the oral cavity, with fewer than 10 cases previously reported in the literature. These lesions typically present asymptomatically shortly after birth but have the potential to cause respiratory and/or feeding difficulties. Treatment is surgical excision, and histopathology is ultimately required to reach a definitive diagnosis and

AIM: To compare perioperative and long-term outcomes of lobectomy versus lung-sparing resection in children undergoing surgery for congenital pulmonary airway malformation (CPAM).
METHODS: A retrospective single-center study was conducted including children who underwent primary surgical resection for postnatally confirmed CPAM between 2005 and 2024. Patients with other congenital lung lesions, bilateral disease, syndromic conditions, or incomplete data were excluded. Perioperative variables, postoperative outcomes, redo surgery, and chest wall anomalies were compared between lobectomy and lu
