On the occurrence of mirror modes at Mercury and their comparison with magnetic holes and ion cyclotron waves.

In the context of this review, civil unrest is defined as disharmony, expressive dissatisfaction and/or disagreement between members of a community, which leads to a situation of competitive aggression that may find expression as disruption of organisation, conflicts, damage to property and injuries. Such a breakdown of harmonious relationships, which may result in property damage and human injuries that may be threatening to life, varies in magnitude from participation of a very few individuals up to the involvement of large crowds of people, which may evolve into a full-scale riot. It is the

Unmasking multiple myeloma first presentation as pericardial effusion with tamponade physiology: a case report.

BACKGROUND: Multiple myeloma is a malignant plasma cell disorder primarily involving the bone marrow and skeleton, leading to anemia, renal dysfunction, and lytic bone lesions. Extramedullary disease, seen in about 9% of cases, reflects aggressive disease biology with poor prognosis. Common sites include the pleura, liver, and gastrointestinal tract, while pericardial involvement is exceedingly rare and often detected postmortem. Fewer than 25 cases of pericardial effusion or cardiac tamponade due to multiple myeloma have been reported, usually in advanced disease. The mechanism likely involve

A case of pleuroparenchymal fibroelastosis without radiological pleural involvement.

BACKGROUND: Pleuroparenchymal fibroelastosis (PPFE) is a rare interstitial lung disease (ILD) characterised by elastosis and fibrosis of the pleura and adjacent subpleural lung parenchyma, generally involving the upper lobes. Atypical presentations have been described, with a predominantly (intra-alveolar) airway-centred fibroelastosis without overt pleural involvement, considered to be 'consistent with PPFE'. The current case illustrates an atypical presentation of PPFE, highlighting the heterogeneity of this disease. CASE PRESENTATION: A 66-year-old Caucasian female, former smoker, presente

Diagnostic performance of triplanar real-time chest magnetic resonance imaging in preschool children - comparison with dedicated lung magnetic resonance imaging.

BACKGROUND: Fast magnetic resonance imaging (MRI) acquisitions with high frame rates can mitigate artefacts caused by physiological motion and bulk patient movement, enabling motion-resolved imaging. OBJECTIVE: This study aims to evaluate the detection performance of triplanar real-time cine fast imaging employing steady-state acquisition (FIESTA), compared to conventional MRI sequences. MATERIALS AND METHODS: This retrospective study included 53 children (median age, 1.9 years; range, 1 day-7.1 years) who underwent clinically indicated 1.5-T chest MRI in sedation between January and Decembe

Diagnostic yield and safety of needle-based COnfocaL LAser endomicroscopy and ultrasound-guided pleural BiOpsy in diagnosing patients with unknown pleuRAl effusion: a protocol for a mulTIcenter, randomized cONtrolled trial (COLLABORATION-II).

BACKGROUND: The diagnosis of exudative pleural effusion is challenging. Although ultrasound-guided pleural biopsy (UGBx) is common, its effectiveness varies, especially in the absence of pleural abnormalities. New imaging techniques have been explored to improve UGBx outcomes, but the results remain inadequate. Recently, needle-based confocal laser endomicroscopy (nCLE) has been used to improve biopsy targeting and diagnostic yield through real-time cellular-level imaging; however, robust evidence is lacking. This trial aims to assess the diagnostic yield and safety of nCLE-assisted UGBx for u

Examination of Salmonella prevalence and slaughter findings in pigs based on rye feeding, coarser feed structure and vaccination against Lawsonia intracellularis under field conditions.

Salmonella remains a food safety concern through pork. This field study assessed the impact of high rye diets and Lawsonia intracellularis (LI) vaccination on Salmonella seroprevalence and pathological findings at slaughter in fattening pigs. Six commercial farms participated in a three-phase trial with varying rye levels and optional immunoprophylaxis against LI. From Phase 1 to 3, rye content increased progressively (pre-, mid-, finishing phase). In Phase 1 (P1), the proportion increased from 5% to 20% to 40%. Phase 2 (P2) showed a higher inclusion level, with ≥30 to ≥40 to 60-70%. In Phase

Case Report: Pembrolizumab unmasks a capillary leak syndrome with chylothorax in early TNBC.

Pembrolizumab has become standard of care in early-stage triple-negative breast cancer (TNBC), but its expanding use may induce rare adverse events involving vascular and lymphatic endothelium. Capillary leak syndrome (CLS) remains an exceptionally rare toxicity of PD-1 blockade, scarcely reported in breast cancer, especially in the adjuvant setting. We report the case of a 40-year-old female with early-stage TNBC who presented with progressive dyspnea, diffuse subcutaneous edema and bilateral pleural and peritoneal effusions after two cycles of adjuvant pembrolizumab. Imaging confirmed polyse

Optimal Number of Pleural Biopsies During Pleuroscopy (OPTIBIOP).

BACKGROUND: There is currently no evidence-based standard for the number of pleural biopsies required during pleuroscopy to achieve diagnostic adequacy. This study evaluated the cumulative diagnostic yield of sequential pleural biopsy aliquots obtained during rigid pleuroscopy. METHODS: We conducted a prospective, single-center observational pilot study of adult patients undergoing pleuroscopy. Biopsy specimens were obtained according to routine practice and submitted in sequential aliquots: aliquot 1 (biopsies 1 to 3), aliquot 2 (4 to 6), aliquot 3 (7 to 9), and aliquot 4 (≥10). Retrospective Review Is the Best View: A Primer for Residents and Radiologists on Missed Findings in Chest Radiography.

Chest radiography (CXR) remains the most frequently performed imaging modality worldwide and serves as the first-line investigation for cardiopulmonary conditions. Despite its widespread use, interpretation is limited by the two-dimensional representation of three-dimensional anatomy, resulting in structural overlap and reduced lesion conspicuity. This review provides a structured, pattern-based approach to commonly overlooked regions on CXR, emphasizing key anatomical blind spots including the lung apices, pleura, trachea, lung bases, hila, mediastinum, retrocardiac region, thoracic skeleton,

[Bilateral Thoracoscopic Surgery for Bilateral Pulmonary Arteriovenous Malformations:Report of a Case].

Pulmonary arteriovenous malformation (PAVM) is a congenital vascular anomaly that forms a shunt between the pulmonary artery and vein. It is frequently detected incidentally in asymptomatic patients. We encountered a 69-year-old female patient with bilateral PAVMs that were incidentally detected on abdominal computed tomography( CT) performed for a detailed examination of appendicitis. Abdominal CT revealed nodular opacities with contrast enhancement in the middle lobe of the right lung and the lingular segment of the left lung. They were further examined, and a diagnosis of PAVMs measuring 14

Population-Based Epidemiology of Congenital Pulmonary Malformations in Early Infancy: A Nationwide Study From Colombia.

BACKGROUND: Structural congenital pulmonary malformations encompass a heterogeneous group of developmental anomalies of the lung, pleura, and mediastinum that may cause significant neonatal morbidity and require specialized care. Population-based healthcare utilization data from middle-income countries remain scarce, limiting health system planning and comparative research. METHODS: We conducted a nationwide, retrospective, population-based study using Colombia's administrative healthcare registry (RIPS) to estimate the encounter-based prevalence, sex-specific distribution, age-standardized r

Cardiovascular outcomes with tirzepatide versus semaglutide in type 2 diabetes.

Preterm birth is associated with the majority of all death and chronic disability related to pregnancy, birth and the neonatal period. The costs to families and to the health care system are enormous. Current approaches to prevent or arrest preterm labour have been unsuccessful. This failure is largely based on our poor understanding of the regulation of the timing and maintenance of parturition. Oxytocin (OT) is the most potent known uterine stimulant. It is produced in the hypothalamus and secreted into the maternal bloodstream. However, OT also is produced within the uterine decidua in late

Diagnosis of malignant pleural effusion using thoracoscopy combined with confocal endomicroscopy: a case report.

Thoracoscopy is widely used for the diagnosis and management of pleural diseases. This technique is valuable because it enables direct visualization of the pleural cavity surface and pathologic biopsy sampling of lesions. However, it is limited in detecting early-stage pleural lesions, carrying a considerable risk of misdiagnosis or missed diagnosis, which may delay treatment and ultimately compromise patient poutcome. Confocal laser endomicroscopy (CLE) is an emerging in vivo microscopy technique that can be integrated with traditional thoracoscopy platforms, providing real-time in vivo obser

Advanced physical simulator for pediatric minimally invasive thoracoscopy training in the treatment of pulmonary sequestration.

Lung sequestration is a rare congenital anomaly characterized by an abnormal mass of lung tissue. It can occur in two forms, either intralobar, where the mass is embedded within the lung tissue, or extralobar, where it develops outside the lung's boundaries. In both cases, removal of this defect involves a complex minimally invasive procedure during which, in brief, the sequestration is isolated and removed. The surgical removal of the pulmonary sequestration represents a complex challenge for surgeons who have to deal with many critical aspects, such as the heartbeat-induced movement of the l

Thoracic ultrasound in hemodialysis: Assessing fluid overload and dialysis efficacy.

Fluid overload in patients with end-stage kidney disease (ESKD) usually results in pleural effusions (PEs). While volume overload remains the primary etiology of PE, other possible causes must be ruled out. Thoracic ultrasonography (TUS) is a practical and efficient method for evaluating volume in patients undergoing hemodialysis (HD), although evidence regarding its usefulness in this context is limited. We aimed to investigate the relationship between pleuropulmonary TUS patterns and overload symptoms and their correlation with optimal HD, as measured by total urea clearance (Kt/V) and the u

Congenital Pleuro-Pleural Communication Presenting as a Simultaneous Bilateral Spontaneous Pneumothorax: A Case Report.

Bilateral spontaneous pneumothorax is a rare condition that accounts for approximately 1% of all pneumothorax cases. Most cases are attributed to underlying lung disease or prior thoracic surgery, as congenital pleuro-pleural communication is rare. We report the case of a 15-year-old girl with bilateral pneumothorax who underwent video-assisted thoracoscopic surgery. Intraoperative air leak testing unexpectedly revealed congenital pleuro-pleural communication. A retrospective review of preoperative computed tomography suggested a potential bilateral pleural communication between the esophagus

Comparison of Lung Ultrasound Findings in Patients with Pulmonary Tuberculosis and Lobar Pneumonia: A Case-control Study.

BACKGROUND: The utility of lung ultrasound (LUS) in diagnosing respiratory disorders is being studied only in recent times. We aimed to describe the ultrasound (USG) features of pulmonary tuberculosis (TB) and compare them with those of lobar pneumonia. In addition, the LUS findings of both diseases were corroborated with chest X-ray findings. METHODS: The study subjects consisted of adult subjects recently diagnosed with pulmonary TB and those diagnosed with lobar pneumonia. Both subsets of patients underwent LUS evaluation. RESULTS: Ninety-six subjects with 64 microbiologically confirmed T

Prognostic value of pulmonary vessel-related structures in rapid progression of idiopathic inflammatory myopathy-associated interstitial lung disease: a retrospective study from two centres.

BACKGROUND: Pulmonary vessel-related structural (PVRS) abnormalities have been implicated in usual interstitial pneumonia; however, their significance in idiopathic inflammatory myopathy-associated interstitial lung disease (IIM-ILD) remains unclear. In this two-centre study, we evaluated the associations between PVRS parameters on high-resolution computed tomography (HRCT) and rapid progression and prognosis in patients with IIM-ILD. METHODS: A total of 578 IIM-ILD patients (412 females; median age, 53 years) were included from retrospective ILD cohorts of two centres. Artificial intelligenc
