Bronchopulmonary and neurenteric forms of foregut anomalies. Imaging for diagnosis and management.

Bronchopulmonary foregut malformations encompass a great variety of anomalies that may arise from abnormal differentiation of the respiratory and alimentary tracts, abnormal separation of the two systems, or abnormal development of blood supply, perhaps singly or in combination, during early embryogenesis. The pulmonary and neurenteric forms share the common features of a pulmonary parenchymal opacification and/or intrathoracic mass, with the addition of vertebral anomalies in the instance of the neurenteric malformations. Plain radiographs serve as the starting point for diagnostic evaluation

Congenital adenomatoid malformation of the lung: current management and prognosis.

A retrospective 10-year review of congenital adenomatoid malformation (CAM) included 10 cases diagnosed in utero by ultrasound and 13 cases that presented postnatally. Two prenatally diagnosed cases were aborted because of associated lethal anomalies. All remaining patients underwent resection. Up to one third of all cases, whether diagnosed prenatally or postnatally, were asymptomatic. Resection is recommended to avoid respiratory distress, infection, or associated malignancy. There were 5 nonsurvivors, including 2 therapeutic abortions and 3 who died postoperatively. All had either polyhydra

[Interstitial pulmonary emphysema. Combined therapeutic approach in a retrospective multidisciplinary study].

Pulmonary interstitial emphysema (PIE) may occur spontaneously, as a complication of endotracheal tube displacement, intrauterine viral pneumonia or massive aspiration of formula. Nevertheless PIE occurs more frequently in neonates requiring mechanical ventilation for RDS. Untoward effects of large air collections in the extra-alveolar spaces are based on decreased perfusion and ventilation of the affected lung tissues, compression of adjacent pulmonary parenchyma and mediastinum, possible air embolism. Sometimes PIE spontaneously regress, but in some instances the process is self perpetuating

The retrotracheal triangle.

The retrotracheal triangle is an area in the posterior superior mediastinum where abnormal structures may be detected in as many as 2% of chest radiographs. The authors describe the anatomy of this space and the abnormalities found therein, including vascular abnormalities (aberrant right subclavian artery, right aortic arch, double aortic arch, true and false aortic aneurysms), thoracic duct lesions, esophageal abnormalities (benign and malignant tumors, congenital atresia, duplication cysts, foreign bodies, diverticula, and achalasia), and miscellaneous entities (intrathoracic thyroid, bronc

Mediastinal cysts and intrathoracic thyroid tumors.

Benign cysts of the mediastinum are thought to be predominantly congenital in origin. Hence, a thorough understanding of their clinicopathologic characteristics is facilitated by knowledge of the embryologic development of the intrathoracic organs, as outlined in this review. Pertinent radiological and pathologic features of these lesions are also discussed. In addition, ectopic thyroid tissue may give rise to mediastinal masses. The salient attributes of these tumors are considered as well.

[Surgical treatment of mediastinal bronchogenic cyst in the adult: a report of 10 cases].

The mediastinal bronchogenic cyst is one of rare congenital foregut anomalies, with low frequency and usually asymptomatic. When secondary infection develops, it rapidly increases in size and may compress the respiratory system, and cause acute respiratory distress, in that condition, emergency surgical intervention will be necessary. After operation which includes a complete cystectomy or a partial resection of the cyst, the prognosis is good. It is difficult to differentiate the bronchogenic cyst from the esophageal cyst on the basis of clinical examination. Correct diagnosis depends on the

[Eight cases of thymic cyst].

Eight cases of thymic cyst were treated in our hospital from Jan. 1967 to Oct. 1989, which were 5.6% of all mediastinal tumors and 35% of congenital cysts. All cases were discovered incidentally by routine chest roentgenograms. All but one had no respiratory symptoms. There were no associated autoimmunological disorders such as myasthenia gravis. These tumors showed water density in computed tomography, low intensity area in T1 and high intensity area in T2 weighted magnetic resonance image. Garium scans were negative in all cases. Operations were performed in all cases for their diagnosis and

[Congenital cysts of the esophageal wall with a respiratory type mucosa].

Eight patients with congenital esophageal cysts are described. They presented with respiratory or gastrointestinal symptoms or an abnormal mediastinal profile on a standard chest film. All cysts were contained in the middle or inferior portions of the thoracic esophagus. They were excised in a submucosal plane. Their coat was composed of a ciliated, respiratory type epithelium. In the embryo and after separation of the tracheal diverticulum, the esophagus is lined with ciliated cells which are able cover a "cystic duplication", which is surrounded by an esophageal muscular coat or can be "incl

[A case of adenocarcinoma of the thymus].

A case of adenocarcinoma of the thymus is reported in this paper. To the best of our knowledge, this is the first case of thymic adenocarcinoma in the world literature. A 51-year-old female was admitted to our hospital because of hyperthyroidism and anterior mediastinal tumor. After control of hyperthyroidism, thoracotomy was performed and the tumor was located in the right lobe of the thymus macroscopically. The tumor was extirpated and the specimen was revealed to be multiple thymic cyst with papillo-tubular adenocarcinoma proliferating in one of the cysts. The variability of the lining cell

Pericardial disease: current diagnosis and management methods.

Diseases of the pericardium can be indolent or have a sudden onset. They may be primary, but more often are secondary to a systemic disease or previous therapy. Pericardial diseases imitate more common cardiac diseases and therefore can be difficult to diagnose. This paper's four case presentations illustrate use of recently developed methods that facilitate diagnosis. Magnetic resonance imaging is useful for the diagnosis of congenital absence of the pericardium. Echocardiography and computed tomography are useful for the diagnosis of pericardial cysts. Diagnosis of cardiac tamponade is aided

[Current status of surgery for mediastinal tumors].

Impact of surgery on the result of mediastinal tumors is variable: In benign tumors such as teratoma, congenital cyst, intrathoracic struma and neurogenic tumor, a complete resection causes a good result. Simultaneous thoracotomy and laminectomy should be performed in the Dumbbell type of neurogenic tumor. Since complete resection of the invasive thymoma promises a good prognosis, excision of all the tumor and invading tissues followed by radiation therapy is recommended in the treatment of thymoma. In pure seminoma resection and radiation therapy is the treatment of choice, while in non-semin

[Cystic duplications of the esophagus in adults. Report of 2 cases].

Oesophageal duplications account for less than 10% of the benign tumors of the oesophagus and for 20% of intestinal duplications. They are most often seen during childhood. In the adult it is more often the non communicating cystic forms, which are prone to complication, which have been reported. Cervical localization is rare and up to now no case has been described in the adult. We report here an asymptomatic case. Our second patient, who had a duplication in the mediastinal region, presented with a cough and dysphagia which completely regressed after excision.

Mediastinal cysts: diagnosis by CT and needle aspiration.

Five patients with bronchogenic or enteric cysts were diagnosed by using a combination of CT and transbronchial or transesophageal needle aspiration. Serous or mucoid fluid was successfully aspirated in each case. Cultures of the fluid were negative for pathogens, and cytologic examinations showed no malignancy. Mediastinoscopy and thoracotomy were avoided in all patients, including one symptomatic patient whose symptoms resolved after needle aspiration. CT with transbronchial or transesophageal needle aspiration represents a practical alternative to surgery in the evaluation of congenital med

Surgical management of thoracic anomalies in infants. Respiratory-tract malformations, congenital chylothorax and mediastinal masses.

The clinical presentation and surgical treatment of thoracic anomalies--developmental malformations of the respiratory tract, congenital chylothorax or mediastinal masses--in 15 infants are reported. The age range at operation was 2 weeks to 8 months. The diagnoses were lobar emphysema (3 cases), bronchogenic cyst (3), cystic adenomatoid malformation (1), enteric duplication (2), hyperplastic thymus (2), neuroblastoma (1), chylothorax (1), cystic lymphangiectasia (1) and tracheal stenosis (1). The most common symptom was respiratory embarrassment, with acute development in half of the cases. T

[Pulmonary and mediastinal bronchogenic cysts].

Bronchogenic cysts are relatively rare anomalies with a congenital origin resulting in an abnormal branching of the tracheo-bronchial tree. They either develop in the pulmonary parenchyma or in the mediastinal region. Between 1975 and 1987, 21 cases of bronchogenic cysts were operated upon. Eighteen of these cysts had a pulmonary localisation and 3 were mediastinal. Only 8 of these were diagnosed pre-operatively by bronchoscopy. The major reason for delay in diagnosis is persistent or recurrent pulmonary infection. Eight cases were treated by cystectomy, 4 by simple wedge resection, 4 by segme
