Concurrent development of a thymic cyst and mediastinal Hodgkin's disease.

A case is presented of a young adult female who presented with nodular sclerosing Hodgkin's disease and a mediastinal mass that proved to be a thymic cyst. Prior to this illness, a chest x-ray had been obtained that did not show a mediastinal mass. This case illustrates that thymic cysts may arise simultaneously with mediastinal Hodgkin's disease, rather than as a result of treatment or being present congenitally.

Mediastinal thymic cyst.

A symptomatic anterior mediastinal mass in a 44-year-old woman was found to be a benign thymic cyst. Complete excision was made by anterolateral thoracotomy. Congenital cysts are benign but differentiation from malignant cystic degeneration should be made.

Neurenteric cysts--a spectrum.

This review encompasses seven patients with clinically important cystic lesions of the gastrointestinal (GI) tract, exhibiting a wide range of vertebral anomalies and connections to the neural canal. Three patients had mediastinal masses connected to lower cervical and upper thoracic anomalous vertebrae with intraspinal extensions. In addition, one of these patients had a separate, juxtapancreatic intestinal duplication cyst. One infant with colonic duplication had a lumbar vertebral anomaly and an epithelial-lined tract between the two. Another patient had a presacral cystic mass which was th

Fine needle aspiration biopsy of peripheral congenital bronchial cyst.

Bronchial cysts usually occur centrally near the mediastinum, but may present as a peripheral "coin" lesion requiring distinction from other causes of coin lesions of the lung. A case is reported in which fine needle aspiration (FNA) biopsy was used to make the diagnosis of such a peripheral bronchial cyst. FNA produced an opaque opalescent fluid; cytologic study of the smears showed abundant bipolar birefringent needle-shaped crystals an a few macrophages. Neither special staining of some smears nor cultures of part of the aspirate revealed the presence of any organism. The cytologic diagnosi

[Thymus surgical pathology in children].

The object of this paper is to briefly review the surgical aspects of the thymic pathology. Thymic cysts and thymic ectopy are usually asymptomatic and are one of the multiple etiologies of cervical masses. Thymic tumors are very rare and may sometimes be difficult to differentiate from simple thymic hyperplasia; being almost always separated from the normal thymic tissue by a dense capsule, they do not tend to extend rapidly. Thymectomy may be indicated in severe myasthenia.

Neurenteric cyst diagnosed by technetium-99m pertechnetate sequential scintigraphy.

Neurenteric cysts are rare congenital anomalies which present as mediastinal tumors associated with vertebra anomalies. Two-thirds of them are lined with gastric mucosa and are potentially life threatening. An exact differential diagnosis is difficult preoperatively but is absolutely necessary because of the grave prognosis if left untreated. We present a case in which [99mTc]pertechnetate sequential scintigraphy demonstrated gastric mucosa in the cyst and helped to confirm the diagnosis. The scintigraphic findings are correlated with radiologic, sonographic, and pathologic features.

Mediastinal carinal bronchogenic cyst: is its mere presence an indication for surgical excision?

A notion has prevailed that carinal bronchogenic cyst and other congenital mediastinal cystic lesions, particularly those occurring in older children and adults, are usually asymptomatic, innocuous, and frequently only an incidental finding on routine chest roentgenogram or postmortem examination. Some physicians, therefore, have adopted a policy of observation for these patients. Our experience, however, as demonstrated in three cases reported herein, and the experience of others, clearly shows that carinal bronchogenic cyst is far from being usually asymptomatic and innocuous, but in fact of

Imaging of foregut duplication cysts.

There are many types of diagnostic imaging studies available for the evaluation of foregut duplication cysts. Esophagography may be helpful in confirming the presence of a mediastinal mass, explaining esophageal symptoms, or defining the location of a lesion, but it is rarely diagnostic. Digital subtraction angiography can successfully be used in place of pulmonary arteriography to define vascular anatomy. Computed tomography is often diagnostic, although the attenuation number may be slightly elevated and not that of water. Computed tomography will clarify the relationship of the mass to adja

[Rare cervicofacial cysts and fistulas in children].

Among the various congenital cervicofacial swellings observed in children, three types are rarely encountered in general practice: auriculobranchial cysts and fistulae, thymic cysts and cystic lymphangiomas. To resolve diagnostic and therapeutic problems related to these rare lesions it is essential that their etiopathogenic bases be understood. Auriculobranchial cysts and fistulae present clinical symptoms (cervical, parotid, auricular) that are only poorly evidenced and are related mainly to infection. Surgical excision requires a wide approach route of the parotidectomy type with preliminar

Bronchial atresia associated with a bronchogenic cyst. Evidence of early appearance of atretic segments.

We report a case of congenital lobar emphysema in an adult due to bronchial atresia and presenting characteristically with a solitary pulmonary nodule due to a mucous plug. In the same patient the presence of a bronchogenic cyst leads to a hypothesis that the atretic segment is the result of an insult occurring during the 5th or 6th week of intrauterine life rather than one occurring after airway development is complete.

Bronchogenic cysts in children.

Radiographic, surgical and pathological features of 33 paediatric patients with bronchogenic cysts are reviewed. These congenital lesions usually presented as spheroid mediastinal masses, near the carina or right paratracheal area, 2-3 cm in diameter with sharply delineated borders. Occasionally, the masses were lobulated or triangular in shape. Some cysts produced considerable airway distortion. All intrapulmonary cysts had connections, sometimes patent, with the trachea or main-stem bronchi. Thin-walled, completely aerated cysts became thick-walled with infection. A few air-filled and solid

A case report of a congenital bronchogenic cyst in the upper mediastinum on the level of the thoracic aperture, which very likely communicates with the oesophagus, is presented. The embryogenesis, aetiopathogeny, diagnostics, location and histological structure of this rare case are discussed.

[Congenital cervical thymic cyst].

Congenital cervical thymic cysts are a very rare cause of tumor of the neck in children. They can exceptionally induce a laryngeal compression. Actual identification of the tumor may sometimes be made only after surgery and histologic examination. Evolution is benign. Their existence seems to be related to the persistence of thymic remnants in the neck.
