The Value of Nasal Nitric Oxide Measurement in the Diagnosis of Primary Ciliary Dyskinesia.

Nasal nitric oxide (nNO) is widely used as a first-line test in the diagnostic evaluation of primary ciliary dyskinesia (PCD). Although low nNO is characteristic of classic PCD forms, its diagnostic performance varies across age groups, measurement techniques, and genetic subtypes. This review summarizes current evidence on the diagnostic accuracy of nNO and its evolving position within PCD guidelines. In recent prospective referral cohorts using comprehensive diagnostic work-ups, pooled sensitivity and specificity remain high, though slightly lower than in earlier, more selected cohorts. Prev

Could a Bronchogenic Cyst Present as an I-131-avid, 99m Tc-Non-Avid Lesion and Mimic Metastatic Thyroid Cancer?

A 54-year-old man with papillary thyroid carcinoma after thyroidectomy underwent a 99m Tc thyroid scan showing no thyroid remnants but a 3.5-cm left lung lesion with faint 99m Tc uptake on SPECT/CT, suggesting metastasis. Post-I-131 therapy scans on days 3 and 7 revealed intense I-131 avidity in the lung lesion. Surgical excision confirmed a benign bronchogenic cyst, negative for thyroglobulin, TTF-1, BRAF V600E, and PAX-8. I-131 uptake was likely NIS-mediated, but independent of PAX-8 or due to mucinous content. This case underscores that bronchogenic cysts should be considered in the differe

Intra-atrial Baffling of an Infradiaphragmatic Scimitar Vein without Circulatory Arrest.

Scimitar syndrome is a rare congenital anomaly characterized by anomalous right pulmonary venous drainage into the inferior vena cava, resulting in a left-to-right shunt. Surgical correction may be achieved by direct reimplantation of the scimitar vein or by intra-atrial baffling, which often requires deep hypothermic circulatory arrest. We report the case of a 47-year-old man with symptomatic scimitar syndrome and an infradiaphragmatic scimitar vein who underwent successful intra-atrial baffling without circulatory arrest. Preoperative imaging demonstrated a significant shunt with dilation of

Clinical immunology in chromatinopathies: a scoping review.

BACKGROUND: Chromatinopathies (CP) are a subclass of monogenic neurodevelopmental disorders caused by germline mutations in genes involved in epigenetic regulation. Symptoms may include neurodevelopmental delay, intellectual disability, autism spectrum disorder, facial dysmorphisms, growth abnormalities, and various congenital anomalies. These disorders often also present with immunopathology. However, a clear insight into immune dysregulation in CPs is currently unavailable. OBJECTIVE: This scoping review aims to synthesize clinical data on anomalies of the immune system, including laborator

Lung ultrasound in children with primary ciliary dyskinesia: is it able to detect structural and functional impairments?

UNLABELLED: Primary ciliary dyskinesia (PCD) is a chronic disorder of mucociliary clearance that leads to recurrent respiratory infections and progressive structural lung disease. Lung ultrasound (LUS) is a radiation-free bedside imaging modality; however, its use in PCD is limited, and no validated scoring system is available. This study aimed to evaluate LUS findings in children with PCD; assess the relationship between LUS aeration score and clinical characteristics, pulmonary function parameters, and chest radiograph severity; and to describe ultrasound-detected atelectatic findings. This

Delayed surgical management of extensive bilateral multilobar congenital pulmonary airway malformation.

Management of bilateral congenital pulmonary airway malformations (CPAM) distributing multiple lobes is challenging since it requires complicated surgical decision-making regarding when and how much abnormal lung parenchyma should be resected with minimal risk of postsurgical respiratory failure. We present a case of antenatally diagnosed bilateral CPAM distributed almost to the entire lungs, which had a favourable outcome with optimised conservative and surgical management. The management was started with maximal ventilatory support, and when and how much of the lesions should be resected was

Pulmonary sequestration in a septuagenarian: a rare congenital lung lesion diagnosed in adulthood.

INTRODUCTION: Pulmonary sequestration (PS) is a rare congenital pulmonary malformation characterized by non-functioning lung tissue that lacks normal communication with the tracheobronchial tree and receives systemic arterial blood supply. While it is often diagnosed in childhood due to recurrent infections, incidental detection in asymptomatic adults is uncommon and can pose a diagnostic challenge. CASE DETAILS: We report the case of a 70-year-old female who was incidentally found to have a left lower lobe mass on routine imaging. Initial CT revealed a 3.2 cm triangular mass in the posteromed

The Impact of Nutritional Status on Lung Function Trajectories in Pediatric Patients With Primary Ciliary Dyskinesia.

RATIONALE: Primary ciliary dyskinesia (PCD) is a phenotypically diverse disease; the impact of nutritional status on clinical trajectories in patients with PCD is not well defined. The aim of this study is to examine the impact of malnutrition on lung function in a pediatric cohort of patients with PCD. METHODS: This was a retrospective cohort study of pediatric patients with a confirmed diagnosis of PCD at the Hospital for Sick Children in Toronto, Canada between 2000 and 2022. Undernutrition was defined as a BMI- or weight-for-age z-score < -1. Mixed effects linear regression models were us

Mathematical reappraisal of the congenital cystic adenomatoid malformation volume ratio.

OBJECTIVE: The CCAM (CPAM) volume ratio (CVR), an ultrasound index to predict hydrops or neonatal demise in fetuses affected with CPAM, is presumed to be independent of gestational age. A CVR of 1.6 has been proposed as predictive of hydrops. The purpose of our study was to assess the fundamental mathematical concept of the CVR. STUDY DESIGN: Using published formulas, the right fetal lung volume was used as a surrogate of a theoretical CPAM involving the entire right lung. The head circumference was estimated using the previously published formula by Chitty et al. The ratio of right lung volu

Severe Bicytopenia and Neutropenic Fever in a Patient With COVID-19 Pneumonia: A Diagnostic Challenge.

Coronavirus disease 2019 (COVID-19) is a multisystemic viral infection that primarily affects the respiratory system but is widely recognized for its diverse extrapulmonary manifestations. Hematologic abnormalities, especially those associated with inflammation and disrupted hemostasis, are prevalent and have been extensively reported. However, few cases of profound neutropenia and severe thrombocytopenia have been documented in the literature, making their occurrence extremely rare. Herein, we present a rare case of a 60-year-old woman who presented with a two-week history of painful oral ulc

Indocyanine green-guided thoracoscopic wedge resection for pulmonary sequestration.

This video tutorial demonstrates a right-sided video-assisted thoracoscopic surgery approach for the management of an intralobar pulmonary sequestration in an adult. Although conservative or endovascular approaches exist, surgical resection is recommended to prevent recurrent infections and mitigate the risk of malignant transformation. While anatomical lobectomy has traditionally been the standard, lung-sparing techniques are increasingly preferred to preserve pulmonary function. The procedure emphasizes the isolation and division of the aberrant systemic arterial supply arising from the desc
