Long-term outcomes after operative versus conservative management of congenital thoracic malformations: a propensity-matched cohort study.

PURPOSE: Management of asymptomatic congenital thoracic malformations (CTMs) remains controversial. Early resection may prevent infection or malignancy and promote compensatory lung growth, but its long-term impact is unclear. This study compared long-term cardiorespiratory and psychosocial outcomes after operative versus conservative management of CTMs and evaluated open versus thoracoscopic surgery. METHODS: Children with congenital pulmonary airway malformation, pulmonary sequestration, or congenital lobar emphysema treated at a tertiary center between 2000 and 2023 were identified retrosp

Enhancing genetic diagnosis of primary ciliary dyskinesia by copy number variants analysis.

BACKGROUND: Primary Ciliary Dyskinesia (PCD) is a rare, genetically heterogeneous disorder. Despite the increasing number of identified causative genes, 20-30% of patients still lack a genetic diagnosis. To date, genetic analyses have primarily focused on single nucleotide variants (SNVs). In this study, we evaluated whether the detection of copy number variants (CNVs) improves the diagnostic yield in our large patient cohort. METHODS: We retrospectively evaluated patients with confirmed or suspected PCD who remained genetically unresolved after next-generation sequencing (NGS). CNV analysis

Advanced Imaging of the Respiratory System.

Respiratory imaging in dogs and cats integrates multiple modalities to noninvasively evaluate thoracic disease. Radiography is the first-line tool but has limitations in assessing dynamic and subtle abnormalities. Advanced imaging such as fluoroscopy, ultrasonography, and computed tomography (CT), enhances characterization of airway, parenchyma, and pleural disorders. CT provides high-resolution cross-sectional detail for lesions of increased or decreased attenuation, nodules, masses, airway collapse, and linear patterns, although definitive diagnosis often requires cytology or histopathology.

Advanced Imaging to Assess Airway Dynamics.

Endoscopy remains the gold standard for assessing dynamic airway abnormalities in pediatric patients, but its invasiveness and variable interoperator agreement have driven interest in imaging-based alternatives. Four-dimensional computed tomography (CT) now enables objective quantification of tracheal collapse during free breathing, while photon-counting CT achieves effective doses approaching chest radiography, fundamentally altering the risk-benefit calculus for serial monitoring. MRI approaches-from real-time cine for obstructive sleep apnea localization to retrospectively gated techniques

Severe Necrotizing Pneumonia as an Unusual Initial Manifestation of Neurofibromatosis Type 1: A Case Presentation.

Neurofibromatosis type 1 (NF1), also known as von Recklinghausen disease, is a relatively common inherited neurogenetic disorder that predisposes individuals to various benign and malignant tumors, primarily in the nervous system. Diagnosis depends on clinical manifestations; however, the clinical presentation of NF1 is highly variable, and complications of NF1 may show great heterogeneity. In this manuscript, we report the case of a 45-year-old male patient who presented with frequent episodes of fever (up to 39°C) and productive cough initially non-responsive to standard-of-care medical trea

Function and Structure Relationships With Inflammation Differ in Two Chronic Suppurative Lung Diseases.

RATIONALE: Cystic fibrosis (CF) and primary ciliary dyskinesia (PCD) are characterized by neutrophilic airway inflammation but differ in clinical features. OBJECTIVES: We investigated relationships of pulmonary and systemic inflammatory markers with functional and structural lung disease. METHODS: Systemic (CRP, IgG, IL-6, and IL-8) and sputum (calprotectin, IL-6, and IL-8) markers were measured at baseline. Relationships were explored with spirometry, lung clearance index and lung computed tomography (CT) scores at baseline and with spirometry 10 years later. RESULTS: 42 patients (21 CF, 2

Low-Dose Radiation Sensitizes Human Nasal Epithelium to Viral Inflammation despite Preserved Structural Integrity.

INTRODUCTION: Patients receiving radiotherapy for nasopharyngeal carcinoma often experience prolonged nasal epithelial dysfunction and recurrent infections. However, the dose-dependent effects of radiation on nasal epithelial integrity and antiviral immune function remain incompletely defined. METHODS: Human nasal epithelial cells (hNECs) were differentiated at an air-liquid interface (ALI) and exposed to gamma irradiation (0, 1, 2, 4, 8, or 16 Gy) on ALI day 0 to define dose-dependent epithelial injury. DNA damage, epithelial cell number, and proliferative capacity were assessed after irradi

Variants in the DNAH11 gene responsible for primary ciliary dyskinesia or probably atypical primary ciliary dyskinesia presenting left-right asymmetry disorder.

Primary ciliary dyskinesia (PCD) is a rare multi-system cilia-related disorder, and approximately 50% of individuals with PCD exhibit left-right asymmetry disorder. The dynein axonemal heavy chain 11 gene (DNAH11) pathogenic variants are responsible for primary ciliary dyskinesia 7, with or without left-right asymmetry disorder. This study aimed to detect the pathogenic variants in three unrelated patients diagnosed with PCD or left-right asymmetry disorder based on the clinical and imaging examinations. Whole exome sequencing, Sanger sequencing, and comprehensive bioinformatics analyses were

Monitoring the Adverse Implications of Coronavirus 2019-induced Pulmonary Complications on Patients' Respiratory Capacity and Physical Abilities with Moderate and Severe Signs during Interval Follow-up.

BACKGROUND: The respiratory system of individuals with coronavirus 2019 (COVID-19) experiences significant strain due to the body's immunological response and inflammation, leading to organ failure. Over time, patients have improvement in symptoms such as radiological abnormalities, pulmonary function impairment, and decline in respiratory and physical abilities. This research aimed to assess the impact of COVID-19 on the pulmonary function and physiological performance capacity of patients with moderate and severe problems who were hospitalized and subsequently released. These parameters were

Intralobar pulmonary sequestration and anomalous systemic arterial supply to the basal segment of the lung: A comparative study of clinical features and 3D imaging.

BACKGROUND: Intralobar pulmonary sequestration (IPS) and anomalous systemic arterial supply to the basal segment of the lung (ABL) are rare congenital anomalies in which a portion of the lung receives its blood supply from a systemic artery. IPS has abnormal bronchial and alveolar structures, whereas ABL does not. We compared the clinical characteristics and management of IPS and ABL, referencing the role of three-dimensional (3D) bronchial imaging. METHODS: We retrospectively reviewed patients diagnosed with IPS or ABL at two Japanese hospitals between January 2007 and September 2024. We col

Potential Risk Factors for Total Anomalous Pulmonary Venous Return: National Birth Defects Prevention Study, 1997-2011 and the Birth Defects Study to Evaluate Pregnancy Exposures, 2014-2021.

BACKGROUND: Total anomalous pulmonary venous return (TAPVR) is a rare heart defect in which the pulmonary veins do not connect to the left atrium, instead draining into the right atrium or directly into other venous systems. We explored factors associated with TAPVR using the National Birth Defects Prevention Study (NBDPS; 1997-2011) and Birth Defects Study To Evaluate Pregnancy exposureS (BD-STEPS; 2014-2021) data. METHODS: We identified potential risk or protective factors from the literature and used logistic regression to calculate crude and adjusted odds ratios (OR) and 95% confidence in

Early-life Gut Microbiota and Allergic Diseases: Immune Regulatory Mechanisms and Intervention Progress.

Over recent decades, the prevalence of allergic diseases has risen steadily, particularly among children, making these conditions a major global public health concern. Increasing evidence suggests that, in addition to genetic factors, gut microbiota dysbiosis is an important contributor to the development and progression of allergic diseases. The gut microbiota plays a critical role in host metabolism, maintenance of mucosal barrier integrity, and regulation of immune homeostasis, and is especially closely linked to immune system development and the establishment of immune tolerance in early l

Intra-atrial tunnel technique for hepatic-draining scimitar syndrome.

Scimitar syndrome is a rare congenital anomaly characterized by abnormal pulmonary venous drainage. This report describes the case of a 15-year-old girl with scimitar syndrome in which the scimitar vein drained into the hepatic vein. Surgical correction was successfully performed using an intra-atrial tunnel technique with right atrial anastomosis of the scimitar vein to reroute the scimitar vein to the left atrium. This case demonstrates the efficacy of this technique in managing complex cases of scimitar syndrome.
