The Stanford Orthodontic Airway Plate: Design and Clinical Implementation in 64 Consecutively Treated Infants With Pierre Robin Sequence.

Mandibular distraction osteogenesis (MDO) is the established standard of care in the United States for severe upper airway obstruction in infants with Pierre Robin Sequence (PRS). While airway plates have been utilized effectively in Europe for decades, this approach was not adopted in the United States until 2020, when an institution-specific protocol was developed to coexist with surgical paradigms. The current study details the design and clinical implementation of the institution's Orthodontic Airway Plate (OAP). By applying orthodontic Visual Treatment Objective principles to maxillofacia

Two Turkish families with type 1 Stickler syndrome carrying novel COL2A1 truncating variants: a case series.

PURPOSE: To describe the ocular and extraocular findings in two unrelated Turkish families with type 1 Stickler syndrome carrying two novel truncating COL2A1 variants and to emphasize the marked intrafamilial phenotypic variability. METHODS: Seven affected individuals from the two families underwent detailed ophthalmic examination including BCVA, slit-lamp biomicroscopy with vitreous phenotype assessment, fundus evaluation, and multimodal retinal imaging. Systemic evaluation included craniofacial, pediatric, orthopedic, and audiological assessments. Index cases underwent clinical exome sequen

"I have to be my child's voice": parents' experiences of continuity of care in cleft speech services.

PURPOSE: Continuity of care is critical for children with cleft palates who require coordinated speech-language pathology input across specialist cleft and community services over time. This study explored parents' experiences of continuity of care in cleft speech services, focusing on their interactions with the cleft team and community speech-language pathologists. METHOD: Thirteen parents of children with cleft palate (aged 1-12 years) from Australia and New Zealand participated in semi-structured interviews. RESULTS: Five global themes and 15 subthemes were developed. The global themes w

Prenatally Diagnosed 7q11.23 Copy Number Variations: A Retrospective Case Series.

BACKGROUND: Williams-Beuren syndrome (WBS; OMIM #194050), caused by 7q11.23 deletions, is well-characterized postnatally, but prenatal manifestations remain poorly defined. This study aims to delineate the prenatal phenotypes, inheritance patterns, and outcomes of 7q11.23 copy number variations (CNVs). METHODS: A retrospective study of 20 prenatal cases with 7q11.23 CNVs diagnosed by SNP array or CNV sequencing (CNV-seq) was conducted. Clinical data, including ultrasound findings, genetic results, and pregnancy outcomes, were analyzed. RESULTS: Classic 7q11.23 deletions (1.42 Mb median size)

[Research progress in regulatory variants in non-coding regions in non-syndromic cleft lip with or without cleft palate].

Non-syndromic cleft lip with or without cleft palate (NSCL/P) is one of the most common congenital craniofacial malformations, characterized by significant genetic heterogeneity and polygenic features. Historically, genetic studies of NSCL/P have primarily focused on coding region variants. However, only approximately 2% of the human genome consists of coding sequences, with the majority of variants located in non-coding regions. Recent large-scale genome-wide association studies have indicated that risk loci associated with NSCL/P are predominantly enriched in regulatory elements within non-c

Barriers and delays in access to cleft surgery among children with cleft deformities in Rwanda: a qualitative study.

OBJECTIVES: Although timely surgery is essential for improving general health and psychosocial outcomes, delays in cleft surgery remain common in low- and middle-income countries, including Rwanda, where little is known about the underlying causes. This study aimed to explore the factors influencing delays in cleft surgery in Rwanda using the four-delay framework. DESIGN: A phenomenological qualitative study was conducted between April and July 2024. We conducted 29 in-depth interviews with 15 caregivers of children with orofacial clefts and 14 healthcare providers involved in cleft care. Tra

Application of team-based learning to support knowledge acquisition and retention of craniofacial content in a graduate speech-language pathology course: A pilot study.

This study examined graduate student confidence, knowledge retention, and perceptions of a craniofacial disorders course delivered using team-based learning (TBL). Seventeen graduate students from a single class participated. Confidence was measured by survey pre-course, post-course, and 6 months later. Knowledge was assessed through exam scores, and retention was evaluated 7 months later using Cranio Q oral questions. Quantitative and qualitative analyses were conducted. Confidence increased from pre-course to post-course and remained elevated at 6 months. Confidence was weakly correlated wit

Complications of tympanostomy tubes in patients with cleft palate.

OBJECTIVES: The primary objective was to determine the prevalence of complications after tympanostomy tube insertion among patients with a cleft palate. The secondary objective was to assess whether patients registered to noninsured health benefits (NIHBs) (all of whom are Indigenous) experience different risk of these complications than patients not registered to NIHB. METHODS: This retrospective cohort study included paediatric patients (<18 years) treated at a tertiary otolaryngology referral centre serving a large population of Indigenous Peoples, including a subset registered to the NIHB

Out of the Laboratory and Into the Clinic: Out-of-Domain Validation of Machine Learning Models for Velopharyngeal Dysfunction Detection.

BACKGROUND: The diagnosis and management of velopharyngeal dysfunction (VPD) is a particularly challenging part of the cleft care timeline, particularly in resource-limited settings. Although machine learning (ML) models offer promising results as screening tools, their real-world clinical viability has yet to be reliably demonstrated. This study aims to systematically compare the capability of several ML models to detect VPD in nonstandardized conditions to simulate real-world clinical testing. METHODS: Eighty-two patients were enrolled under standardized acoustic conditions and partitioned

The Suggestive Association Between the NOG rs227731 Polymorphism and Non-Syndromic Cleft Lip With or Without Palate Subtypes in a Japanese Cohort.

Non-syndromic cleft lip with or without palate (NSCL/P) ranks among the most prevalent congenital craniofacial anomalies globally. In Japan, reported rates of cleft lip and/or palate range from 1 in 500 to 1 in 700 live births. This study aims to explore potential sex-specific interactions of NOG rs227731 across NSCL/P subtypes in a Japanese population. This case-control study included 420 participants recruited from the Cleft Lip and Palate Centre at Aichi Gakuin Dental Hospital in Nagoya, Japan. The study population was divided into three groups: 110 individuals with non-syndromic cleft lip

Combined Multi-Axis Tessier Clefts: Surgical Management and Early Functional Outcomes From a Tertiary Center.

BackgroundRare craniofacial clefts are among the most complex anomalies encountered in reconstructive surgery. Although isolated Tessier clefts are well described, patients with multiple independent craniofacial cleft axes remain exceptionally rare, and their anatomical patterns, staged reconstructive burden, and early functional outcomes remain incompletely characterized.Patients and MethodsA retrospective review was conducted of patients with combined craniofacial clefts treated at a tertiary craniofacial center between 2015 and 2024. Combined clefts were defined as 2 or more independent cra
