The "Dynamic Tongue Contraction Technique" for Diagnosis of Soft Palate Cleft in Cases of Cleft Lip and Palate Sequence.

OBJECTIVE: To present and evaluate the dynamic tongue contraction technique as a novel prenatal sonographic method for detecting soft palate clefts in fetuses with a cleft lip and palate (CLP) sequence. METHODS: This prospective cross-sectional study was conducted at a tertiary care center between September 2023 and September 2024. Seven fetuses with prenatally diagnosed CLP (16-27 weeks' gestation) and 28 control fetuses (14-32 weeks' gestation) underwent 2D ultrasound scans. Three experienced sonographers assessed soft palate integrity using a novel axial-oblique view at the tongue base dur

A prospective study of the medial cleft edge as a cause of soft palate shortening and a simple technique to restore length during palatoplasty.

INTRODUCTION AND OBJECTIVES: A thin rim of fibrotic thickening of the medial cleft edge with distinctive histopathological features is a constant finding in palatal clefts. We hypothesized that this scar-like tissue acts as a contracture band by tethering the soft palate and contributes to its anteroposterior shortening. We aimed to quantify the lengthening achieved by releasing it during palatoplasty. DESIGN: Prospective observational study. SETTING: Specialized cleft center in a tertiary care hospital in Southern India. PARTICIPANTS: Consecutive patients who underwent cleft palate repair

Comparisons of longitudinal speech outcomes in children born with cleft palate ± cleft lip with and without additional conditions at 5 and 10 years -a registry study.

BACKGROUND: Speech outcome differences in children with cleft palate ± cleft lip (CP ± L) with ( +) and without (-) additional conditions (i.e., diagnosed syndromes, additional malformations, and intellectual disabilities) are not fully explored. The Swedish cleft lip and palate (CLP) registry enables a longitudinal approach. Thus, the purpose was to compare longitudinal speech outcomes in children with different cleft types, with and without additional conditions, at ages 5 and 10 years, based on registry data. METHODS: Included were all children with oral clefts involving the palate born in

Prenatal diagnosis of micrognathia and postnatal identification of cleft soft palate: A case report.

This case report details the prenatal diagnosis of fetal micrognathia using advanced ultrasound techniques and highlights its role in anomaly detection and perinatal management. A 33-year-old primigravida presented for routine antenatal care at 30 weeks of gestation. Ultrasound revealed mandibular hypoplasia with an Inferior Facial Angle (IFA) of 46.2° (below the diagnostic threshold of 50°) and a reduced Jaw Index, confirming micrognathia. Three-dimensional ultrasound provided additional visualization of the retronasal triangle, where the absence of a normal mandibular gap further supported t

Evaluation of postoperative outcomes in palatoplasty with three soft palate cleft repairs.

The aim of this study was to compare the postoperative clinical and functional outcomes of palatoplasty with three soft palate cleft repairs and analyse the factors potentially impacting these outcomes. A retrospective analysis was conducted on a consecutive series of 337 patients who underwent primary cleft palate repair by palatoplasty modified with either Furlow Z-plasty (P-FZP, n = 77), intravelar veloplasty (P-IVV, n = 110), or combined intravelar veloplasty-Furlow Z-plasty (P-IVV-FZP, n = 150). The postoperative outcomes evaluated included wound healing (complete closure/fistula) and vel

Surgical treatment of velopharyngeal dysfunction: Incidence and associated factors in the Swedish cleft palate population.

INTRODUCTION: Speech in children with cleft palate can be affected by velopharyngeal dysfunction, which persists after primary palate repair. The incidence of surgery to correct velopharyngeal dysfunction in this patient group has previously been reported as 2.6-37%. We aimed to investigate the incidence of velopharyngeal dysfunction surgery in Swedish children with cleft palate and to examine potential associations of independent variables with this incidence. METHODS: In this cohort study, we analysed data from the Swedish cleft lip and palate quality registry for 1093 children with cleft p

Clinical application of endoscopic soft palate augmentation in the treatment of velopharyngeal insufficiency.

Velopharyngeal structure augmentation with the injection of autologous fat tissue into the nasal mucosa of the soft palate has been reported previously. However, as the injection points in the velopharyngeal space cannot be observed directly, these injections may be difficult to perform accurately. This report describes a new endoscope-assisted approach in which the materials for velopharyngeal structure augmentation are administered while observing the injection points directly, also enabling adjustment of the amount of material injected. A case series of five patients aged 8-16 years who und

Trio-based GWAS identifies novel associations and subtype-specific risk factors for cleft palate.

Orofacial clefts (OFCs) are the most common craniofacial birth defects and are often categorized into two etiologically distinct groups: cleft lip with or without cleft palate (CL/P) and isolated cleft palate (CP). CP is highly heritable, but there are still relatively few established genetic risk factors associated with its occurrence compared to CL/P. Historically, CP has been studied as a single phenotype despite manifesting across a spectrum of defects involving the hard and/or soft palate. We performed GWAS using transmission disequilibrium tests using 435 case-parent trios to evaluate br

The Utility of the Ultrasound "Superimposed-Line" Sign at the Junction of the Vomer and Maxilla in First-Trimester Screening for Fetal Cleft Palate: A Case-Control Study.

INTRODUCTION: The current retrospective case-control study evaluates the diagnostic value of screening for a fetal cleft palate by using the ultra-sound-based observation of the "superimposed-line" sign appearing at the junction of the vomer and maxilla in the first trimester of pregnancy. METHODS: Retrospective analyses were performed of ultrasonographic images of nuchal translucency obtained during the first trimester of pregnancy (11-13+6 weeks) from 45 fetuses with a cleft palate later confirmed following parturition or induced labor (cases) and 4,500 normal fetuses confirmed after partur

Antimicrobial Peptides and Interleukins in Cleft Soft Palate.

Cleft palate is one of the most common and well-studied congenital anomalies; however, the role of protective tissue factors in its pathophysiology is still debated. The aim of our study was to evaluate interleukin and antimicrobial peptide appearance and distribution in cleft palate. Eight soft palate samples were obtained during veloplasty procedures. Immunohistochemical staining was applied to detect HBD-2-, HBD-3-, HBD-4-, LL-37-, IL-10-, and CD-163-positive cells via light microscopy. For statistical evaluation, the Mann-Whitney U test and Spearman's rank correlation coefficient were used

Clinical course of a Japanese patient with developmental delay linked to a small 6q16.1 deletion.

There is only one report of patients with developmental delay due to a 6q16.1 deletion that does not contain the SIM1 gene. A 3-year-old female showed strabismus, cleft soft palate, hypotonia at birth, and global developmental delay. Exome sequencing detected a de novo 6q16.1 deletion (chr6: 99282717-100062596) (hg19). The following genes were included in this region: POU3F2, FBXL4, FAXC, COQ3, PNISR, USP45, TSTD3, CCNC, and PRDM13.

Cleft Palate and Aortic Dilatation as Clues for Loeys-Dietz Syndrome.

Loeys-Dietz syndrome (LDS) is a rare autosomal-dominant disorder of the connective tissue with some typical vascular findings, skeletal manifestations, craniofacial features, and cutaneous findings with a wide phenotypic spectrum. Six different genes are involved in LDS and the diagnosis is based on the identification of a heterozygous pathogenic variant in TGFBR1, TGFBR2, SMAD3, TGFB2, TGFB3, or SMAD2 in children with suggestive findings. These genes distinguish LDS into six classes (LDS1-LDS6, respectively). Delay in diagnosis of Loeys-Dietz syndrome may be associated with an adverse prognos

A novel nonsense variant in ARID1B causing simultaneous RNA decay and exon skipping is associated with Coffin-Siris syndrome.

Coffin-Siris syndrome (CSS) is a congenital disorder that is characterized by an absent/hypoplastic fifth distal phalanx, psychomotor developmental delay, and coarse facial features. One of the causative genes, ARID1B (AT-rich interactive domain-containing protein 1B), encodes components of the BAF chromatin remodeling complexes. Here, we report a case of a 3-year 8-month-old male with a novel nonsense variant (NM_001374820.1:c.4282C > T, p.(Gln1428*)) in the ARID1B gene, which was identified with whole-exome sequencing. He showed clinical symptoms of cleft soft palate, distinctive facial feat

An orthodontic perspective on Larsen syndrome.

BACKGROUND: Larsen syndrome (LS) is a rare disorder of osteochondrodysplasia. In addition to large-joint dislocations, craniofacial anomalies are typical characteristics. In this report, we performed orthodontic analyses, including skeletal and occlusal evaluations, to examine whether the craniofacial skeletal morphology leads to the craniofacial anomalies in LS. CASE PRESENTATION: A 5 year old Japanese girl who was clinically diagnosed with LS was referred to the orthodontic clinic in the Fukuoka Dental College Medical and Dental Hospital because of a malocclusion. Clinical findings at birth

Microbial contamination profile change over a 4-year period in nonoperated cleft soft palate.

AIMS: Surgical site infection is a major concern in cleft soft palate. Knowledge of the type, number and antimicrobial resistance of pathogens present preoperatively contribute to treatment success. The aim of this study is to determine whether or not the microbial contamination (diversity) preoperatively has changed since 2015. METHODS AND RESULTS: Swabs were taken from the surgical site in 103 consecutive patients who presented for primary repair of the soft palate cleft. These were sent for microscopy, culture and sensitivity testing. Swabs were taken before disinfecting the site. Results
