An uncommon cleft subtype of unilateral cleft lip and palate.

The finding that the vomer plays a crucial role in maxillary growth suggests that the bilateral cleft configuration of unilateral cleft lip and palate (UCLP), in which the vomer is detached from the non-cleft-side secondary hard palate, negatively influences palatal development, and this hypothesis was tested. Sixty persons with complete UCLP, including those with the vomer detached from (n = 30, b-UCLP) and attached to (n = 30, u-UCLP) the secondary hard palate, were analyzed morphologically, with the use of cast models taken at 10 days, 3 mos, and 12 mos of age. The anterio-posterior palatal

Complete bilateral facial cleft (Tessier 4) with corneal staphyloma: a rare association.

The oro-ocular cleft no. 4 according to the Tessier classification is one of the rarest facial cleft, and to this day, few cases have been reported in the literature. We describe the case of a 9-month-old girl with a complete bilateral facial cleft. On the right cornea protruded a hard lesion, a corneal staphyloma. We describe the 3 primary surgical steps used to restore the possibility of satisfactory feeding, to promote language acquisition, and to protect vision in the nonaffected eye. The psychological and social aspects of severe facial deformities in developing countries are also tackled

Evaluation of the progress of therapy in patients with cleft lip, jaw and palate, using voice analysis--a pilot study.

BACKGROUND: The defective morphology of the hard and soft palate in patients with cleft lip, jaw and/or palate in conjunction with speech, voice and hearing disorders can considerably restrict the ability to communicate. Changes in vocal timbre and the centripetal displacement of articulation are characteristic of cleft palate speech. There has not been a uniform diagnostic method in cleft centers to date which makes possible the analysis, documentation and comparison of changes in timbre. In this study we assessed a computer-aided evaluation process to determine objective vocal timbre paramet

Maxillary growth controversies after two-stage palatal repair with delayed hard palate closure in unilateral cleft lip and palate patients: perspectives from literature and personal experience.

OBJECTIVE: To analyze published papers dealing with delayed hard palate repair within a two-stage palatal surgery protocol in treatment of cleft lip and palate. Timing of the procedures, methods used, as well as growth results were considered. METHOD: By utilizing this information in relation to knowledge about normal maxillary development, efforts were made to explain differences in growth outcome between different investigations. Particularly, follow-up reports of unilateral cleft lip and palate patients with records up to at least 10 years of age were studied. RESULTS: Most papers reporte

Delayed closure of the hard palate leads to speech problems and deleterious maxillary growth.

BACKGROUND: Hard palate cleft closure has been associated with maxillary hypoplasia. The Schweckendiek procedure offers delayed hard palate closure to avoid early subperiosteal dissection and palatal scarring. This study sought to compare single-stage versus delayed hard palate closure for speech outcome and maxillary growth. METHODS: A retrospective outcome study was performed of unilateral cleft lip and palate patients with either delayed hard palate repair with a pinned-retained speech prosthesis (Schweckendiek repair) (group 1, delayed hard palate repair, 1978 to 1983) or single-stage cle

[Nasal sequels of unilateral clefts: analysis and management].

Usually, the nasal sequels of unilateral cleft patient are just considered as an esthetic problem to be addressed after the growth spurt of adolescence. This very narrow vision has led the cleft lip and palate treatment to a deadend. Actually, nasal sequels are the worst in terms of consequence on facial growth. 75% of complete unilateral cleft children are more oral than nasal breathers. Today, we know about the bad consequences of oral breathing on facial growth. It is not surprising to observe a high rate of small maxilla with cleft maxilla scars. In the fetus, the unilateral cleft nose def

Cephalometric evaluation of long-term craniofacial development in unilateral cleft lip and palate patients treated with delayed hard palate closure.

This long-term cephalometric study aimed to evaluate the craniofacial development of patients with a complete unilateral cleft lip and palate treated with a two-stage palatal closure, including delayed closure of the hard palate. Prediction models for cephalometric outcome at age 18 years were developed with the help of cephalometric values at 9 and 12 years. The objective need for surgery at age 18 was predicted from cephalometric values at age 9. Cephalograms of 43 consecutive patients with a complete unilateral cleft lip and palate were analysed at 9, 12 and 18 years. The patient group show

One- or two-stage palate closure in patients with unilateral cleft lip and palate: comparing cephalometric and occlusal outcomes.

OBJECTIVE: To assess facial growth and dentoalveolar development in two groups of patients with complete unilateral cleft lip and palate. Primary surgical treatment differed in the timing of hard palate closure. DESIGN: Forty-three patients with unilateral cleft lip and palate were examined. Twenty-two patients underwent early one-stage closure of the hard and soft palate cleft (mean age 23.0 +/- 4.7 months); in 21 patients, the hard palate closure was delayed to 86.3 +/- 39.2 months of age. Lateral cephalograms and dental casts were consecutively analyzed at four stages between 6 and 18 year

The nature of feeding in infants with unrepaired cleft lip and/or palate compared with healthy noncleft infants.

OBJECTIVE: Feeding difficulties are reported widely in infants with cleft lip and/ or palate. There is, however, a paucity of objective information about the feeding patterns of these infants. This study compared patterns of feeding in infants with unrepaired cleft lip and palate with healthy noncleft infants of a similar age. SETTING: North Thames Regional Cleft Centre. The noncleft cohort was recruited from West Middlesex University Hospital, a general hospital with similar demographics. PARTICIPANTS: Fifty newborn infants with nonsyndromic complete unilateral cleft lip and palate or a cle

Bilateral cleft lip and palate: improved maxillary and dental development.

BACKGROUND: Maxillary growth deficiency is still a problem in children with repaired cleft lip-cleft palate, and little progress has been made on research for its prevention. An increase in the early diagnosis of maxillary growth deficiency prompted this study to obtain an early indication of the efficacy of a new protocol for better dental and maxillary development. METHODS: A new method for repair of the hard palate in infants with bilateral cleft lip-cleft palate that made innovative use of mucosa of the nasal septum and involved less extensive surgery was coupled with the repair of the li

Alveolar bone formation in patients with unilateral and bilateral cleft lip and palate after early secondary gingivoalveoloplasty: long-term results.

BACKGROUND: The Milan surgical protocol includes the use of an early secondary gingivoalveoloplasty together with hard palate closure at 18 to 36 months, to avoid later bone grafting. The goal of this study was to evaluate the long-term quality of ossification in patients who have undergone early secondary gingivoalveoloplasty. METHODS: The samples consisted of panoramic radiographs of 87 unilateral cleft lip-cleft palate and 29 bilateral cleft lip-cleft palate patients. The records available allowed for a longitudinal and a cross-sectional evaluation of the ossification in the cleft area. Al

Cleft oronasal fistula: a review of treatment results and a surgical management algorithm proposal.

BACKGROUND: Oronasal fistulas (ONF) following cleft palate repair are commonly encountered and remain a challenging problem. With reported recurrence rates between 33% and 37%, this urges us to critically evaluate the current treatment and propose a surgical management protocol. METHODS: A retrospective study of patients treated for ONF by a single surgeon between 1995 and 2005 was performed. Data regarding cleft type, age at palate repair, complications, location and size of fistula, tissue condition, surgical technique employed, and success rate were gathered. RESULTS: There were 64 patien

Oblimersen: Augmerosen, BCL-2 antisense oligonucleotide - Genta, G 3139, GC 3139, oblimersen sodium.

Oblimersen is an antisense oligonucleotide developed by Genta for systemic use as an injection. It comprises a phosphorothioate backbone linking 18 modified DNA bases. Oblimersen targets the first six codons of Bcl-2 mRNA to form a DNA/RNA complex. The duplex is subsequently recognised as a foreign message and is cleaved enzymatically, thereby destroying the Bcl-2 message. The Bcl-2 protein, which is a potent inhibitor of apoptosis, is overexpressed in many cancers, including follicular lymphomas, breast, colon and prostate cancers, and intermediate-/high-grade lymphomas. Unilateral complete cleft lip and palate repair using lip adhesion combined with a passive intraoral alveolar molding appliance: surgical results and the effect on the maxillary alveolar arch.

BACKGROUND: A number of combined maxillary orthopedic and surgical treatment protocols have been proposed for the initial phase of therapy for infants with a complete cleft lip and palate. METHODS: Lip adhesion was used in combination with a passive intraoral molding appliance to treat a unilateral complete cleft lip and palate. The proposed protocols are lip adhesion, along with positioning of a passive alveolar molding appliance, at 4 to 6 weeks of age, then definitive cheiloplasty at 4 to 5 months of age, and palatoplasty at 12 months of age. Twenty-five patients with a complete cleft lip
