A detailed description of soft tissue morphology in unilateral cleft lip and palate cases--a study applying a new soft tissue analysis.

More than 1,500 CLP patients have been treated in Berne using a protocol which has always remained fundamentally the same. Even though--from birth to 20 years of age--the finest possible plastic surgery, orthodontics and prosthodontics were undertaken, the specialists involved considered that the patient, at the end of treatment, still possessed a CLP appearance to some extent. Neither the cephalometric analyses, nor the occlusal findings, however, provided any explanation for this, and a new method of soft tissue analysis was therefore developed to provide a detailed picture of the tissue ove

One-stage closure of the entire primary palate.

Timing of the closure of the anterior palate and alveolus is a subject of debate. Late repair of this defect is complicated by high fistula formation and subjects the patient to the problems of palate fistula for extended periods of time. We have utilized a single procedure performed when the child is 3 months of age that completely closes the anterior hard palate and alveolus along with the cleft lip. Our series consisted of 61 consecutive patients with unilateral clefts of the primary and secondary palate. Mucosal turnover flaps from the vomer along with lateral nasal mucosal flaps provide t

Cleft lip and palate patients prior to delayed closure of the hard palate: evaluation of maxillary morphology and the effect of early stimulation on pre-school speech.

Speech and maxillary development were analysed in two groups of patients with unilateral cleft lip and palate; both groups had early jaw orthopaedic treatment and a surgical regimen that included two-stage lip surgery (mean ages of 2 and 19 months) and soft palate repair (8 months). Closure of the hard palate was postponed until the children were 8 to 10 years of age. The first group comprised 10 consecutive patients who were analysed at 5 and 7 years of age, and the second group seven patients who were studied at the age of 5. Both groups were thus investigated before the repair of the cleft

Maxillary growth after two-stage palatal closure in complete (unilateral and bilateral) clefts of the lip and palate from infancy until 10 years of age.

The purpose of the study is to clarify the maxillary growth effects following different types of palatal closure in complete clefts of the lip and palate from infancy to 10 years of age. Lip repair, carried out at 5 months in one stage, was accomplished by Tennison's procedure. These patients were then assigned randomly to each of the 4 experimental groups according to the types of clefts and of palatal closure. One group of 14 patients in unilateral cases (Unil-S) and another group of 8 patients in bilateral cases (Bil-S) received mucoperiosteal palatal push-back procedure in a single stage a

Adaptive functional changes in the swallowing pattern of the tongue following expansion of the maxillary dental arch in subjects with and without cleft palate.

In seven subjects with unilateral cleft lip and palate (UCLP), two subjects without cleft but with complete lingual crossbite, and two subjects with normal occlusion, tongue contact patterns against the hard palate, lingual surfaces of maxillary and mandibular teeth, and mandibular lingual mucosa during swallowing were analyzed three-dimensionally using electropalatography (EPG), before and after lateral expansion of the maxillary dental arch and after 3 months of retention. Before maxillary lateral expansion, all of the subjects with cleft palate showed no palate-tongue contact at rest, and l

Presurgical orthopaedic treatment using hard plates.

Our concept in the treatment of unilateral clefts of lip, alveolus and palate includes early maxillary orthopaedic treatment starting immediately after birth until the end of the first year of life and the surgical closure of the lip at three months and of the soft palate at twelve months. Hard palate and alveolus are closed after three years by secondary osteoplasty in the alveolar region. The appliance is made of hard acrylic resin and is adjusted monthly to allow for growth. It thus encourages passive orthopaedic guidance of maxillary growth. The local compatibility of the plate was excelle

[Prosthetic reconstruction of the secondary deformed maxillary arch in a patient with congenital cleft].

A case history of a bilateral cleft lip and palate male patient is presented. He was operated late at the age of his twelve, without any previous orthodontic treatment. Surgical reconstruction included, lip, hard and soft palate, removal of the premaxilla, and later on the pharingoplasty. At the age of twenty years he was prosthetically rehabilitated. Nine inner telescope crowns were made in "Palador" silver alloy, and nine outer in chrom-cobalt splinted together and connected with the skeletal frame. Intermaxillary relations were recorded and established in a manner used in the construction o

Features of maxillary arch and nasal cavity in infancy and their influence on deciduous occlusion in unilateral cleft lip and palate.

The purpose of this study was to test the statistical relationship between certain preoperative characteristics of the maxilla and nasal cavity and later occlusal development in patients born with complete unilateral cleft lip and palate. The sample consisted of 51 patients and 22 of them had undergone surgical treatment which included vomer flap and pushback palatal repair, while the remaining 29 patients were treated with a routine characterized by delayed closure of the hard palate. Twelve variables related to 4 different maxillary and nasal areas were recorded from maxillary casts and fron

Roentgenocephalometric study of complete unilateral cleft lip and palate patients treated with delayed closure of the hard palate--a preliminary report.

Cephalometric radiographs of a sample of complete unilateral cleft lip and palate patients (15 males, 15 females, aged 6.5 to 7.5 years) were analysed. In most cases, an adhesion cheiloplasty was done at 3 months of age, followed by definitive cheiloplasty at 6 to 12 months, then by posterior palatoplasty at 1.5 years. The hard palate was not repaired till the age of 5-7 years. This preliminary study indicated that: (1) The growth of maxilla and mandible shows no sex difference, except the anteroposterior maxillary length, which is longer in the cleft males. (2) The maxilla of our group is mor

Effects of cleft lip and palate on the nasal airway in children.

Clefts of the lip and palate often produce significant nasal deformities and reduced nasal airway size. The purpose of this study was to assess how type of cleft affects nasal cross-sectional area and mode of breathing. The pressure-flow technique was used to estimate nasal airway size and modified inductive plethysmography was used to determine percent of nasal breathing in 60 children with cleft lip and palate aged 6 to 15 years. Ninety-five normal children served as controls. The data demonstrate that nasal size decreased among cleft types as follows: children with bilateral cleft lip and p

Occurrence of oronasal fistulas in operated cleft palate patients.

Oronasal fistulas, a troublesome complication, often occur after cleft palate repair. Seventy-three patients in a series of 346 cases of cleft palate (21%) were found to have fistulas, most located at the junction of hard and soft palate (42%). Langenbeck's method of cleft palate repair resulted in more fistulas than Wardill's method. Fistulas occurred more frequently in bilateral clefts than in the unilateral type. Nasality was found to be the most common symptom in patients with oronasal fistulas. No treatment was needed for 17 patients, 10 were given obturators, and surgical repair was perf

Caudally-based single-layer septum-vomer flap for cleft palate closure.

The surgical technique using a single-layer caudally-based septum-vomer flap for cleft palate surgery, especially for unilateral cleft lip and palate cases, is described and introduced in this paper. The advantages and disadvantages of this primary technique are discussed and compared with other types of vomer-flap closures for the hard palate. Additionally, a lip flap, for anterior nasal floor closure, is introduced for the uninterrupted closure of the cleft of the premaxilla and palate.

Facial morphology and occlusion at the stage of early mixed dentition in cleft lip and palate patients treated with delayed closure of the hard palate.

Cephalometric radiographs and dental study casts were analyzed in a group of 23 seven-year-old cleft lip and palate patients, 16 with unilateral and 7 with bilateral cleft. The patients' primary surgical procedures had been completed except for closure of the cleft in the hard palate. For comparison, similar records from another group of patients, 18 with unilateral and 8 with bilateral cleft lip and palate, were studied. In these cases the cleft of the hard palate had been repaired in infancy, using a vomer flap procedure. The results indicated that midfacial growth and dental occlusion of th

Malocclusion in complete unilateral and bilateral cleft lip and palate. The results of a change in the surgical procedure.

In 1972 the surgical interference with the cleft in the hard palate was changed to a one-layer closure by a vomer flap. The purpose of the present investigation was to examine the effect of this change in the surgical management on the width of the maxillary dental arch and the frequency of malocclusion. Two groups of CLP children were examined: group 1 consisted of 58 children operated on before and in 1972. Group 2 consisted of 72 children operated on after 1972. All of the children had been operated on by the same surgeon. The remainder of the surgical procedures was the same in the two gro

Maxillary development in cleft palate patients with special reference to the effects of operation.

Thirty-one patients whose complete unilateral clefts of lip and palate had been repaired by early operation were followed up for 18 years. The results showed that repair of the hard palate is followed by deformity of the maxilla in a sufficient number of patients to suggest that we need to look for improved methods of repair of the hard palate.

Visual training and correction of articulation disorders by use of dynamic palatography: serial observation in a case of cleft palate.

The dynamic palatograph is an electrical apparatus that generates a visual display of constantly changing palatolingual contact as a function of time, using an artificial palatal plate with affixed electrodes. This paper describes a technique of speech therapy incorporating dynamic palatography for a cleft palate patient. The patient, a 6-year-old Japanese girl with a repaired unilateral cleft lip and palate, had been judged to demonstrate articulation disorders involving contact of the tongue with the hard palate or alveolus following surgical improvement of velopharyngeal function. Prior to

Elongated pharyngeal flap in extensive clefts of the hard and soft palate.

In some cases of extensive palatal defects surgical closure may be regarded as unfeasible, and the condition treated with an obturator prosthesis. In such a case the cleft can be closed in one operation by means of a pharyngeal flap elongated through a pharyngotomy according to Bengt Johanson (1966). Eleven patients who had used obturators were operated on between 1957 and 1978. The mean age of the patients was 39 years. All patients were cleft lip and palate or cleft palate cases; two unilateral, five bilateral, and four with an isolated cleft palate. In most of these patients a temporary tra

Three-dimensional nasal morphology in adult bilateral cleft palate patients: a morphanalytic study.

The residual anomalies of the soft and hard tissues of the nose were studied in a group of adult bilateral cleft palate patients. Standard morphanalytic techniques were used to produce objective and accurate pictorial representations of the mean nasal deformities in three dimensions. The spatial positions of these mean outlines were tested statistically against normal means. Mean bilateral cleft nasal outlines were shown to have symmetric lateral and superior displacement of both alae with nasal blunting and retrusion and rounding of the nasal dorsum. The analytic techniques used indicated tha

Cleft palate repair by double opposing Z-plasty.

In an attempt to improve speech results following palate repair while allowing adequate maxillary growth, a palatoplasty using two opposing Z-plasties of the soft palate, one of the oral and one of the nasal layers, has been used in 22 infants. Eight patients had unilateral cleft lip and palate, eight had bilateral cleft lip and palate, and six had cleft palate. The Z-plasties facilitate effective dissection and redirection of the palatal muscles to produce an overlapping muscle sling and lengthen the velum without using tissue from the hard palate, which permits hard palate closure without pu
