Effects of cleft lip and palate on the nasal airway in children.

Clefts of the lip and palate often produce significant nasal deformities and reduced nasal airway size. The purpose of this study was to assess how type of cleft affects nasal cross-sectional area and mode of breathing. The pressure-flow technique was used to estimate nasal airway size and modified inductive plethysmography was used to determine percent of nasal breathing in 60 children with cleft lip and palate aged 6 to 15 years. Ninety-five normal children served as controls. The data demonstrate that nasal size decreased among cleft types as follows: children with bilateral cleft lip and p

Occurrence of oronasal fistulas in operated cleft palate patients.

Oronasal fistulas, a troublesome complication, often occur after cleft palate repair. Seventy-three patients in a series of 346 cases of cleft palate (21%) were found to have fistulas, most located at the junction of hard and soft palate (42%). Langenbeck's method of cleft palate repair resulted in more fistulas than Wardill's method. Fistulas occurred more frequently in bilateral clefts than in the unilateral type. Nasality was found to be the most common symptom in patients with oronasal fistulas. No treatment was needed for 17 patients, 10 were given obturators, and surgical repair was perf

Elongated pharyngeal flap in extensive clefts of the hard and soft palate.

In some cases of extensive palatal defects surgical closure may be regarded as unfeasible, and the condition treated with an obturator prosthesis. In such a case the cleft can be closed in one operation by means of a pharyngeal flap elongated through a pharyngotomy according to Bengt Johanson (1966). Eleven patients who had used obturators were operated on between 1957 and 1978. The mean age of the patients was 39 years. All patients were cleft lip and palate or cleft palate cases; two unilateral, five bilateral, and four with an isolated cleft palate. In most of these patients a temporary tra

Cleft palate repair by double opposing Z-plasty.

In an attempt to improve speech results following palate repair while allowing adequate maxillary growth, a palatoplasty using two opposing Z-plasties of the soft palate, one of the oral and one of the nasal layers, has been used in 22 infants. Eight patients had unilateral cleft lip and palate, eight had bilateral cleft lip and palate, and six had cleft palate. The Z-plasties facilitate effective dissection and redirection of the palatal muscles to produce an overlapping muscle sling and lengthen the velum without using tissue from the hard palate, which permits hard palate closure without pu

A bilateral total cleft of lip and hard and soft palate and transverse cleft face with an atypical paramedian defect of the lower lip and lower jaw on the right side. A case report of a new syndrome?

A three-month-old male baby who has a complete bilateral cleft of the lip and hard and soft palate, and a right unilateral transverse facial cleft, with an atypical paramedian defect of the lower lip and jaw is reported in detail. As these associated malformations are very rare, it may represent a new dysmorphic syndrome.

Island soft palatoplasty for early reconstruction of the posterior muscular ring.

Restoration of the continuity of the posterior muscular ring and production of adequate palatal length have been the prime objectives cleft surgeons have sought in their drive to obtain good speech. Twelve patients with cleft palates varying from incomplete clefts of the secondary palate to wide clefts associated with unilateral and bilateral lip deformities have been repaired utilizing an early radical soft palate release from the hard palate. The element not stressed in prior early soft palatoplasties is this full-thickness release of the soft palate from its abnormal insertion along the cle

A follow-up study of cleft children treated with vomer flap as part of a three-stage soft tissue surgical procedure. Facial morphology and dental occlusion.

The development of the face and jaws in cleft patients, treated with a three-stage surgical procedure including a single layer vomer flap, was studied by analysing cephalometric radiographs and dental casts. The material consisted of 13 patients with complete bilateral cleft lip and palate and 50 cases with complete unilateral cleft lip and palate, operated on 1964--1970. At the follow-up the average patient in both cleft categories demonstrated a maxillary retrognathia and in the unilateral cleft sample also a facial skeletal profile straighter than normal, though not as pronounced as we had

Cleft palate and congenital palatopharyngeal incompetency in mandibulofacial dysostosis: frequency and problems in treatment.

In a series of 25 cases of mandibulofacial dysostosis, 7 patients had isolated clefts of the palate, 1 had a complete unilateral cleft of the lip and palate, and 8 had congenital palatopharyngeal incompetency (CPI). The CPI appeared in four forms: (a) complete agenesis of the soft palate, (b) foreshortening of the soft palate associated with a submucous defect of the hard palate, (c) submucous defect of the hard palate with adequate palatal length but inadequate elevation in speech, and (d) lack of adequate palatal elevation in the absence of a submucous defect or reduced length of the soft pa
