TXNRD1-driven enteric glial remodeling promotes neuronal dysfunction in Hirschsprung disease.

Hirschsprung disease (HSCR) is characterized by distal intestinal aganglionosis and persistent dysfunction of the enteric nervous system after surgical treatment. Here, we integrated single-cell transcriptomics, network-based virtual perturbation analysis, proteomics, human tissue validation, enteric glial cell assays, neuron-glia co-culture systems, and an Ednrb conditional knockout (cKO) mouse model to investigate TXNRD1-dependent enteric glial remodeling in HSCR. We found that thioredoxin reductase 1 (TXNRD1) enrichment was associated with expansion of an SPP1-high/CXCL9-low enteric glial s

A diagnostic cascade from cholecystitis to Meckel's diverticulum rupture: a case report.

BACKGROUND: Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract, yet it rarely becomes symptomatic in adults. Perforation is an uncommon and often overlooked complication that may mimic more frequent abdominal conditions, particularly in the postoperative setting. MD presenting shortly after unrelated laparoscopic surgery is exceptionally rare and can pose a significant diagnostic challenge. CASE PRESENTATION: A 36-year-old woman presented with acute right upper quadrant pain and was diagnosed with calculous cholecystitis. She underwent laparoscopic

Atypical presacral masses in Currarino syndrome: a case series.

INTRODUCTION: Currarino syndrome is a rare congenital disorder classically defined by the triad of sacral anomaly, anorectal malformation, and a presacral mass. While anterior meningoceles and teratomas are the most commonly reported lesions, an expanding body of literature suggests a broader spectrum of presacral pathology. We present three cases from our institution that expand the phenotypic range of presacral masses associated with Currarino syndrome, including lesions not previously well described in this population. CASE PRESENTATION: Three cases are presented. Final pathologic diagnose

Objective quantification of suckling biomechanics in infants with and without tongue-tie using hydraulic differential-pressure recording and machine learning.

INTRODUCTION: Objective assessment of infant suckling biomechanics remains limited despite decades of pressure-based measurement research, and tongue-tie (ankyloglossia) is still largely evaluated using semi-quantitative anatomical scoring. We developed a hydraulic differential-pressure device - the electronic baby bottle (EBB) - designed to record real-time net intraoral mechanical load during nutritive suckling through a fluid-filled sensing cavity with a controlled air inclusion coupled to a differential pressure transducer. This study aimed to: (1) describe the EBB measurement principle, (

Zebrafish hoxba and hoxbb clusters are redundantly required for posterior pharyngeal pouch development.

BACKGROUND: Pharyngeal pouch formation is fundamental to vertebrate craniofacial development, but molecular mechanisms underlying posterior pouch formation remain incompletely understood. Retinoic acid (RA) signaling is required for posterior pharyngeal development, and Hox genes are established RA targets. However, their genetic requirement in posterior pouch formation has not been established. RESULTS: Zebrafish lacking both hoxba and hoxbb clusters-teleost-specific duplicates of the ancestral HoxB cluster-exhibited severe loss of posterior pharyngeal pouches and associated pharyngeal skele

The impact of nephrectomy on liver and contralateral kidney volume in patients with ADPKD; a case series.

PURPOSE: A substantial number of patients with autosomal dominant polycystic kidney disease (ADPKD) undergo nephrectomy. A decrease in intra-abdominal pressure after nephrectomy might hypothetically accelerate the growth of other polycystic organs and accelerate the rate of eGFR decline of the remaining kidney. In this case-series, we investigated the change in kidney and liver volume in patients before and after nephrectomy, and in a control group without nephrectomy. METHODS: In this retrospective case series kidney and liver volumes were measured using an automated stereological approach (

Expanding the Donor Pool in Pediatric Transplantations: Successful Domino Liver Transplantation From the Youngest Reported Pediatric DCD Donor Using Normothermic Regional Perfusion in the United States.

INTRODUCTION: Normothermic regional perfusion (NRP) during donation after circulatory death (DCD) improves abdominal organ utilization by limiting warm ischemia time. Its application in young pediatric donors remains exceptionally rare; the youngest previously reported DCD-NRP donor in the United States was 5 years of age. METHODS: We describe DCD-NRP organ procurement from a 23-month-old donor and the subsequent use of the liver graft in a domino liver transplantation involving two pediatric recipients. The kidneys were recovered en bloc. The donor, supported on veno-arterial extracorporeal

Successful Liver Transplantation in a Child With Invasive Aspergillus terreus Cholangitis Identified via Plasma Metagenomic Next-Generation Sequencing.

BACKGROUND: Ascending cholangitis, one of the most common complications in patients with biliary atresia (BA) after Kasai hepatoportoenterostomy (HPE), impacts native liver survival. Typically, enteric bacterial flora are the source of infection, and fungal infections of the biliary tree are rare. CASE REPORT: We report the case of a 3-year-old female with BA status post Kasai HPE who developed Aspergillus terreus cholangitis (complicated by hepatic vein thrombus) identified through plasma metagenomic next-generation sequencing (NGS) of microbial cell-free deoxyribonucleic acid (DNA). Meckel's diverticulum leading to axial torsion and volvulus of the small bowel.

A Meckel's diverticulum is a blind-ending pouch that arises on the antimesenteric border of the terminal ileum. It is the most common gastrointestinal anomaly and the result of an incomplete obliteration of the vitelline duct during the first 8 weeks of gestation. The 'rule of two' is a mnemonic which refers to key characteristics of Meckel diverticula: it is found in 2% of the population, it is 2 inches long, 2 feet from the ileocaecal valve and twice as common in males. Only 2%-4% of those with a Meckel's develop complications in their lifetime. The majority present in early childhood with c

H-type anorectovaginal fistula with anterior ectopic anus in a 15-month-old girl: a case report.

INTRODUCTION AND IMPORTANCE: H-type anorectovaginal fistula is a rare anorectal malformation characterized by an abnormal communication between the rectum and vagina despite a normally formed or near-normal anus. Diagnosis may be delayed due to the absence of obvious external anatomical abnormalities, making clinical recognition essential, particularly in resource-limited settings. CASE PRESENTATION: We report a 15-month-old girl presenting with fecal vaginal discharge and chronic constipation since early infancy. Clinical examination revealed an anterior ectopic stenotic anus with vaginal fe

Population Differences in the Epidemiology, Phenotype, and Genetics of Hirschsprung Disease in the United States.

IMPORTANCE: Understanding population differences in epidemiology, clinical presentation, and genetic architecture remains a major challenge for all rare genetic disorders. Hirschsprung disease (HSCR), despite being the commonest cause of neonatal intestinal obstruction, has been poorly studied with respect to its significant heterogeneity across U.S. populations. OBJECTIVE: To characterize self-identified race and ethnicity differences in HSCR incidence, clinical presentation, and genetic architecture in the United States from diverse data sources. DESIGN SETTING AND PARTICIPANTS: We used re

One-year outcomes for patients undergoing antegrade continence enema procedures without prior rectal enema therapy: a Pediatric Colorectal and Pelvic Learning Consortium study.

PURPOSE: Antegrade continence enemas (ACE) delivered through surgically placed channels help achieve social continence in children with colorectal disorders. Historically, a successful rectal enema program was required before ACE creation, but high-volume centers increasingly perform ACE upfront, particularly for patients intolerant of rectal enemas. This study describes a cohort of patients who underwent upfront ACE for bowel management within a multicenter consortium of high-volume pediatric colorectal centers, with 30-day and one-year outcomes. METHODS: We retrospectively evaluated patient

Giant cystic lymphangioma in the pancreatic tail-splenic hilum region managed by en bloc resection: A case report.

RATIONALE: Lymphangioma is a benign lymphatic malformation that occurs predominantly in children and at superficial sites. Intra-abdominal lymphangioma is uncommon, and involvement of the pancreatic tail-splenic hilum region is particularly rare. Such lesions may mimic pancreatic or peripancreatic cystic disease, making preoperative diagnosis challenging. PATIENT CONCERNS: We report a 43-year-old woman who presented with persistent dull left upper abdominal pain for 2 days. Laboratory tests showed nonspecific inflammatory and mild hepatobiliary abnormalities. DIAGNOSES: Ultrasonography and c

A novel variant of pancreaticobiliary maljunction: Complete opening of the common bile duct into the duct of Santorini in a patient with pancreas divisum.

Pancreaticobiliary maljunction (PBM) is a rare congenital anomaly in which the pancreatic and bile ducts unite outside the duodenal wall, forming an abnormally long common channel that permits reciprocal reflux of pancreatic juice and bile. This leads to a high risk of recurrent pancreatitis, cholangitis, biliary stone formation, and biliary tract malignancy. Type D PBM, particularly when associated with complete pancreas divisum and aberrant bile duct drainage, represents an exceptionally rare and complex variant. We report the case of a 20-year-old female with a long-standing history of recu

Concomitant Meckel's Diverticulum With Focal Paneth Cell Metaplasia and Acute Appendicitis Presenting as Chronic Periumbilical Pain in a Child.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract but remains diagnostically challenging in children because of its varied and nonspecific clinical presentation. Concomitant Meckel's diverticulum and acute appendicitis are uncommon and may further obscure diagnosis. We report a 9-year-old boy who presented with chronic periumbilical abdominal pain of 3 months' duration, with acute worsening over 3 days accompanied by vomiting. Physical examination revealed localized right periumbilical tenderness without signs of peritonitis. Laboratory investigations de
