Antiseizure Medication Polytherapies During Pregnancy and the Risk of Congenital Malformations.

BACKGROUND AND OBJECTIVES: The use of antiseizure medication (ASM) polytherapy has become increasingly common. However, it is known that there is an elevated risk of major congenital malformations (MCMs) in newborns after maternal use of select ASMs. This study characterizes the risk of MCMs in infants born to women using common ASM polytherapies during the first trimester. METHODS: The study population was pregnant women enrolled in the North American AED Pregnancy Registry between 1997 and 2024. Phone interviews at enrollment, 7 months' gestation, and postpartum collected data on ASM use an

How Often is Speech Surgery Recommended After Le Fort I Advancement in Patients With Preoperative Borderline Velopharyngeal Insufficiency?

BACKGROUND: Children with repaired cleft lip and palate (CLP) often develop maxillary hypoplasia requiring Le Fort I (LF1) advancement. Patients with preoperative velopharyngeal insufficiency (VPI) are at high risk for worsening velopharyngeal function after LF1 advancement, while those with normal preoperative function are at low risk. However, speech outcomes in patients with preoperative borderline VPI are poorly understood. PURPOSE: The study purpose was to estimate the frequency of recommended speech surgery among subjects with preoperative borderline VPI following LF1 advancement and it

Global, regional, and national burden and trends of congenital birth defects from 1990 to 2021: epidemiological trends, health inequalities, and COVID-19 impact.

BACKGROUND: Congenital birth defects remain a leading cause of mortality and disability in younger populations aged <14 years, however, little comprehensive or recent evidence of the burden of congenital birth defect and its subtypes are available globally. Additionally, the extent of regional health inequality and the effects of the coronavirus disease 2019 (COVID-19) pandemic on congenital birth defects remain unclear. This study leverages 2021 Global Burden of Disease (GBD) data to conduct a comprehensive assessment of the global epidemiological profile, health inequality, and the repercuss

Comparative assessment of the infraorbital canal course in patient with cleft lip and palate with control group: a CBCT study.

AIM: This study aimed to assess the course of the infraorbital canal (IOC) in cleft lip and palate (CLP) patients compared to a matched control group using CBCT imaging, evaluating the potential anatomical variations that may influence surgical risks in the maxillofacial region. MATERIALS AND METHODS: In this cross-sectional study, CBCT scans of 67 CLP patients (50 unilateral, 17bilateral) were evaluated alongside 67 age- and sex-matched controls. IOC courses were classified into three types using the Ference method: Type I (within sinus roof), Type II (juxtaposed), and Type III (protruded).

Advanced Transdermal Patches with Folate Nanoparticles: A Breakthrough in Prenatal Supplementation.

OBJECTIVE: One of the causes of congenital cleft lip/palate abnormalities is maternal folic acid deficiency. A common issue during the first trimester is nausea, vomiting, and, in some cases, hyperemesis gravidarum, which can hinder optimal folic acid intake. The application of transdermal patches represents a significant advancement in systemic drug delivery through intact skin. This study aimed to formulate and evaluate a folic acid nanoparticle transdermal patch that effectively bypasses gastrointestinal absorption, addressing a critical need during the first trimester of pregnancy. MATERI

Comprehensive analysis of de novo variants across 2,497 orofacial cleft trios reveals novel genetic drivers of disease.

BACKGROUND: Orofacial clefts (OFCs) and other palate abnormalities (PAs) are among the most common birth defects worldwide and are characterized by the abnormal formation of the lip and/or palate. Genetic studies have traditionally classified OFC cases as either syndromic, involving OFCs alongside other congenital anomalies, or nonsyndromic, which represent the majority of cases and occur in isolation. Emerging genomic evidence indicates that genes traditionally associated with syndromic forms of OFC can also harbor variants contributing to isolated cases, challenging the notion of a strict di

Mirror-image Bilateral Complete and Incomplete Cleft Lip and Alveolus in Monozygotic Twins: A Case Report.

Cleft lip and/or palate (CL/P) represent one of the most common congenital malformations in the oral and maxillofacial region, and various factors are associated with the development of CL/P and its subtypes. Despite an increased concordance rate in monozygotic twins, mirror-image cleft formations represent a very rare phenotype, for which only very few case reports exist in the literature. The present case report describes a female pair of monozygotic twins from Germany who were born with mirror-image bilateral complete and incomplete cleft lip and alveolus.

Transforming growth factor alpha polymorphism and cleft lip and palate in North Indian Population.

INTRODUCTION: One of the most common congenital malformations, with widespread racial and regional variation, is an orofacial cleft. Cleft lip and palate (CLP) are a congenital anomaly that results in incomplete or non-fusion of the lip and/or palate during the embryonic development of the child. Many studies support that the occurrence of CLP is attributed to a combination of factors of environment and genes. Earlier studies had studied many candidate genes for CLP in different populations but not all are significant to every population. Transforming growth factor alpha (TGFA) genes had been

Congenital Superior Lumbar Hernia (Grynfeltt-Lesshaft Hernia) Associated With Orofacial, Limb, and Rib Anomalies: A Neonatal Case Report.

Congenital superior lumbar hernia (Grynfeltt-Lesshaft hernia) is an extremely rare defect of the posterior abdominal wall in neonates and is frequently associated with other congenital anomalies. We report a one-month-old female presenting with a right-sided lumbar swelling since birth. Clinical examination revealed a large, reducible mass measuring 8 × 10 cm in the superior lumbar triangle. Associated anomalies included a complete right-sided cleft lip, right thumb hypoplasia with radial deviation, and absence of the lower right thoracic ribs. Abdominal ultrasound demonstrated bowel loops wit

Bilateral superior labial artery embolization for arteriovenous malformation.

Lip arteriovenous malformations (AVMs) are typically present at birth and can lead to progressive tissue destruction and functional and cosmetic issues. Their management is challenging due to the lack of standardized protocols and may involve both surgical and non-surgical approaches. The authors present a case of upper lip AVM with feeders bilaterally from both facial arteries and their superior labial branch arteries. Bilateral Gelfoam embolization of the labial arteries followed by surgical resection was performed. The upper lip was reconstructed using a bilateral advancement flap. The pati

Eustachian tube balloon dilation for middle ear effusion in children with cleft lip and palate.

PROBLEM: Eustachian tube balloon dilation is increasingly recognised as a minimally invasive option for middle ear effusion. However, its role in children with cleft lip and palate remains under-explored. METHODS: We prospectively evaluated 14 cleft lip and palate children (28 ears) with middle ear effusion. Group 1 (intervention) comprised 14 ears that underwent Eustachian tube balloon dilation with intranasal corticosteroids, while Group 2 (control) included 14 ears treated with intranasal corticosteroids alone. Oral microbiome dysbiosis and innate immune dysregulation as determinants of oronasal fistula after primary cleft palate repair.

Oronasal fistula complicates 15-55% of primary cleft palate repairs, with recurrence rates approaching 43% after secondary closure, and global fistula rates have risen despite decades of iterative technical refinement, a trend that mechanical closure quality alone cannot explain. This narrative review synthesizes evidence from PubMed/MEDLINE, Scopus, and Web of Science from inception through April 2026 to argue that ONF is increasingly recognizable as a biologically mediated complication in which oral microbiome dysbiosis and innate immune dysregulation are primary, historically underrecognize
