Response to Letter Regarding Cordray et al. (2023) and Brief Commentary on the Ankyloglossia Debate.

PURPOSE: Ankyloglossia (tongue tie) is common, and its clinical relevance is hotly debated among multidisciplinary health care providers including pediatricians, lactation consultants, otolaryngologists, and speech-language pathologists. The literature focuses on breastfeeding symptoms; less evidence exists to clarify the spectrum of possible symptoms after infancy. We conducted a scoping review of potentially related symptoms in pediatric patients who presented for clinical evaluation of untreated ankyloglossia, with the aim of mapping symptoms that providers may want to evaluate and identify

Incorrect Statements and Citing Errors Regarding the Effect of Ankyloglossia on Speech in Cordray et al., 2023.

PURPOSE: There is a great deal of misinformation among professionals about the effect (or lack thereof) of ankyloglossia on speech. Consequently, many children undergo unnecessary frenotomy procedures that do not improve their speech and may cause unnecessary complications. This letter points out several erroneous statements in Cordray et al. (2023) about the effect of ankyloglossia on speech. In addition, it identifies articles that were incorrectly cited to support these statements. This letter also reveals significant errors of omission, including the results of the systematic review from t

Management of Postoperative Hematoma Following a Lingual Frenectomy: A Case Report.

Ankyloglossia, commonly known as tongue-tie, is a congenital condition characterized by a short, thick lingual frenum that restricts tongue mobility, leading to functional and developmental issues. Surgical intervention, such as a lingual frenectomy, is commonly used to restore tongue function. However, postoperative complications like hematomas can pose significant risks. This case report discusses the management of a sublingual hematoma in a 15-year-old male following a conventional lingual frenectomy. Postoperatively, the patient developed swelling, erythema, and restricted tongue movement,

Experiences of Breastfeeding Mothers of Infants With Ankyloglossia: A Qualitative Study.

BACKGROUND: Ankyloglossia, or limited tongue mobility due to a restrictive sublingual frenulum, can complicate breastfeeding. Treatment typically involves a frenotomy, a procedure that cuts the restrictive tissue parallel to the tongue. RESEARCH AIM: To describe the experiences of breastfeeding mothers with children affected by ankyloglossia, before any treatment for the condition. METHOD: This was a qualitative descriptive study. Using purposive sampling, 20 participants were recruited. Data were collected through individualized semi-structured interviews. The interviews were recorded and t

Incidence of oral tie diagnosis and treatment in Southwestern Ontario: A survey study.

BACKGROUND: Ankyloglossia describes a short lingual frenulum restricting tongue mobility. There are no universally accepted diagnostic criteria, and the rate of diagnosis and treatment are increasing. Many frenotomies are performed in the outpatient setting, making it difficult to estimate ankyloglossia incidence. This study aims to estimate the rate of diagnosis and common indications cited for treatment of ankyloglossia in a large catchment in Southwestern Ontario, Canada. METHODS: A survey was administered to all patients presenting to a pediatric otolaryngology clinic and five community p

Effectiveness of orofacial myofunctional therapy in improving orofacial function and oral habits: a scoping review.

BACKGROUND: The effectiveness of orofacial myofunctional therapy (OMT) has yet to be confirmed in the literature. This scoping review aimed to answer the question, "What evidence exists to support the effectiveness of OMT in treating/managing orofacial myofunctional disorders (OMDs) affecting orofacial structures' function and oral habits?" METHODS: A librarian at the University of Alberta, Canada, developed a comprehensive search strategy and applied it to 6 databases and grey literature. The reference lists of included studies were cross-checked. Two independent reviewers screened the retri

Analyzing ankyloglossia information on TikTok: A cross- sectional study.

INTRODUCTION: The American Academy of Otolaryngology-Head and Neck Surgery Foundation (AAO-HNSF) developed a clinical consensus on ankyloglossia in children to improve understanding of controversies and increase family-centered care. The breadth of information on ankyloglossia available on social media and the consistency with AAO-HNSF consensus remains underexplored. METHODS: A cross-sectional analysis was performed on the top 100 publicly available TikTok videos using the search term "tongue tie." Video metrics and stakeholders were determined and were classified as informational, anecdotal

The Prevalence of Oral Anomalies Among Healthy Newborns at a Gynecological Obstetric Hospital in Quito, Ecuador: An Observational, Cross-Sectional Study.

Objectives: Early observations often fail to detect anomalies that may impact the health and quality of life of newborns. This study aimed to determine the prevalence of oral anomalies in newborns and explore their possible associations with sociodemographic factors. Methods: A cross-sectional study was conducted in Quito, Ecuador, analyzing a sample of 241 newborns. The presence of oral anomalies was recorded, and their association with sex, birth weight, maternal age, and gestational factors was evaluated. Results: The prevalence of oral anomalies was 72.3%, with Epstein's pearls being the m

Avoiding frenotomies and improving health equity: Findings from a paradigm shift newborn feeding difficulties model.

IMPORTANCE: Newborn feeding difficulties are increasingly attributed to ankyloglossia, leading to a surge in frenotomy procedures despite limited high-level evidence supporting its effectiveness. Disparities in access to appropriate feeding assessments contribute to potential surgical overtreatment and inequities in care. OBJECTIVE: To evaluate the impact of a paradigm shift newborn feeding difficulties model on clinical care patterns and health equity. DESIGN, SETTING, AND PARTICIPANTS: A retrospective cohort study of infants (<6 months) with ankyloglossia-related ICD-9/ICD-10 codes at a te

Maternal Experiences and Challenges in Breastfeeding Infants with Tongue-Tie: A Systematic Review.

INTRODUCTION: Tongue-tie, clinically referred to as ankyloglossia, is a common condition characterized by an unusually short lingual frenulum and is associated with breastfeeding difficulties. Mothers of infants with tongue-ties are at increased risk of not only painful breastfeeding and poor latching, but also increased maternal stress and decreased maternal-infant bonding. The purpose of this systematic review is to examine the literature on maternal experiences and wellbeing while feeding infants with tongue-ties. METHODS: We searched MEDLINE, Web of Science and EMBASE for relevant article

Allergic Reactions to Tetracaine-Phenylephrine in Neonatal Lingual Frenotomy: Clinical Insights.

BACKGROUND Infants born with a short lingual frenulum, a condition known as ankyloglossia or tongue-tie, can experience breastfeeding difficulties and often require surgical frenotomy to restore functional mobility. Topical anesthesia is commonly used to minimize discomfort during the procedure, and adverse reactions are rare. This case report describes a 48-day-old female infant who experienced an allergic reaction to topical anesthesia with tetracaine and phenylephrine during frenotomy. CASE REPORT A 48-day-old infant with ankyloglossia and breastfeeding difficulties was referred for special

Association between ankyloglossia and pediatric obstructive sleep apnea: A retrospective cohort study using a global health network.

OBJECTIVE: Pediatric obstructive sleep apnea (OSA) affects an estimated 1-3 % of children, corresponding to over 40 million cases globally. While obesity and adenotonsillar hypertrophy are well-established pediatric risk factors, the potential role of ankyloglossia remains underexplored. We evaluated whether a diagnosis of ankyloglossia is associated with an increased risk of developing OSA in children. METHODS: Retrospective cohort study of 3,535,879 pediatric patients from the TriNetX Global Collaborative Network (143 healthcare organizations). Two cohorts were defined based on the presence
