Investigating the optimum timeline for final cure assessment of treated Visceral Leishmaniasis patients in Bangladesh.

Bangladesh became the first country globally to receive validation for the public health elimination of Visceral Leishmaniasis (VL). However, while VL cases have declined, sequels such as treatment failure and relapse are increasing. The current practice of assessing VL cure at six months post-treatment may delay the identification of sequelae. This study aims to determine the time required for VL patients to achieve complete clinical recovery and parasitological clearance, contributing to sustainable elimination efforts. We enrolled 50 confirmed VL patients from VL-endemic subdistricts of Mym

Sand Fly Fauna (Diptera: Psychodidae): Association Between Climatic Variables and Natural Leishmania Infection in Araçatuba, Brazil.

Visceral leishmaniasis (VL) is a zoonosis of major public health importance. In urban areas, Lutzomyia longipalpis is the primary vector of Leishmania (L.) infantum. This study assessed the seasonality, spatiotemporal distribution, and climatic factors associated with L. longipalpis abundance in Araçatuba, São Paulo State, and detected Leishmania spp. DNA in captured females. Monthly collections were conducted from March 2023 to February 2024 in 72 households across eight urban areas using CDC-type light traps placed indoors and in peridomestic environments. A total of 1641 specimens (1516 mal

Toward Eliminating Leishmaniasis as a Public Health Problem in Yemen: Advocating for a One Health Approach.

Leishmaniasis is among the most neglected tropical diseases in Yemen, where both cutaneous and visceral clinical forms represent a public health challenge. Leishmania tropica is the main cause of cutaneous leishmaniasis, whereas L. donovani and L. infantum cause visceral leishmaniasis in the country. Although leishmaniasis has been recognized for decades, its actual burden in Yemen remains poorly defined, largely due to weak surveillance, constraints in diagnostic services, and the lack of a national control program. Conflict-related disruptions in healthcare, poverty, poor housing, and malnut

Identification of risk areas for visceral leishmaniasis in a municipality in the Triangulo Mineiro region.

BACKGROUND OBJECTIVES: Considered a major public health problem and one of the most neglected tropical diseases, leishmaniasis has long been a challenge for health systems around the world, given its wide geographical distribution and ability to affect populations in situations of socioeconomic vulnerability. The aim of this study was to investigate the phlebotomine fauna and environmental aspects in different ecotopes in Uberaba, Minas Gerais, Brazil and to evaluate peridomiciliary risk factors, as well as to sensitize the population about the disease. METHODS: The systematic collections too

From Pancytopenia to Diagnosis: Visceral Leishmaniasis Identified Through Primary Care Assessment.

Visceral leishmaniasis is a potentially life-threatening systemic parasitic infection caused by protozoa of the genus Leishmania and transmitted by sandflies. Although uncommon in Portugal, it should be considered in the differential diagnosis of prolonged fever associated with pancytopenia and hepatosplenomegaly, even in immunocompetent individuals. We report a case of a previously healthy young adult who initially presented to an urgent Family Medicine consultation in primary care, where pancytopenia was identified and prompted timely referral to secondary care. Diagnosis was confirmed by th

Nucleoside Analogue with Thymidine Nucleobase Inhibits Leishmania infantum and Depolarizes the Plasma Membrane Potential In Vitro.

Visceral leishmaniasis, caused by the protozoan parasites Leishmania infantum or L. donovani, remains a lethal neglected tropical disease without effective therapy. Current drugs available are toxic, leading to severe side effects, treatment abandonment, and resistance, underscoring the urgent need for new therapeutic options. As Leishmania spp. are auxotrophic for purines, they rely on the purine salvage pathway for their nucleotide biosynthesis. Consequently, nucleoside analogues (NAs) represent a promising class of compounds for the design of antiparasitic agents. This study aimed to invest

Multiscale-Aware Graph Embedding Approach Uncovers LC-61, a Potent Anti-Leishmania infantum Compound.

Visceral leishmaniasis caused by Leishmania infantum remains a lethal disease with few therapeutic options, necessitating innovative computational methods and approaches to accelerate drug discovery. Here, we present a graph neural network (GNN) framework incorporating well-established multiscale mechanisms to improve the identification of novel antileishmanial compounds. Across two classificatory antileishmanial data sets, our GNNs demonstrated significant improvements in predictive performance, with area under the receiver operating characteristic curve (AUC) increases of 2.2-29.2% on the im

Successful Treatment of Cutaneous Leishmania infantum Infection with Immunocryosurgery after Failure of the Respective Monotherapies.

Herein, we present a case of cutaneous leishmaniasis successfully treated with a modified immunocryosurgery protocol. A 46-year-old male Caucasian presented with two nodules on the forehead of eight-month duration. He reported an unsuccessful attempt to treat the lesions with a session of cryosurgery elsewhere. During the clinical examination, two adjacent hard mobile nodules that did not infiltrate the underlying tissues were notable. Their diameter was 10 mm and 15 mm, respectively. There were no clinical, laboratory or imaging data in favor of a systemic infection. Histology and polymerase

Characterization of GH18 chitinase in Leishmania braziliensis: expression, structural insights, and implications for vaccine and therapeutic development.

Chitinases, a group of glycosyl hydrolases (GHs), catalyze the degradation of chitin by releasing N-acetylglucosamine subunits. GHs can be found in all three domains of life. Among the three GH families (GH18, GH19, and GH20), GH18 chitinases are the most conserved and extensively studied. These enzymes have been implicated in nutrition, immune modulation, invasion, and virulence across diverse pathogens. In some parasitic protists, GH18 chitinases are essential for transmission. However, in kinetoplastids, including Leishmania spp., these enzymes remain poorly characterized. This study aimed

[Facial granulomatous lesion in a traveler].

We report the case of a 49-year-old man presenting with a 5-month history of an ulcerated frontal plaque, initially interpreted as granulomatous dermatitis. Histopathological re-evaluation, guided by a history of exposure in an endemic area, revealed features consistent with cutaneous leishmaniasis. This case highlights the importance of histopathological analysis in non-endemic settings, as well as key points in the differential diagnosis of granulomatous lesions.

Oral 18-methoxycoronaridine activity in simian and murine Leishmania amazonensis infection.

INTRODUCTION: Leishmaniasis remains a major unmet medical need, with limited oral options and persistent constraints related to efficacy, tolerability, and treatment duration. We investigated 18-methoxycoronaridine (18-MC), an iboga-type indole alkaloid, as an oral candidate against Leishmania amazonensis, integrating rodent and non-human primate efficacy, toxicology, and pharmacokinetics to support clinical translation. METHODS: Antileishmanial activity was first assessed in BALB/c mice treated orally with 18-MC (20 mg/kg/day). Translational efficacy was evaluated in non-human primates recei

Climate change and risk of cutaneous leishmaniasis: an artificial neural network-based approach.

Climate change significantly impacts the prevalence of vector-borne diseases, including cutaneous leishmaniasis (CL). This study evaluates the impact of climate change on CL by artificial neural network (ANN) in the central part of Iran. The Gamma was employed to estimate the least mean squared error and optimal lag time for predictions. General circulation models (GCMs) are essential tools for simulating future climate conditions with two pathways: RCP2.6 and RCP8.5. ANNs were used to simulate the various impacts of climate change on CL infection. We revealed that temperature and precipitatio

Furanocoumarin-rich extracts from Trichocline plicata as natural photosensitizers for the photodynamic inactivation of Leishmania amazonensis: Insights into ROS- and DNA-mediated mechanisms.

Cutaneous leishmaniasis (CL) is a neglected tropical disease with limited therapeutic options due to toxicity, resistance, and high costs. Photodynamic therapy offers a promising alternative; however, the systematic evaluation of furanocoumarin-rich plant extracts in Leishmania models remains limited. Despite the well-established photodynamic properties of individual furanocoumarins, no systematic evaluation of Trichocline plicata extracts has been performed in protozoan parasites. This study aimed to provide an integrated evaluation of the chemical composition, intrinsic photophysical propert

Post-kala-azar dermal leishmaniasis: insights into underlying pathogenic mechanisms and genetic landscape.

Post-apparently successful treatment visceral leishmaniasis (VL), caused by protozoan parasite Leishmania donovani, is often followed by a dermal manifestation among patients known as post-kala-azar dermal leishmaniasis (PKDL). Although non-fatal disorder PKDL manifests itself clinically with a spectrum of cutaneous lesions, including macular, papular, nodular, or polymorphic types, that appear following apparent cure from VL. The absence of reliable non-invasive diagnostic techniques contributes to the underreporting of PKDL, particularly in rural regions. Individuals affected by PKDL may act

Antigenic profiling of Leishmania amazonensis microsomal fraction to support vaccine development.

Vaccination remains the most promising long-term strategy for controlling leishmaniasis, as individuals who recover from primary infection generally develop protective immunity. However, no vaccine is currently available for human use. In this study, we identified two immunogenic proteins that may account for the protective effect previously observed with the Leishmania amazonensis microsomal fraction (MicF), which confers protection in murine models. Both proteins were expressed in Escherichia coli and used to immunize mice, eliciting specific humoral responses and promoting elevated producti

Insights into drug resistance in Leishmania: Mechanisms, therapeutics, and clinical case studies.

BACKGROUND AND PURPOSE: Leishmaniasis, a neglected tropical disease caused by the protozoan parasite Leishmania, remains a significant public health concern in endemic regions. The disease manifests in various forms, including cutaneous, mucocutaneous, and visceral leishmaniasis, each associated with specific Leishmania species and influenced by host immune responses. Over the past few decades, treatment for leishmaniasis has relied on a limited range of drugs, including pentavalent antimonials, amphotericin B formulations, miltefosine, and paromomycin. However, widespread drug resistance, par
