Treatment and Long-Term Outcomes of Children with Co-Occurring Childhood Apraxia of Speech and Velopharyngeal Dysfunction.

A retrospective review of multidisciplinary velopharyngeal insufficiency (VPI) Clinic records from 2010 to 2024 was completed. Inclusion criteria for the review were diagnosis of childhood apraxia of speech (CAS) and documented evaluation for velopharyngeal dysfunction (VPD). Adequacy of velopharyngeal closure and perceptual judgment of resonance were identified as primary outcome measures. Surgical interventions and speech outcomes were also described. This review identified 74 children who were diagnosed with CAS and evaluated for VPD in the VPI Clinic. For the subset (n = 50) of children wh

Standardized Method for Description of Cleft Lip and Palate Phenotypes and Its Implementation in Daily Practice: Validation Study.

BackgroundCleft lip and palate are the most common congenital craniofacial anomalies. There are several classifications of cleft lip and palate that have been proposed over the years; however, only a few have found clinical application. We presented classification of a cleft lip and palate that is comprehensive and uniform, and it can precisely define the vast majority of the clefts.MethodsFive hundred and twenty-one consecutive patients underwent their primary surgical procedure for cleft lip and palate deformities. These intraoperative diagnoses were translated into abbreviated diagnoses bas

Otopalatodigital Syndrome Type 2: A Case Report.

Otopalatodigital syndrome type 2 (OPD2) is a rare, typically lethal X-linked congenital disorder associated with craniofacial, skeletal, and visceral malformations. Because of its rarity, each case provides valuable insights into neonatal care, genetic counseling, and anticipatory guidance. A male infant diagnosed prenatally with OPD2 after ultrasound revealed multiple anomalies, and genetic testing confirmed the pathogenic variant. The mutation was de novo, as there was no family history of X-linked inheritance. The infant exhibited cleft palate, skeletal deformities, omphalocele, thoracic hy

Toward Understanding the Role of miRNAs in Cleft Palate Only: Observations from Patient Tissues and In Vitro Assays.

Cleft palate only (CPO) is a multifactorial craniofacial malformation with significant genetic and epigenetic contributions. Among these, microRNAs (miRNAs) have emerged as key regulators of palate development, although their alterations in CPO remain incompletely characterized. In this study, we performed a comprehensive miRNA expression analysis on palatal tissues from an Italian cohort of non-syndromic CPO patients, compared with a human embryonic palatal mesenchymal (HEPM) cell line. Using the NanoString® nCounter® platform for miRNA profiling, we identified significant deregulation of sev

Parental knowledge and barriers to cleft lip and palate care: a cross-cultural study from the Middle East and South Asia.

BACKGROUND: Orofacial clefts, including cleft lip and/or palate (CLP), are common congenital anomalies that can cause feeding difficulties, speech articulation issues, dental problems, and ear complications. Timely treatment is essential to improve the quality of life for affected children. This study investigates parental knowledge, perceptions, and barriers to care for children with CLP across Saudi Arabia, Jordan, Egypt, and Pakistan. It explores cultural and systemic factors influencing healthcare-seeking behavior and offers recommendations to reduce cross-cultural disparities in awareness

Methods for designing patient-specific templates for optimized fitting of autologous bone augmentations in alveolar cleft osteoplasty.

BACKGROUND: Following the current gold standard, autologous cancellous bone is used as part of alveolar cleft osteoplasty. To fill the bone defects, autologous bone material is harvested from the iliac crest using manual tools such as shepard chisels or trephine drills. The bone augmentations obtained in this way have a simple geometry, usually cylinders, and must then be manually adapted to the defect to be filled by the surgeon using surgical forceps and scissors. There are no established routines for the manufacturing of patient-specific, cost-effective surgical cutting guides. However, the

Enhancing precision or adding complexity? The impact of surgical microscope use in palatal cleft repair: A prospective comparative study.

The use of an operating microscope in cleft palate repair offers improved visualization, enhanced illumination, and ergonomic advantages. However, its clinical adoption remains limited. This study compares surgical efficiency, intraoperative visualization, and complication rates between microscope and surgical loupe use. A prospective, quasi-randomized controlled study was conducted on 40 patients under the age of two years with isolated nonsyndromic secondary incomplete cleft palate. Patients were allocated to either surgical loupe magnification or operating microscope use in an alternating s

Early maxillary arch development in unilateral cleft lip and palate: a retrospective 3D morphometric analysis following nasoalveolar molding and cheiloplasty.

BACKGROUND: Early assessment of maxillary arch development is crucial in infants with unilateral cleft lip and palate (UCLP). This study aimed to evaluate three-dimensional changes in the maxillary arch following modified nasoalveolar molding (NAM) and cheiloplasty using digital model analysis. METHODS: This retrospective study included 25 infants with complete UCLP who underwent modified NAM therapy prior to cheiloplasty. Digital maxillary models were obtained at two time points: T1 on the day of cheiloplasty at 3–6 months of age and T2 on the day of palate repair at 9–18 months of age. Three

A Novel Nonsense Variant in TBX15 Transcription Factor Suggests an Expanded Genetic Spectrum Of Submucous Cleft Palate.

INTRODUCTION AND AIMS: Submucous cleft palate (SMCP) is a congenital craniofacial malformation. While TBX22 is a known pathogenic gene, the potential role of TBX15 in SMCP remains unclear. This study aimed to investigate the genetic etiology of SMCP in a multigenerational family and explore the possible involvement of TBX15. METHODS: Whole-exome sequencing (WES) was performed on a 4-generation Chinese family with SMCP. Functional characterization of the identified variant was conducted through protein localization and transcriptional activity assays. Furthermore, single-nucleus RNA sequencing

Maternal exposure to volatile organic compounds and the risk of congenital anomalies: A systematic review and meta-analysis.

Volatile organic compounds (VOCs) are widespread environmental and occupational pollutants with potential teratogenic effects. Growing evidence indicates that maternal exposure to VOCs during pregnancy may increase the risk of congenital anomalies in offspring. This systematic review aims to evaluate and synthesize the association between maternal exposure to benzene, toluene, xylene, and formaldehyde during pregnancy and the occurrence of congenital anomalies, including neural tube defects (NTDs), congenital heart defects (CHDs), and cleft lip and/or palate (CL/P). A systematic search was con

National prevalence of postoperative opioid prescription following cleft palate repair.

The prevalence of postoperative opioid prescribing following palatoplasty is largely unknown. We queried the National Surgical Quality Improvement Program-Pediatric (NSQIP-P) 2023 database to evaluate opioid prescribing at discharge and perioperative factors associated with opioid use in children aged 0-36 months undergoing palatoplasty. A total of 2026 patients (mean age 13.5 ± 4.6 months) were included. Approximately half of the patients (52.6%) received an opioid prescription at discharge, most commonly oxycodone (73.7%). Multivariable analysis demonstrated that longer anesthesia duration (

Effect of Nasoalveolar Molding on Skeletal Growth in Individuals With Bilateral Cleft Lip and Palate During the Mixed Dentition Stage.

BACKGROUND: Nasoalveolar molding (NAM) is widely used to improve surgical outcomes in cleft lip and palate by reshaping nasal cartilage and aligning the alveolar cleft. However, its lasting impacts on skeletal growth in individuals with bilateral cleft lip and palate (BCLP) remain unclear. METHODS: This retrospective study reviewed individuals with nonsyndromic BCLP born between 1997 and 2017. Two groups were compared: NAM-treated (n=33) and control (n=45). Lateral cephalometric x-rays taken in the mixed dentition before orthodontic intervention or alveolar bone grafting were compared between

Acute airway obstruction in a paediatric patient with Wolf-Hirschhorn syndrome requiring emergency tracheostomy.

Wolf-Hirschhorn syndrome (WHS) is an uncommon genetic disorder that often presents with characteristic facial features, developmental delay and potential airway anomalies, which can complicate the anaesthetic management. We present a case of WHS in early childhood who came with severe upper airway obstruction due to grade 4 adenotonsillar hypertrophy and required an emergency tracheostomy. The situation was made even more challenging by craniofacial abnormalities, a submucosal cleft palate and a history of laryngomalacia. Anaesthesia was induced with inhalational sevoflurane and intravenous ke

Double Opposing Z-plasties to Repair a Cleft Palate: A Personal Account.

Dr. Leonard T. Furlow Jr.'s description of the double-opposing Z-plasty remains one of the most impactful contributions to cleft palate surgery, redefining expectations for velopharyngeal competence and functional outcomes. In this reflective manuscript originally published by Advances in Plastic and Reconstructive Surgery in 1998, Dr. Furlow offers a rare, personal account of how the operation emerged-not from a single breakthrough, but from years of surgical observation, mentorship, and an evolving understanding of palatal anatomy and Z-plasty biomechanics. His narrative traces the transitio
