Temporomandibular joint morphology in cleft lip and palate: evidence from a CBCT-based volumetric analysis.

BACKGROUND: Cleft lip and palate (CLP) may influence mandibular development and temporomandibular joint (TMJ) morphology; however, evidence on this association in adults remains limited. This study aimed to investigate condylar and glenoid fossa morphology in adults with CLP using cone-beam computed tomography (CBCT). METHODS: CBCT scans of 27 adults with CLP and 34 age- and sex-matched controls were retrospectively analyzed. Volumetric and linear measurements of the mandibular condyle and glenoid fossa were obtained using ITK-SNAP software by manual segmentation. Condylar volume, height, wid

Recombinant Human Bone Morphogenetic Protein-2 (rhBMP-2) as an Alternative to Autologous Bone Graft in Pediatric Craniofacial Reconstruction: A Systematic Review.

BACKGROUND: Craniofacial reconstruction in pediatricpatients commonly uses autologous bone grafts, such as iliac crest bone grafts (ICBG). Recombinant human bone morphogenetic protein-2 (rhBMP-2) infused allografts may reduce operative time and donor-site morbidity. This review assesses whether rhBMP-2 is a viable alternative. METHODS: A literature search of PubMed, Ovid, and Web of Science (2008-2023) identified studies on rhBMP-2 in pediatric craniofacial reconstruction. Randomized controlled trials, prospective or retrospective case series, and case reports were included; reviews, animal,

Case Reviews for Two Families With Unique Variants in TBX22 Causing Abruzzo-Erickson Syndrome.

The purpose of this study is to explore the phenotypic spectrum observed in individuals and between families with confirmed variants in the T-Box Transcription Factor 22 gene (TBX22). Pathogenic variants in TBX22 have been identified in individuals with classic X-linked cleft palate (CPX) and also in Abruzzo-Erickson Syndrome (ABERS). We compare the phenotypic features of a newly suspected family with ABERS to those of the original family with ABERS to help determine if family-specific pathogenic variants in TBX22 are the cause of ABERS. Furthermore, we discuss possible mechanisms of action of

Life Beyond Surgery: Postoperative Quality of Life in Patients With Cleft Lip/Palate.

BACKGROUND: Cleft lip/palate (CL/P) affects patients' appearance, function, and mental health. The CLEFT-Q is the first condition-specific tool to assess quality of life (QoL) in CL/P patients, yet limited data exist on postoperative outcomes, especially in nonwestern populations. PURPOSE: The purpose of this study was to measure the QoL of CL/P patients who had undergone secondary surgical correction for deformities following cheiloplasty. STUDY DESIGN, SETTING, SAMPLE: A prospective cross-sectional study was conducted at the Plastic Surgery Hospital from July 2023 to June 2024, enrolling i

Association of the Disheveled 2 (DVL2) Gene c.2044delC Variant with Increased Risk of Canine Cleft Palate.

Canine congenital cleft palate is one of the most common craniofacial anomalies in dogs, characterized by a failure of the palatal shelves to fuse properly during fetal development, leading to abnormal communication between the oral and nasopharyngeal cavities. Patients with cleft palate can experience significant challenges including feeding difficulties, aspiration pneumonia, and respiratory distress, all of which can profoundly impact quality of life and, in severe cases, survival. The clinical management of cleft palate in dogs involves a combination of medical and surgical interventions.

Multiple congenital malformations in a litter of feline fetuses associated with gestational exposure to a synthetic estrus-suppressing progestin.

BACKGROUND: Congenital malformations in domestic cats are traditionally considered uncommon; however, recent evidence suggests they may represent an underrecognized cause of neonatal mortality. Most reported anomalies occur as isolated defects in individual animals, while the occurrence of multiple severe malformations within the same fetus or affecting an entire litter appears to be rare. Exposure to exogenous substances during critical windows of embryonic development has been proposed as a potential contributing factor. Reports describing congenital abnormalities in feline fetuses following

Primary Language Spoken at Home and Speech Outcomes Among Children With Cleft Palate.

OBJECTIVES: Following cleft palate repair, early detection of maladaptive compensatory mechanisms and hypernasality may determine the need for additional intervention. Our goal is to understand if primary language influences rates of subsequent intervention among children with cleft palate in the United States. METHODS: Within a single urban cleft team, patients with a history of palate repair evaluated at least 2 years postoperatively were analyzed using Fisher's exact test. RESULTS: There were 124 patients representing 6 languages: 101 English, 9 Spanish, and 14 other (Vietnamese, Haitian

A comparative analysis of secondary unilateral and bilateral cleft lip nasal deformities: From anatomical characteristics to repair strategies.

Secondary cleft lip nasal deformity presents a persistent challenge in reconstructive surgery, with fundamental differences existing between unilateral and bilateral presentations that dictate distinct management approaches. This systematic review delineates the contrasting anatomical underpinnings of these deformities, where unilateral cases are characterized by marked asymmetry and bilateral cases by a symmetric deficiency marked by severe columellar shortening. It further synthesizes the evolution of assessment methodologies, from traditional anthropometry to advanced three-dimensional (3D)

Perceptions of Interdisciplinary Cleft Treatment: A Qualitative Pilot Study of Children With Cleft Palate and Their Parents.

BACKGROUND: Treating children with a cleft palate with or without a cleft lip (CP±L) requires an interdisciplinary approach. While this coordinated, long-term treatment approach intends to enhance children's quality of life (QoL), it may still be perceived as burdensome for the children and their environment. AIMS: This study investigated the perspectives of both children with a CP±L and their parents, with a particular focus on the psychological and financial impact of interdisciplinary cleft treatment. METHODS: A total of 10 parents (mean age of 48.0 years) and 7 children with CP±L (mean a

Timing of Secondary Palate Fusion in Human Embryos and Fetuses.

INTRODUCTION: The human face develops through a complex sequence of growth and fusion events involving multiple pharyngeal arch derivatives. Disruptions in these processes can result in congenital anomalies such as cleft lip and/or palate (CL/P), which occur in approximately 1.6 per 1000 live births in Europe. Patients with CL/P often experience difficulties regarding feeding and speech, and studies suggest an increased risk of psychosocial difficulties. Understanding the morphological development and timing of secondary palate formation is essential for clarifying the pathogenesis of CL/P and

Let-7c-5p Drives Collagen Gene Repression Linking Phenobarbital Teratogenicity to Cleft Palate.

The treatment of seizures during pregnancy remains challenging due to the teratogenic effects of many anticonvulsants. Phenobarbital (PB), despite its known risks, is widely used and associated with congenital malformations such as cleft palate (CP). CP is a common craniofacial defect resulting from disrupted palatogenesis, influenced by genetic and environmental factors, including maternal medication. miRNAs are key post-transcriptional regulators implicated in palate development and possibly modulated by teratogenic agents. This study investigated whether miRNA dysregulation observed in CP p

Behind the Screens-Social Media and Psychosocial Outcomes in Adolescents With Cleft Lip/Palate.

BACKGROUND: Social media influences adolescent self-image and confidence, yet its impact on psychosocial outcomes in adolescents with cleft lip and/or palate remains poorly defined. This population may be particularly vulnerable given the importance of craniofacial features to self-perception during adolescence. Negative self-perceptions have been linked to psychosocial distress and interest in revisional surgery, whereas social media may also provide peer support and cleft-related information. This study examined social media use patterns and their association with psychosocial functioning in

A cost analysis of speech-language pathology applications for telepractice during the COVID-19 pandemic.

BACKGROUND: Cleft lip and palate are common congenital anomalies that impose significant healthcare and economic burdens. The COVID-19 pandemic disrupted conventional speech-language therapy, necessitating the use of telepractice interventions. The Application for Articulation Therapy-Thai (AAT-T) was developed to address these limitations. This study aimed to estimate the economic costs and resource use associated with delivering AAT-T during the pandemic. METHODS: A descriptive micro-costing analysis was conducted using retrospective data from a clinical efficacy trial of AAT-T performed bet
