HOXA3 Knockdown Impairs hESC-Derived Thymic Epithelial-like Cell Differentiation and CXCL12-Associated Early T-Lineage Support.

Thymic epithelial cells (TECs) provide essential signals for T-cell development, yet the mechanisms governing human TEC differentiation and the acquisition of T-cell-supporting function remain incompletely understood. Using a three-stage system that directs human embryonic stem cells (hESCs) through definitive endoderm and third pharyngeal pouch endoderm (3PPE) toward a thymic epithelial-like lineage, we investigated the role of HOXA3 during thymic epithelial-like cell (TEC-like cell) differentiation. HOXA3 knockdown during 3PPE-to-TEC-like cell transition impaired thymic epithelial differenti

Zebrafish hoxba and hoxbb clusters are redundantly required for posterior pharyngeal pouch development.

BACKGROUND: Pharyngeal pouch formation is fundamental to vertebrate craniofacial development, but molecular mechanisms underlying posterior pouch formation remain incompletely understood. Retinoic acid (RA) signaling is required for posterior pharyngeal development, and Hox genes are established RA targets. However, their genetic requirement in posterior pouch formation has not been established. RESULTS: Zebrafish lacking both hoxba and hoxbb clusters-teleost-specific duplicates of the ancestral HoxB cluster-exhibited severe loss of posterior pharyngeal pouches and associated pharyngeal skele

Coral rubble facilitates feeding opportunities for invertivorous reef fishes on tropical coral reefs.

BACKGROUND: Endoscopic stapling to treat pharyngeal pouch is a relatively new technique with the potential to reduce the morbidity associated with the open approach for pharyngeal pouch. Despite enthusiasm for the endoscopic approach there have been no series reported in Australia, and descriptions of outcomes and benefits are currently anecdotal. The aim of the present study was to determine the outcome associated with endoscopic stapling of pharyngeal pouch in an Australian setting. METHODS: All patients admitted for endoscopic stapling for a pharyngeal pouch between 1998 and 2002 by surgeo

Paxillin is crucial for thymus and parathyroid development by regulating the architecture of the third pharyngeal pouch endoderm.

The paxillin (PXN) protein is a key component of focal adhesions in which it primarily functions as a molecular scaffold to spatiotemporally integrate diverse signalling networks to transduce intracellular responses. In this study, using loss-of-function genetics in mice, we investigated whether the Pxn gene has a role in the morphogenesis of the pharyngeal apparatus and whether it interacts with the Tbx1 gene, which encodes a key transcription factor required in pharyngeal development. Conditional deletion of Pxn in the Tbx1 expression domain did not cause cardiac defects. Instead, the germli

Resolving thyroid lineage cell trajectories merging into a dual endocrine gland in mammals.

The thyroid has a remarkable evolution, transforming from an exocrine constituent of the chordate endostyle to an endocrine gland in basal vertebrates. In mammals, a second endocrine cell type of presumed neural crest origin appears, although recent lineage tracing has firmly established thyroid C-cells are also endoderm-derived. Here, we characterize the global gene expression in both embryonic thyroid lineages at single-cell level and identify lineage-specific transcription factors and their network regulation of target genes implicated in thyroid development into a dual endocrine organ. A Multimodal Morphological and Immunohistochemical Analysis of the Human Thymus-Parathyroid Relationship.

Although the thymus and parathyroid glands originate from the third pharyngeal pouch, their postnatal relationship has been predominantly interpreted in embryological terms, with limited structural evidence supporting a functional interaction. The aim of this study was to explore whether multimodal observations support a previously underrecognized structural relationship between the thymus and parathyroid glands. For this purpose, microCT-based three-dimensional reconstruction was used to assess their spatial and branching high-attenuation structures, and the findings were correlated with hist

Hypoparathyroidism with a TBX1 variant: reconsidering genetic etiologies in the absence of 22q11.2 deletions.

Hypoparathyroidism is most commonly postsurgical, but autoimmune and genetic causes should be considered in the absence of prior neck surgery. T-box transcription factor 1 (TBX1) is a dosage-sensitive regulator of pharyngeal pouch and parathyroid development. While DiGeorge syndrome classically results from a 22q11.2 deletion, emerging evidence suggests that heterozygous TBX1 variants may cause deletion-negative, partial phenotypes with variable penetrance. We report a 25-year-old man with congenital sensorineural hearing loss who presented with chronic hypoparathyroidism after an initial hypo

A Transcriptomic Analysis of Human iPSC-Derived Parathyroid Lineage Cells Reveals Limited Maturation Beyond the Parathyroid-Thymic Primordium.

Several protocols have been reported for inducing parathyroid differentiation from human pluripotent stem cells in vitro. However, the efficiency of terminal differentiation into mature parathyroid cells remains limited. In this study, we performed comparative transcriptomic analyses of parathyroid lineage-induced human induced pluripotent stem (iPS) cells to clarify their developmental stage identity. Comparative analyses with adult parathyroid adenoma, used as a surrogate reference for mature parathyroid cells, and thymuses revealed that the differentiated cells exhibited gene expression pat

Three-dimensional structure and epithelial changes of the piriform sinus fistula in cotton rats.

Piriform sinus fistula (PSF) is a rare congenital anomaly derived from the third or fourth pharyngeal pouch. However, its developmental mechanisms remain poorly understood owing to the absence of suitable animal models. Cotton rats (Sigmodon hispidus) spontaneously develop PSF, providing a unique opportunity to investigate its anatomical origin and postnatal remodeling. In this study, we performed a three-dimensional reconstruction of serial histological sections from two inbred strains, HIS/Mz with third-pouch-derived PSF and HIS/Hiph with fourth-pouch-derived PSF. The third-pouch PSF opened

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Pax1a-EphrinB2a pathway in the first pharyngeal pouch controls hyomandibular plate formation by promoting chondrocyte formation in zebrafish.

The hyomandibular (HM) cartilage securing the lower jaw to the neurocranium in fish is a craniofacial skeletal element whose shape and function have changed dramatically in vertebrate evolution, yet the genetic mechanisms shaping this cartilage are less understood. Using mutants and rescue experiments in zebrafish, we reveal a previously unappreciated role of Pax1a in the anterior HM plate formation through EphrinB2a. During craniofacial development, pax1a is expressed in the pharyngeal endoderm from the pharyngeal segmentation stage to chondrocyte formation. Loss of pax1a leads to defects in

Zenker Peroral Endoscopic Myotomy Is Safe and Effective for the Management of Pharyngeal Pouch: A Multicentre Retrospective Cohort Study.

UNLABELLED: <p>Introduction: A Zenker diverticulum (ZD) is a sac-like outpouching at the pharyngo-oesophageal junction, which is characterised by dysphagia and regurgitation. Surgery has been the predominant treatment but carries the burden of invasiveness, complications, and recurrence. Zenker peroral endoscopic myotomy (Z-POEM) is an alternative minimally invasive endoscopic treatment for ZD. We report on the safety and efficacy of Z-POEM in the UK. METHODS: Consecutive patients undergoing Z-POEM by expert interventional endoscopists at four UK referral centres were retrospectively analysed

Video Capsule Endoscopy: Think Pharyngeal Pouch - Retention of a Video Capsule in a 94-Year-Old in the Upper Gastrointestinal Tract.

Video capsule endoscopy (VCE) provides endoluminal visualisation of the small bowel. It is often regarded as well-tolerated, non-invasive, and safe across all age groups, with the main, albeit rare, complication of retention of the capsule - normally in the small bowel or lower gastrointestinal (GI) tract. Retention in the upper GI tract is extremely rare; here, we present a case of a 94-year-old who experienced capsule retention in a previously undiagnosed pharyngeal pouch. This appears to be the first case report detailing video capsule retention in a pharyngeal pouch involving a patient of

Beyond cysts - organization of epithelial networks in the murine thymus.

The thymus originates from the third pharyngeal pouch endoderm, which also gives rise to respiratory tract elements. Here, we examined intrathymic cystic structures, long considered remnants of organogenesis. Through sequential histology and ultrastructural imaging, we uncovered that these 'cysts' are in fact continuous and structured epithelial networks embedded within the thymic parenchyma. These networks follow a conserved 'head-neck-funnel-tentacle' architecture spanning the trabeculae, cortex, corticomedullary junction (CMJ) and medulla. The head, typically glandular and ciliated, connect

Fate and Specification of the Ventral Endoderm in and Outside the Foregut of the Avian Embryo at 9-12 Somite Stage.

The fate and specification of the endoderm lining the ventral foregut and outside the foregut cranial to the anterior intestinal portal of the 9-12 somite stage avian embryo were analyzed to determine whether this endoderm was specified according to its fate. The ventral foregut endoderm contributed to the ventral endoderm of the gut from the pharynx to the duodenum, and to the thyroid, respiratory organ, and liver. Pre-pharyngeal cells were distributed throughout the ventral foregut, with pre-thyroid cells lying medially in the anterior to middle part and pre-pharyngeal pouch cells lying late

Cross-Expression of Thymic and Parathyroid Hormone Receptors Supports the Hypothesis of a Parathyroid-Thymus Port System.

The thymus and parathyroid glands share a common embryological origin from the third pharyngeal pouch, yet their potential morphological and functional interconnections remain insufficiently explored. We conducted a comparative study integrating immunohistochemistry (IHC) and SEM on human thymic tissue, parathyroid adenomas, and parathyroid tissue excised during thyroidectomy. IHC staining targeted Thymosin-α1, CaSR, and PTH1R, with semi-quantitative evaluation of staining intensity and distribution. SEM analysis was performed at multiple magnifications to assess stromal organization and micro

HOXA3 functions as the on-off switch to regulate the development of hESC-derived third pharyngeal pouch endoderm through EPHB2-mediated Wnt pathway.

OBJECTIVES: Normal commitment of the endoderm of the third pharyngeal pouch (3PP) is essential for the development and differentiation of the thymus. The aim of this study was to investigate the role of transcription factor HOXA3 in the development and differentiation of 3PP endoderm (3PPE) from human embryonic stem cells (hESCs). METHODS: The 3PPE was differentiated from hESC-derived definitive endoderm (DE) by mimicking developmental queues with Activin A, WNT3A, retinoic acid and BMP4. The function of 3PPE was assessed by further differentiating into functional thymic epithelial cells (TEC

Squamous Cell Carcinoma Arising in a Zenker's Diverticulum.

Squamous cell carcinoma (SCC) developing in a Zenker's diverticulum (ZD) is an uncommon condition. The preferred treatment for SCC in the pharyngeal pouch is complete diverticulum resection. Only histopathological evaluation of the pouch can rule out SCC. Here, we present a case of a 62-year-old male patient, who was evaluated for repeated episodes of aspiration and dysphagia, and diagnosed to have a large ZD, the patient underwent Zenker's diverticulectomy with cricopharyngeal myotomy with wide margins due to clinically suspicious specimen. A Potential Role of fgf4, fgf24, and fgf17 in Pharyngeal Pouch Formation in Zebrafish.

In vertebrates, Fgf signaling is essential for the development of pharyngeal pouches, which controls facial skeletal development. Genetically, fgf3 and fgf8 are required for pouch formation in mice and zebrafish. However, loss-of-function phenotypes of fgf3 and fgf8 are milder than expected in mice and zebrafish, which suggests that an additional fgf gene(s) would be involved in pouch formation. Here, we analyzed the expression, regulation, and function of three fgfs, fgf4, fgf24, and fgf17, during pouch development in zebrafish. We find that they are expressed in the distinct regions of phary

The safety and efficacy of endoscopic approaches for the management of Zenker's diverticulum: a multicentre retrospective study.

INTRODUCTION: Minimally invasive endoscopic options are safe and effective alternatives to surgery for the treatment of symptomatic Zenker's diverticulum (ZD). However, there is no consensus on the gold-standard approach. We compared the safety and efficacy of Zenker's peroral endoscopic myotomy (Z-POEM), flexible diverticulotomy (FD), and rigid diverticulotomy (RD) for the management of ZD. METHODS: Patients undergoing treatment for ZD at three UK tertiary referral centres were identified and analysed between 2013 and 2023. Patient demographics, procedural details, clinical success, and 30-d
