Four cases of bilateral congenital choanal atresia are presented. Three of them (75 per cent) were shown by Electric Response Audiometry (ERA) tests to have, in addition, congenital nerve deafness; and two had other congenital anomalies. Thus, congenital nerve deafness should be considered as a possible congenital anomaly associated with choanal atresia. The embryology of the branchial (pharyngeal) region is reviewed in terms of the embryonic pharynx, branchial apparatus and derivatives, tongue, larynx, and trachea. Derivatives of the branchial pouches, grooves (clefts), and arches are analyzed in relation to each other and to the first arch syndrome. Other common head and neck defects attributable to abnormal development of the branchial region are also discussed, particularly those defects involving the incomplete degeneration or the abnormal migration of branchial tissues.

Three cases of congenital palatal incompetence in the absence of the usual stigmata associated with hypernasality are presented. In two instances, mucosal webbing of the posterior pillars was noted and in one case the palatopharyngeus muscle was displaced. Hypernasal voice quality was apparent in each patient. A z-plasty on both sides of the posterior pillars is suggested as the treatment of choice when no other morphologic abnormalities are present.

The superiorly based and inferiorly based posterior pharyngeal flaps are compared. Reasons for choosing one or the other in specific cases are given. The complications with each are enumerated. The results obtained in speech comparing the two flaps in two retrospective series and one prospective series of cases failed to show a significant difference between them. Therefore, with little difference in the results and some good reasons for preferring one or the other under certain conditions, it would seem logical to select the one best suited for the particular problem.

Ninety-eight cleft palate subjects were studied to ascertain the eventual adolescent and adult hearing. There were 72 complete clefts of lip and palate and 26 clefts of the palate alone. The incidence or side of ear disease did not appear to be influenced by this type of cleft. Five patients evaluated via multi-view videofluoroscopy were found to have incongruous movements between the velum and lateral aspects of the pharyngeal walls. A case of cor pulmonale occurred as a complication of a simultaneous palatoplasty and a superiorly-based pharyngeal flap. The patient, like two others found in the literature, had micrognathia. The cor pulmonale was not recognized in the immediate postoperative period but presented two years later with "failure to thrive." Therefore, surgeons must make other members of the health care team aware of this possible complication of combined palatoplasty and pharyngeal flap operations.

The important otolaryngological manifestations of arthrogryposis multiplex congenita (AMC), which heretofore have not been described in otolaryngological and other specialty journals, are reviewed. Thirty-seven patients with AMC were studied. Nine of these patients with neurogenic AMC, had otolaryngological manifestations. Six of these had Pierre Robin-like syndromes. During the past three years, forty-eight patients, ranging from five to thirty years of age, have undergone bilateral transverse pharyngeal pedicles to the palate. Thirty-nine patients had not had pharyngeal flaps prior to this procedure and the remaining nine patients had failure of production of acceptable speech after undergoing inferiorly or superiorly based flap operations. The question posed in this study was, are there measures which are predictive of the need for further palatal management? Articulation scores, lateral x-rays and clinical judgments of velopharyngeal competency were examined for 75 subjects in retrospect to determine their predictive value. Each of the measures resulted in at least 90% appropriate predictions. The combination of articulation score and lateral x-ray rating appeared to be the best predictor for this particular sample in that subsequent treatment was correctly predicted for 96% of the subjects. In an attempt to minimize postoperative scar contracture and shrinkage after a pharyngeal flap operation, a folded flap operation was devised. Twenty-four patients were evaluated for voice quality after primary palatal repair by the Millard island flap procedure.
