Neural Crest.

This chapter discusses the role of cardiac neural crest cells in the formation of the septum that divides the cardiac arterial pole into separate systemic and pulmonary arteries. Further, cardiac neural crest cells directly support the normal development and patterning of derivatives of the caudal pharyngeal arches, including the great arteries, thymus, thyroid, and parathyroids. Recently, cardiac neural crest cells have also been shown to indirectly influence the development of the secondary heart field, another derivative of the caudal pharynx, by modulating signaling in the pharynx. The con

Palatal dysgenesis in a feline causing complete obstruction of the nasopharynx: A case report.

The nasopharyngeal disease is common in felines. Nasopharyngeal stenosis is uncommonly a congenital problem, with most cases being secondary to other diseases. An 8-month-old male neutered domestic shorthair presented with a chief complaint of chronic nasal congestion, open-mouth breathing, and discharge. CT was performed, and palatal dysgenesis resulting in complete nasopharyngeal obstruction, secondary nasopharyngitis, and rostral nasal turbinate lysis were found. This was confirmed by an endoscopic evaluation of the nasopharynx region, which demonstrated a lack of connection between the nas

Neural network analysis of pharyngeal sounds can detect obstructive upper respiratory disease in brachycephalic dogs.

Brachycephalic obstructive airway syndrome (BOAS) is a highly prevalent respiratory disease affecting popular short-faced dog breeds such as Pugs and French bulldogs. BOAS causes significant morbidity, leading to poor exercise tolerance, sleep disorders and a shortened lifespan. Despite its severity, the disease is commonly missed by owners or disregarded by veterinary practitioners. A key clinical sign of BOAS is stertor, a low-frequency snoring sound. In recent years, a functional grading scheme has been introduced to semi-objectively grade BOAS based on the presence of stertor and other abn

Palatal relaxing incisions versus nasopharyngeal relaxing incisions in Palatoplasty.

BACKGROUND: Bone denudation after conventional relaxing incisions could be a critical factor in inhibiting maxillofacial growth. To address this, alternative relaxing incisions were designed. Thus, this study aimed to compare the effectiveness of palatal relaxing incisions versus nasopharyngeal relaxing incisions in enhancing postoperative outcomes. MATERIALS AND METHODS: A retrospective cohort study was conducted, involving a total of 120 patients divided into three groups: 40 patients have received modified Furlow palatoplasty with nasopharyngeal relaxing incisions (M.F + N.P.I palatoplasty

Differential Diagnosis of a Pharyngeal Fricative and Therapeutic Monitoring of Velopharyngeal Function Using Magnetic Resonance Imaging.

PURPOSE: Speech disorders associated with velopharyngeal dysfunction (VPD) are common. Some require surgical management, while others are responsive to speech therapy. This is related to whether the speech error is obligatory (passive) or compensatory (active). Accurate identification of speech errors is necessary to facilitate timely and appropriate intervention. Recent studies have supported the role of magnetic resonance imaging (MRI) in the assessment process for VPD. The purpose of this study was to utilize MRI to support differential diagnosis and treatment planning in a child presenting

Linguistic validation of a questionnaire for the screening of OSAS in a paediatric population with Down Syndrome.

AIM: Obstructive sleep apnoea syndrome (OSAS) is a common clinical condition in which the throat narrows or collapses repeatedly during sleep, causing obstructive sleep apnoea events. This disorder is present in the paediatric population with a prevalence estimated between 1.2% and 5.8%. Down Syndrome (DS) is the most common chromosomal alteration associated with mental disability and characterised by other clinical manifestations, and its incidence is estimated at 1/800 births worldwide. Most of the craniofacial features typical of DS represent a risk factor for the development of OSAS. Routi

Cranial Versus Cervical Spine Magnetic Resonance Imaging in Adult Chiari Malformation Type I Diagnostics: Is There a Difference in Tonsil Ectopia Length?

BACKGROUND: Chiari malformation type 1 (CM-1) diagnosis is based on measurement of the cerebellar tonsils on cranial or cervical spine magnetic resonance imaging (MRI). However, imaging parameters of cranial and cervical spine MRI could differ because spine MRI has greater resolution. METHODS: We conducted a retrospective chart review of 161 patients of a single neurosurgeon for adult CM-I consultation between February 2006 and March 2019. Patients were selected based on receiving both cranial and cervical spine MRI within a month of each other to determine tonsillar ectopia length for CM-1.

IS SURGICAL TREATMENT OF VELOPHARYNGEAL INSUFFICIENCY BY MYOMUCOSAL POSTERIOR PHARYNGEAL FLAP A CONTRAINDICATION FOR NASAL INTUBATION? A NARRATIVE REVIEW.

Velopharyngeal insufficiency is a disorder where the soft palate directs the air through the nose. It is often present in patients with previous cleft or short palate, but also in many other conditions. Symptoms are primarily to be found in speech, with very distinct nasal sound. After clinical evaluation and nasal endoscopy, surgery is considered. Several surgical techniques are in use, with posterior pharyngeal flap pharyngoplasty being most widely used. This method leaves the base of the posterior pharyngeal flap attached to the posterior pharyngeal wall, with two lateral ports on each side

Prenatal diagnosis of pyriform sinus fistula using ultrasonography by detecting the communication to the pharynx.

OBJECTIVE: Pyriform sinus fistula (PSF) is a congenital anomaly which originates from the pharyngeal pouch. PSF is initially recognized as a cyst around the fetal neck, but accurate prenatal diagnosis of the disease is challenging. We aimed to report the key findings and tips in accurately distinguishing PSF from other differential diagnosis by which enables detection of the communication of the nuchal cyst and the pharynx. CASE REPORT: We report a case in which we were able to diagnose PSF as early as 18 weeks of gestation with ultrasonography. We used epiglottis as a landmark, and detected

Reliability and Agreement in the Cerebellar Tonsil Tip Localization: Two Methods Using the McRae Line Concept in MRI.

BACKGROUND: The cerebellar tonsil tip position (TP) is a common parameter used for the radiologic diagnosis of Chiari malformation type 1 (CM1). However, these paramedian structures are usually not properly visualized in the midsagittal section. Such mismatch may be a source of bias in TP measurements based on the McRae line (ML) traced between median craniometric points. This study aims to evaluate the intraoperator and interoperator reliability and agreement of 2 protocols to trace the ML in magnetic resonance imaging (MRI) for the cerebellar tonsil tip localization, adding a 3-dimensional m

Patterns of velopharyngeal closure during speech in individuals with normal habitual resonance: A nasoendoscopic analysis.

OBJECTIVE: Velopharyngeal valve closure is essential for adequate speech intelligibility as well as for other activities. The variations in the contribution of different components of the velopharyngeal port walls produce different closure patterns. The aim of this cross-sectional study is to identify the prevalence of the different velopharyngeal closure patterns in Arabic-speaking individuals with no perceived hypernasality or velopharyngeal dysfunction. METHODS: After verification of selection criteria, 100 subjects with age range between 15 and 60 years underwent nasoendoscopic examinatio

Verifying Efficacy of Self-Inhalation Training for Velopharyngeal Dysfunction.

UNLABELLED: Velopharyngeal closure in healthy adults during different tasksObjective: Velopharyngeal dysfunction causes not only resonance problems (so-called "hypernasality") but also dysphagia, particularly in the elderly. In our previous study, we developed a new inhaling training method to objectively improve velopharyngeal function using the measurement of peak inspiratory flow (PIF) rate, which was effective in all patients. In this study, we clarify the degree of velopharyngeal closure to determine the efficacy of our training to improve the closure mechanism. METHODS: Three healthy vo
