The impact of right-sided aortic arch on surgical outcomes in esophageal atresia: A systematic review.

OBJECTIVE: The management of esophageal atresia and tracheoesophageal fistula (EA/TEF) in the presence of a right-sided aortic arch (RAA) remains a significant surgical challenge. This systematic review aims to analyze the diagnostic trends, surgical approaches, and clinical outcomes for EA/TEF patients with an associated RAA. METHODS: A comprehensive and systematic literature search was performed across major electronic databases, including PubMed, MEDLINE and Embase and systematic review was conducted in accordance with PRISMA guidelines, evaluating studies published between January 2000 an

Health-related quality of life following thoracoscopic repair in children with long-gap esophageal atresia.

BACKGROUND: By retaining the native esophagus and minimizing postoperative morbidity, the thoracoscopic approach contributes to improved functional outcomes for patients with esophageal atresia (EA). The aim of this study was to evaluate HRQoL-including digestive, respiratory, and feeding outcomes in children treated exclusively by a thoracoscopic approach, with a specific focus on comparing outcomes between patients with LGEA (Gross type A and B) and those with DTEF (Gross type C or D). MATERIALS AND METHODS: LGEA was treated using the internal traction technique with staged repair, 25 child

Congenital Esophageal Stenosis Presenting During Weaning in a 5-Month-Old Infant.

BACKGROUND/OBJECTIVES: Congenital esophageal stenosis (CES) is a rare condition that may present with feeding difficulties in infancy, often becoming evident during weaning. We report a case of CES presenting with progressive dysphagia during the introduction of solid foods. METHODS: Clinical evaluation, contrast esophagography, endoscopy, and magnetic resonance imaging were performed to characterize the stenosis and guide management. RESULTS: Imaging revealed a fixed narrowing of the mid-distal esophagus with minimal passage of contrast and proximal dilatation. Endoscopy confirmed a non-tra

Feasibility of magnetic compression-mediated transection of rabbit xiphoid cartilage: A simulation study for tracheobronchial remnant-type congenital esophageal stenosis.

BACKGROUND: Tracheobronchial remnant (TBR)-type congenital esophageal stenosis (CES) is a rare infantile anomaly characterized by ectopic tracheobronchial cartilage within the esophageal wall. Magnetic compression devices are effective for soft-tissue approximation, but their ability to transect cartilage remains unproven. This study assessed magnetic compression-mediated transection of rabbit xiphoid cartilage. METHODS: Eighteen rabbits were divided into control, sham-operated, and experimental groups (n = 6 each). Experimental rabbits received magnetic compression devices to compress the xi

Successful one-stage thoracoscopic repair of esophageal atresia with tracheoesophageal fistula and associated foregut duplication cyst in a neonate: a case report and review of the literature.

BACKGROUND: Esophageal atresia with tracheoesophageal fistula (EA-TEF) complicated by foregut duplication (FD) cyst is an exceedingly rare congenital anomaly. Since the first description in 1956, only 20 cases have been reported, with open thoracotomy representing the standard surgical approach. We present a case of successful one-stage thoracoscopic repair of this complex association in a neonate. CASE PRESENTATION: A male neonate weighing 2080 g was diagnosed with Gross type C EA-TEF. At 3 days of age, right thoracoscopic exploration identified a 1-cm FD cyst on the distal esophageal wall.

Robotic-Assisted Thoracoscopic Circumferential Resection of Congenital Esophageal Stenosis in Under 10 kg Patient.

Congenital esophageal stenosis is a rare and heterogenous malformation often associated with esophageal atresia (EA). Failure of conservative endoscopic treatment leads to surgery. Transthoracic approach is rarely advocated due to the peridiaphragmatic localization of the stricture. Robotic-assisted thoracoscopic surgery (RATS) may offer enhanced precision in confined spaces; its application in patients under 10 kg is still considered a challenge. We report the first description of RATS resection of cartilaginous congenital stenosis and esophagoesophagostomy in a 9.5 kg patient. A female patie

Sustained efficacy and long-term outcomes of autologous oral mucosal epithelial cell sheet transplantation for pediatric esophageal anastomotic restenosis.

Refractory anastomotic restenosis following surgery for congenital esophageal atresia (CEA) or congenital esophageal stenosis (CES) remains a serious complication in pediatric surgery. Repeated endoscopic balloon dilatation (EBD) under general anesthesia often impairs growth, feeding function, and overall quality of life (QOL). We have developed a regenerative therapy using autologous oral mucosal epithelial cell sheets. This study aimed to evaluate the long-term safety and efficacy of epithelial cell sheet transplantation for refractory esophageal restenosis in pediatric patients. Between 201

Mortality and Factors Associated with Death in Children with Congenital Gastrointestinal Malformations-A Five-Year Retrospective Study from Romania (2020-2024).

Background/Objectives: Congenital gastrointestinal malformations (CGIMs) remain a significant cause of pediatric mortality, particularly during the neonatal period in resource-limited settings. Mortality varies widely, across conditions and healthcare systems, yet data from Eastern European centers remain limited. We aimed to determine mortality during index admission and identify baseline factors associated with death among children with CGIMs treated at our tertiary surgical center, including the COVID-19 pandemic years. Methods: We conducted a single-center retrospective study (January 2020

Trends and outcomes of thoracoscopic esophageal atresia and tracheoesophageal fistula repair: a retrospective analysis 2016-2022.

PURPOSE: Thoracoscopic repair of esophageal atresia (EA) and tracheoesophageal fistula (TEF) is a technically demanding operation. We hypothesize thoracoscopic repair of EA with TEF over time has increased, with improved or equivalent outcomes compared to open. METHODS: NSQIP-P identified patients < 30 days with EA and TEF who underwent repair of both defects from 2016 to 2022. Patients were categorized into (1) intention to treat (thoracoscopic/thoracoscopic converted to open); and (2) thoracoscopic only, both compared to open only patients. Coarsened exact matching was performed. Pre/post-ma

Endoscopic Trichloroacetic Acid Chemocauterization for Recurrent Tracheoesophageal Fistula: A Single-Center Retrospective Cohort Study.

BACKGROUND: Tracheoesophageal fistula (TEF) is a rare congenital anomaly commonly associated with esophageal atresia (EA-TEF). Management involves early surgical repair. TEF recurrence is not rare and repeat surgery carries significant complication risks. Endoscopic treatment is an alternative to iterative surgery. This retrospective case series describes the diagnosis, management, and outcomes of children treated with endoscopic chemocauterization for RTEF after EA-TEF surgical repair (RTEF) or isolated congenital TEF in our center. METHODS: We reviewed the charts of all patients who underwe

Surgical Strategies for Native Esophagus Preservation in Long-Gap Esophageal Atresia: A 20-Year Population-Based Study.

Background/Objectives: Long-gap esophageal atresia (LGEA) is most commonly associated with Gross types A and B, which are usually diagnosed preoperatively, but may also be encountered in Gross type C, where the long gap is recognized intraoperatively, during fistula ligation. This study aimed to describe and explore outcomes associated with two native esophagus-preserving procedures, delayed primary anastomosis (DPA) and the Foker procedure (FP), and to assess their respective roles in the management of LGEA. Methods: We analyzed a population-representative cohort of 38 patients with LGEA trea

Preservation of the Native Esophagus in Long-Gap Esophageal Atresia: Case Presentation.

Esophageal atresia type 1 is a rare birth defect that involves the loss of esophageal continuity without the presence of a tracheoesophageal fistula. In prenatal testing, polyhydramnios and the absence of a gastric bubble were observed. At birth, sialorrhea, cyanosis, and difficulty progressing the orogastric tube were noted. Although minimally invasive surgery with preservation of the native esophagus represents an innovative strategy to reduce morbidity and improve quality of life, it hasn't yet become the standard management due to its technical requirements and low volume of global cases.

Intravascular Large B-Cell Lymphoma Presenting with Rhabdomyolysis and Inflammatory Myositis: A Diagnostic Challenge.

INTRODUCTION: Intravascular large B-cell lymphoma (IVLBCL) is a rare and aggressive extranodal lymphoma characterised by the proliferation of lymphoma cells within small blood vessels. Distinctively, IVBCL typically lacks the lymphadenopathy, discrete tumour masses, or circulating lymphoma cells characteristic of other lymphomas. This results in substantial diagnostic delays and historically high rates of postmortem diagnosis. Common clinical features of IVLBCL include fever of unknown origin, cytopenias, elevated lactate dehydrogenase, and neurological dysfunction. This case highlights the di

SMAD6 is required for normal foregut development in humans and frogs.

Esophageal atresia and tracheoesophageal fistula (EA/TEF) are congenital malformations of the foregut. We identified two distinct variants in the BMP/TGFβ repressor SMAD6 in two individuals exhibiting EA/TEF. We investigated the function of SMAD6 in tracheoesophageal development using two orthogonal approaches in Xenopus embryos, both resulting in foregut malformations similar to those observed in EA/TEF. We then used human pluripotent stem cell-derived foregut epithelium and mesenchyme to explore the separate roles of SMAD6 in these germ layers. CRISPR-mediated disruption of human SMAD6 cause

Understanding in conditions of neutrality: a closer look at its objects.

Studies in vitro have shown that phosphorylated translation initiation factor 2 alpha (TIF 2 alpha) may have several functions, including regulation of protein synthesis, control of cell death and procurement of resistance to oxidative stress in nerve cells. These properties may have implications in certain human neurodegenerative diseases, such as Alzheimer's disease (AD) and Creutzfeldt-Jakob's disease (CJD), in which oxidative stress appears to be involved in the process of neurodegeneration and neurone death. Single and double-labelling immunohistochemistry to phosphorylated TIF 2 alpha, p
