Delayed Thoracoscopic Repair Following Abdominal Esophageal Banding in a 539-g Infant With Triple Atresia: A Case Report.

Triple atresia, comprising esophageal atresia, duodenal atresia, and anorectal malformation, has no established management strategy in extremely low-birth-weight (ELBW) infants. We report successful staged treatment in a 539-g infant with severe respiratory failure. Transabdominal esophageal banding and gastrostomy on Day 0 controlled airflow through the tracheoesophageal fistula without thoracic intervention. Duodeno-duodenostomy and transverse colostomy were performed on Day 18 (680 g), permitting enteral feeding. After growth to 1924 g, thoracoscopic repair of Gross type C esophageal atresi

CT-guided three-dimensional reconstruction with predictive modeling for personalized esophageal anastomosis in type C esophageal atresia.

BACKGROUND: Anastomotic stricture remains the most common and clinically significant complication following primary repair of type C esophageal atresia. Despite various technical modifications, postoperative narrowing at the anastomotic site continues to occur. We describe an approach incorporating preoperative three-dimensional CT reconstruction to enable individualized planning of the esophageal anastomosis. AIMS AND OBJECTIVES: To evaluate the feasibility, safety, and early clinical outcomes of a modified esophageal anastomosis guided by preoperative radiological and CT-based measurements

Esophageal atresia type III? Intraoperative surprise and different approaches.

A newborn was referred due to clinical and radiological suspicion of esophageal atresia (EA) type III. Surgery revealed an esophagus without evident interruptions; however, intraoperative advancement of the nasogastric tube was unsuccessful, and the distal esophagus inflated with each ventilation, indicating the presence of a distal fistula. An intraoperative esophago-tracheobronchoscopy showed a proximal esophageal pouch with a tiny tracheoesophageal fistula and a large distal tracheoesophageal fistula. The esophageal ends were blind but overlapping, with no external discontinuity observed. W

Feeding difficulties in children with esophageal atresia: A parent-reported multicenter study.

OBJECTIVE: Feeding difficulties (FDs) are common among children with esophageal atresia (EA) and tracheoesophageal fistula (TEF), but knowledge about their prevalence and risk factors is limited. This multicenter study aimed to assess the prevalence, subtypes, and associated factors of FD in children with EA/TEF. METHODS: Parents of children who underwent surgery for EA/TEF in four tertiary centers in Israel (2005-2022) completed a structured questionnaire. Pediatric feeding disorder (PFD) was diagnosed by means of the Montreal Children's Hospital Feeding Scale and classified by consensus cri

Preservation of the azygos vein versus ligation of the azygos vein during surgical repair of esophageal atresia-tracheoesophageal fistula - A systematic review and meta-analysis.

INTRODUCTION: Esophageal atresia-tracheoesophageal fistula (EA-TEF) is amongst the most common life-threatening developmental anomalies. For the surgical repair of esophageal atresia-tracheoesophageal fistula (EA-TEF), several interventional modifications have been suggested. One such modification is the preservation of the azygos vein, a key draining vein for the esophagus and surrounding structures. However, there is no clear consensus on the subject. OBJECTIVE: To evaluate whether preservation of the azygous vein offers clinical benefits compared with its ligation during primary repair of

Membranous esophageal atresia without tracheoesophageal fistula in a neonate: a rare case report and literature review.

BACKGROUND: Membranous esophageal atresia (MEA) without tracheoesophageal fistula (TEF) is a rare form of esophageal atresia (EA) that is often misdiagnosed as Gross type A esophageal atresia (isolated EA). However, the management strategies for these two conditions differ substantially. We present a case initially diagnosed as Gross type A EA, in which intraoperative recognition of key findings led to a revised diagnosis of MEA without TEF, enabling a single-stage repair and avoiding unnecessary staged reconstruction. Additionally, we have reviewed the literature to summarize the clinical cha

Revisiting Turkish Esophageal Atresia Regisrty data for quality indicators.

AIM: The Turkish Esophageal Atresia Registry (TEAR) data were revisited using quality indicators (QIs) to evaluate quality care provided to patients with esophageal atresia (EA). METHODS: Among 36 centers registering data in the TEAR database, only those treating more than four patients per year were included. Based on predefined QIs, each center was assessed for structural, procedural and outcome indicators. Mean percentages were calculated for each QI. Centers with a lower-than-mean percentage for adverse outcomes were determined as 'meeting' that QI, while those with higher percentage were

Fiber-Guided Red Light-Emitting Diode Localization of an Esophageal Blind End in Long-Gap Esophageal Atresia Surgery: A Case Report.

INTRODUCTION: Esophageal atresia (EA) Gross type A is a congenital malformation in which both the upper and lower esophagus have blind ends without a tracheoesophageal fistula. The gap between the segments is often long (long-gap EA), making intraoperative localization challenging. We report a case in which the lower esophageal blind end was readily localized using Tumguide (Otsuka Pharmaceutical Factory, Tokushima, Japan), a device that transmits biologically transparent red light from a light-emitting diode (LED) through an optical fiber. CASE PRESENTATION: The patient was a 2-month-old gir

Parents' experiences of condition management in children born with esophageal atresia-tracheoesophageal fistula during their early childhood.

BACKGROUND: Esophageal atresia-tracheoesophageal fistula (EA-TEF) is a rare congenital malformation of the esophagus and airways. Following surgical repair, children commonly face ongoing aerodigestive morbidity. To address the limited research in this area, this study explored parents’ experiences managing EA-TEF during early childhood, a time marked by high morbidity, and evolving parenting roles. METHOD: As part of a larger project, five standardized focus groups were conducted with 22 parents of children aged 6 months to 7 years treated at a quaternary pediatric surgical center in U.S. Dis

Development of a lifelong core outcome set for oesophageal atresia ± tracheoesophageal fistula: the OCELOT study.

BACKGROUND: Despite anatomical correction, people born with oesophageal atresia±tracheoesophageal fistula (OA-TOF) experience lifelong morbidity. Core outcome sets (COSs) are recognised as a means of improving research quality and, as a consequence, improving patient outcomes; one was not available for this population. OBJECTIVE: The scope of the study was to develop a COS for people born with OA-TOF that would be applicable regardless of age or geographic location. STUDY DESIGN: Patient input was paramount to this study. For long-list generation, in addition to the systematic review (SR), p

Risk factors for anastomotic complications following thoracoscopic repair of type III esophageal atresia in neonates: a single-center retrospective cohort study.

BACKGROUND: Although thoracoscopic repair in China has emerged as a preferred and increasingly adopted minimally invasive approach for esophageal atresia (EA), anastomotic complications remain a major concern, with reported incidences ranging from 15% to 80%. However, the independent risk factors for these complications in a homogeneous surgical population remain poorly characterized. METHODS: This single-center retrospective cohort study recruited neonates with type III EA who underwent primary thoracoscopic repair. Comprehensive demographic, operative, and anatomic variables were gathered.

Management of postoperative leaks and infections in patients with esophageal atresia.

Esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) remains a common, complex congenital anomaly with high survival but significant postoperative morbidity. Despite advances in surgical technique and neonatal care, one of the main contributors to postoperative morbidity is infection with anastomotic leakage occurring in up to 25 % of cases. Leakage of the esophageal anastomosis is associated with mediastinal and thoracic infection, prolonged hospitalization, and long-term sequelae. Prompt recognition and management can minimize short- and long-term complications for these p

Is there any difference between surgical and endoscopic interventions related to the outcome of recurrent tracheoesophageal fistulas? A systematic review.

BACKGROUND: Recurrent tracheoesophageal fistula (rTEF) is a rare but serious complication after esophageal atresia (EA) repair. Although various surgical and endoscopic techniques are available, the optimal management remains controversial due to limited data and heterogeneous outcomes. PURPOSE: To systematically review and compare the outcomes of surgical and endoscopic interventions for rTEF in pediatric patients. METHODS: A systematic literature search was conducted in PubMed, MEDLINE, and EMBASE for studies published between January 2015 and January 2025. Twenty-seven eligible studies in

Revisiting fundoplication in esophageal atresia: Implications for nutrition, stricture management, and wrap type.

BACKGROUND: Patients with esophageal atresia with or without tracheoesophageal fistula (EA-TEF) frequently require fundoplication to manage gastroesophageal reflux disease (GERD). Data is limited on whether fundoplication improves GERD-related outcomes and if outcomes vary by type of fundoplication. METHODS: This is a retrospective cohort study of patients <18 years-old who underwent EA-TEF repair and fundoplication at a single institution from January 2013 to September 2025. In the first aim, we compared GERD symptoms, medications, nutrition, and esophageal stricture dilation rate before and
