Congenital anomalies of the digestive system, particularly in the esophagus and stomach, present significant challenges to neonatal health due to their complex management and associated risks of morbidity and mortality. This narrative review explores esophageal and gastric anomalies, addressing the embryology, prevalence, diagnostic methods, and treatment approaches, with an emphasis on early detection and improved outcomes. Because of recent advances in prenatal imaging, such as ultrasonography and magnetic resonance, diagnostic accuracy has increased, allowing better clinical planning and ma

Delays along the referral and care pathway for neonatal tracheoesophageal fistula in low-resource settings remain understudied despite their impact on preventable mortality. This case series was done at Pakistan Institute of Medical Sciences. All neonates with confirmed tracheoesophageal fistula were included in the study, enrolling 15 patients, two of which were excluded. Data collection included admission variables, prehospital and in-hospital care timelines. Misdiagnosis occurred in 46.15% of the cases. Time to correct diagnosis from first health-care contact was shorter in survivors (24) t

OBJECTIVE: To evaluate the safety and efficacy of the "spaghetti" dissection technique in thoracoscopic repair of congenital type C esophageal atresia (EA).
METHODS: A total of 58 neonates with confirmed type C EA who underwent thoracoscopic surgery at Huai'an Maternity and Child Health Hospital from March 2019 to June 2025 were consecutively enrolled. Patients were randomly assigned to two groups: (1) Group A (n = 30): Thoracoscopic "spaghetti" dissection technique was used for proximal esophageal dissection followed by end-to-end esophageal anastomosis; (2) Group B (n = 28): Traditional tho

Aortopulmonary window (APW) is a rare conotruncal cardiac anomaly that is rarely diagnosed prenatally. We report a case of APW identified by detailed fetal echocardiography and confirmed after birth, associated with esophageal atresia with distal tracheoesophageal fistula, duodenal atresia, and anal atresia. Prenatal imaging demonstrated an abnormal communication between the ascending aorta and the main pulmonary artery, allowing delivery planning at a tertiary center and early multidisciplinary management. This case highlights the importance of careful assessment of the great arteries during

BACKGROUND: Esophageal atresia with tracheoesophageal fistula (EA-TEF) complicated by foregut duplication (FD) cyst is an exceedingly rare congenital anomaly. Since the first description in 1956, only 20 cases have been reported, with open thoracotomy representing the standard surgical approach. We present a case of successful one-stage thoracoscopic repair of this complex association in a neonate.
CASE PRESENTATION: A male neonate weighing 2080 g was diagnosed with Gross type C EA-TEF. At 3 days of age, right thoracoscopic exploration identified a 1-cm FD cyst on the distal esophageal wall.

Background/Objectives: Congenital gastrointestinal malformations (CGIMs) remain a significant cause of pediatric mortality, particularly during the neonatal period in resource-limited settings. Mortality varies widely, across conditions and healthcare systems, yet data from Eastern European centers remain limited. We aimed to determine mortality during index admission and identify baseline factors associated with death among children with CGIMs treated at our tertiary surgical center, including the COVID-19 pandemic years. Methods: We conducted a single-center retrospective study (January 2020

PURPOSE: Thoracoscopic repair of esophageal atresia (EA) and tracheoesophageal fistula (TEF) is a technically demanding operation. We hypothesize thoracoscopic repair of EA with TEF over time has increased, with improved or equivalent outcomes compared to open. METHODS: NSQIP-P identified patients < 30 days with EA and TEF who underwent repair of both defects from 2016 to 2022. Patients were categorized into (1) intention to treat (thoracoscopic/thoracoscopic converted to open); and (2) thoracoscopic only, both compared to open only patients. Coarsened exact matching was performed. Pre/post-ma

BACKGROUND: Esophageal atresia (EA), with or without tracheoesophageal fistula, remains associated with substantial early mortality and long-term morbidity. Reported incidence and outcomes vary across Asia, but interpretation is complicated by differences in case capture, referral pathways, diagnostic capacity, and neonatal intensive care availability.
METHODS: We performed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020-aligned systematic narrative review of observational studies from Asian countries published up to March 2024 (PubMed and Google Scholar). Studies

BACKGROUND: Tracheoesophageal fistula (TEF) is a rare congenital anomaly commonly associated with esophageal atresia (EA-TEF). Management involves early surgical repair. TEF recurrence is not rare and repeat surgery carries significant complication risks. Endoscopic treatment is an alternative to iterative surgery. This retrospective case series describes the diagnosis, management, and outcomes of children treated with endoscopic chemocauterization for RTEF after EA-TEF surgical repair (RTEF) or isolated congenital TEF in our center.
METHODS: We reviewed the charts of all patients who underwe

Esophageal atresia type 1 is a rare birth defect that involves the loss of esophageal continuity without the presence of a tracheoesophageal fistula. In prenatal testing, polyhydramnios and the absence of a gastric bubble were observed. At birth, sialorrhea, cyanosis, and difficulty progressing the orogastric tube were noted. Although minimally invasive surgery with preservation of the native esophagus represents an innovative strategy to reduce morbidity and improve quality of life, it hasn't yet become the standard management due to its technical requirements and low volume of global cases.

Esophageal atresia and tracheoesophageal fistula (EA/TEF) are congenital malformations of the foregut. We identified two distinct variants in the BMP/TGFβ repressor SMAD6 in two individuals exhibiting EA/TEF. We investigated the function of SMAD6 in tracheoesophageal development using two orthogonal approaches in Xenopus embryos, both resulting in foregut malformations similar to those observed in EA/TEF. We then used human pluripotent stem cell-derived foregut epithelium and mesenchyme to explore the separate roles of SMAD6 in these germ layers. CRISPR-mediated disruption of human SMAD6 cause

Studies in vitro have shown that phosphorylated translation initiation factor 2 alpha (TIF 2 alpha) may have several functions, including regulation of protein synthesis, control of cell death and procurement of resistance to oxidative stress in nerve cells. These properties may have implications in certain human neurodegenerative diseases, such as Alzheimer's disease (AD) and Creutzfeldt-Jakob's disease (CJD), in which oxidative stress appears to be involved in the process of neurodegeneration and neurone death. Single and double-labelling immunohistochemistry to phosphorylated TIF 2 alpha, p

INTRODUCTION: Tracheoesophageal fistula with esophageal atresia (TEF/EA) carries long-term gastrointestinal morbidity, including strictures, gastroesophageal reflux disease (GERD), and dysmotility, all of which predispose to chronic inflammation. Gastric signet-ring-cell adenocarcinoma (SRC) is rare in young adults and unusual in the context of congenital foregut anomalies.
CASE PRESENTATION: A female in her twenties with repaired TEF/EA had progressive dysphagia, recurrent food impactions, and severe distal esophageal stricture requiring frequent dilations. During routine surveillance esopha

Esophageal atresia (EA) is a rare congenital anomaly of the foregut. Several anatomic variants have been described, ranging from isolated esophageal discontinuity to forms associated with tracheoesophageal fistula, with some variants occurring far less frequently than others. A premature infant (33 weeks, 1.5 kg) presented with a gasless abdomen and a nasogastric tube coiled at T4. Bronchoscopy revealed a Grade 1 laryngeal cleft without fistula, consistent with type A EA, and a gastrostomy was placed. The initial six-vertebral gap shortened to 2.5 over six weeks. Collis gastroplasty and thorac
