Endoscopic tracheoesophageal fistula closure-Electrocautery combined with esophageal clip application in pediatric patients.

OBJECTIVE: A tracheoesophageal fistula (TEF) is an abnormal communication between the esophagus and trachea, most often associated with esophageal atresia (EA), a rare congenital malformation affecting 1 in 2400-4500 live births. While surgical repair of EA/TEF is typically completed in infancy, recurrent or missed TEF can be diagnosed later. Open repair can be technically challenging, often requiring re-do thoracotomy or neck dissection. We describe our center's experience, using endoscopic electrocautery in combination with esophageal clips for closure of TEF. METHODS: We conducted a retros

[Clinical Challenges in the Diagnosis and Management of Fetal Esophageal Atresia: From Prenatal Diagnosis and Surgical Intervention to Prognostic Management].

Congenital esophageal atresia (EA), a severe malformation of the digestive tract, has an incidence of approximately 1 in every 2500 to 3000 live births. It is characterized by an interruption in the continuity of the esophagus and is often accompanied by tracheoesophageal fistula (TEF) . The etiology of EA is complex. With continuous advancements in prenatal imaging technology, fetal medicine, and neonatal surgery, the prenatal detection rate, perinatal management, and postoperative outcomes of EA have improved. However, the clinical management of EA still faces significant challenges, includi

Induced Pluripotent Stem Cells as a Tool to Decipher the Normal and Abnormal Development of the Esophagus and Trachea from Normal Morphogenesis to Esophageal Atresia, Tracheomalacia, and Laryngo-Tracheal Clefts.

The development of the esophagus and trachea following the septation of the anterior foregut is a highly regulated process involving bidirectional communication between the endoderm and mesoderm. Signaling pathways such as the Bone Morphogenetic Protein family, Wnt/β-catenin, Sonic Hedgehog, and Fibroblast Growth Factor family mediate this complex crosstalk to induce the dorsal-ventral patterning of the anterior foregut as well as lineage specification. Even though the mechanisms are not fully understood, dysregulation of signaling pathways may lead to congenital malformations such as tracheom

Usefulness of Transparent Illumination Device in Esophageal Atresia for the Detection of the Distal Esophagus.

INTRODUCTION: Pure esophageal atresia without a tracheoesophageal fistula (Gross type A) is technically demanding, particularly with respect to intraoperative identification of the distal esophageal pouch. Biologically transparent illumination (BTI) devices have been used to visualize nasogastric tubes in pediatric patients; however, their application during esophageal reconstruction has not been reported. CASE PRESENTATION: A male neonate with prenatally suspected esophageal atresia was diagnosed postnatally with Gross type A esophageal atresia. After gastrostomy and serial bougienage of bot

Perioperative risk factors for short- and long-term respiratory morbidity in patients with esophageal atresia.

PURPOSE: Respiratory complications are a major source of morbidity in esophageal atresia (EA), yet predictors of short- and long-term respiratory outcomes are not well defined. This study aimed to identify perioperative factors associated with respiratory morbidity in EA/TEF patients. METHODS: We conducted a retrospective cohort study of 242 EA patients treated at a single tertiary center from 2005 to present. Short-term respiratory morbidity was defined as the need for supplemental respiratory support at 30 days postoperatively from primary EA repair. Long-term morbidity was determined based

Left Cervical Approach for Unrecognized Proximal Tracheoesophageal Fistula in an Adolescent following Neonatal Esophageal Atresia Repair: A Case Report.

INTRODUCTION: Esophageal atresia with both proximal and distal tracheoesophageal fistulas (TEF) is rare, and a proximal TEF may be overlooked during the initial neonatal surgery. However, reports describing the surgical management of residual proximal TEF beyond infancy are limited. Herein, we report a case of a residual proximal TEF that was unrecognized during neonatal repair of esophageal atresia, remained undiagnosed until adolescence, and was successfully resected using a left cervical approach. CASE PRESENTATION: A 14-year-old boy with recurrent aspiration pneumonia was diagnosed with a

Aberrant Right Subclavian Artery in Infants With Esophageal Atresia: A Single-Institution Study of Incidence and Outcomes.

INTRODUCTION: Esophageal atresia (EA) is associated with aortic arch anomalies, including an aberrant right subclavian artery (ARSA) in 3.4-12%. ARSA complicates EA repair due to proximity to the esophageal anastomosis and poses risks during subsequent interventions for strictures. This study characterizes management, complications, and outcomes of EA patients with ARSA. METHODS: Retrospective review (2012-2023) of EA patients with ARSA at a single institution. Data on diagnosis, management and complications were analyzed with descriptive statistics. RESULTS: Among 128 EA patients, 12 (9.4%)

Eosinophilic esophagitis: An aerodigestive perspective.

Eosinophilic esophagitis (EoE) is an immune-mediated inflammatory condition of the esophagus that is characterized by esophageal dysfunction and distinct histopathologic findings of elevated eosinophils in the esophageal mucosa. EoE can present in the aerodigestive patient with a variety of symptoms ranging from feeding difficulty to extraesophageal symptoms such as chronic cough. Routine evaluation for EoE should be performed in high-risk patients such as esophageal atresia/tracheoesophageal fistula and should be considered for those undergoing evaluation for laryngotracheal reconstruction su

An atypical variant of congenital bronchoesophageal fistula associated with esophageal atresia and right main bronchial atresia and right lung hypoplasia in a preterm neonate: a case report.

BACKGROUND: Congenital bronchoesophageal fistula (BEF) is a rare communicating bronchopulmonary foregut malformation (CBPFM) characterized by an abnormal connection between the esophagus and a bronchial segment or lung tissue. While esophageal atresia (EA), with or without tracheoesophageal fistula, occurs in approximately 2-3 per 10,000 live births, EA associated with CBPFM/congenital BEF is far rarer, with no established population-based incidence and only several dozen reported CBPFM cases in the literature. Most congenital BEFs present in childhood or adulthood with chronic, nonspecific re

Congenital aerodigestive fistula with extraordinarily delayed adult presentation: a case report and review of endoscopic closure for bronchoesophageal fistula.

BACKGROUND: Congenital aerodigestive fistulae (ADF) represent extremely rare developmental malformations derived from disrupted embryological separation of the primitive foregut, which can be classified by lesion location into tracheoesophageal fistulae, bronchoesophageal fistulae (BEF), and other subtypes. While typically diagnosed in neonates, isolated BEF without esophageal atresia ("H-type") may remain undetected for decades, presenting in adulthood with chronic, stereotyped symptoms. Endoscopic interventions for congenital BEF remain poorly characterized, with limited evidence regarding p
