Cervical esophagogastric anastomosis using a retrosternal gastric conduit in an adult patient with congenital type A long-gap esophageal atresia after 18 years of gastrostomy: a case report.

BACKGROUND: Definitive reconstruction in adulthood after 18 years of gastrostomy dependence for congenital type A long-gap esophageal atresia is extremely rare. We report an adult patient who had been reliant on gastrostomy since birth and regained sustained oral intake following definitive reconstruction, emphasizing the preoperative exclusion of tracheoesophageal fistula (TEF), individualized route selection, and tailored cervical anastomosis. CASE PRESENTATION: An 18-year-old woman with type A LGEA underwent neonatal thoracotomy without definitive repair, followed by gastrostomy. At 2 year

Antimicrobial resistance and vaccine-preventable infections: concurrent health security challenges after the double earthquake in Venezuela.

SCIENTIFIC BACKGROUND: Meningiomas are the most common benign intracranial neoplasms with a slow growth presented as the intracranial lesion. These tumors are without any symptoms for a long time. At the time of diagnosis it is frequently an asymptomatic tumor. In that case the therapist may well suggest a wait-and-see strategy. The therapy of meningiomas focuses firstly on the microsurgical treatment. Volume reduction can be achieved immediately after treatment. Stereotactic radiosurgery is an important non-invasive treatment option for recurrent tumors or meningiomas with partial resection.

[Interest as Catalyzer: The Role of Interest in the Use of Incremental Scaffolds in Inquiry-Based Learning].

BACKGROUND: Use of human mesenchymal stromal cells (MSCs) is a promising strategy for cell therapy in injured tissues recovery. However, MSCs acquire genetic changes when cultivated in vitro that make them more susceptible to undergo neoplastic transformation. Therefore, genomic integrity of stem cells should be monitored carefully for the use in basic research and clinical trials, including karyotype analysis to confirm the absence of genetic instability. Here, we report a case of a male 67-year-old patient selected to join the study: "Autologous transplantation of mesenchymal cells for treat

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Specific Findings of Blood Perfusion on Anastomosed Esophagus of Neonatal Esophageal Atresia and Tracheoesophageal Fistula Using Indocyanine Green Fluorescence During Thoracoscopic Surgery.

We herein report a neonatal case showing specific findings of blood perfusion in the anastomosed esophagus of esophageal atresia (EA) and tracheoesophageal fistula (TEF) using indocyanine green (ICG) fluorescence during thoracoscopic surgery. The patient was a 3054 g, 0-day neonatal boy diagnosed with EA-TEF based on a coil-up sign of the nasogastric tube. Thoracoscopic surgery was performed on Day 4 after birth. After TEF transection, esophageal anastomosis was performed using interrupted sutures. ICG was administered intravenously to confirm blood perfusion at the anastomotic site. Initially

Experience in the treatment of type C congenital esophageal atresia using a staged approach.

BACKGROUND: In select patients with type C esophageal atresia, primary anastomosis is not appropriate and a staged approach is required. We aim to summarize our experience in the management of type C EA using a staged approach. METHODS: A retrospective chart-review of patients with type C EA admitted to Beijing Children's Hospital between July 2020 to October 2023 were conducted. Those diagnosed with type C EA who were not amendable to primary anastomosis were included for analysis. Clinical information was recorded, and follow- up was performed. RESULTS: Seven (five boys) patients with type

Outcome of Recurrent Tracheoesophageal Fistula Treatment After Esophageal Atresia Repair.

INTRODUCTION: Recurrent Tracheoesophageal Fistula (rTEF) is a complication of Esophageal Atresia (EA) that can lead to severe respiratory symptoms. RTEF can be corrected via endotracheal treatment (ET) or surgical treatment (ST). The efficacy of these techniques varies in literature. This study presents the outcome of rTEF treatment after EA correction at the Wilhelmina Children's Hospital in Utrecht. METHODS: From 2000 until 2023, patients with EA (n = 251) underwent thoracoscopic EA correction by using the native esophagus at the Wilhelmina Children's Hospital in Utrecht. All EA patients wh

Respiratory Outcomes in Children Born With Esophageal Atresia With or Without Tracheoesophageal Fistula: A Retrospective Longitudinal Cohort Study.

BACKGROUND: The respiratory health of children born with esophageal atresia with or without tracheoesophageal fistula (EA/TEF) can be compromised. PURPOSE: We aimed to study how respiratory outcomes in EA/TEF cases change with increasing age and over time. METHODS: We performed a retrospective cohort study of children with EA/TEF treated in our centre from 1991 to 2022 and compared their respiratory outcomes to those of date-of-birth matched controls. Respiratory outcomes up to 2022 were identified using International Classification of Diseases codes (ICD-9). Odds ratios (OR), rate ratios (R

Does thoracoscopic repair of type C esophageal atresia require emergency treatment?

BACKGROUND: Thoracoscopic repair of esophageal atresia (EA) with tracheoesophageal fistula (TEF) has been performed with increasing frequency. Although many children have underwent surgery within three days after birth, the optimal timing for operation remains undetermined. This study aimed to investigate the appropriate timing for thoracoscopic repair of type C EA and its mid-term clinical outcomes. METHOD: We retrospectively analyzed 142 patients with EA between April 2009 and April 2023. A total of 109 patients with type C EA who underwent thoracoscopic one-stage repair surgery were includ

Early Esophagogram and Esophagoscopy: Evaluating Anastomotic Integrity in Successfully Treated Type 3 Esophageal Atresia Survivors.

INTRODUCTION: Anastomotic narrowing post-Type 3 esophageal atresia (EA) primary repair has been linked to life-threatening respiratory complications and failure to thrive during infancy. It becomes important to find some method to ensure anastomotic adequacy in these patients. We in the study here aimed to find the role of follow-up esophagogram and esophagoscopy in detecting anastomotic stricture (AS) in the early stage and to find whether these help in reducing the morbidity in these patients. MATERIALS AND METHODS: Neonates with EA Type 3, who were successfully managed with primary anastom

Diagnosis and Management of Long-term Gastrointestinal Complications in Pediatric Esophageal Atresia/Tracheoesophageal Fistula.

PURPOSE OF REVIEW: Esophageal atresia/tracheoesophageal fistula (EA/TEF) causes gastrointestinal (GI) complications that can begin in infancy and extend into adulthood. The etiology of these is frequently multifactorial, and they can adversely affect feeding, growth, respiratory health, and overall quality of life. This review highlights the importance of early diagnosis, multidisciplinary surveillance, and individualized treatment strategies to effectively manage these complications and improve long-term outcomes for affected children. RECENT FINDINGS: Recent studies have focused on the deve

Factors Associated With Short- and Long-term Survival in Oesophageal Atresia With Tracheoesophageal Fistula.

BACKGROUND: Risk stratification scores for oesophageal atresia with tracheoesophageal fistula (OA-TOF) are useful to predict survival. Scores from single-centre series may lack external validity. We aimed to model survival in OA-TOF and validate a new risk score using an international, multicentre cohort of patients. METHOD: Retrospective data (2000-2022) from 4 tertiary centres were used; a multivariable logistic regression model for survival to discharge was derived from single centre data, and then validated on the whole dataset using bootstrapping. In addition, Cox regression was used to

3D printed synthetic models for training minimally invasive pediatric surgery - our own experience and literature review.

Pediatric surgery is a medical specialty focused on the diagnosis, treatment, and postoperative care of children with congenital and acquired anomalies and diseases. The goal of pediatric surgeons is to ensure that children receive the best possible care while minimizing the risks and complications associated with surgical procedures. Contemporary pediatric surgeons face many challenges, including a decline in the number of children with congenital developmental defects, economic pressures, and efforts to increase efficiency, leading to reduced time spent on individual surgeries. This can limi

Long-Term Aerodigestive Morbidities After Esophageal Atresia/Tracheoesophageal Fistula Repair.

PURPOSE: Short-term complications, such as gastroesophageal reflux disease (GERD) and anastomotic stricture, are well documented in esophageal atresia/tracheoesophageal fistula (EA/TEF) survivors; however, long-term aerodigestive morbidities remain poorly understood. This study evaluated the long-term aerodigestive morbidities of EA/TEF survivors through propensity-matched analysis using a large international electronic health record (EHR) database. METHODS: EA/TEF cases from 2004 to 2024 were identified in the TriNetX EHR database, which encompasses over 100 million patients from 86 internat

The boy who lived: staged repair of congenital diaphragmatic hernia with esophageal atresia and tracheoesophageal fistula in a 32-week, 1.5 kg infant, and review of the literature.

Congenital diaphragmatic hernia (CDH) associated with esophageal atresia (EA) and tracheoesophageal fistula (TEF) is a rare and often fatal combination with reported survival rates of 6%-26%. We aim to analyze the literature on left sided CDH with EA and TEF and report our experience, hypothesizing that delaying right chest approach for EA/TEF repair improves outcomes. We report a case of a 1.5 kg 32-week patient who survived a staged approach of initial CDH repair and abdominal control of TEF with gastrostomy to water seal and vessel loop encircling the gastroesophageal junction followed by E
