Resource use in infants undergoing esophageal atresia repair at major children's hospitals.

BACKGROUND: Esophageal atresia with or without tracheoesophageal fistula is a resource-intensive neonatal surgical condition; however, the magnitude of the institutional cost burden is not well understood. In this study, we used a national database to measure resource use associated with the initial management of esophageal atresia at major children's hospitals nationwide. METHODS: The Pediatric Health Information System database was queried for infants undergoing esophageal atresia repair at 1 of 47 children's hospitals in the United States between 2014 and 2021. Those undergoing concomitant

Clinical Utility of Indocyanine Green Fluorescence in Neonatal Surgery: A Single-Center Study and Systematic Review.

The use of indocyanine green (ICG) fluorescence in neonatal procedures is limited to specific pathologies, with variability in its application, highlighting the need to expand its indications and standardize administration protocols. We present our experience and review of the literature.Prospective assessment (2019-2023) of ICG-navigated neonatal surgeries. Administration routes included intravenous, endoluminal, and intracatheter. ICG dosages were variable according to the indication. The results were prospectively registered, focusing on its ability to achieve the desired goal. A systematic

Clinical challenges and outcomes of thoracoscopic versus open repair in esophageal atresia: a single-center retrospective comparative study.

PURPOSE: This study aimed to compare clinical outcomes between thoracoscopic surgery (TR) and open surgery (OR) for esophageal atresia with distal tracheoesophageal fistula (EA with distal TEF) and to evaluate the feasibility of TR. METHODS: We retrospectively analyzed the clinical data of 42 patients who underwent primary surgery for EA with distal TEF from January 2012 to December 2020. We compared the OR and TR groups based on patient characteristics, intraoperative outcomes, and early and late postoperative outcomes. RESULTS: The TR group had longer operation times and a higher risk for

Mind the Missing Gap: A Cervical Variant of Type A Esophageal Atresia.

Background: Esophageal atresia (EA) type A, characterized by the absence of a tracheoesophageal fistula and typically presenting with a long esophageal gap, usually requires staged repair. Methods: We report a rare case of a newborn with type A EA in which both the proximal and distal esophageal pouches were unexpectedly close and located in the cervical region. This anatomical variant allowed for a successful primary anastomosis through a cervical approach. Results: Initial imaging was misleading, and the true anatomy was clarified only through thoracoscopic exploration, underscoring the impo

Technique for Consistent Selective Left Mainstem Intubation in Neonatal and Infantile Surgery.

Selective left mainstem intubation can be challenging in neonates and infants due to airway fragility, limited device options, and the natural inclination toward right-sided placement. This paper describes a sample case series demonstrating a technique that combines patient positioning, a Seldinger-like approach using a soft stylet, and real-time fluoroscopic guidance for optimal left mainstem intubation. The method has become an institutional standard for procedures requiring single-lung ventilation, including esophageal atresia with tracheoesophageal fistula repair and congenital pulmonary a

Acid suppression after esophageal atresia repair: Some infants do benefit.

BACKGROUND: Acid suppression is used to protect against anastomotic strictures after esophageal atresia/tracheoesophageal fistula (EA/TEF) repair, but recent studies have contested this benefit. We aimed to compare rates of anastomotic stricture following EA/TEF repair among patients who received and did not receive acid suppression in a nationwide sample. METHODS: We queried the Pediatric Health Information System for infants undergoing EA/TEF repair between 2010 and 2022. Acid suppression was defined as receipt of an H2 blocker or proton pump inhibitor on the day of discharge or longer than

A Comparative Study of the Clinical Characteristics of Esophageal-Pulmonary Fistula in Children.

OBJECTIVES: Esophageal-pulmonary fistula (EPF) is a rare complication after esophageal surgery, especially following esophageal atresia/tracheoesophageal fistula (EA/TEF) repair in neonates. Its clinical characteristics remain unclear, and the etiology is still under investigation. This study aims to describe the clinical features of EPF by comparing it with traditional recurrent TEF (rTEF) after EA repair. METHODS: We retrospectively analyzed children with rTEF after esophageal atresia repair from September 2017 to December 2023. Patients were grouped into EPF and TEF types for comparison. S

Experiences of symptom burden among young children born with esophageal atresia-tracheoesophageal fistula: a US focus group study.

BACKGROUND: Children born with esophageal atresia-tracheoesophageal fistula (EA-TEF) can suffer from aerodigestive morbidity that impairs their quality of life and can persist into adulthood. Ameliorating their symptom burden requires a thorough understanding of the symptom experiences that children have early in life. We aimed to explore parents' experiences of their children's aerodigestive symptom burden during the first years of life after being born with EA-TEF. This exploration also aimed to help determine whether a disease-specific measurement of symptom burden is needed. METHOD: Five

Long-Term Nutritional Status in Children Born With Esophageal Atresia.

BACKGROUND AND OBJECTIVES: Previous studies have reported early undernutrition and growth delay in children born with esophageal atresia (EA). Our goal was to assess their long-term nutritional status. METHODS: We longitudinally studied 204 children born with EA (91% with tracheoesophageal fistula) from birth to 16 years. Undernutrition (Z-score for body mass index [BMI] < -2 SD), and stunting (Z-score for height for age [HFA] < -2 SD) were evaluated. Risk factors were analyzed using regression models for interval-censored data. RESULTS: The mean Z-BMI and Z-HFA were lower in children with E

Internal traction as an effective bridge to successful anastomosis in complex esophageal atresia.

BACKGROUND: Internal traction is an evolving strategy for patients with long-gap esophageal atresia (LGEA) and esophageal atresia (EA) patients with a distal tracheo-esophageal fistula in whom primary anastomosis cannot be achieved. This study aimed to evaluate outcomes and complications following placement of internal traction sutures in EA. METHODS: A retrospective review was performed of consecutive patients with EA undergoing internal traction suture placement at a single center over an 11-year period (2014-2025). Data were collected from a prospectively maintained institutional EA databa

Benefits and barriers to using a portable thoracoscopic esophageal atresia simulator as perceived by surgical trainees.

INTRODUCTION: Simulation-based education (SBE) offers a controlled training environment for complex procedures, such as thoracoscopic repair of esophageal atresia and tracheo-esophageal fistula (EA/TEF). Identifying and overcoming the barriers to SBE is imperative for surgical training programs to be effective and efficient. This study investigated surgical trainees' attitudes to a portable thoracoscopic EA/TEF simulator and changes in the perceptions of surgical trainees following its use as a measure of its benefits and possible barriers for learning. METHODS: Twenty New Zealand surgical tr

Failed Extubation After Primary Repair of Type C Esophageal Atresia: Frequency and Risk Factors.

PURPOSE: Reintubation after esophageal atresia and tracheoesophageal fistula (EA/TEF) repair is believed to increase the risk of anastomotic injury. However, some newborns fail initial extubation and require reintubation. This study explores the frequency and risk factors predictive of extubation failure in newborns after primary type C EA/TEF repair. We further evaluate the effect of extubation management strategies, such as the timing of extubation and the use of supportive non-invasive modalities. METHODS: We conducted a single-center review of newborns with type C EA/TEF between 2010 and
