Congenital anomalies of the aortic arch often lead to tracheoesophageal compression during childhood. Occasionally, symptoms may not emerge until adulthood, appearing for the first time at any age due to age-related or pathological dilation of the aortic segments. The oldest patient described with symptoms at the onset was 76 years old. Surgical intervention is complex and poses challenges. This case study describes a patient with a right mirror-image branching aortic arch, Kommerell's diverticulum, and a ligamentum arteriosum originating from the diverticulum. The patient first experienced sy

BACKGROUND/OBJECTIVES: Congenital esophageal stenosis (CES) is a rare condition that may present with feeding difficulties in infancy, often becoming evident during weaning. We report a case of CES presenting with progressive dysphagia during the introduction of solid foods.
METHODS: Clinical evaluation, contrast esophagography, endoscopy, and magnetic resonance imaging were performed to characterize the stenosis and guide management.
RESULTS: Imaging revealed a fixed narrowing of the mid-distal esophagus with minimal passage of contrast and proximal dilatation. Endoscopy confirmed a non-tra

BACKGROUND: Esophageal atresia with tracheoesophageal fistula (EA-TEF) complicated by foregut duplication (FD) cyst is an exceedingly rare congenital anomaly. Since the first description in 1956, only 20 cases have been reported, with open thoracotomy representing the standard surgical approach. We present a case of successful one-stage thoracoscopic repair of this complex association in a neonate.
CASE PRESENTATION: A male neonate weighing 2080 g was diagnosed with Gross type C EA-TEF. At 3 days of age, right thoracoscopic exploration identified a 1-cm FD cyst on the distal esophageal wall.

Congenital esophageal stenosis is a rare and heterogenous malformation often associated with esophageal atresia (EA). Failure of conservative endoscopic treatment leads to surgery. Transthoracic approach is rarely advocated due to the peridiaphragmatic localization of the stricture. Robotic-assisted thoracoscopic surgery (RATS) may offer enhanced precision in confined spaces; its application in patients under 10 kg is still considered a challenge. We report the first description of RATS resection of cartilaginous congenital stenosis and esophagoesophagostomy in a 9.5 kg patient. A female patie

Refractory anastomotic restenosis following surgery for congenital esophageal atresia (CEA) or congenital esophageal stenosis (CES) remains a serious complication in pediatric surgery. Repeated endoscopic balloon dilatation (EBD) under general anesthesia often impairs growth, feeding function, and overall quality of life (QOL). We have developed a regenerative therapy using autologous oral mucosal epithelial cell sheets. This study aimed to evaluate the long-term safety and efficacy of epithelial cell sheet transplantation for refractory esophageal restenosis in pediatric patients. Between 201

Diversion stomas are done to protect the patients from anastomosis-related complications after low anterior resection, particularly after neoadjuvant chemoradiotherapy. Problems with these temporary stomas are the significant deterioration in quality of life along with medical and surgical complications. Diversion ileostomy is the most commonly performed diversion procedure. Reversal of stoma is usually done after completion of adjuvant chemotherapy. Studies looking into the safety of early stoma reversal have shown conflicting results. The objective of this work is to study the advantages, di

Dieulafoy's lesion is a rare but potentially life-threatening cause of gastrointestinal bleeding because of an abnormally dilated submucosal artery that erodes the overlying epithelium without a primary ulceration. Typically located in the proximal stomach near the esophagogastric junction, Dieulafoy's lesions can also occur throughout the whole gastrointestinal tract, including the oesophagus, small bowel, and colon. Rare cases have been reported in the respiratory tract. Dieulafoy's lesion accounts for approximately 3.5% of gastrointestinal bleeding cases, with mortality rates ranging from 2

BACKGROUND: Definitive reconstruction in adulthood after 18 years of gastrostomy dependence for congenital type A long-gap esophageal atresia is extremely rare. We report an adult patient who had been reliant on gastrostomy since birth and regained sustained oral intake following definitive reconstruction, emphasizing the preoperative exclusion of tracheoesophageal fistula (TEF), individualized route selection, and tailored cervical anastomosis.
CASE PRESENTATION: An 18-year-old woman with type A LGEA underwent neonatal thoracotomy without definitive repair, followed by gastrostomy. At 2 year

Complete communicating tubular oesophageal duplication is a rare congenital foregut anomaly that may remain undiagnosed until adolescence because symptoms can be intermittent. A 17-year-old boy with long-standing intermittent solid-food dysphagia presented with moderate haematemesis. Upper gastrointestinal endoscopy identified an ulcerated inflammatory stricture 26 cm from the incisors, an accessory orifice, and a diverticulum-like opening. Biopsies showed ulcerative and regenerative changes with glandular and intestinal metaplasia without dysplasia. Barium oesophagography and cervicothoracoab

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

OBJECTIVES: Esophageal atresia (EA) is a significant congenital anomaly, with most survivors experiencing esophageal dysmotility. Currently, there is no reliable way to predict which patients will develop significant, life-threatening dysmotility. Using high-resolution impedance manometry (HRIM), this study aimed to characterize the common pressure topography patterns in children with repaired EA.
METHODS: This prospective longitudinal cohort study focused on children (<18 years) with repaired EA. Utilizing HRIM, esophageal motility patterns were studied. Repeat manometric assessments were pe

CONTEXT: Esophageal atresia (EA), with or without tracheoesophageal fistula (TEF), is a rare congenital anomaly. Long-gap esophageal atresia (LGEA) and failed primary repair of TEF often necessitate esophageal replacement in children. This study assesses the outcomes of staged isoperistaltic esophageal replacement using a gastric tube, particularly in resource-limited settings.
AIMS: The aim of this study was to evaluate the outcomes of staged isoperistaltic esophageal replacement using a gastric tube in children with LGEA and failed primary repair of TEF, particularly in resource-limited set

INTRODUCTION: Congenital esophageal stenosis (CES) is a rare clinical condition found in 1 in 25000-50000 live births. Surgical treatment is required when endoscopic balloon dilatation is ineffective. Laparoscopic and thoracoscopic approaches are selected based on lesion location. Gastroesophageal reflux (GER) is often observed as a postoperative complication that necessitates additional fundoplication. We report a case of CES in the lower third of the esophagus that was treated with simultaneous thoracoscopic resection and Thal fundoplication using the right thoracic approach.

Esophageal atresia (EA), with or without tracheoesophageal fistula, is the most common congenital anomaly of the esophagus. Surgical correction is the primary treatment, however, up to 80% of patients experience anastomotic stenosis, with esophageal balloon dilation (EBD) being the first-line treatment. Unfortunately, some patients develop refractory anastomotic stenosis (RAS), defined by the presence of strictures after three EBD sessions. The endoscopic management of RAS remains controversial. Although EA itself is rare, the incidence of postoperative stricture is significant, contributing t

Achalasia cardia is an uncommon esophageal motility disorder that is often misdiagnosed as gastroesophageal reflux disease (GERD). Although rare in infancy, it presents unique diagnostic and therapeutic challenges compared to adults. We describe four infants who presented with early-onset vomiting and feeding difficulties and were diagnosed with achalasia cardia based on characteristic findings on timed barium esophagogram (TBE). None of our patients had features of Allgrove (Triple-A) syndrome, which is characterized by achalasia, alacrimia, and adrenal insufficiency. Serial controlled radial

RATIONALE: Long gap esophageal atresia (LGEA) is a rare congenital malformation. Magnamosis represents a minimally invasive technique for LGEA to achieve esophageal recanalization for feeding. Few cases had been reported worldwide.
PATIENT CONCERNS: An infant was diagnosed as congenital esophageal atresia by using prenatal ultrasound imaging including the absence of gastric bubble and polyhydramnios.
DIAGNOSES: LGEA was confirmed after birth by esophagography through the gastrostomy and esophagus simultaneously.
INTERVENTIONS: Laparoscopic gastrostomy was performed on the 3rd day after birt

This case report presents a woman in her 20s with VACTERL association who presented with acute on chronic 10-year pelvic pain and haematometra. Initially, diagnostic imaging suspected a didelphic uterus with a dilated right horn on CT imaging. However, after physical examination and further imaging, further diagnosis of unicornuate uterus with non-communicating horn was noted. After years of expectant management, the patient began to experience severe malnutrition and metabolic derangements. After significant patient-centred discussions regarding pain relief, the patient was refractory to oral

Persistent Fourth Right Aortic Arch (PRAA) with retention of the left ductus arteriosus constitutes the most prevalent clinically significant form of vascular ring anomalies in both dogs and cats, accounting for approximately 95% of aortic arch developmental anomalies. This anomaly causes oesophageal compression in weaned animals, leading to restricted passage of solid food, postprandial regurgitation, and potential development of acquired megaoesophagus. Notably, neonatal or lactating animals usually do not exhibit these consequences. While PRAA is well documented in dogs and cats, its occurr

BACKGROUND AND OBJECTIVE: Data on file with the US FDA, and other published studies, suggest that the selective cyclo-oxygenase (COX)-2 inhibitor NSAID rofecoxib has a greater hypertensive adverse effect than other NSAIDs, including celecoxib. In this study we describe a pharmacoepidemiologic analysis of spontaneous adverse event reports of acute, clinically serious hypertension (as defined by hospitalisation) reported in association with rofecoxib, celecoxib, nabumetone and oxaprozin. The objective of this analysis is to assess whether postmarketing data are consistent with results of clinica
