Congenital esophageal stenosis presenting as noncardiac, esophageal chest pain.

A case of a 31-year-old female with congenital esophageal stenosis presenting with symptoms of chest pain caused by esophageal dysmotility is described. The involved segment in congenital esophageal stenosis has a characteristic thickening of the muscularis propria layer, as seen by EUS examination. In these patients, symptoms of dysphagia can be managed with esophageal dilation and noncardiac esophageal chest pain responds to pharmacotherapy with diltiazem.

[Cor triatriatum associated with inter-atrial defect: diagnosis by transesophageal echocardiography].

Cor triatriatum (CT) is a rare malformation consisting of a fibromuscolar membrane that subdivides the left atrium in a postero-superior (or accessory) chamber and an antero-inferior chamber (true left atrium, containing the left atrial appendage). In its classic form, the accessory chamber receives the pulmonary veins and communication with the left atrium is accomplished by way of one or more fenestrations in the membrane. The malformation is usually isolated, but in about one in four patients is associated with other congenital defects of a complex nature. We describe a case of CT associate

Usefulness of transesophageal echocardiography in the pediatric catheterization laboratory.

Transesophageal echocardiography was performed in 51 children (aged 2 to 14 years, mean 4; weight 9 to 50 kg, mean 21) undergoing elective diagnostic or therapeutic cardiac catheterization. The interventional procedures were percutaneous balloon dilation of pulmonary (n = 8) and aortic (n = 2) valve stenosis, percutaneous closure of patent ductus arteriosus (n = 8), and attempted occlusion of Pott's anastomosis by the double umbrella device (n = 1). The diagnostic catheterizations were performed on preoperative children of whom 5 had undergone previous palliative procedures. Precise placement

Left aortic arch and right ligamentum arteriosum causing esophageal obstruction in a dog.

A 10-week-old, mixed-breed dog was examined because of persistent regurgitation. Results of thoracic radiography and fluoroscopy of the esophagus, using a liquid suspension of barium sulfate, were suggestive of a vascular ring anomaly. During left lateral thoracotomy, a left aortic arch was identified, but the cause of the esophageal obstruction could not be determined. Results of endoscopic examination of the esophagus suggested a compressive lesion originating from the right side of the esophagus. An anomalous artery could not be identified on an aortogram. Post-mortem examination revealed a

Oesophageal compliance in naturally occurring canine megaoesophagus.

The passive biomechanical property of oesophageal compliance (OC) was measured in 8 naturally occurring cases of canine megaoesophagus, 8 matched control and 7 vagotomised control dogs. Of the 8 dogs with megaoesophagus, 6 had congenital idiopathic megaoesophagus and 2 had secondary megaoesophagus attributable to generalised skeletal muscle disease. Stepwise distension of the whole oesophagus was employed for measurement of OC at the 4.0 and 8.0 mL/kg injected volume steps within the control volume range (0 to 12.0 mL/kg). At both injected volume steps OC was higher in megaoesophagus dogs than

[Congenital stenosis of the esophagus. A rare diagnosis in children and adults].

The authors report about 3 cases of congenital stenosis of the upper third of the esophagus. This is an exceptional condition, for which 3 main types are described: a mucous diaphragm, to be compared with membranous atresia, fibrous stenosis and stenosis caused by bronchial heterotopy. The location of the stenosis in the upper third of the esophagus is extremely rare. It results from a disorder in the revascularization of the esophageal tube. The diagnosis is established in the newborn, and less frequently in children and adults, in cases of vomiting, regurgitation or dysphagia. It is based on

[Dynamic intracavitary echoes studied by transthoracic and transesophageal echocardiography].

One hundred consecutive echocardiograms performed on adults using transthoracic and transesophageal (TEE) transducers were analyzed in order to determine the prevalence of spontaneous contrast (SC) and the factors associated with it. Indications for study included complex congenital cardiopathy, evaluation of valve lesions or prosthetic valves, dissecting aortic aneurysm, cardiac masses and evaluation of ventricular function. SC was observed only with TEE and appeared in 25% of cases, most frequently in left atrium. Factors associated with contrast included mitral valve disease and replacement

Dissection of the esophagus after dilatation of a cervical diaphragm.

The authors report a case of a very large esophageal mucosal dissection observed during an esophagoscopy performed 7 days after dilatation of a cervical esophageal web. They treated this dissection by positioning an endoesophageal balloon which obtained the fast, overlapping closure of the torn mucosal layers and at the same time allowed both gastric drainage and enteral feeding via a pump.

Familial multiple congenital esophageal rings: report of an affected father and son.

Multiple esophageal rings are a very unusual cause of dysphagia. We report a case of a 35-yr-old male with multiple esophageal rings and severe dysphagia, whose father had similar symptoms. Our patient underwent esophageal dilation on one occasion and is asymptomatic at 1-yr followup. We speculate that multiple esophageal rings may be of congenital origin.

[Esophageal stenosis and bullous epidermolysis. Endoscopic treatment].

Esophageal lesions are often present in congenital epidermolysis bullosa. Bullae formation, ulceration and oedema ultimately will lead to stricture formation. Surgery is useful only for slowly evolutive forms. To prevent the risk of developing complete occlusion to the human of the esophagus, dilatations are performed under endoscopic control, that allows through nutrition without risk of local complication.

At modelling a congenital heart failure--defect of the interatrial septum during late stages of the experiment, dilatation of the pulmonary artery and certain changes in the microcirculatory bed of the lungs, oesophagus, stomach and colon are observed. It has been demonstrated histologically and morphometrically that in adaptive reactions of the blood bed an important role belongs to the arteriolar part and metabolic link of the microcirculatory bed of the organs investigated. The changes in the pulmonary vessels are insufficiently expressed and possibility of hypertension in the lesser circul

A case of early carcinoma of the oesophagus in association with annular pancreas.

A Chinese patient presented with a 3-week history of vomiting. Pre-operative barium swallow and upper endoscopy and biopsy revealed a squamous cell carcinoma of the lower oesophagus. Operative findings included a relatively early carcinoma of the oesophagus and an annular pancreas with dilated proximal duodenum. Oesophagectomy was performed and the whole stomach used for reconstruction. A pylorojejunostomy provided drainage for the stomach as well as decompression of the proximal duodenum. This patient's congenital anomaly, with its attendant symptoms, led to the early diagnosis and treatment

Distal congenital esophageal stenosis associated with esophageal atresia.

A series of six patients with congenital esophageal stenosis associated with esophageal atresia (EA) and distal tracheoesophageal fistula is presented. Three patients required only repeated dilatations, and have had good results. Two patients required limited resections of the distal esophagus, with excellent results. One patient died following a Heller myotomy. Tracheobronchial rests were present in the distal esophagus in the latter three patients. Diagnosis of congenital distal esophageal stenosis following repair of EA requires a high index of suspicion and a careful review of previous eso

Intraoperative transesophageal echocardiography--an indispensable tool in valve reconstruction and congenital anomalies.

Transesophageal echocardiography (TEE) with color flow mapping has become an excellent tool for evaluating valve reconstruction and correction of congenital anomalies intraoperatively. From February 1990 to September 1990 we have utilized TEE intraoperatively in 14 consecutive patients. Their ages ranged from 25 to 77 years, with a mean age of 58.7 years. There were eight males and six females. Twenty-one pathological entities were identified intraoperatively: mitral regurgitation due to posterior leaflet pathology in four; anterior leaflet pathology in four; annular dilatation in two; left to

Anatomic and functional abnormalities of the esophagus in infants surviving congenital diaphragmatic hernia.

As more infants with congenital diaphragmatic hernia (CDH) survive by means of extracorporeal membrane oxygenation (ECMO), new clinical observations are being made. We report anatomic and functional abnormalities of the esophagus in these infants. Twenty-two of 30 infants with CDH survived. Three of 7 non-ECMO-treated infants and 14 of 17 ECMO-treated infants had an air- or fluid-filled mediastinal mass that was confirmed by an upper gastrointestinal series as ectatic esophagus. Contrast studies demonstrated severe gastroesophageal reflux. Intra-esophageal pH monitoring confirmed significant r

Temporary banding of the gastroesophageal juncture in a very small neonate with esophageal atresia and tracheoesophageal fistula.

A 700 gm, small-for-gestational-age infant with severe respiratory distress, esophageal atresia, and Type C tracheoesophageal fistula was treated with initial banding of the gastroesophageal juncture followed by a gastrostomy. Primary repair was completed later, when the infant had stabilized and grown. Continuous air pressure had distended the fistula, thus dilating the distal esophagus and facilitating repair. In very small, high-risk infants with Type C esophageal atresia and tracheoesophageal fistula, banding of the distal esophagus enables enteral feeding and time for growth while protect

[Congenital esophageal diverticulum].

The congenital or acquired variety of esophageal diverticulum is a rare childhood disease. Reported is a case-study which deals with a five year old patient who arrived at our hospital complaining of a constricture at the cricopharyngeal level. A month later he returned to the hospital with dysphasia and regurgitation; the X-rays and endoscopic diagnosis showed stenosis of the esophagus and the presence of an esophageal diverticulum. Esophageal dilatations were carried out until an acceptable esophageal diameter was reached. Later on, the surgical removal of the diverticulum was performed with
