In this study, 41 randomly chosen patients aged 15 to 35 years (mean 22 years) were carefully examined. As primary operations there were 13 membrane excisions, five duodenoduodenostomies, 22 duodenojejunostomies, and one gastrojejunostomy. Twenty-eight patients were symptom-free, ten admitted some discomfort, three had major pains, including one with a history of duodenal ulcer. Reoperation for adhesion ileus had been performed in six patients, in the early postoperative phase in one instance. At late follow-up barium meals (N = 41) showed completely normal findings in two cases only, hiatal h

Extracorporeal membrane oxygenation (ECMO) for survivors of surgical repair of congenital diaphragmatic hernia was used in 14 neonates. All showed marked esophageal dilatation on postoperative chest radiographs; the dilated esophagus simulated an air- or fluid-filled mass. All patients were later shown to have marked gastroesophageal reflux as well. The finding suggests a problem in swallowing exists before birth, which may explain the recent correlation between diaphragmatic hernia and hydramnios. The observation of a mediastinal mass in neonates with congenital diaphragmatic hernia may repre

The purpose of this paper is to report our analysis of four generations of a family with congenital strictures of the lower esophagus associated with leiomyoma of the gastrointestinal tract. Two members of the family died of sarcomatous degeneration of the leiomyoma and one is still alive 2 years after resection of the malignancy. Three family members had surgical repair of the congenital stricture. Surgical repair was unnecessary in one, for whom repeated dilatations alleviated the symptoms. Since there were no direct female descendants in this family, it is unknown whether this hereditary di

Over the past 30 years, the authors reconstructed the esophagus with the right colon in 85 patients who had congenital and benign disease. The first individual operated upon in 1955 was asymptomatic. Nine patients had congenital tracheoesophageal fistula with atresia, four esophageal varices, 31 advanced obliterative esophagitis, and 23 corrosive destruction. In seven patients, severe esophagitis followed esophagogastrectomy, four had unsuccessful operations for achalasia, and seven had colon bypass following esophageal perforation. Eleven early nonfatal complications occurred. Remote nonfatal

Congenital persistent right aortic arch was diagnosed as the cause of weight loss and regurgitation in 2 dogs, aged 2.5 and 8 years, respectively. The first dog had 2 brief episodes of regurgitation that resolved spontaneously before the most recent onset of signs and diagnosis. The second dog had no clinical signs attributed to persistent right aortic arch until 2 months before the diagnosis was made. Dogs born with persistent right aortic arch typically have clinical signs of esophageal stenosis around the time of weaning. Evidence from the 2 dogs in this report indicate that clinical signs

From 1964 to 1985 inclusive, gastric tubes have been constructed in 36 infants and children to replace a congenitally defective esophagus or an esophagus with acquired irreparable damage. This report reviews our entire series to date covering a 21-year experience. There were 24 boys and 12 girls ranging in age from 3 months to 17 years. Twenty-two patients were operated on because of congenital esophageal defects and 14 for acquired lesions. A proximally based antiperistaltic tube was constructed in 32 instances, and a distally based isoperistaltic tube made in four. Thirty tubes were passed r

An infant with cricopharyngeal achalasia, recurrent aspiration pneumonia, and severe failure to thrive was fed for 5 months through a nasogastric tube. She responded promptly to dilatations of the spastic cricopharyngeal muscle. Normal oral feedings followed the treatment with progressive weight gain and no further pulmonary complications over a follow-up period of 1 1/2 years. Dilatations were used in the infant described here, in analogy to the well-known success of treatment by dilatations in infants with achalasia of the lower esophagus.

Strictures of the esophagus are difficult and challenging problems. Whether a result of caustic ingestion, trauma, or surgery, or a congenital abnormality, the patient suffers a disruption of the fundamental act of swallowing. The seven cases of stricture of the cervical esophagus presented here were not alleviated by the usual methods of antegrade and retrograde dilation. Each patient underwent a two-stage reconstruction of the esophagus. The strictures in four patients were secondary to a caustic agent, two were congenital, and one was of unknown etiology. All patients no longer require dila

A full-term infant developed dysphagia, vomiting, and regurgitation immediately after birth. On examination at 5 months of age, he was malnourished and had radiographically proven megaesophagus. On direct examination his blood was positive for Trypanosoma cruzi. Additionally an enzyme immunoassay for T. cruzi IgM antibodies was positive. He underwent corrective surgery for the megaesophagus but died post-operatively of aspiration pneumonia. At autopsy, the esophagus was markedly dilated. There was an extreme reduction of neurons in Auerbach's plexus, and inflammation of this plexus and the mus

In 11 years (1971-1981) 1.040 cases of anomalies of the cardio-hiatal sector have been controlled in the Children's Hospital of the Social Security of Barcelona, 101 patients were operated. Surgical indication was based on the following facts: size of the hernia, poor clinical evolution with postural treatment, or infants of more than 2 years who also presented a high gastric acidity or a clearly pathological pHmetry, or, lastly, a social cause. We have used a much more physiological technique than that of Nissen, based on the obtention of a large segment of the abdominal esophagus on which ch

Computed tomographic (CT) studies of the cardiovascular system were performed on 850 cases at our institute during the last five years. The aorta was clearly demonstrated by CT from the aortic root to the bifurcation of the iliac artery, and in most cases the main aortic branches including the coronary, brachiocephalic, common carotid, subclavian, celiac, superior mesenteric, renal and iliac arteries were satisfactorily evaluated (Fig. 1-3). Therefore CT renders us encouraging to detect the abnormality of these arteries. This paper described the CT findings of congenital anomalies of the great

This communication describes one case treated by the authors and reviews 27 reports of congenital esophageal stenosis caused by tracheobronchial remnants. Histologic examination of the case showed respiratory mucous glands with respiratory epithelium and hyaline cartilage. The anomalies presumably result from failure of normal embryonic separation of the respiratory tract from the primitive esophagus. Primary resection is recommended as definitive treatment. Dilatation of the stenosis affords only temporary relief.

Radiological features of the oesophagus of 7 normal horses and 13 with oesophageal lesions are described. The use of barium sulphate as a contrast agent and the techniqes of its administration are discussed. It is suggested that chronic intermittent oesophageal impaction with food material was a predisposing factor in the development of a localised oesophageal dilation in 3 horses and that one other probably resulted from an injury. A similar dilatation was seen which resulted from oesophageal constriction by a vascular ring. Megaoesophagus was seen in 2 ponies associated with grass sickness a

This communication describes two new cases of esophageal malformation situated at the usual level of atresias. In these two stenosis, the superior segment overlapped the inferior segment a certain length, and the inferior segment adhered closely to the trachea. When recognized, this malformation responds well to instrumental dilatation.

Functional hindrances in passage of the oesophageal tract are particularly recognised in achalasia, which occurs in less than 5% of children. The megaoesophagus accompanying massive gastro-oesophageal reflux can also be a consequence of functional disturbances. In addition, peptic stenosis of the oesophagus usually causes dilatation of this organ. Other organic stenoses are just as rare, whether they are congenital membranous or fibromuscular stenoses, or occur as secondary narrowing of the oesophagus, following accidental acid-burns. Treatment of choice in achalasia is cardiomyotomy with inse

A two-month-old female foal with duodenal stenosis is described. The foal was in poor condition and showed an abnormal stable behaviour. She stood preferably with her head stretched out and reposing on the mare's back or on the stable ridge. Salivation was an important symptom. Clinical and radiological examination of the oral cavity, the pharynx, and the oesophagus showed no abnormalities, except for the antiperistaltic wave along the oesophagus. Because of her bad condition the filly was euthanized. At post mortem examination, apart from leukoplakia, an erosive ulcerative inflammation was fo

Myasthenia gravis is characterized by muscle weakness, which is alleviated by rest and by anticholinesterase drugs. There are two forms of the disease in the dog, acquired and congenital. The acquired form occurs either in young adults, or in older animals that have developed mediastinal tumors. Clinically, there is weakness of the muscles of the limbs, neck, and head, together with dilatation of the esophagus. In some cases, circulating antibody to acetylcholine receptor is present and the amount of receptor in the end-plates is decreased. It is thought to be an autoimmune disease. The congen
