Management of congenital microgastria with a jejunal reservoir pouch.

A 3-mo-old female presented with growth retardation, vomiting, reflux esophagitis, recurrent aspiration pneumonias, and was found to have megaesophagus and microgastria. After the failure of conservative therapy a double-lumen jejunal (Hunt-Lawrence) pouch with distal Roux-en-Y anastomosis was anastomosed to the stomach to increase the gastric reservoir. One year later, there has been progressive weight gain, the megaesophagus and gastroesophageal reflux have lessened significantly, pneumonia has not recurred, and the tracheobronchitis and esophagitis have resolved. This suggests that the gast

Esophageal atresia and tracheoesophageal fistula.

Fifty-seven babies were surgically treated for esophageal atresia and tracheoesophageal fistula between 1968 and 1978. Forty-eight (84%) had proximal esophageal atresia and a distal tracheoesophageal fistula, 2 (4%) had proximal and distal esophageal atresia and no tracheosophageal fistula, and 7 (12%) had a tracheosophageal fistula without esophageal atresia. Primary repair was accomplished in 43 patients (75%), colon interposition was required in 5 (9%), while the remainder had staged or palliative reapirs. Forty-six (81%) survived surgical treatment. All 21 babies in Waterston Category A, 9

Vascular ring anomalies: case report and brief review.

During early fetal life six pair of aortic arches surround the esophagus and trachea. Normal maturation and selective regression of these structures form the adult vasculature. Abnormal location or development of the aortic arches may result in pressure on adjacent organs. Vascular ring anomalies must be considered with any patient with a history of regurgitating food shortly after eating. Physical examination, test feedings, survey and contrast radiographs may give an accurate impression of the problem but a final diagnosis can only be made following surgical exploration. In the case presente

The flaccid trachea and tracheoesophageal congenital anomalies.

The patient with an esophageal atresia associated with a distal tracheoesophageal fistula, is often found to have an abnormally soft trachea. The use of contrast material as an aid in the diagnosis of this congenital anomaly demonstrates radiologically the compressive effect that a dilated and hypertrophied proximal esophageal pouch has on the trachea of the newborn. It is postulated that this chronic compressive force plays a part in retarding the development of the trachea during fetal life. The important decompressive effect of a distal tracheoesophageal fistula allowing nonphysiologic esca

Intramural pseudodiverticulosis of the esophagus.

Intramural pseudodiverticulosis of the esophagus is a rare benign disease, developing most frequently in older males, but occurring in both sexes, at all ages. In some patients symptoms from early childhood suggest congenital origin. The characteristic appearance of "pinhead" outpouching from the lumen of the esophagus is seen with contrast esophagram. The major symptom is dysphagia with episodic exacerbations, often with foreign body lodgement. The pseudodiverticula are formed by abnormally dilated adnexal glands and ducts which lie in the submucosa of the esophagus. Secondary infection, most

Midesophageal web in adults.

Stenotic webs in the midesophagus of adults are rare. Including the present patient, only 6 have been described in the literature in English. Clinical presentation is characterized by dysphagia and insidious weight loss in a patient usually over 40 years old. The cause of the lesion is unknown. Based on knowledge of the embryological development of the esophagus and the absence of microscopical inflammatory changes in true webs, a congenital rather than inflammatory basis seems likely. Treatment of midesophageal webs can be accomplished by dilation, divulsion through an esophagoscope, or trans

High peptic stricture of the oesophagus.

Fifty-seven patients with high peptic stricture and the lower oesophagus lined by columnar epithelium are considered from the clinical point of view. Information from 115 cases of low stricture is introduced for comparison. The average age of adult patients was 62 years with a sex incidence of 36 females to 21 males. There is little difference between the symptoms of high and low strictures. Radiologically, the majority of high strictures are short and smooth but other types are illustrated. Carcinoma and congenital mid-oesophageal web are considered in the differential diagnosis. There was an

Columnar cell-lined esophagus: assessment of etiology and treatment. A 22 year experience.

Among 1,000 patients with hiatal hernias were 45 who had biopsy-proved columnar-lined esophagus (CLE). Twenty-one were male and 24 female, with bimodal age ranges peaking at zero to 10 and 48 to 80 years. Of the first decade patients, all boys, 2 were brothers. While 44 had dysphagia, one third also had iron-deficiency anemia. All had x-ray-proved sliding hiatal hernias, with esophageal stricturing at the squamous cell-columnar cell interface. In 43 cases this area was 35 cm. or less from the upper jaw. The epithelial histology showed simple, tubular, mucus-secreting glands (45 cases), goblet

[Congenital stenosis of the esophagus (personal case and review)].

Starting from personal observation of a case of congenital stenosis of the oesophagus, the Authors proceed to a review of the scanty literature on the subject. After a discussion of the most outstanding aspects of differential diagnosis, the frequent necessity of histological verification, especially in adolescence and adult age, in order to obtain a correct understanding of the nature of the case, is stressed. On the basis of an essentially favourable prognosis, the Authors maintain that, where medical and dilating treatment is insufficient, surgery is indicated and evidence the straight-forw

Congenital oesophageal strictures due to cartilaginous rings.

Two cases of distal oesophageal stricture secondary to tracheo-bronchial remnants containing cartilage are presented. The clinical histories and radiographic appearances of these lesions are discussed as is the postulated embryogenesis. A characteristic finding, when present, is the linear collection of barium projecting horizontally from the area of stricture, representing respiratory epithelial ducts.

Aberrant origin of left pulmonary artery (vascular sling). Report of the clinical and anatomic features in three patients.

The clinical and anatomical findings in 3 patients with aberrant origin of the left pulmonary artery from the right pulmonary artery ("vascular sling") are presented. All 3 children symptoms of severe respiratory distress shortly after birth. In 2 children the correct diagnosis was suggested from the roentgenogram of the thorax, because of an indentation in the anterior wall of the esophagus. The diagnosis was further endorsed by selective angiography of the aberrant left pulmonary artery. The second patient is of particular interest, since the correct diagnosis was missed because of the prese

Congenital achalasia of the oesophagus in children.

Nine cases of congenital achalasia of the cardia in children are reported, of which four were below 1 year old and recording the youngest age of 3 months. A hereditary element is present in four children. Association with Hirschsprung's disease of the colon in one case and with hiatal hernia in another case, favours a more generalized neuromuscular pathology than a simple spasm of the cardia.

Fiberoptic endoscopic diagnosis and treatment of a congenital esophageal diaphragm.

A 9-month-old infant suffering from dysphagia to solids presented with esophageal bolus obstruction. Using an adult fiberoptic esophagoscope, a congenital web was found at the junction of the middle and distal thirds of the esophagus. Previous direct rigid esophagoscopy and roentgenologic examination had not visualized the web. The web was successfully ruptured and dilated by inserting a guide wire under fiberscopic control and then inserting metal olives, followed later by blind mercury bougienage. This report illustrates the use of adult fiberoptic instruments for diagnosis and treatment in
