Biodegradable and self-expanding metal stents in the treatment of children with strictures after esophageal atresia repair.

PURPOSE: This study compared the efficacy and complications of biodegradable stents (BDS) versus self-expandable fully covered metal stents (SEMS) in pediatric patients with strictures after esophageal atresia repair. METHODS: The charts of children with esophageal atresia (EA) undergoing stent treatment for anastomotic stricture were retrospectively reviewed. Primary outcomes included time to reintervention (TTR) and procedural success, defined as no necessity of further intervention with a patent esophageal lumen at the time of follow-up. Secondary outcomes included stent-related complicati

Type of pledget and suture technique relative to esophageal tensile strength in long-gap esophageal atresia.

PURPOSE: We aimed to investigate whether pledget presence, pledget composition, and full versus partial thickness suture technique affect the force esophageal tissue can withstand before tearing. METHODS: A right thoracotomy was performed on 10 deceased fetal sheep, 110-114 days gestation. The esophagus was divided at the level of the carina and 2 cm of tissue was removed to mimic long-gap esophageal atresia. The pouches were sutured with horizontal mattress non-absorbable braided polyester suture using one of three methods: polytetrafluoroethylene (PTFE) pledgets, bovine pericardial pledgets

Health-related quality of life following thoracoscopic repair in children with long-gap esophageal atresia.

BACKGROUND: By retaining the native esophagus and minimizing postoperative morbidity, the thoracoscopic approach contributes to improved functional outcomes for patients with esophageal atresia (EA). The aim of this study was to evaluate HRQoL-including digestive, respiratory, and feeding outcomes in children treated exclusively by a thoracoscopic approach, with a specific focus on comparing outcomes between patients with LGEA (Gross type A and B) and those with DTEF (Gross type C or D). MATERIALS AND METHODS: LGEA was treated using the internal traction technique with staged repair, 25 child

Sustained efficacy and long-term outcomes of autologous oral mucosal epithelial cell sheet transplantation for pediatric esophageal anastomotic restenosis.

Refractory anastomotic restenosis following surgery for congenital esophageal atresia (CEA) or congenital esophageal stenosis (CES) remains a serious complication in pediatric surgery. Repeated endoscopic balloon dilatation (EBD) under general anesthesia often impairs growth, feeding function, and overall quality of life (QOL). We have developed a regenerative therapy using autologous oral mucosal epithelial cell sheets. This study aimed to evaluate the long-term safety and efficacy of epithelial cell sheet transplantation for refractory esophageal restenosis in pediatric patients. Between 201

Co-Occurring Non-Cardiac Congenital Anomalies Among Cases With Congenital Heart Defects.

Cases with congenital heart defects (CHD) often have other associated anomalies. The aim of this investigation was to assess the prevalence and the types of co-occurring anomalies in CHD in a well-defined population. The anomalies co-occurring with CHD were ascertained in all live births, stillbirths and terminations of pregnancy for fetal anomaly during 29 years in 387,067 consecutive pregnancies of known outcome in the area covered by our population-based registry of congenital anomalies. Of the 4640 cases with CHD ascertained during this period (prevalence of 119.9 per 10,000), 1228 (26.5%)

Genital and urinary tract malformations, associated morbidity, and need for urogenital surgery among children with esophageal atresia: a retrospective cohort study.

PURPOSE: Genital malformations are increasingly recognized when a VACTERL is present, but remain insufficiently described in children with esophageal atresia (EA). This study aimed to determine the prevalence and spectrum of genital and urinary tract malformations in children with EA, and to describe associated morbidity, need for additional surgery, and association with Gross classification. METHODS: Retrospective cohort study including all children undergoing surgery for EA at a tertiary center of pediatric surgery between 2012-2023. Children deceased within five days of birth were excluded

Perioperative risk factors for short- and long-term respiratory morbidity in patients with esophageal atresia.

PURPOSE: Respiratory complications are a major source of morbidity in esophageal atresia (EA), yet predictors of short- and long-term respiratory outcomes are not well defined. This study aimed to identify perioperative factors associated with respiratory morbidity in EA/TEF patients. METHODS: We conducted a retrospective cohort study of 242 EA patients treated at a single tertiary center from 2005 to present. Short-term respiratory morbidity was defined as the need for supplemental respiratory support at 30 days postoperatively from primary EA repair. Long-term morbidity was determined based

A Rare Case of an Abdominal Pedunculated Bronchogenic Cyst.

Bronchogenic cysts (BC) are rare congenital malformations that result from abnormal budding of the primitive foregut. Most of these cysts are located in the mediastinum or the lungs. Rarely, abdominal BCs result from embryonic budding migrating below the diaphragm prior to the fusion of the pleuroperitoneal membranes. We present a case of a 34-year-old male patient undergoing evaluation for bronchopneumopathy. Preoperative imaging revealed a well-circumscribed cystic lesion adjacent to the colon, which failed to provide a definitive diagnosis due to its atypical location and nonspecific radiol

Intrapulmonary Bronchogenic Cyst Adjacent to the Inferior Pulmonary Ligament Mimicking a Paraesophageal Mediastinal Cyst: A Case Report.

Bronchogenic cysts are rare congenital foregut malformations that most commonly arise in the mediastinum. Intrapulmonary bronchogenic cysts are less frequent and may present diagnostic challenges when located adjacent to mediastinal structures. We report a case of an intrapulmonary bronchogenic cyst that closely mimicked a paraesophageal mediastinal cyst on preoperative imaging. A 72-year-old man was referred to our department after a cystic lesion was incidentally detected during a routine health examination. CT demonstrated a 37-mm well-circumscribed cystic lesion in the right lower thorax.

Laparoscopic transhiatal management of a rare case of esophageal lung and medialized stomach associated with heterotaxy - a technical case report.

INTRODUCTION: Esophageal lung is a rare and complex congenital malformation, in which the bronchus of one or several pulmonary lobes arises from the esophagus. It is frequently associated with other malformations. Surgical management needs individual approaches, is often complex and depends on concomitant disorders. CASE DESCRIPTION: A 3-month-old male was referred for further management of a left lower esophageal pulmonary lobe associated with a laryngotracheal cleft, congenital heart disease, heterotaxy, medialized stomach with additional microgastria and severe gastroesophageal reflux lead

Eosinophilic esophagitis: An aerodigestive perspective.

Eosinophilic esophagitis (EoE) is an immune-mediated inflammatory condition of the esophagus that is characterized by esophageal dysfunction and distinct histopathologic findings of elevated eosinophils in the esophageal mucosa. EoE can present in the aerodigestive patient with a variety of symptoms ranging from feeding difficulty to extraesophageal symptoms such as chronic cough. Routine evaluation for EoE should be performed in high-risk patients such as esophageal atresia/tracheoesophageal fistula and should be considered for those undergoing evaluation for laryngotracheal reconstruction su

Congenital aerodigestive fistula with extraordinarily delayed adult presentation: a case report and review of endoscopic closure for bronchoesophageal fistula.

BACKGROUND: Congenital aerodigestive fistulae (ADF) represent extremely rare developmental malformations derived from disrupted embryological separation of the primitive foregut, which can be classified by lesion location into tracheoesophageal fistulae, bronchoesophageal fistulae (BEF), and other subtypes. While typically diagnosed in neonates, isolated BEF without esophageal atresia ("H-type") may remain undetected for decades, presenting in adulthood with chronic, stereotyped symptoms. Endoscopic interventions for congenital BEF remain poorly characterized, with limited evidence regarding p

In Vitro Fertilization and VACTERL Birth Defects.

IMPORTANCE: The VACTERL (vertebral defects, anal atresia, cardiac defects, tracheo-esophageal fistula, renal defects, and limb defects) pattern of birth defects is a complex congenital condition with largely unknown causes. Studies suggest a link between in vitro fertilization (IVF) and birth defects, including VACTERL, but it remains unknown whether IVF has a unique association with VACTERL. OBJECTIVE: To characterize the association between IVF and VACTERL. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included 1 555 936 live births in 4 US States (Massachusetts, New York, North Car

HLA genotype, clinical and immunofluorescence findings of 30 patients with type VII collagen antibody positive subepidermal bullous disease.

Epidermolysis bullosa acquisita (EBA) and mucous membrane pemphigoid are mucocutaneous blistering disorders that are associated with antibodies that target type VII collagen (anti-col7). There are relatively few studies that closely investigate the clinical phenotype and HLA genotype of patients who are anti-col7 positive. We therefore characterized all patients with anti-col7-positive subepidermal bullous disease that were treated at our centre. In total, 30 patients were identified, 29 had mechanobullous skin lesions and all had oral ulceration. The most common sites for oral lesions were th

Surgical treatment of pulmonary hydatidosis: six years of experience and results at the National Hospital Guillermo Almenara Irigoyen, Lima, Peru.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The Canadian Experience With Magnetic Esophageal Anastomosis: A Report From the Canadian Consortium for Research in Pediatric Surgery (CanCORPS).

OBJECTIVE: Magnetic anastomosis is an innovative technique for establishing esophageal continuity in infants born with esophageal atresia. Few case series featuring this technology have been published, with even fewer reports on complications. We present the entire Canadian experience with this approach. METHODS: A nationwide retrospective review of all esophageal atresia patients treated with magnetic anastomoses in Canada was conducted through the Canadian Consortium for Research in Pediatric Surgery (CanCORPS). RESULTS: Magnetic anastomoses were attempted in nine patients including four T

Staged thoracoscopic internal traction approach for early repair of long-gap esophageal atresia (LGEA) with distal tracheoesophageal fistula (TEF).

BACKGROUND: Long-gap esophageal atresia (LGEA) can complicate the management of esophageal atresia (EA) with or without a tracheoesophageal fistula (TEF). This series describes a short interval, staged, thoracoscopic internal traction approach for LGEA with distal TEF to manage complex anastomotic tension or an anatomically impossible esophageal anastomosis. METHODS: A retrospective review (2018-2024) was performed across four tertiary centers to identify patients with LGEA and distal TEF, managed with a staged, thoracoscopic internal traction approach. In the first stage, the TEF was ligated
