Immunopeptidomics of cutaneous leishmaniasis patients reveals the natural antigenic landscape.

INTRODUCTION: Cutaneous leishmaniasis (CL) is a skin disease caused by Leishmania infection, for which no licensed human vaccine exists. Protective immunity is largely T cell-mediated and depends on antigen presentation by MHC molecules, yet the naturally presented epitopes during human disease remain poorly defined. METHODS: To address this gap, we performed mass spectrometry-based immunopeptidomics on lesional biopsies from 27 Ethiopian CL patients spanning the full clinical spectrum. RESULTS: We newly identified 333 MHC-I and 247 MHC-II epitopes from 398 L. aethiopica proteins, including

Guanylate-binding proteins balance iNOS/Arg-1 in myeloid cells during L. major infection and promote host defense to infection.

Cutaneous leishmaniasis (CL) is a debilitating neglected tropical disease characterized by lesions that can range from self-healing to permanent disfiguration. A predominant Th1 response, which stimulates IFN-γ production, is crucial for parasite control during self-healing CL. While IFN-γ primarily activates macrophages to produce nitric oxide via inducible nitric oxide synthase (iNOS), leading to parasite control, it also activates other downstream pathways involved in cell autonomous immunity. One such pathway is the activation of guanylate-binding proteins (Gbps), a class of interferon-ind

Host and Parasite-Targeted Mechanisms of Gedunin Against Cutaneous Leishmaniasis: In Vitro and In Vivo Evaluation.

BACKGROUND: The main pharmacological interventions for cutaneous leishmaniasis (CL) are associated with various adverse effects. The present study aims to comprehensively evaluate the antileishmanial impact of gedunin (GN) and its effects on the expression of immune and virulence-related genes. METHODS: The antileishmanial efficacy and synergistic effects of GN, both as a monotherapy and in combination with glucantime (GT), were evaluated against the intracellular amastigote stages of Leishmania tropica. Additionally, the impact of GN on the expression of genes related to immunomodulation and

Metabolically reprogrammed eosinophils impair T cell immunity and cause chronic skin infection.

Eosinophils exhibit antimicrobial, cytotoxic and immunoregulatory effects, but our knowledge of their transcriptional and functional heterogeneity is still limited, especially in non-intestinal tissues. Here, we used a mouse model of chronic cutaneous inflammation elicited by the protozoan pathogen Leishmania mexicana to investigate the function and transcriptional dynamics of skin eosinophils. Infection of C57BL/6 mice triggered local and systemic eosinophilia that was driven by type 2 innate lymphoid cells and interleukin-5. Genetic and pharmacological eosinophil depletion led to an enhanced

Cutaneous Leishmaniasis in Patients under Biologic Therapies: A Spanish CLINI-AEDV Multicentre Study.

Anti-TNFα agents have been associated with the reactivation of granulomatous infections, including leishmaniasis. Cases of atypical or recurrent cutaneous (CL) or mucocutaneous leishmaniasis (MCL) have been reported in patients on biologics. We conducted a retrospective analysis of a multicenter database to describe the clinical characteristics and management of CL and MCL in patients receiving biological therapy in an endemic area. Clinical features, diagnostics, therapies, and outcomes were analyzed. Seventy-one patients were included (63 CL and 8 MCL). The most common underlying condition w

Epidemiology and clinical spectrum of leishmaniasis due to Leishmania infantum in Tunisia: Current status, challenges, and perspectives.

BACKGROUND: This review focuses on leishmaniasis caused by Leishmania (L.) infantum in Tunisia, a vector-borne parasitic disease transmitted through the bite of infected female sandflies. Leishmaniasis manifests as a spectrum of clinical forms, ranging from benign cutaneous lesions, to a severe and potentially fatal visceral form. In Tunisia, L. infantum is the etiological agent of visceral leishmaniasis (VL) and cutaneous leishmaniasis (CL). While CL typically manifests as a single, small facial lesion, atypical forms are sometimes observed. VL primarily affects children under the age of five

Unravelling complex interactions during Toxoplasma, Plasmodium, and Leishmania co-infections in French Guiana.

Toxoplasma gondii, Plasmodium spp., and Leishmania spp. collectively infect billions and cause millions of deaths worldwide. In French Guiana, the geographical coexistence of malaria, acute toxoplasmosis, and cutaneous leishmaniasis may result in sequential or simultaneous infections. This study assesses the seroprevalence of protozoan multi-infections and their impact on host responses compared to individual infections. We analyzed plasma samples and clinical data from patients with these diseases and healthy controls (n = 253) collected at Cayenne Hospital Centre between 2012 and 2022. Serop

A lightweight deep learning framework for reliable microscopy-based diagnosis of cutaneous leishmaniasis.

Cutaneous leishmaniasis (CL) is a neglected tropical and zoonotic disease affecting both human and animal health, for which microscopic examination of Giemsa-stained slides remains the diagnostic reference standard despite being time-consuming and operator-dependent. In this study, we developed a lightweight, calibration-aware deep learning framework for automated amastigote detection and slide-level diagnostic probability estimation from microscopy images. A U-Net architecture with a MobileNetV2 encoder was employed for pixel-level parasite segmentation using a weakly supervised pseudo-labeli

Safety and efficacy of intralesional amphotericin B in the treatment of Old World cutaneous leishmaniasis: a nonrandomized open-label prospective study.

BACKGROUND: Cutaneous leishmaniasis (CL) is a neglected tropical disease with limited accessible treatment options. Although pentavalent antimonials remain the gold standard, emergence of drug resistance and recurrent stockouts pose significant challenges. Amphotericin B is a well-established treatment of visceral leishmaniasis; however, its use in Old World CL (OWCL), particularly via the intralesional route and using conventional formulations, has been studied only anecdotally to the best of our knowledge. OBJECTIVES: To evaluate the safety and efficacy of intralesional amphotericin B deoxy

Operationalising One Health for epidermal parasitic skin diseases: threshold-triggered mass drug administration and a minimal indicator set.

Epidermal parasitic skin diseases (EPSDs)-including scabies, pediculosis, cutaneous leishmaniasis, tungiasis and cutaneous larva migrans-affect millions worldwide yet remain neglected within global health programmes. Control is hindered by delayed diagnosis, fragmented service delivery, weak surveillance and limited integration of human, animal and environmental interventions. This review operationalises a One Health approach for EPSDs by proposing a pragmatic, threshold-triggered control pathway supported by a minimal indicator set suitable for district-level implementation. Evidence publishe

Fifteen-Year Evolution of Chronic Cutaneous Leishmaniasis Mimicking Other Dermatoses.

Cutaneous leishmaniasis is an endemic parasitic infection in Colombia that typically presents as ulcerated skin lesions, although atypical variants can mimic diverse dermatosis leading to misdiagnosis and delayed treatment. We report the case of a 32-year-old woman with a 15-year history of progressive granulomatous and exophytic tumor-like plaques involving her lower face and neck. She was initially misdiagnosed with lymphocutaneous sporotrichosis and treated unsuccessfully with itraconazole for 1 year. The chronic facial involvement during her formative years resulted in major aesthetic and

Revisiting Leishmania (Viannia) lainsoni Silveira, Shaw, Braga, and Ishikawa, 1987: current knowledge and emerging perspectives.

More than three decades after its description, Leishmania (Viannia) lainsoni Silveira, Shaw, Braga, and Ishikawa, 1987 remains the least explored species of the subgenus Viannia Lainson and Shaw, 1987 within the genus Leishmania Ross, 1903. Although long considered rare and of limited clinical impact, recent records reveal a wider geographic range, diverse vector associations, and increasing involvement in American cutaneous leishmaniasis (ACL). Advances in molecular and genomic studies have clarified its taxonomy and uncovered notable biological and genetic variability. This review synthesize
